Changing the variable names for ioatdma_device to be consistently named
ioat_dma instead of device/dma in order to avoid confusion and distinct
from struct device. This will clearly indicate that it is an
ioatdma_device. This also make all the naming consistent that the dma
device is ioat_dma and all the channels are ioat_chan.

Kill the common ioatdma channel structure and everything that is not
dma_chan to be ioat_dma_chan. Since we don't have to worry about v1
and v2 ioatdma anymore this makes it much cleaner and obvious for
maintenance.

If the allocation order is 16, then the u16 count will overflow and wrap
to zero when assigned the value 1 << 16.
Change the type of 'total_descs' to int, so that it is large enough to
store a value equal or greater than 1 << 16.

If the allocation order is 16, then the u16 index will overflow and wrap
to zero instead of being equal or greater than 1 << 16. The loop
condition will always be true, and the loop will run until all the
memory resources are depleted.
Change the type of index 'i' to u32, so that it is large enough to store
a value equal or greater than 1 << 16.

The dmaengine core assumes that async DMA devices will only be removed
when they not used anymore, or it assumes dma_async_device_unregister()
will only be called by dma driver exit routines. But this assumption is
not true for the IOAT driver, which calls dma_async_device_unregister()
from ioat_remove(). So current IOAT driver doesn't support device
hot-removal because it may cause system crash to hot-remove an inuse
IOAT device.
To support CPU socket hot-removal, all PCI devices, including IOAT
devices embedded in the socket, will be hot-removed. The idea solution
is to enhance the dmaengine core and IOAT driver to support hot-removal,
but that's too hard.
This patch implements a hack to disable IOAT devices under hotplug-capable
CPU socket so it won't break socket hot-removal.

This allows claiming of non-RAID channels as a private channel. This
prevents breakage of MDRAID using the IOATDMA channels via
async_tx but also allows agents such as NTB to claim channels
exclusively for its usages.

The err completion callback is missing from the error handler. Two
reasons we never hit this. On Xeon because the hw err workaround, the
completion happens on a NULL descriptor so we don't do callback on the
PQ descriptor. On Atom we have DWBES support and thus the callback already
happened or we don't halt on error, so that was take cared of. But this code
needs to be corrected for future error handlers.

Per commit "77873803363c net_dma: mark broken" net_dma is no longer used
and there is no plan to fix it.
This is the mechanical removal of bits in CONFIG_NET_DMA ifdef guards.
Reverting the remainder of the net_dma induced changes is deferred to
subsequent patches.

As result of deprecation of MSI-X/MSI enablement functions
pci_enable_msix() and pci_enable_msi_block() all drivers
using these two interfaces need to be updated to use the
new pci_enable_msi_range() or pci_enable_msi_exact()
and pci_enable_msix_range() or pci_enable_msix_exact()
interfaces.
Function pci_enable_msix() returns a tri-state value while
pci_enable_msi_exact() is a canonical zero/-errno variant.
The former is being phased out in favor of the latter.
In case of 'ioat' there (should be) no difference.

Since commit 7787380336 "net_dma: mark broken" we no longer pin dma
engines active for the network-receive-offload use case. As a result
the ->free_chan_resources() that occurs after the driver self test no
longer has a NET_DMA induced ->alloc_chan_resources() to back it up. A
late firing irq can lead to ksoftirqd spinning indefinitely due to the
tasklet_disable() performed by ->free_chan_resources(). Only
->alloc_chan_resources() can clear this condition in affected kernels.
This problem has been present since commit 3e037454bc "I/OAT: Add
support for MSI and MSI-X" in 2.6.24, but is now exposed. Given the
NET_DMA use case is deprecated we can revisit moving the driver to use
threaded irqs. For now, just tear down the irq and tasklet properly by:
1/ Disable the irq from triggering the tasklet
2/ Disable the irq from re-arming
3/ Flush inflight interrupts
4/ Flush the timer
5/ Flush inflight tasklets

1/ Bartlomiej and Dan finalized a rework of the dma address unmap
implementation.
2/ In the course of testing 1/ a collection of enhancements to dmatest
fell out. Notably basic performance statistics, and fixed / enhanced
test control through new module parameters 'run', 'wait', 'noverify',
and 'verbose'. Thanks to Andriy and Linus for their review.
3/ Testing the raid related corner cases of 1/ triggered bugs in the
recently added 16-source operation support in the ioatdma driver.

The implementation of ioat3_irq_reinit has two bugs:
1/ The mode is incorrectly set to MSIX for the MSI case
2/ The 'dev_id' parameter to free_irq is the ioatdma_device not the channel in

Once we have determined that we will not have all of our desired msix
vectors there is no point in attempting a single msix allocation. The
driver will already need to read registers to determine the source of
the interrupt the fact that it is msix is moot. Fallback directly to
msi.

Use a single cache for all sed allocations. No need to make it per
channel. This also avoids the slub_debug warnings for multiple caches
with the same name.
Switching to dmam_pool_create() to fix leaking the dma pools on
initialization failure and lets us kill ioat3_dma_remove().

When performing continuations there are implied sources that need to be
added to the source count. Quoting dma_set_maxpq:

The array to lookup the sed pool based on the number of sources
(pq16_idx_to_sedi) is 16 entries and expects a max source index.
However, we pass the total source count which runs off the end of the
array when src_cnt == 16. The minimal fix is to just pass src_cnt-1,
but given we know the source count is > 8 we can just calculate the sed
pool by (src_cnt - 2) >> 3.

Commit 48a9db4 (3.11) removed the memset op in the xor selftest for ioatdma.
The issue is that with the removal of that op, it never replaced the memset
with a CPU memset. The memory being operated on is expected to be zeroes but
was not. This is causing the xor selftest to fail.

have been introduced in January 2007 by commit 7405f74bad ("dmaengine:

v3.3 provides support for write back descriptor error status. This allows
reporting of errors in a descriptor field. In supporting this, certain
errors such as P/Q validation errors no longer halts the channel. The DMA
engine can continue to execute until the end of the chain and allow software
to report the "errors" up the stack. We are also going to mask those error
interrupts and handle them when the "chain" has completed at the end.

v3.3 introduced 16 sources PQ operations. This also introduced super extended
descriptors to support the 16 srcs operations. This patch adds support for
the 16 sources ops and in turn adds the super extended descriptors for those
ops.
5 SED pools are created depending on the descriptor sizes. An SED can be a 64
bytes sized descriptor or larger and must be physically contiguous. A kmem
cache pool is created for allocating the software descriptor that manages the
hardware descriptor. The super extended descriptor will take place of extended
descriptor under certain operations and be "attached" to the op descriptor
during operation. This is a new feature for ioatdma v3.3.

CB3.2 and earlier hardware has silicon bugs that are no longer needed with
the new hardware. We don't have to use a NULL op to signal interrupt for
RAID ops any longer. This code make sure the legacy workarounds only happen on
legacy hardware.

The Intel Atom S1200 family ioatdma changed the channel reset behavior.
It does a reset similar to PCI FLR by resetting all the MSIX
registers. We have to re-init msix interrupts because of this. This
workaround is only specific to this platform and is not expected to carry
over to the later generations.

Adding Haswell PCI device IDs for ioatdma and simplify the detection of
certain Xeon CPUs that has alignment bugs so that modifications can be
changed at a single place going forward.

This patch is meant to be a follow-up for a patch originally submitted under
the title "ioat: Do not enable DCA if tag map is invalid". It was brought to
my attention that the preferred approach for BIOS workarounds is to set the
taint flag for TAINT_FIRMWARE_WORKAROUND for systems that require BIOS
workarounds.
This change makes it so that the DCA workarounds for broken BIOSes will now
use WARN_TAINT_ONCE(1, TAINT_FIRMWARE_WORKAROUND, ...) instead of just
printing a message via dev_err.

There is a race that can hit during __cleanup() when the ioat->head pointer is
incremented during descriptor submission. The __cleanup() can clear the
PENDING flag when it does not see any active descriptors. This causes new
submitted descriptors to be ignored because the COMPLETION_PENDING flag is
cleared. This was introduced when code was adapted from ioatdma v1 to ioatdma
v2. For v2 and v3, IOAT_COMPLETION_PENDING flag will be abandoned and a new
flag IOAT_CHAN_ACTIVE will be utilized. This flag will also be protected under
the prep_lock when being modified in order to avoid the race.

The PCI IDs for IvyBridge IOAT DMA needs to go into a header file since
dma_v3.c looks them up for certain hardware workarounds. Need to add to the
alignment workaround for IOAT 3.2 since it wasn't fixed in IVB.

CONFIG_HOTPLUG is going away as an option. As a result, the __dev*
markings need to be removed.
This change removes the use of __devinit, __devexit_p, __devinitconst,
and __devexit from these drivers.

I have encountered several systems that have an invalid tag map. This
invalid tag map results in only two tags being generated 0x1F which is
usually applied to the first core in a Hyper-threaded pair and 0x00 which
is applied to the second core in a Hyper-threaded pair. The net result of
all this is that DCA causes significant cache thrash because the 0x1F tag
will send traffic to the second socket, which the 0x00 tag will not DCA tag
the frame resulting in no benefit.
For now the best solution from the driver's perspective is to just disable
DCA if the tag map is invalid. The correct solution for this would be to
have the BIOS on affected systems updated so that the correct tags are
generated for a given APIC ID.

The alloc order can be up to 16 and 1 << 16 will over flow the 16bit

Starting with v3.2 Jonathan reports that Xen crashes loading the ioatdma
driver. A debug run shows:

...which shows that in this environment GFP_KERNEL memory may be backed
by a 64-bit dma address. This breaks the driver's assumption that an
unsigned long should be able to contain the physical address for
descriptor memory. Switch to dma_addr_t which beyond being the right
size, is the true type for the data i.e. an io-virtual address
inidicating the engine's last processed descriptor.

Ensure all DMA engine drivers initialize their cookies in the same way,
so that they all behave in a similar fashion. This means their first
issued cookie will be 2 rather than 1, and will increment to INT_MAX
before returning 1 and starting over.

Now that we have the completed cookie in the dma_chan structure, we
can consolidate the tx_status functions by providing a function to set
the txstate structure and returning the DMA status. We also provide
a separate helper to set the residue for cookies which are still in
progress.

For versions of the device that implement operation-types 0x87, 0x88
(IOAT_OP_XOR, IOAT_OP_XOR_VAL) this map determines whether a given
source is located in the base or extended descriptor. Source addresses
6 through 8 require an extended descriptor, hence 0xe0, not 0xd0. No
shipping hardware currently implements these operation types.

Fix up trivial conflict in drivers/dma/dw_dmac.c
For certain system configurations a 5 usec udelay before checking I/OAT DMA
channel status is sometimes not sufficient, resulting in a false failure
status and unnecessary freeing of channel resources. Conversely, for many
configurations 5 usec is longer than necessary.
Loop for up to 20 usec waiting for successful status before failing.

After discovering that wide use of prefetch on modern CPUs
could be a net loss instead of a win, net drivers which were
relying on the implicit inclusion of prefetch.h via the list
headers showed up in the resulting cleanup fallout. Give
them an explicit include via the following $0.02 script.

Commit 0793448 "DMAENGINE: generic channel status v2" changed the interface for
how dma channel progress is retrieved. It inadvertently exported an internal
helper function ioat_tx_status() instead of ioat_dma_tx_status(). The latter
polls the hardware to get the latest completion state, while the helper just
evaluates the current state without touching hardware. The effect is that we
end up waiting for completion timeouts or descriptor allocation errors before
the completion state is updated.
iperf (before fix):
[SUM] 0.0-41.3 sec 364 MBytes 73.9 Mbits/sec
iperf (after fix):
[SUM] 0.0- 4.5 sec 499 MBytes 940 Mbits/sec
This is a regression starting with 2.6.35.

On some platforms (MacPro3,1) the BIOS assigns the ioatdma device to the
incorrect iommu causing faults when the driver initializes. Add a quirk
to catch this misconfiguration and try falling back to untranslated
operation (which works in the MacPro3,1 case).
Assuming there are other platforms with misconfigured iommus teach the
ioatdma driver to treat initialization failures as non-fatal (just fail
the driver load and emit a warning instead of triggering a BUG_ON).
This can be classified as a boot regression since 2.6.32 on affected
platforms since the ioatdma module did not autoload prior to that
kernel.

There are cases where cacheline-unaligned raid operations can hang the
dma channel. Simply disable these operations by increasing the
alignment constraints published to async_tx. The raid456 driver always
issues page aligned requests, so the only in-kernel user of the ioatdma
driver that is affected by this change is dmatest.

percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files. percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.
percpu.h -> slab.h dependency is about to be removed. Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability. As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.
* Scan files for gfp and slab usages and update includes such that
only the necessary includes are there. ie. if only gfp is used,
gfp.h, if slab is used, slab.h.
* When the script inserts a new include, it looks at the include
blocks and try to put the new include such that its order conforms
to its surrounding. It's put in the include block which contains
core kernel includes, in the same order that the rest are ordered -
alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
doesn't seem to be any matching order.
* If the script can't find a place to put a new include (mostly
because the file doesn't have fitting include block), it prints out
an error message indicating which .h file needs to be added to the
file.
The conversion was done in the following steps.
1. The initial automatic conversion of all .c files updated slightly
over 4000 files, deleting around 700 includes and adding ~480 gfp.h
and ~3000 slab.h inclusions. The script emitted errors for ~400
files.
2. Each error was manually checked. Some didn't need the inclusion,
some needed manual addition while adding it to implementation .h or
embedding .c file was more appropriate for others. This step added
inclusions to around 150 files.
3. The script was run again and the output was compared to the edits
from #2 to make sure no file was left behind.
4. Several build tests were done and a couple of problems were fixed.
e.g. lib/decompress_*.c used malloc/free() wrappers around slab
APIs requiring slab.h to be added manually.
5. The script was run on all .h files but without automatically
editing them as sprinkling gfp.h and slab.h inclusions around .h
files could easily lead to inclusion dependency hell. Most gfp.h
inclusion directives were ignored as stuff from gfp.h was usually
wildly available and often used in preprocessor macros. Each
slab.h inclusion directive was examined and added manually as
necessary.
6. percpu.h was updated not to include slab.h.
7. Build test were done on the following configurations and failures
were fixed. CONFIG_GCOV_KERNEL was turned off for all tests (as my
distributed build env didn't work with gcov compiles) and a few
more options had to be turned off depending on archs to make things
build (like ipr on powerpc/64 which failed due to missing writeq).
* x86 and x86_64 UP and SMP allmodconfig and a custom test config.
* powerpc and powerpc64 SMP allmodconfig
* sparc and sparc64 SMP allmodconfig
* ia64 SMP allmodconfig
* s390 SMP allmodconfig
* alpha SMP allmodconfig
* um on x86_64 SMP allmodconfig
8. percpu.h modifications were reverted so that it could be applied as
a separate patch and serve as bisection point.
Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

Convert the device_is_tx_complete() operation on the
DMA engine to a generic device_tx_status()operation which
can return three states, DMA_TX_RUNNING, DMA_TX_COMPLETE,
DMA_TX_PAUSED.
