Quinn Tran f3f1938bb6 scsi: qla2xxx: Fix N2N link up fail
During link up/bounce, qla driver would do command flush as part of
cleanup.  In this case, the flush can intefere with FW state.  This patch
allows FW to be in control of link up.

There are instances, though rare, where a LOGO request cannot be sent out
and the thread in free session done can wait indefinitely. Fix this by
putting an upper bound to sleep.

Quinn Tran 2037ce49d3 scsi: qla2xxx: Fix stale session
On fast cable pull, where driver is unable to detect device has disappeared
and came back based on switch info, qla2xxx would not re-login while remote
port has already invalidated the session.  This causes IO timeout.  This
patch would relogin to remote device for RSCN affected port.

Quinn Tran ce0ba496dc scsi: qla2xxx: Fix stuck login session
Login session was stucked on cable pull. When FW is in the middle PRLI
PENDING + driver is in Initiator mode, driver fails to check back with FW to
see if the PRLI has completed. This patch would re-check with FW again to
make sure PRLI would complete before pushing forward with relogin.

Himanshu Madhani da48b82425 scsi: qla2xxx: Fix message indicating vectors used by driver
This patch updates log message which indicates number of vectors used by
the driver instead of displaying failure to get maximum requested
vectors. Driver will always request maximum vectors during
initialization. In the event driver is not able to get maximum requested
vectors, it will adjust the allocated vectors. This is normal and does not
imply failure in driver.

Martin Wilck 3cf92f4bfc scsi: qla2xxx: cleanup trace buffer initialization
Avoid code duplication between qla2x00_alloc_offload_mem() and
qla2x00_alloc_fw_dump() by moving the FCE and EFT buffer allocation and
initialization to separate functions. Cleanly track failure and success by
making sure that the ha->eft, ha->fce and respective eft_dma, fce_dma
members are set if and only if the buffers are properly allocated and
initialized. Avoid pointless buffer reallocation.  Eliminate some goto
statements. Make sure the fce_enabled flag is cleared when the FCE buffer
is freed.

Martin Wilck edbd56472a scsi: qla2xxx: qla2x00_alloc_fw_dump: set ha->eft
In qla2x00_alloc_fw_dump(), an existing EFT buffer (e.g. from previous
invocation of qla2x00_alloc_offload_mem()) is freed.  The buffer is then
re-allocated, but without setting the eft and eft_dma fields to the new
values.

Bill Kuzeja 26fa656e9a scsi: qla2xxx: Fix gnl.l memory leak on adapter init failure
If HBA initialization fails unexpectedly (exiting via probe_failed:), we
may fail to free vha->gnl.l. So that we don't attempt to double free, set
this pointer to NULL after a free and check for NULL at probe_failed: so we
know whether or not to call dma_free_coherent.

Instead of calling qla2x00_fcport_event_handler() and letting the switch
statement inside that function decide which other function to call, call
the latter function directly. Remove the event member from the event_arg
structure because it is no longer needed. Remove the
qla_handle_els_plogi_done() function because it is never called.

Bart Van Assche 0dcec41acb scsi: qla2xxx: Make sure that aborted commands are freed
The LIO core requires that the target driver callback functions
.queue_data_in() and .queue_status() call target_put_sess_cmd() or
transport_generic_free_cmd(). These calls may happen synchronously or
asynchronously. Make sure that one of these LIO functions is called in case
a command has been aborted. This patch avoids that the code for removing a
session hangs due to commands that do not make progress.

Bart Van Assche 85cffefa09 scsi: qla2xxx: Fix a race condition between aborting and completing a SCSI command
Instead of allocating a struct srb dynamically from inside .queuecommand(),
set qla2xxx_driver_template.cmd_size such that struct scsi_cmnd and struct
srb are contiguous. Do not call QLA_QPAIR_MARK_BUSY() /
QLA_QPAIR_MARK_NOT_BUSY() for SRBs associated with SCSI commands. That is
safe because scsi_remove_host() is called before queue pairs are deleted
and scsi_remove_host() waits for all outstanding SCSI commands to finish.

Bart Van Assche 6c18a43e3c scsi: qla2xxx: Enable type checking for the SRB free and done callback functions
Since all pointers passed to the srb_t.done() and srb_t.free() functions
have type srb_t, change the type of the first argument of these functions
from void * into struct srb *. This allows the compiler to verify the

Introduce the be_id_t and le_id_t data types for Fibre Channel source and
destination ID formats supported by the firmware instead of using an
uint8_t[3] array. Introduce functions for converting from and to the
port_id_t data types. This patch does not change the behavior of the
qla2xxx driver but improves source code readability and also allows the
compiler to verify the endianness of Fibre Channel IDs.

commands times out. This patch fixes a small bug: it modifies

function failed.

I'm not sure how this happened but the patch that was intended to fix abort
handling was incomplete. This patch fixes that patch as follows:

 - If aborting the SCSI command failed, wait until the SCSI command
   completes.

 - Return SUCCESS instead of FAILED if an abort attempt races with SCSI
   command completion.

 - Since qla2xxx_eh_abort() increments the sp reference count by calling
   sp_get(), decrement the sp reference count before returning.

Quinn Tran 03cc44bf68 scsi: qla2xxx: Allow NVMe IO to resume with short cable pull
Current driver report dev_loss_tmo to 0 for NVMe devices with short cable
pull.  This causes NVMe controller to be freed along with NVMe namespace.
The side affect is IO would stop.  By not setting dev_loss_tmo to 0, NVMe
namespace would stay until cable is plugged back in.  This allows IO to
resume afterward.

Arun Easi 9e744591ef scsi: qla2xxx: Fix NVMe port discovery after a short device port loss
The following sequence of event leads to NVME port disappearing:

    - device port shut
    - nvme_fc_unregister_remoteport
    - device port online
    - remote port delete completes
    - relogin is scheduled
    - "post gidpn" message appears due to rscn generation # mismatch

In short, if a device comes back online sooner than an unregister
completion, a mismatch in rscn generation number occurs, which is not
handled correctly during device relogin. Fix this by starting with a redo
of GNL.

When ql2xextended_error_logging is enabled, the re-plugged device's
discovery stops with the following messages printed:

Quinn Tran 3a4b6cc733 scsi: qla2xxx: Fix premature timer expiration
For any qla2xxx async command, the SRB buffer is used to send it. In
setting up the SRB buffer, the timer for this command is started before all
memory allocation has finished.  Under low memory pressure, memory alloc
can go to sleep and not wake up before the timer expires. Once timer has
expired, the timer thread will access uninitialize fields resulting into
NULL pointer crash.

This patch fixes this crash by moving the start of timer after everything
is setup.

In qla2x00_alloc_fcport(), fcport is assigned to NULL in the error
handling code on line 4880:
    fcport = NULL;

Then fcport is used on lines 4883-4886:
    INIT_WORK(&fcport->del_work, qla24xx_delete_sess_fn);
	INIT_WORK(&fcport->reg_work, qla_register_fcport_fn);
	INIT_LIST_HEAD(&fcport->gnl_entry);
	INIT_LIST_HEAD(&fcport->list);

Thus, possible null-pointer dereferences may occur.

To fix these bugs, qla2x00_alloc_fcport() directly returns NULL
in the error handling code.

These bugs are found by a static analysis tool STCheck written by us.

Note on the SPDX license tag conversion conflicts: the SCSI tree had
done its own SPDX conversion, which in some cases conflicted with the
treewide ones done by Thomas & co.

In almost all cases, the conflicts were purely syntactic: the SCSI tree
used the old-style SPDX tags ("GPL-2.0" and "GPL-2.0+") while the
treewide conversion had used the new-style ones ("GPL-2.0-only" and
"GPL-2.0-or-later").

In these cases I picked the new-style one.

In a few cases, the SPDX conversion was actually different, though.  As
explained by James above, and in more detail in a pre-pull-request
thread:

 "The other problem is actually substantive: In the libsas code Luben
  Tuikov originally specified gpl 2.0 only by dint of stating:

  * This file is licensed under GPLv2.

  In all the libsas files, but then muddied the water by quoting GPLv2
  verbatim (which includes the or later than language). So for these
  files Christoph did the conversion to v2 only SPDX tags and Thomas
  converted to v2 or later tags"

So in those cases, where the spdx tag substantially mattered, I took the
SCSI tree conversion of it, but then also took the opportunity to turn
the old-style "GPL-2.0" into a new-style "GPL-2.0-only" tag.

Similarly, when there were whitespace differences or other differences
to the comments around the copyright notices, I took the version from
the SCSI tree as being the more specific conversion.

Finally, in the spdx conversions that had no conflicts (because the
treewide ones hadn't been done for those files), I just took the SCSI
tree version as-is, even if it was old-style.  The old-style conversions
are perfectly valid, even if the "-only" and "-or-later" versions are
perhaps more descriptive.

Quinn Tran baf23eddbf scsi: qla2xxx: move IO flush to the front of NVME rport unregistration
On session deletion, current qla code would unregister an NVMe session
before flushing IOs. This patch would move the unregistration of NVMe
session after IO flush. This way FC-NVMe layer would not have to wait for
stuck IOs. In addition, qla2xxx would stop accepting new IOs during session
deletion.

Quinn Tran 4c2a2d0178 scsi: qla2xxx: Fix NVME cmd and LS cmd timeout race condition
This patch uses kref to protect access between fcp_abort path and nvme
command and LS command completion path.  Stack trace below shows the abort
path is accessing stale memory (nvme_private->sp).

When command kref reaches 0, nvme_private & srb resource will be
disconnected from each other.  Any subsequence nvme abort request will not
be able to reference the original srb.

The crash is due to a bad entry in the nvme_rport_list. This list is not
protected, and when a remoteport_delete callback is called, driver
traverses the list and crashes.

Actually, the list could be removed and driver could traverse the main
fcport list instead. Fix does exactly that.

Quinn Tran 5386a4e6c7 scsi: qla2xxx: Add cleanup for PCI EEH recovery
During EEH error recovery testing it was discovered that driver's reset()
callback partially frees resources used by driver, leaving some stale
memory.  After reset() is done and when resume() callback in driver uses
old data which results into error leaving adapter disabled due to PCIe
error.

This patch does cleanup for EEH recovery code path and prevents adapter
from getting disabled.

Bart Van Assche a861b49273 scsi: qla2xxx: Avoid that qlt_send_resp_ctio() corrupts memory
The "(&ctio->u.status1.sense_data)[i]" where i >= 0 expressions in
qlt_send_resp_ctio() are probably typos and should have been
"(&ctio->u.status1.sense_data[4 * i])" instead. Instead of only fixing
these typos, modify the code for storing sense data such that it becomes
easy to read. This patch fixes a Coverity complaint about accessing an
array outside its bounds.

Since fc_remote_port_delete() must be called with interrupts enabled, do
not disable interrupts when calling that function. Remove the lockin calls
from around the put_sess() call. This is safe because the function that is
called when the final reference is dropped, qlt_unreg_sess(), grabs the
proper locks. This patch avoids that lockdep reports the following:

Bart Van Assche 219d27d714 scsi: qla2xxx: Fix race conditions in the code for aborting SCSI commands
In the *_done() functions, instead of returning early if sp->ref_count >=
2, only decrement sp->ref_count. In qla2xxx_eh_abort(), instead of deciding
what to do based on the value of sp->ref_count, decide which action to take
depending on the completion status of the firmware abort. Remove srb.cwaitq
and use srb.comp instead. In qla2x00_abort_srb(), call
isp_ops->abort_command() directly instead of calling qla2xxx_eh_abort().

Bart Van Assche 1b1e68d2a9 scsi: qla2xxx: Remove unnecessary locking from the target code
All callbacks from the target core into the qla2xxx driver and also all I/O
completion functions are serialized per command. Since .cmd_sent_to_fw and
.trc_flags are only modified from inside these functions it is not
necessary to protect it with locking. Remove the superfluous locking.

Remove the function qlt_abort_cmd_on_host_reset() because it can do the
following, all of which can cause a kernel crash:

- DMA unmapping while DMA is in progress.
- Call target_execute_cmd() while DMA is in progress.
- Call transport_generic_free_cmd() while the LIO core owns a command.

Instead of trying to abort a command asynchronously, set the 'aborted' flag
and handle the abort after the hardware has passed control back to the
tcm_qla2xxx driver.

Quinn Tran 2137490f21 scsi: qla2xxx: Fix device staying in blocked state
This patch fixes issue reported by some of the customers, who discovered
that after cable pull scenario the devices disappear and path seems to
remain in blocked state. Once the device reappears, driver does not seem to
update path to online. This issue appears because of the defer flag
creating race condition where the same session reappears.  This patch fixes
this issue by indicating SCSI-ML of device lost when
qlt_free_session_done() is called from qlt_unreg_sess().

Hannes Reinecke a6a6d0589a scsi: scsi_transport_fc: nvme: display FC-NVMe port roles
Currently the FC-NVMe driver is leverating the SCSI FC transport class to
access the remote ports. Which means that all FC-NVMe remote ports will be
visible to the fc transport layer, but due to missing definitions the port
roles will always be 'unknown'.  This patch adds the missing definitions to
the fc transport class to that the port roles are correctly displayed.

Andrew Vasquez e81d1bcbde scsi: qla2xxx: Further limit FLASH region write access from SysFS
Recent ISPs have larger and more complex flash-write semantics
(secure-access and signing). The BSG interfaces support these semantics for
all ISPs and is exclusively used by QLogic user-space tools. Limit
flash-write operations to ISPs <= 25xx.

Andrew Vasquez 5cbdae10bf scsi: qla2xxx: Fix incorrect region-size setting in optrom SYSFS routines
Commit e6f77540c0 ("scsi: qla2xxx: Fix an integer overflow in sysfs
code") incorrectly set 'optrom_region_size' to 'start+size', which can
overflow option-rom boundaries when 'start' is non-zero.  Continue setting
optrom_region_size to the proper adjusted value of 'size'.

Giridhar Malavali 740e29358e scsi: qla2xxx: Set the SCSI command result before calling the command done
This patch tries to address race condition between abort handler and
completion handler. When scsi command result is set by both abort and
completion handler, scsi_done() is only called after refcount on SRB
structure goes to zero. The abort handler sets this result prematurely even
when the refcount is non-zero value. Fix this by setting SCSI cmd->result
before scsi_done() is called.

Quinn Tran 4705f10e82 scsi: qla2xxx: Fix FC-AL connection target discovery
Commit 7f147f9bfd ("scsi: qla2xxx: Fix N2N target discovery with Local
loop") fixed N2N target discovery for local loop.  However, same code is
used for FC-AL discovery as well. Added check to make sure we are bypassing
area and domain check only in N2N topology for target discovery.

There are special %pad and %pap format strings in Linux that we could use
here, but since the driver already does 64-bit arithmetic on the values,
using a plain 'u64' seems more consistent here.

Note: A possible related issue may be that the driver possibly checks the
wrong kind of overflow: when an IOMMU is in use, buffers that cross a
32-bit boundary in physical addresses would still be mapped into dma
addresses within the low 4GB space, so I suspect that we actually want to
check sg_dma_address() instead of sg_phys() here.

There is no null check for qpair after it is assigned in the for-loop.

Bill Kuzeja f233e8c000 scsi: qla2xxx: Move debug messages before sending srb preventing panic
When sending an srb with qla2x00_start_sp, the sp can complete and be freed
by the time we log the debug message saying we sent it. This can cause a
panic if sp gets reused quickly or when running a kernel that poisons freed
memory.

This was partially fixed by (not every case was addressed):

Sawan Chandak dcbf8f8087 scsi: qla2xxx: Restore FAWWPN of Physical Port only for loop down
When loop was made down explicitly due to cable pull, then for N2N toplogy,
if FAWWPN BIT is enabled by user, then it would restore some default
(garbage) value for Physical port WWPN, so this show garbage WWPN for the
port. Fix is, to restore physical port WWPN, if it is fabric
configuration. When loop is explicitly made down, and FAWWPN feature is
enabled, then driver need to restore original flashed WWPN.

Giridhar Malavali 50b812755e scsi: qla2xxx: Fix DMA error when the DIF sg buffer crosses 4GB boundary
When SGE buffer containing DIF information crosses 4G boundary, it results
in DMA error. This patch fixes this issue by calculating SGE buffer size
and if it crosses 4G boundary, driver will split it into multiple SGE
buffers to avoid DMA error.

Ming Lei f0783d43dd scsi: qla2xxx: Use correct number of vectors for online CPUs
When SCSI-MQ is enabled, in some case system would present
nr_possible_cpus() which is greater than requested vectors by the
driver. This results into driver being able to get larger number of MSI-X
vectors than actual online CPUs.  Driver then uses
pci_alloc_irq_vectors_affinity() to assign 1:1 mapping and affinity for
each MSI-x vector to CPUs. When the command is submitted using MSI-x
vector, assigned to offline CPU, it results in an ABTS and system
hang. This hang is result of a driver not being able to process interrupt
on a vector assigned to an Off-line CPUs

This patch fixes this issue by setting irq_offset value for the
blk_mq_pci_map_queues() to use only those CPUs which has CPU mask affinity
assigned and are online. By using the irq_offset value, driver will allow
online cpumask to decide which vectors are used in blk_mq_pci_map_queues().

Anatoliy Glagolev 17b18eaa6f scsi: qla2xxx: deadlock by configfs_depend_item
The intent of invoking configfs_depend_item in commit 7474f52a82
("tcm_qla2xxx: Perform configfs depend/undepend for base_tpg")
was to prevent a physical Fibre Channel port removal when
virtual (NPIV) ports announced through that physical port are active.
The change does not work as expected: it makes enabled physical port
dependent on target configfs subsystem (the port's parent), something
the configfs guarantees anyway.

Besides, scheduling work in a worker thread and waiting for the work's
completion is not really a valid workaround for the requirement not to call
configfs_depend_item from a configfs callback: the call occasionally
deadlocks.

Thus, removing configfs_depend_item calls does not break anything and fixes
the deadlock problem.
