The SIMCom SIM5218 and compatible devices have 5 USB interfaces, only 4
of which are serial ports. The fifth is a network interface supported
by the qmi-wwan driver. Furthermore, the serial ports do not support
modem control signals. Add driver_info flags to reflect this.

The Quectel EM12 is a Cat. 12 LTE modem. It behaves in the exactly the
same way as the EP06 (including the dynamic configuration behavior), so
the same checks on reserved interfaces, etc. are needed.

device-id flag.
This allows for the handling of devices whose interface numbers can
change (e.g. Quectel EP06) to be contained in the device-id table.

The Quectel EP06 (and EM06/EG06) LTE modem supports updating the USB
configuration, without the VID/PID or configuration number changing.
When the configuration is updated and interfaces are added/removed, the
interface numbers are updated. This causes our current code for matching
EP06 not to work as intended, as the assumption about reserved
interfaces no longer holds. If for example the diagnostic (first)
interface is removed, option will (try to) bind to the QMI interface.
This patch improves EP06 detection by replacing the current match with
two matches, and those matches check class, subclass and protocol as
well as VID and PID. The diag interface exports class, subclass and
protocol as 0xff. For the other serial interfaces, class is 0xff and
subclass and protocol are both 0x0.
The modem can export the following devices and always in this order:
diag, nmea, at, ppp. qmi and adb. This means that diag can only ever be
interface 0, and interface numbers 1-5 should be marked as reserved. The
three other serial devices can have interface numbers 0-3, but I have
not marked any interfaces as reserved. The reason is that the serial
devices are the only interfaces exported by the device where subclass
and protocol is 0x0.
QMI exports the same class, subclass and protocol values as the diag
interface. However, the two interfaces have different number of
endpoints, QMI has three and diag two. I have added a check for number
of interfaces if VID/PID matches the EP06, and we ignore the device if
number of interfaces equals three (and subclass is set).

The dwm-158 interface 4 and 5 doesn't answer to the AT commands
and doesn't appears a option interface.
Tested on openwrt distribution (kernel 4.14 using the old blacklist

Reimplement interface masking using device flags stored directly in the
device-id table. This will make it easier to add and maintain device-id
entries by using a more compact and readable notation compared to the
current implementation (which manages pairs of masks in separate
blacklist structs).
Two convenience macros are used to flag an interface as either reserved
or as not supporting modem-control requests:

For now, we limit the highest maskable interface number to seven, which

follows:

Interface 4 is QMI and interface 5 is ADB, so they are blacklisted.
This patch should also be considered for -stable. The QMI-patch for this
modem is already in the -stable-queue.

Now that the SPDX tag is in all USB files, that identifies the license
in a specific and legally-defined manner. So the extra GPL text wording
can be removed as it is no longer needed at all.
This is done on a quest to remove the 700+ different ways that files in
the kernel describe the GPL license text. And there's unneeded stuff
like the address (sometimes incorrect) for the FSF which is never
needed.
No copyright headers or other non-license-description text was removed.

It's good to have SPDX identifiers in all files to make it easier to
audit the kernel tree for correct licenses.
Update the drivers/usb/ and include/linux/usb* files with the correct
SPDX license identifier based on the license text in the file itself.
The SPDX identifier is a legally binding shorthand, which can be used
instead of the full boiler plate text.

Only the option driver implements the send_setup callback; it uses the
SET_CONTROL_LINE_STATE request in CDC ACM to generate DTR/RTS signals
on the port. This is not driver-specific though and is needed by other
drivers, so move the function to the usb_wwan driver (with formatting
tweaks), and replace the callback pointer with a flag that enables the
request.

Tests with a Sierra Wireless MC7355 have shown that 1199:9041 devices
also require the option_send_setup() code to be used on the USB
interface for the AT port to make unsolicited response codes work
correctly. Move these devices from the qcserial driver to the option
driver like it has been done for the 1199:68c0 devices in commit
d80c0d1418 ("USB: qcserial/option: make
AT URCs work for Sierra Wireless MC73xx").

MC73xx") to the option driver and add a MC73xx-specific blacklist

1. the sendsetup code is not used for the DIAG/DM and NMEA interfaces
2. the option driver does not attach to the QMI/network interfaces

The zte_ev driver is based on code (once) distributed by ZTE that still
appears to originally have been reverse-engineered and bolted onto the
generic driver.
A closer analysis of the zte_ev setup code reveals that it consists of
standard CDC requests (SET/GET_LINE_CODING and SET_CONTROL_LINE_STATE)
but unfortunately fails to get some of those right. In particular, as
reported by Lei Liu, it fails to lower DTR/RTS on close. It also appears
that the control requests lack the interface argument.
Since line control is already handled properly by the option driver, and
the SET/GET_LINE_CODING requests appears to be redundant (amounts to a
SET 9600 8N1) let's remove the redundant zte_ev driver.
Also move the remaining ZTE PIDs to the generic option modem driver.

This reverts commit 73228a0538 ("USB: option,zte_ev: move most ZTE
CDMA devices to zte_ev").
Move the IDs of the devices that were previously driven by the option
driver back to that driver.
As several users have reported, the zte_ev driver is causing random
disconnects as well as reconnect failures.
A closer analysis of the zte_ev setup code reveals that it consists of
standard CDC requests (SET/GET_LINE_CODING and SET_CONTROL_LINE_STATE)
but unfortunately fails to get some of those right. In particular, as
reported by Liu Lei, it fails to lower DTR/RTS on close. It also appears
that the control requests lack the interface argument.
Note that the zte_ev driver is based on code (once) distributed by ZTE

Since line control is already handled properly by the option driver, and
the SET/GET_LINE_CODING requests appears to be redundant (amounts to a
SET 9600 8N1), this is a first step in ultimately removing the redundant
zte_ev driver.
Note that AC2726 had already been moved back to option, and that some
IDs were in the device table of both drivers prior to the commit being
reverted.

This VIA Telecom baseband processor is used is used by by u-blox in both the
FW2770 and FW2760 products and may be used in others as well.

Do not log normal interrupt-urb shutdowns as errors.
The option driver has always been logging any nonzero interrupt-urb
status as an error, including when the urb is killed during normal
operation.
Commit 9096f1fbba ("USB: usb_wwan: fix potential NULL-deref at
resume") moved the interrupt urb submission from port probe and release
to open and close, thus potentially increasing the number of these
false-positive error messages dramatically.

The driver does not implement set_termios so the operation can be left
unset (tty will do the tty_termios_copy_hw for us).
Note that the send_setup call is bogus as it really only sets DTR/RTS
to their current values.

ZTE AC2726 EVDO modem drops ppp connection every minute when driven by
zte_ev but works fine when driven by option. Move the support for AC2726

Interface 1 on this device isn't for option to bind to otherwise an oops
on usb_wwan with log flooding will happen when accessing the port:
tty_release: ttyUSB1: read/write wait queue active!
It doesn't seem to respond to QMI if it's added to qmi_wwan so don't add
it there - it's likely used by the card reader.
