Linus Torvalds 9657752cb5 Merge branch 'perf-core-for-linus' of git://git.kernel.org/pub/scm/linux/kernel/git/tip/tip
Pull perf updates from Ingo Molnar:
 "Kernel side changes:

   - Add branch type profiling/tracing support. (Jin Yao)

   - Add the PERF_SAMPLE_PHYS_ADDR ABI to allow the tracing/profiling of
physical memory addresses, where the PMU supports it. (Kan Liang)

   - Export some PMU capability details in the new
     /sys/bus/event_source/devices/cpu/caps/ sysfs directory. (Andi
     Kleen)

   - Aux data fixes and updates (Will Deacon)

   - kprobes fixes and updates (Masami Hiramatsu)

   - AMD uncore PMU driver fixes and updates (Janakarajan Natarajan)

On the tooling side, here's a (limited!) list of highlights - there
were many other changes that I could not list, see the shortlog and
  git history for details:

  UI improvements:

   - Implement a visual marker for fused x86 instructions in the
annotate TUI browser, available now in 'perf report', more work
needed to have it available as well in 'perf top' (Jin Yao)

Further explanation from one of Jin's patches:

             │ ┌──cmpl $0x0,argp_program_version_hook
       81.93 │ ├──je 20
             │ │ lock cmpxchg %esi,0x38a9a4(%rip)
             │ │↓ jne 29
             │ │↓ jmp 43
       11.47 │20:└─→cmpxch %esi,0x38a999(%rip)

That means the cmpl+je is a fused instruction pair and they should
     be considered together.

   - Record the branch type and then show statistics and info about in
     callchain entries (Jin Yao)

Example from one of Jin's patches:

        # perf record -g -j any,save_type
        # perf report --branch-history --stdio --no-children

        38.50% div.c:45 [.] main div

                   compute_flag div.c:28 (cycles:2)

                   rand rand.c:28 (RET CROSS_2M cycles:1)
                   __random random.c:298 (cycles:1)
                   __random random.c:297 (COND_BWD CROSS_2M cycles:1)
                   __random random.c:295 (cycles:1)
                   __random random.c:295 (COND_BWD CROSS_2M cycles:1)
                   __random random.c:295 (cycles:1)
                   __random random.c:295 (RET CROSS_2M cycles:9)

  namespaces support:

   - Add initial support for namespaces, using setns to access files in
namespaces, grabbing their build-ids, etc. (Krister Johansen)

  perf trace enhancements:

   - Beautify pkey_{alloc,free,mprotect} arguments in 'perf trace'
     (Arnaldo Carvalho de Melo)

   - Add initial 'clone' syscall args beautifier in 'perf trace'
     (Arnaldo Carvalho de Melo)

   - Ignore 'fd' and 'offset' args for MAP_ANONYMOUS in 'perf trace'
     (Arnaldo Carvalho de Melo)

   - Beautifiers for the 'cmd' arg of several ioctl types, including:
sound, DRM, KVM, vhost virtio and perf_events. (Arnaldo Carvalho de
     Melo)

   - Add PERF_SAMPLE_CALLCHAIN and PERF_RECORD_MMAP[2] to 'perf data'
CTF conversion, allowing CTF trace visualization tools to show
callchains and to resolve symbols (Geneviève Bastien)

   - Beautify the fcntl syscall, which is an interesting one in the
sense that infrastructure had to be put in place to change the
formatters of some arguments according to the value in a previous
one, i.e. cmd dictates how arg and the syscall return will be
     formatted. (Arnaldo Carvalho de Melo

  perf stat enhancements:

   - Use group read for event groups in 'perf stat', reducing overhead
when groups are defined in the event specification, i.e. when using
{} to enclose a list of events, asking them to be read at the same
time, e.g.: "perf stat -e '{cycles, instructions}'" (Jiri Olsa)

  pipe mode improvements:

   - Process tracing data in 'perf annotate' pipe mode (David
     Carrillo-Cisneros)

   - Add header record types to pipe-mode, now this command:

        $ perf record -o - -e cycles sleep 1 | perf report --stdio --header

Will show the same as in non-pipe mode, i.e. involving a perf.data
     file (David Carrillo-Cisneros)

Vendor specific hardware event support updates/enhancements:

   - Update POWER9 vendor events tables (Sukadev Bhattiprolu)

   - Add POWER9 PMU events Sukadev (Bhattiprolu)

   - Support additional POWER8+ PVR in PMU mapfile (Shriya)

   - Add Skylake server uncore JSON vendor events (Andi Kleen)

   - Support exporting Intel PT data to sqlite3 with python perf
scripts, this is in addition to the postgresql support that was
     already there (Adrian Hunter)"

Jiri Slaby 30d6e0a419 futex: Remove duplicated code and fix undefined behaviour
There is code duplicated over all architecture's headers for
futex_atomic_op_inuser. Namely op decoding, access_ok check for uaddr,
and comparison of the result.

Remove this duplication and leave up to the arches only the needed
assembly which is now in arch_futex_atomic_op_inuser.

This effectively distributes the Will Deacon's arm64 fix for undefined
behaviour reported by UBSAN to all architectures. The fix was done in
commit 5f16a046f8 (arm64: futex: Fix undefined behaviour with
FUTEX_OP_OPARG_SHIFT usage). Look there for an example dump.

And as suggested by Thomas, check for negative oparg too, because it was
also reported to cause undefined behaviour report.

Note that s390 removed access_ok check in d12a29703 ("s390/uaccess:
remove pointless access_ok() checks") as access_ok there returns true.
We introduce it back to the helper for the sake of simplicity (it gets
optimized away anyway).

Paul E. McKenney 952111d7db arch: Remove spin_unlock_wait() arch-specific definitions
There is no agreed-upon definition of spin_unlock_wait()'s semantics,
and it appears that all callers could do just as well with a lock/unlock
pair. This commit therefore removes the underlying arch-specific
arch_spin_unlock_wait() for all architectures providing them.

Willem de Bruijn 76851d1212 sock: add SOCK_ZEROCOPY sockopt
The send call ignores unknown flags. Legacy applications may already
unwittingly pass MSG_ZEROCOPY. Continue to ignore this flag unless a
socket opts in to zerocopy.

Introduce socket option SO_ZEROCOPY to enable MSG_ZEROCOPY processing.
Processes can also query this socket option to detect kernel support
for the feature. Older kernels will return ENOPROTOOPT.

Max Filippov 6d0f581d17 xtensa: fix cache aliasing handling code for WT cache
Currently building kernel for xtensa core with aliasing WT cache fails
with the following messages:

  mm/memory.c:2152: undefined reference to `flush_dcache_page'
  mm/memory.c:2332: undefined reference to `local_flush_cache_page'
  mm/memory.c:1919: undefined reference to `local_flush_cache_range'
  mm/memory.c:4179: undefined reference to `copy_to_user_page'
  mm/memory.c:4183: undefined reference to `copy_from_user_page'

This happens because implementation of these functions is only compiled
when data cache is WB, which looks wrong: even when data cache doesn't
need flushing it still needs invalidation. The functions like
__flush_[invalidate_]dcache_* are correctly defined for both WB and WT
caches (and even if they weren't that'd still be ok, just slower).

Fix this by providing the same implementation of the above functions for
both WB and WT cache.

Masahiro Yamada 22c92aee6f xtensa: move generic-y of exported headers to uapi/asm/Kbuild
Since commit fcc8487d47 ("uapi: export all headers under uapi
directories"), all (and only) headers under uapi directories are
exported, but asm-generic wrappers are still exceptions.

To complete de-coupling the uapi from kernel headers, move generic-y
of exported headers to uapi/asm/Kbuild.

With this change, "make headers_install" will just need to parse
uapi/asm/Kbuild to build up exported headers.

Also, move "generic-y += kprobes.h" up in order to keep the entries
sorted.

Linus Torvalds f72e24a124 This is the first pull request for the new dma-mapping subsystem
In this new subsystem we'll try to properly maintain all the generic
code related to dma-mapping, and will further consolidate arch code
 into common helpers.

 This pull request contains:

  - removal of the DMA_ERROR_CODE macro, replacing it with calls
to ->mapping_error so that the dma_map_ops instances are
more self contained and can be shared across architectures (me)
  - removal of the ->set_dma_mask method, which duplicates the
->dma_capable one in terms of functionality, but requires more
    duplicate code.
  - various updates for the coherent dma pool and related arm code
    (Vladimir)
  - various smaller cleanups (me)

Pull dma-mapping infrastructure from Christoph Hellwig:
"This is the first pull request for the new dma-mapping subsystem

In this new subsystem we'll try to properly maintain all the generic
code related to dma-mapping, and will further consolidate arch code

  This pull request contains:

   - removal of the DMA_ERROR_CODE macro, replacing it with calls to
->mapping_error so that the dma_map_ops instances are more self
contained and can be shared across architectures (me)

   - removal of the ->set_dma_mask method, which duplicates the
->dma_capable one in terms of functionality, but requires more
     duplicate code.

   - various updates for the coherent dma pool and related arm code

fixups for some __get/put_user usages that were reported. Nothing huge,
just lots of development by a number of different developers, full
 details in the shortlog.

All of these have been in linux-next for a while. There will be a merge
issue with the arm-soc tree in the include/linux/platform_data/atmel.h

Vladimir Murzin 07c75d7a6b drivers: dma-mapping: allow dma_common_mmap() for NOMMU
Currently, internals of dma_common_mmap() is compiled out if build is
done for either NOMMU or target which explicitly says it does not
have/want coherent DMA mmap. It turned out that dma_common_mmap() can
be handy in NOMMU setup (at least for ARM).

This patch converts exitent NOMMU targets to use ARCH_NO_COHERENT_DMA_MMAP,
thus when CONFIG_MMU is gone from dma_common_mmap() their behaviour stays
unchanged.

ARM is not converted to ARCH_NO_COHERENT_DMA_MMAP because it 1)
already has mmap callback which can handle (at some extent) NOMMU 2)
already defines dummy pgprot_noncached() for NOMMU build.

c6x and frv stay untouched since they already have ARCH_NO_COHERENT_DMA_MMAP.

David Herrmann 28b5ba2aa0 net: introduce SO_PEERGROUPS getsockopt
This adds the new getsockopt(2) option SO_PEERGROUPS on SOL_SOCKET to
retrieve the auxiliary groups of the remote peer. It is designed to
naturally extend SO_PEERCRED. That is, the underlying data is from the
same credentials. Regarding its syntax, it is based on SO_PEERSEC. That
is, if the provided buffer is too small, ERANGE is returned and @optlen
is updated. Otherwise, the information is copied, @optlen is set to the
actual size, and 0 is returned.

While SO_PEERCRED (and thus `struct ucred') already returns the primary
group, it lacks the auxiliary group vector. However, nearly all access
controls (including kernel side VFS and SYSVIPC, but also user-space
polkit, DBus, ...) consider the entire set of groups, rather than just
the primary group. But this is currently not possible with pure
SO_PEERCRED. Instead, user-space has to work around this and query the
system database for the auxiliary groups of a UID retrieved via
SO_PEERCRED.

Unfortunately, there is no race-free way to query the auxiliary groups
of the PID/UID retrieved via SO_PEERCRED. Hence, the current user-space
solution is to use getgrouplist(3p), which itself falls back to NSS and
whatever is configured in nsswitch.conf(3). This effectively checks
which groups we *would* assign to the user if it logged in *now*. On
normal systems it is as easy as reading /etc/group, but with NSS it can
resort to quering network databases (eg., LDAP), using IPC or network
communication.

Long story short: Whenever we want to use auxiliary groups for access
checks on IPC, we need further IPC to talk to the user/group databases,
rather than just relying on SO_PEERCRED and the incoming socket. This
is unfortunate, and might even result in dead-locks if the database
query uses the same IPC as the original request.

So far, those recursions / dead-locks have been avoided by using
primitive IPC for all crucial NSS modules. However, we want to avoid
re-inventing the wheel for each NSS module that might be involved in
user/group queries. Hence, we would preferably make DBus (and other IPC
that supports access-management based on groups) work without resorting
to the user/group database. This new SO_PEERGROUPS ioctl would allow us
to make dbus-daemon work without ever calling into NSS.

Hugh Dickins 1be7107fbe mm: larger stack guard gap, between vmas
Stack guard page is a useful feature to reduce a risk of stack smashing
into a different mapping. We have been using a single page gap which
is sufficient to prevent having stack adjacent to a different mapping.
But this seems to be insufficient in the light of the stack usage in
userspace. E.g. glibc uses as large as 64kB alloca() in many commonly
used functions. Others use constructs liks gid_t buffer[NGROUPS_MAX]
which is 256kB or stack strings with MAX_ARG_STRLEN.

This will become especially dangerous for suid binaries and the default
no limit for the stack size limit because those applications can be
tricked to consume a large portion of the stack and a single glibc call
could jump over the guard page. These attacks are not theoretical,
unfortunatelly.

Make those attacks less probable by increasing the stack guard gap
to 1MB (on systems with 4k pages; but make it depend on the page size
because systems with larger base pages might cap stack allocations in
the PAGE_SIZE units) which should cover larger alloca() and VLA stack
allocations. It is obviously not a full fix because the problem is
somehow inherent, but it should reduce attack space a lot.

One could argue that the gap size should be configurable from userspace,
but that can be done later when somebody finds that the new 1MB is wrong
for some special case applications. For now, add a kernel command line
option (stack_guard_gap) to specify the stack gap size (in page units).

Implementation wise, first delete all the old code for stack guard page:
because although we could get away with accounting one extra page in a
stack vma, accounting a larger gap can break userspace - case in point,
a program run with "ulimit -S -v 20000" failed when the 1MB gap was
counted for RLIMIT_AS; similar problems could come with RLIMIT_MLOCK
and strict non-overcommit mode.

Instead of keeping gap inside the stack vma, maintain the stack guard
gap as a gap between vmas: using vm_start_gap() in place of vm_start
(or vm_end_gap() in place of vm_end if VM_GROWSUP) in just those few
places which need to respect the gap - mainly arch_get_unmapped_area(),
and and the vma tree's subtree_gap support for that.

Aleksa Sarai 54ebbfb160 tty: add TIOCGPTPEER ioctl
When opening the slave end of a PTY, it is not possible for userspace to
safely ensure that /dev/pts/$num is actually a slave (in cases where the
mount namespace in which devpts was mounted is controlled by an
untrusted process). In addition, there are several unresolvable
race conditions if userspace were to attempt to detect attacks through
stat(2) and other similar methods [in addition it is not clear how
userspace could detect attacks involving FUSE].

Resolve this by providing an interface for userpace to safely open the
"peer" end of a PTY file descriptor by using the dentry cached by
devpts. Since it is not possible to have an open master PTY without
having its slave exposed in /dev/pts this interface is safe. This
interface currently does not provide a way to get the master pty (since
it is not clear whether such an interface is safe or even useful).

Max Filippov e5c86679d5 xtensa: don't use linux IRQ #0
Linux IRQ #0 is reserved for error reporting and may not be used.
Increase NR_IRQS for one additional slot and increase
irq_domain_add_legacy parameter first_irq value to 1, so that linux
IRQ #0 is not associated with hardware IRQ #0 in legacy IRQ domains.
Introduce macro XTENSA_PIC_LINUX_IRQ for static translation of xtensa
PIC hardware IRQ # to linux IRQ #. Use this macro in XTFPGA platform
data definitions.

This fixes inability to use hardware IRQ #0 in configurations that don't
use device tree and allows for non-identity mapping between linux IRQ #
and hardware IRQ #.

Nicolas Dichtel fcc8487d47 uapi: export all headers under uapi directories
Regularly, when a new header is created in include/uapi/, the developer
forgets to add it in the corresponding Kbuild file. This error is usually
detected after the release is out.

In fact, all headers under uapi directories should be exported, thus it's

Max Filippov 2b83878dd7 xtensa: make __pa work with uncached KSEG addresses
When __pa is applied to virtual address in uncached KSEG region the
result is incorrect. Fix it by checking if the original address is in
the uncached KSEG and adjusting the result. It looks better than masking
off bits because pfn_valid would correctly work with new __pa results
and it may be made working in noMMU case, once we get definition for
uncached memory view.

This is required for the dma_common_mmap and DMA debug code to work
correctly: they both indirectly use __pa with coherent DMA addresses.
In case of DMA debug the visible effect is false reports that an address
mapped for DMA is accessed by CPU.

startup. It doesn't always make sense. In many cases vectors location
can be automatically decided at kernel link time and code can be placed
right there. This is especially useful for XIP kernel.

Max Filippov 9a736fcb09 xtensa: clean up bootable image build targets
Currently xtensa uses 'zImage' as a synonym of 'all', but in fact xtensa
supports three targets: 'Image' (ELF image with reset vector), 'zImage'
(compressed redboot image) and 'uImage' (U-Boot image).
Provide separate 'Image', 'zImage' and 'uImage' make targets that only
build corresponding image type. Make 'all' build all images appropriate
for a platform.

Max Filippov 4ab18701c6 xtensa: move parse_tag_fdt out of #ifdef CONFIG_BLK_DEV_INITRD
FDT tag parsing is not related to whether BLK_DEV_INITRD is configured
or not, move it out of the corresponding #ifdef/#endif block.
This fixes passing external FDT to the kernel configured w/o
BLK_DEV_INITRD support.

Vegard Nossum 3fce371bfa mm: add new mmget() helper
Apart from adding the helper function itself, the rest of the kernel is
converted mechanically using:

  git grep -l 'atomic_inc.*mm_users' | xargs sed -i 's/atomic_inc(&\(.*\)->mm_users);/mmget\(\1\);/'
  git grep -l 'atomic_inc.*mm_users' | xargs sed -i 's/atomic_inc(&\(.*\)\.mm_users);/mmget\(\&\1\);/'

This is needed for a later patch that hooks into the helper, but might
be a worthwhile cleanup on its own.

Luis R. Rodriguez 7d134b2ce6 kprobes: move kprobe declarations to asm-generic/kprobes.h
Often all is needed is these small helpers, instead of compiler.h or a
full kprobes.h. This is important for asm helpers, in fact even some
asm/kprobes.h make use of these helpers... instead just keep a generic
asm file with helpers useful for asm code with the least amount of
clutter as possible.

Likewise we need now to also address what to do about this file for both
when architectures have CONFIG_HAVE_KPROBES, and when they do not. Then
for when architectures have CONFIG_HAVE_KPROBES but have disabled
CONFIG_KPROBES.

Right now most asm/kprobes.h do not have guards against CONFIG_KPROBES,
this means most architecture code cannot include asm/kprobes.h safely.
Correct this and add guards for architectures missing them.
Additionally provide architectures that not have kprobes support with
the default asm-generic solution. This lets us force asm/kprobes.h on
the header include/linux/kprobes.h always, but most importantly we can
now safely include just asm/kprobes.h on architecture code without
bringing the full kitchen sink of header files.

Two architectures already provided a guard against CONFIG_KPROBES on its
kprobes.h: sh, arch. The rest of the architectures needed gaurds added.
We avoid including any not-needed headers on asm/kprobes.h unless
kprobes have been enabled.

In a subsequent atomic change we can try now to remove compiler.h from
include/linux/kprobes.h.

During this sweep I've also identified a few architectures defining a
common macro needed for both kprobes and ftrace, that of the definition
of the breakput instruction up. Some refer to this as
BREAKPOINT_INSTRUCTION. This must be kept outside of the #ifdef
CONFIG_KPROBES guard.

Linus Torvalds ac1820fb28 This is a tree wide change and has been kept separate for that reason.
Bart Van Assche noted that the ib DMA mapping code was significantly
similar enough to the core DMA mapping code that with a few changes
it was possible to remove the IB DMA mapping code entirely and
switch the RDMA stack to use the core DMA mapping code. This resulted
in a nice set of cleanups, but touched the entire tree. This branch
will be submitted separately to Linus at the end of the merge window
as per normal practice for tree wide changes like this.

Pull exception table module split from Paul Gortmaker:
 "Final extable.h related changes.

This completes the separation of exception table content from the
module.h header file. This is achieved with the final commit that
removes the one line back compatible change that sourced extable.h
  into the module.h file.

The commits are unchanged since January, with the exception of a
couple Acks that came in for the last two commits a bit later. The
changes have been in linux-next for quite some time[1] and have got
widespread arch coverage via toolchains I have and also from
additional ones the kbuild bot has.

Maintaners of the various arch were Cc'd during the postings to
lkml[2] and informed that the intention was to take the remaining arch
specific changes and lump them together with the final two non-arch
specific changes and submit for this merge window.

The ia64 diffstat stands out and probably warrants a mention. In an
earlier review, Al Viro made a valid comment that the original header
separation of content left something to be desired, and that it get
fixed as a part of this change, hence the larger diffstat"

Frederic Weisbecker b672592f02 sched/cputime: Remove generic asm headers
cputime_t is now only used by two architectures:

	* powerpc (when CONFIG_VIRT_CPU_ACCOUNTING_NATIVE=y)
	* s390

And since the core doesn't use it anymore, we don't need any arch support
from the others. So we can remove their stub implementations.

A final cleanup would be to provide an efficient pure arch
implementation of cputime_to_nsec() for s390 and powerpc and finally
remove include/linux/cputime.h .

   - Get rid of cycles_t and use a plain u64. The type is not really

   - Micro-optimize mutexes some more. (Waiman Long)

Francis Yan 1c885808e4 tcp: SOF_TIMESTAMPING_OPT_STATS option for SO_TIMESTAMPING
This patch exports the sender chronograph stats via the socket
SO_TIMESTAMPING channel. Currently we can instrument how long a
particular application unit of data was queued in TCP by tracking
SOF_TIMESTAMPING_TX_SOFTWARE and SOF_TIMESTAMPING_TX_SCHED. Having
these sender chronograph stats exported simultaneously along with
these timestamps allow further breaking down the various sender
limitation. For example, a video server can tell if a particular
chunk of video on a connection takes a long time to deliver because
TCP was experiencing small receive window. It is not possible to
tell before this patch without packet traces.

To prepare these stats, the user needs to set
SOF_TIMESTAMPING_OPT_STATS and SOF_TIMESTAMPING_OPT_TSONLY flags
while requesting other SOF_TIMESTAMPING TX timestamps. When the
timestamps are available in the error queue, the stats are returned
in a separate control message of type SCM_TIMESTAMPING_OPT_STATS,
in a list of TLVs (struct nlattr) of types: TCP_NLA_BUSY_TIME,
TCP_NLA_RWND_LIMITED, TCP_NLA_SNDBUF_LIMITED. Unit is microsecond.

Christian Borntraeger 79ab11cdb9 locking/core: Introduce cpu_relax_yield()
For spinning loops people do often use barrier() or cpu_relax().
For most architectures cpu_relax and barrier are the same, but on
some architectures cpu_relax can add some latency.
For example on power,sparc64 and arc, cpu_relax can shift the CPU
towards other hardware threads in an SMT environment.
On s390 cpu_relax does even more, it uses an hypercall to the
hypervisor to give up the timeslice.
In contrast to the SMT yielding this can result in larger latencies.
In some places this latency is unwanted, so another variant
"cpu_relax_lowlatency" was introduced. Before this is used in more
and more places, lets revert the logic and provide a cpu_relax_yield
that can be called in places where yielding is more important than
latency. By default this is the same as cpu_relax on all architectures.

Chris Metcalf 6727ad9e20 nmi_backtrace: generate one-line reports for idle cpus
When doing an nmi backtrace of many cores, most of which are idle, the
output is a little overwhelming and very uninformative. Suppress
messages for cpus that are idling when they are interrupted and just
emit one line, "NMI backtrace for N skipped: idling at pc 0xNNN".

We do this by grouping all the cpuidle code together into a new
.cpuidle.text section, and then checking the address of the interrupted
PC to see if it lies within that section.

This commit suitably tags x86 and tile idle routines, and only adds in
the minimal framework for other architectures.

Max Filippov a4c6be5ad1 xtensa: disable MMU initialization option on MMUv2 cores
MMU initialization option is currently ignored on MMUv2 cores, but it is
used in Kconfig to select kernel load and start addresses. This choice
is not available for MMUv2 cores as they have hardwired TLB entries.
Disable MMU initialization option for known MMUv2 cores so that they get
correct kernel load/start address by default.

Max Filippov 549409b4b5 xtensa: ISS: allow simdisk to use high memory buffers
ISS kernel by default has only low memory. But it may be configured to
support high memory and started in a simulator with more than 128M of
RAM. Simdisk driver in such configuration can get IO request with a
high memory page. There may be no TLB entry for that page, only page
table entry. However simulators don't do pagewalking, so such IO request
will fail. Touch IO buffer in the buffer read/write loop so that a TLB
entry is likely there when read or write simcall is invoked.

is the CCOUNT clock. Use old platform_calibrate_ccount call as a fallback

Drop no longer needed code that updates CPU clock-frequency property in

Dave Hansen e8c24d3a23 x86/pkeys: Allocation/free syscalls
This patch adds two new system calls:

int pkey_alloc(unsigned long flags, unsigned long init_access_rights)
	int pkey_free(int pkey);

These implement an "allocator" for the protection keys
themselves, which can be thought of as analogous to the allocator
that the kernel has for file descriptors. The kernel tracks
which numbers are in use, and only allows operations on keys that
are valid. A key which was not obtained by pkey_alloc() may not,
for instance, be passed to pkey_mprotect().

These system calls are also very important given the kernel's use
of pkeys to implement execute-only support. These help ensure
that userspace can never assume that it has control of a key
unless it first asks the kernel. The kernel does not promise to
preserve PKRU (right register) contents except for allocated
pkeys.

The 'init_access_rights' argument to pkey_alloc() specifies the
rights that will be established for the returned pkey. For
instance:

	pkey = pkey_alloc(flags, PKEY_DENY_WRITE);

will allocate 'pkey', but also sets the bits in PKRU[1] such that
writing to 'pkey' is already denied.

The kernel does not prevent pkey_free() from successfully freeing
in-use pkeys (those still assigned to a memory range by
pkey_mprotect()). It would be expensive to implement the checks
for this, so we instead say, "Just don't do it" since sane
software will never do it anyway.

Any piece of userspace calling pkey_alloc() needs to be prepared
for it to fail. Why? pkey_alloc() returns the same error code
(ENOSPC) when there are no pkeys and when pkeys are unsupported.
They can be unsupported for a whole host of reasons, so apps must
be prepared for this. Also, libraries or LD_PRELOADs might steal
keys before an application gets access to them.

This allocation mechanism could be implemented in userspace.
Even if we did it in userspace, we would still need additional
user/kernel interfaces to tell userspace which keys are being
used by the kernel internally (such as for execute-only
mappings). Having the kernel provide this facility completely
removes the need for these additional interfaces, or having an
implementation of this in userspace at all.

Note that we have to make changes to all of the architectures
that do not use mman-common.h because we use the new
PKEY_DENY_ACCESS/WRITE macros in arch-independent code.

1. PKRU is the Protection Key Rights User register. It is a
usermode-accessible register that controls whether writes
and/or access to each individual pkey is allowed or denied.

Krzysztof Kozlowski 00085f1efa dma-mapping: use unsigned long for dma_attrs
The dma-mapping core and the implementations do not change the DMA
attributes passed by pointer. Thus the pointer can point to const data.
However the attributes do not have to be a bitfield. Instead unsigned
long will do fine:

1. This is just simpler. Both in terms of reading the code and setting
attributes. Instead of initializing local attributes on the stack
and passing pointer to it to dma_set_attr(), just set the bits.

2. It brings safeness and checking for const correctness because the
   attributes are passed by value.
