Sreekanth Reddy 3627dba57f [SCSI] mpt2sas: Change in MPI2_RAID_ACTION_SYSTEM_SHUTDOWN_INITIATED notification methodology
The intent of this patch is to perform a graceful shutdown of target drives even if
volume doesn't exits. Changes done in this patch

1. Removed the check for the presence of volumes before sending down
   MPI2_RAID_ACTION_SYSTEM_SHUTDOWN_INITIATED. Therefore, this RAID action
   would be sent if the card is IR Firmware.

2. The MPI2_RAID_ACTION_SYSTEM_SHUTDOWN_INITIATED is sent even when the
   system undergoes suspend (in addition to remove/shutdown which was already
   present)

Sreekanth Reddy 6409a7d000 [SCSI] mpt2sas: Null pointer deference possibility in mpt2sas_ctl_event_callback function
Added a check to identify if mpi_reply is NULL in mpt2sas_ctl_event_callback()
and return without proceeding if it is the case.

Also modified the following functions to return void instead of 0 or 1
as returning those values from events perspective doesn't make sense.
* _base_async_event()
* mpt2sas_ctl_event_callback()
* mpt2sas_scsih_event_callback()

Hans Verkuil f66b2a1c7f [media] cx88: Fix regression: CX88_AUDIO_WM8775 can't be 0
Cards using the wm8775 specify that in their card struct. Those that do not
use it leave the audio_chip field to 0. Unfortunately, the CX88_AUDIO_WM8775
enum is 0 as well, so boards that do not have the wm8775 still try to load
and use that driver. Change it to 1 to fix this.
This regression was introduced in commit facd23664f.

Arun Kumar K d0b1c31349 [media] exynos-gsc: Register v4l2 device
Gscaler video device registration was happening without reference to
a parent v4l2_dev causing probe to fail. The patch creates a parent
v4l2 device and uses it for the gsc m2m video device registration.
This fixes regression introduced with comit commit 1c1d86a1ea
[media] v4l2: always require v4l2_dev, rename parent to dev_parent

Arun Kumar K 3396b096c5 [media] exynos4-is: Fix fimc-lite bayer formats
The 10-bit and 12-bit Bayer output formats supported by FIMC-LITE
actually use 16 bits where the extra bits are padded with zeros.
The patch corrects buffer allocation for these two formats by
modifying the depth field. This prevents memory corruption by the
output DMA due to insufficient buffer size.

Alexey Khoroshilov 2e923a0527 [media] hdpvr: fix iteration over uninitialized lists in hdpvr_probe()
free_buff_list and rec_buff_list are initialized in the middle of hdpvr_probe(),
but if something bad happens before that, error handling code calls hdpvr_delete(),
which contains iteration over the lists (via hdpvr_free_buffers()).
The patch moves the lists initialization to the beginning and by the way fixes
goto label in error handling of registering videodev.
Found by Linux Driver Verification project (linuxtesting.org).

Lubomir Rintel 3080072460 [media] usbtv: Fix deinterlacing
The image data is laid out a bit more weirdly and thus needs more work to
properly interlace. What we get from hardware is V4L2_FIELD_ALTERNATE, but
since userspace support for it is practically nonexistent, thus we make
V4L2_FIELD_INTERLACED from it so that it's more easily interpreted.

Sachin Kamat 8a09a4cc9b [media] s5p-g2d: Fix registration failure
Commit 1c1d86a1ea ("[media] v4l2: always require v4l2_dev,
rename parent to dev_parent") expects v4l2_dev to be always set.
It converted most of the drivers using the parent field of video_device
to v4l2_dev field. G2D driver did not set the parent field. Hence it got
left out. Without this patch we get the following boot warning and G2D
driver fails to register the video device.

John Sheu 19c2813c28 [media] s5p-mfc: Fix input/output format reporting
The video encode/decode paths have duplicated logic between
VIDIOC_TRY_FMT and VIDIOC_S_FMT that should be de-duped.  Also, video
decode reports V4L2_PIX_FMT_NV12MT_16X16 output format, regardless of
what the actual output has been set at.  Fix this.

Mike Dunn 1e185c7aaf pwm: pxa: Use module_platform_driver
Commit 76abbdde2d

    pwm: Add sysfs interface

causes a kernel oops due to a null pointer dereference on PXA platforms.
This happens because the class added by the patch is registered in a
subsys_initcall (initcall4), but the pxa pwm driver is registered in
arch_initcall (initcall3). If the class is not registered before the
driver probe function runs, the oops occurs in device_add() when the
uninitialized pointers in struct class are dereferenced.  I don't see a
reason that the driver must be an arch_initcall, so this patch makes it
a regular module_platform_driver (initcall6), preventing the oops.

Parsing the device tree may cause probing to be deferred. Doing this as
early as possible prevents any other resources from being requested and
enabled, therefore reducing the need to cleanup on deferred probe while
at the same time not wasting precious CPU cycles determining if probing
needs to be deferred or not.

Thierry Reding 03c961ba6d gpu: host1x: Check for valid host1x pointer
Under rare circumstances it can happen that the host1x driver's .probe()
doesn't finish properly, in which case the device's driver-specific data

Li Zhong 942e443127 module: Fix mod->mkobj.kobj potentially freed too early
DEBUG_KOBJECT_RELEASE helps to find the issue attached below.

After some investigation, it seems the reason is:
The mod->mkobj.kobj(ffffffffa01600d0 below) is freed together with mod
itself in free_module(). However, its children still hold references to
it, as the delay caused by DEBUG_KOBJECT_RELEASE. So when the
child(holders below) tries to decrease the reference count to its parent
in kobject_del(), BUG happens as it tries to access already freed memory.

This patch tries to fix it by waiting for the mod->mkobj.kobj to be
really released in the module removing process (and some error code
paths).

Takashi Iwai b054087dba ALSA: hda - Re-setup HDMI pin and audio infoframe on stream switches
When the transcoder:port mapping on Haswell HDMI/DP audio is changed
during the stream playback, the sound gets lost.  Typically this
problem is seen when the user switches the graphics mode from eDP+DP
to DP-only configuration, where CRTC 1 is used for DP in the former
while CRTC 0 is used for the latter.

The graphics controller notifies the change via the normal ELD update
procedure, so we get the intrinsic event.  For enabling the sound
again, the HDMI audio driver needs to reset the pin and set up the
audio infoframe again.

This patch achieves it by:
- keep the current status of channels and info frame setup in per_pin
  struct,
- check the reconnection in the intrinsic event handler,
- reset the pin and the re-invoke hdmi_setup_audio_infoframe()
  accordingly.

The hdmi_setup_audio_infoframe() function has been changed, too, so
that it can be invoked without passing the substream instance.

The patch is mostly based on the work by Mengdong Lin.

Naresh Gottumukkala cffce99051 RDMA/ocrdma: Dont use PD 0 for userpace CQ DB
Create_CQ verb doesn't provide a PD pointer.  So, until now we are
creating all (both userspace and kernel) CQ DB regions from PD0.  This
will result in mmapping PD0 to applications.  A rogue userspace
application can mess things up.

Also more serious issues is even the be2net NIC uses PD0.

This patch addresses this problem by:

1) Create a PD page for every userspace application when the
   alloc_ucontext is called. This will be destroyed in
   dealloc_ucontext.
2) All CQs for that context will use the PD allocated in ucontext.
3) The first create_PD call from application will result in returning
   the PD address from its ucontext (no new PD will be created).
4) For subsecquent create_pd calls from application, we create new PDs for
   the application.
