Ingo Molnar b17b01533b sched/headers: Prepare for new header dependencies before moving code to <linux/sched/debug.h>
We are going to split <linux/sched/debug.h> out of <linux/sched.h>, which
will have to be picked up from other headers and a couple of .c files.

Create a trivial placeholder <linux/sched/debug.h> file that just
maps to <linux/sched.h> to make this patch obviously correct and
bisectable.

Include the new header in the files that are going to need it.

Apart from adding the helper function itself, the rest of the kernel is
converted mechanically using:

Luis R. Rodriguez 7d134b2ce6 kprobes: move kprobe declarations to asm-generic/kprobes.h
Often all is needed is these small helpers, instead of compiler.h or a
full kprobes.h.  This is important for asm helpers, in fact even some
asm/kprobes.h make use of these helpers...  instead just keep a generic
asm file with helpers useful for asm code with the least amount of
clutter as possible.

Likewise we need now to also address what to do about this file for both
when architectures have CONFIG_HAVE_KPROBES, and when they do not.  Then
for when architectures have CONFIG_HAVE_KPROBES but have disabled
CONFIG_KPROBES.

Right now most asm/kprobes.h do not have guards against CONFIG_KPROBES,
this means most architecture code cannot include asm/kprobes.h safely.
Correct this and add guards for architectures missing them.
Additionally provide architectures that not have kprobes support with
the default asm-generic solution.  This lets us force asm/kprobes.h on
the header include/linux/kprobes.h always, but most importantly we can
now safely include just asm/kprobes.h on architecture code without
bringing the full kitchen sink of header files.

Two architectures already provided a guard against CONFIG_KPROBES on its
kprobes.h: sh, arch.  The rest of the architectures needed gaurds added.
We avoid including any not-needed headers on asm/kprobes.h unless
kprobes have been enabled.

In a subsequent atomic change we can try now to remove compiler.h from
include/linux/kprobes.h.

During this sweep I've also identified a few architectures defining a
common macro needed for both kprobes and ftrace, that of the definition
of the breakput instruction up.  Some refer to this as
BREAKPOINT_INSTRUCTION.  This must be kept outside of the #ifdef
CONFIG_KPROBES guard.

Mike Rapoport 897ab3e0c4 userfaultfd: non-cooperative: add event for memory unmaps
When a non-cooperative userfaultfd monitor copies pages in the
background, it may encounter regions that were already unmapped.
Addition of UFFD_EVENT_UNMAP allows the uffd monitor to track precisely
changes in the virtual memory layout.

Since there might be different uffd contexts for the affected VMAs, we
first should create a temporary representation for the unmap event for
each uffd context and then notify them one by one to the appropriate
userfault file descriptors.

The event notification occurs after the mmap_sem has been released.

Michal Hocko 6d23f8a5d4 arch, mm: remove arch specific show_mem
We have a generic implementation for quite some time already.  If there
is any arch specific information to be printed then we should add a
callback called from the generic code rather than duplicate the whole
show_mem.

The current code has resulted in the code duplication and the output
divergence which is both confusing and adds maintainance costs.

Let's just get rid of this mess.

Frederic Weisbecker b672592f02 sched/cputime: Remove generic asm headers
cputime_t is now only used by two architectures:

	* powerpc (when CONFIG_VIRT_CPU_ACCOUNTING_NATIVE=y)
	* s390

And since the core doesn't use it anymore, we don't need any arch support
from the others. So we can remove their stub implementations.

A final cleanup would be to provide an efficient pure arch
implementation of cputime_to_nsec() for s390 and powerpc and finally
remove include/linux/cputime.h .

Peter Zijlstra 9e3d6223d2 math64, timers: Fix 32bit mul_u64_u32_shr() and friends
It turns out that while GCC-4.4 manages to generate 32x32->64 mult
instructions for the 32bit mul_u64_u32_shr() code, any GCC after that
fails horribly.

Fix this by providing an explicit mul_u32_u32() function which can be
architcture provided.

Chris Metcalf 14e73e78ee tile: use __ro_after_init instead of tile-specific __write_once
The semantics of the old tile __write_once are the same as the
newer generic __ro_after_init, so rename them all and get rid
of the tile-specific version.

This does not enable actual support for __ro_after_init,
which had been dropped from the tile architecture before the
initial upstreaming was done, since we had at that time switched
to using 16MB huge pages to map the kernel.

Chris Metcalf 870ee4ff56 tile: remove #pragma unroll from finv_buffer_remote()
This directive was put in the kernel source before the "pragma
unroll" support for tilegx gcc was upstreamed.  Remove it for
now, and we can put it back later if/when the compiler support
is upstreamed.  This avoids a warning when building the kernel.

This routine is not on a hot path in any case, so the extra
optimization here was mostly just for its own sake.

Chris Metcalf e658a6f14d tile: avoid using clocksource_cyc2ns with absolute cycle count
For large values of "mult" and long uptimes, the intermediate
result of "cycles * mult" can overflow 64 bits.  For example,
the tile platform calls clocksource_cyc2ns with a 1.2 GHz clock;
we have mult = 853, and after 208.5 days, we overflow 64 bits.

Since clocksource_cyc2ns() is intended to be used for relative
cycle counts, not absolute cycle counts, performance is more
importance than accepting a wider range of cycle values.  So,
just use mult_frac() directly in tile's sched_clock().

Commit 4cecf6d401 ("sched, x86: Avoid unnecessary overflow
in sched_clock") by Salman Qazi results in essentially the same
generated code for x86 as this change does for tile.  In fact,
a follow-on change by Salman introduced mult_frac() and switched
to using it, so the C code was largely identical at that point too.

Peter Zijlstra then added mul_u64_u32_shr() and switched x86
to use it.  This is, in principle, better; by optimizing the
64x64->64 multiplies to be 32x32->64 multiplies we can potentially
save some time.  However, the compiler piplines the 64x64->64
multiplies pretty well, and the conditional branch in the generic
mul_u64_u32_shr() causes some bubbles in execution, with the
result that it's pretty much a wash.  If tilegx provided its own
implementation of mul_u64_u32_shr() without the conditional branch,
we could potentially save 3 cycles, but that seems like small gain
for a fair amount of additional build scaffolding; no other platform
currently provides a mul_u64_u32_shr() override, and tile doesn't
currently have an <asm/div64.h> header to put the override in.

Additionally, gcc currently has an optimization bug that prevents
it from recognizing the opportunity to use a 32x32->64 multiply,
and so the result would be no better than the existing mult_frac()
until such time as the compiler is fixed.

For now, just using mult_frac() seems like the right answer.

Christian Borntraeger 79ab11cdb9 locking/core: Introduce cpu_relax_yield()
For spinning loops people do often use barrier() or cpu_relax().
For most architectures cpu_relax and barrier are the same, but on
some architectures cpu_relax can add some latency.
For example on power,sparc64 and arc, cpu_relax can shift the CPU
towards other hardware threads in an SMT environment.
On s390 cpu_relax does even more, it uses an hypercall to the
hypervisor to give up the timeslice.
In contrast to the SMT yielding this can result in larger latencies.
In some places this latency is unwanted, so another variant
"cpu_relax_lowlatency" was introduced. Before this is used in more
and more places, lets revert the logic and provide a cpu_relax_yield
that can be called in places where yielding is more important than
latency. By default this is the same as cpu_relax on all architectures.

Chris Metcalf e123386bc3 tile: handle __ro_after_init like parisc does
The tile architecture already marks RO_DATA as read-only in
the kernel, so grouping RO_AFTER_INIT_DATA with RO_DATA, as is
done by default, means the kernel faults in init when it tries
to write to RO_AFTER_INIT_DATA.  For now, just arrange that
__ro_after_init is handled like __write_once, i.e. __read_mostly.

Chris Metcalf 6727ad9e20 nmi_backtrace: generate one-line reports for idle cpus
When doing an nmi backtrace of many cores, most of which are idle, the
output is a little overwhelming and very uninformative.  Suppress
messages for cpus that are idling when they are interrupted and just
emit one line, "NMI backtrace for N skipped: idling at pc 0xNNN".

We do this by grouping all the cpuidle code together into a new
.cpuidle.text section, and then checking the address of the interrupted
PC to see if it lies within that section.

This commit suitably tags x86 and tile idle routines, and only adds in
the minimal framework for other architectures.

Chris Metcalf 511f838945 arch/tile: adopt the new nmi_backtrace framework
Previously tile was rolling its own method of capturing backtrace data
in the NMI handlers, but it was relying on running printk() from the NMI
handler, which is not always safe.  So adopt the nmi_backtrace model
(with the new cpumask extension) instead.

So we can call the nmi_backtrace code directly from the nmi handler,
move the nmi_enter()/exit() into the top-level tile NMI handler.

The semantics of the routine change slightly since it is now synchronous
with the remote cores completing the backtraces.  Previously it was
asynchronous, but with protection to avoid starting a new remote
backtrace if the old one was still in progress.

Vineet Gupta 51a021244b atomic64: no need for CONFIG_ARCH_HAS_ATOMIC64_DEC_IF_POSITIVE
This came to light when implementing native 64-bit atomics for ARCv2.

The atomic64 self-test code uses CONFIG_ARCH_HAS_ATOMIC64_DEC_IF_POSITIVE
to check whether atomic64_dec_if_positive() is available.  It seems it
was needed when not every arch defined it.  However as of current code
the Kconfig option seems needless

 - for CONFIG_GENERIC_ATOMIC64 it is auto-enabled in lib/Kconfig and a
   generic definition of API is present lib/atomic64.c
 - arches with native 64-bit atomics select it in arch/*/Kconfig and
   define the API in their headers

So I see no point in keeping the Kconfig option

Compile tested for:

Josh Poimboeuf 0d025d271e mm/usercopy: get rid of CONFIG_DEBUG_STRICT_USER_COPY_CHECKS
There are three usercopy warnings which are currently being silenced for
gcc 4.6 and newer:

1) "copy_from_user() buffer size is too small" compile warning/error

   This is a static warning which happens when object size and copy size
   are both const, and copy size > object size.  I didn't see any false
   positives for this one.  So the function warning attribute seems to
   be working fine here.

   Note this scenario is always a bug and so I think it should be
   changed to *always* be an error, regardless of
   CONFIG_DEBUG_STRICT_USER_COPY_CHECKS.

2) "copy_from_user() buffer size is not provably correct" compile warning

   This is another static warning which happens when I enable
   __compiletime_object_size() for new compilers (and
   CONFIG_DEBUG_STRICT_USER_COPY_CHECKS).  It happens when object size
   is const, but copy size is *not*.  In this case there's no way to
   compare the two at build time, so it gives the warning.  (Note the
   warning is a byproduct of the fact that gcc has no way of knowing
   whether the overflow function will be called, so the call isn't dead
   code and the warning attribute is activated.)

   So this warning seems to only indicate "this is an unusual pattern,
   maybe you should check it out" rather than "this is a bug".

   I get 102(!) of these warnings with allyesconfig and the
   __compiletime_object_size() gcc check removed.  I don't know if there
   are any real bugs hiding in there, but from looking at a small
   sample, I didn't see any.  According to Kees, it does sometimes find
   real bugs.  But the false positive rate seems high.

3) "Buffer overflow detected" runtime warning

   This is a runtime warning where object size is const, and copy size >
   object size.

All three warnings (both static and runtime) were completely disabled
for gcc 4.6 with the following commit:

  2fb0815c9e ("gcc4: disable __compiletime_object_size for GCC 4.6+")

That commit mistakenly assumed that the false positives were caused by a
gcc bug in __compiletime_object_size().  But in fact,
__compiletime_object_size() seems to be working fine.  The false
positives were instead triggered by #2 above.  (Though I don't have an
explanation for why the warnings supposedly only started showing up in
gcc 4.6.)

So remove warning #2 to get rid of all the false positives, and re-enable
warnings #1 and #3 by reverting the above commit.

Furthermore, since #1 is a real bug which is detected at compile time,
upgrade it to always be an error.

Having done all that, CONFIG_DEBUG_STRICT_USER_COPY_CHECKS is no longer
needed.

Josh Poimboeuf 9a7c348ba6 ftrace: Add return address pointer to ftrace_ret_stack
Storing this value will help prevent unwinders from getting out of sync
with the function graph tracer ret_stack.  Now instead of needing a
stateful iterator, they can compare the return address pointer to find
the right ret_stack entry.

Note that an array of 50 ftrace_ret_stack structs is allocated for every
task.  So when an arch implements this, it will add either 200 or 400
bytes of memory usage per task (depending on whether it's a 32-bit or
64-bit platform).

Chris Metcalf c14b4bcfd9 tile: support static_key usage in non-module __exit sections
Previously, all the __exit sections were just dropped by the link phase.
However, if there are static_key (jump label) constructs in __exit
sections that are not modules, the link fails with the message:

   `.exit.text' referenced in section `__jump_table' of xxx.o:
   defined in discarded section `.exit.text' of xxx.o

Support this usage by keeping the .exit.text sections in the final image
if JUMP_LABEL is defined, then discarding them once initialization is
complete.

Krzysztof Kozlowski 00085f1efa dma-mapping: use unsigned long for dma_attrs
The dma-mapping core and the implementations do not change the DMA
attributes passed by pointer.  Thus the pointer can point to const data.
However the attributes do not have to be a bitfield.  Instead unsigned
long will do fine:

1. This is just simpler.  Both in terms of reading the code and setting
   attributes.  Instead of initializing local attributes on the stack
   and passing pointer to it to dma_set_attr(), just set the bits.

2. It brings safeness and checking for const correctness because the
   attributes are passed by value.

Andy Lutomirski 7e7814180b signal: consolidate {TS,TLF}_RESTORE_SIGMASK code
In general, there's no need for the "restore sigmask" flag to live in
ti->flags.  alpha, ia64, microblaze, powerpc, sh, sparc (64-bit only),
tile, and x86 use essentially identical alternative implementations,
placing the flag in ti->status.

Replace those optimized implementations with an equally good common
implementation that stores it in a bitfield in struct task_struct and
drop the custom implementations.

Additional architectures can opt in by removing their
TIF_RESTORE_SIGMASK defines.

Mel Gorman 11fb998986 mm: move most file-based accounting to the node
There are now a number of accounting oddities such as mapped file pages
being accounted for on the node while the total number of file pages are
accounted on the zone.  This can be coped with to some extent but it's
confusing so this patch moves the relevant file-based accounted.  Due to
throttling logic in the page allocator for reliable OOM detection, it is
still necessary to track dirty and writeback pages on a per-zone basis.

Mel Gorman 599d0c954f mm, vmscan: move LRU lists to node
This moves the LRU lists from the zone to the node and related data such
as counters, tracing, congestion tracking and writeback tracking.

Unfortunately, due to reclaim and compaction retry logic, it is
necessary to account for the number of LRU pages on both zone and node
logic.  Most reclaim logic is based on the node counters but the retry
logic uses the zone counters which do not distinguish inactive and
active sizes.  It would be possible to leave the LRU counters on a
per-zone basis but it's a heavier calculation across multiple cache
lines that is much more frequent than the retry checks.

Other than the LRU counters, this is mostly a mechanical patch but note
that it introduces a number of anomalies.  For example, the scans are
per-zone but using per-node counters.  We also mark a node as congested
when a zone is congested.  This causes weird problems that are fixed
later but is easier to review.

In the event that there is excessive overhead on 32-bit systems due to
the nodes being on LRU then there are two potential solutions

1. Long-term isolation of highmem pages when reclaim is lowmem

   When pages are skipped, they are immediately added back onto the LRU
   list. If lowmem reclaim persisted for long periods of time, the same
   highmem pages get continually scanned. The idea would be that lowmem
   keeps those pages on a separate list until a reclaim for highmem pages
   arrives that splices the highmem pages back onto the LRU. It potentially
   could be implemented similar to the UNEVICTABLE list.

   That would reduce the skip rate with the potential corner case is that
   highmem pages have to be scanned and reclaimed to free lowmem slab pages.

2. Linear scan lowmem pages if the initial LRU shrink fails

   This will break LRU ordering but may be preferable and faster during
   memory pressure than skipping LRU pages.

James Hogan cdf8b46330 tile: Define AT_VECTOR_SIZE_ARCH for ARCH_DLINFO
AT_VECTOR_SIZE_ARCH should be defined with the maximum number of
NEW_AUX_ENT entries that ARCH_DLINFO can contain, but it wasn't defined
for tile at all even though ARCH_DLINFO will contain one NEW_AUX_ENT for
the VDSO address.

This shouldn't be a problem as AT_VECTOR_SIZE_BASE includes space for
AT_BASE_PLATFORM which tile doesn't use, but lets define it now and add
the comment above ARCH_DLINFO as found in several other architectures to
remind future modifiers of ARCH_DLINFO to keep AT_VECTOR_SIZE_ARCH up to
date.

Michal Hocko f45eebc25e tile: get rid of superfluous __GFP_REPEAT
__GFP_REPEAT has a rather weak semantic but since it has been introduced
around 2.6.12 it has been ignored for low order allocations.

pgtable_alloc_one uses __GFP_REPEAT flag for L2_USER_PGTABLE_ORDER but
the order is either 0 or 3 if L2_KERNEL_PGTABLE_SHIFT for HPAGE_SHIFT.
This means that this flag has never been actually useful here because it
has always been used only for PAGE_ALLOC_COSTLY requests.

Linus Torvalds b235beea9e Clarify naming of thread info/stack allocators
We've had the thread info allocated together with the thread stack for
most architectures for a long time (since the thread_info was split off
from the task struct), but that is about to change.

But the patches that move the thread info to be off-stack (and a part of
the task struct instead) made it clear how confused the allocator and
freeing functions are.

Because the common case was that we share an allocation with the thread
stack and the thread_info, the two pointers were identical.  That
identity then meant that we would have things like

	ti = alloc_thread_info_node(tsk, node);
	...
	tsk->stack = ti;

which certainly _worked_ (since stack and thread_info have the same
value), but is rather confusing: why are we assigning a thread_info to
the stack? And if we move the thread_info away, the "confusing" code
just gets to be entirely bogus.

So remove all this confusion, and make it clear that we are doing the
stack allocation by renaming and clarifying the function names to be
about the stack.  The fact that the thread_info then shares the
allocation is an implementation detail, and not really about the
allocation itself.

This is a pure renaming and type fix: we pass in the same pointer, it's
just that we clarify what the pointer means.

The ia64 code that actually only has one single allocation (for all of
task_struct, thread_info and kernel thread stack) now looks a bit odd,
but since "tsk->stack" is actually not even used there, that oddity
doesn't matter.  It would be a separate thing to clean that up, I
intentionally left the ia64 changes as a pure brute-force renaming and
type change.

Chris Metcalf 893d66192c tile: support gcc 7 optimization to use __multi3
The tip gcc includes an optimization mode that converts
64-bit divides into 128-bit multiplies using __multi3.
Export the symbol so that modules can find it.  We just
export unconditionally without worrying about the gcc
version, since the symbol has been in libgcc forever and
the function is less than 300 bytes.

Peter Zijlstra b7271b9f3e locking/atomic, arch/tile: Fix tilepro build
The tilepro change wasn't ever compiled it seems (the 0day built bot
also doesn't have a toolchain for it).

Make it work.

The thing that makes the patch bigger than desired is namespace
collision with the C11 __atomic builtin functions. So rename the
tilepro functions to __atomic32.

Peter Zijlstra 726328d92a locking/spinlock, arch: Update and fix spin_unlock_wait() implementations
This patch updates/fixes all spin_unlock_wait() implementations.

The update is in semantics; where it previously was only a control
dependency, we now upgrade to a full load-acquire to match the
store-release from the spin_unlock() we waited on. This ensures that
when spin_unlock_wait() returns, we're guaranteed to observe the full
critical section we waited on.

This fixes a number of spin_unlock_wait() users that (not
unreasonably) rely on this.

I also fixed a number of ticket lock versions to only wait on the
current lock holder, instead of for a full unlock, as this is
sufficient.

Furthermore; again for ticket locks; I added an smp_rmb() in between
the initial ticket load and the spin loop testing the current value
because I could not convince myself the address dependency is
sufficient, esp. if the loads are of different sizes.

I'm more than happy to remove this smp_rmb() again if people are
certain the address dependency does indeed work as expected.

Note: PPC32 will be fixed independently

Petr Mladek 42a0bb3f71 printk/nmi: generic solution for safe printk in NMI
printk() takes some locks and could not be used a safe way in NMI
context.

The chance of a deadlock is real especially when printing stacks from
all CPUs.  This particular problem has been addressed on x86 by the
commit a9edc88093 ("x86/nmi: Perform a safe NMI stack trace on all
CPUs").

The patchset brings two big advantages.  First, it makes the NMI
backtraces safe on all architectures for free.  Second, it makes all NMI
messages almost safe on all architectures (the temporary buffer is
limited.  We still should keep the number of messages in NMI context at
minimum).

Note that there already are several messages printed in NMI context:
WARN_ON(in_nmi()), BUG_ON(in_nmi()), anything being printed out from MCE
handlers.  These are not easy to avoid.

This patch reuses most of the code and makes it generic.  It is useful
for all messages and architectures that support NMI.

The alternative printk_func is set when entering and is reseted when
leaving NMI context.  It queues IRQ work to copy the messages into the
main ring buffer in a safe context.

__printk_nmi_flush() copies all available messages and reset the buffer.
Then we could use a simple cmpxchg operations to get synchronized with
writers.  There is also used a spinlock to get synchronized with other
flushers.

We do not longer use seq_buf because it depends on external lock.  It
would be hard to make all supported operations safe for a lockless use.
It would be confusing and error prone to make only some operations safe.

The code is put into separate printk/nmi.c as suggested by Steven
Rostedt.  It needs a per-CPU buffer and is compiled only on
architectures that call nmi_enter().  This is achieved by the new
HAVE_NMI Kconfig flag.

The are MN10300 and Xtensa architectures.  We need to clean up NMI
handling there first.  Let's do it separately.

Hugh Dickins fd8cfd3000 arch: fix has_transparent_hugepage()
I've just discovered that the useful-sounding has_transparent_hugepage()
is actually an architecture-dependent minefield: on some arches it only
builds if CONFIG_TRANSPARENT_HUGEPAGE=y, on others it's also there when
not, but on some of those (arm and arm64) it then gives the wrong
answer; and on mips alone it's marked __init, which would crash if
called later (but so far it has not been called later).

Straighten this out: make it available to all configs, with a sensible
default in asm-generic/pgtable.h, removing its definitions from those
arches (arc, arm, arm64, sparc, tile) which are served by the default,
adding #define has_transparent_hugepage has_transparent_hugepage to
those (mips, powerpc, s390, x86) which need to override the default at
runtime, and removing the __init from mips (but maybe that kind of code
should be avoided after init: set a static variable the first time it's
called).

Joonsoo Kim 0139aa7b7f mm: rename _count, field of the struct page, to _refcount
Many developers already know that field for reference count of the
struct page is _count and atomic type.  They would try to handle it
directly and this could break the purpose of page reference count
tracepoint.  To prevent direct _count modification, this patch rename it
to _refcount and add warning message on the code.  After that, developer
who need to handle reference count will find that field should not be
accessed directly.

Vivien Didelot f81ec90fe9 net: dsa: mv88e6xxx: factorize the switch driver
Now that all drivers support the same set of functions and the same
setup code, drop every model-specific DSA switch driver and replace them
with a common mv88e6xxx driver.

This merges the info tables into one, removes the function exports, the
model-specific files, and update the defconfigs.

James Hogan b0da6d4415 asm-generic: Drop renameat syscall from default list
The newer renameat2 syscall provides all the functionality provided by
the renameat syscall and adds flags, so future architectures won't need
to include renameat.

Therefore drop the renameat syscall from the generic syscall list unless
__ARCH_WANT_RENAMEAT is defined by the architecture's unistd.h prior to
including asm-generic/unistd.h, and adjust all architectures using the
generic syscall list to define it so that no in-tree architectures are
affected.

Alexander Potapenko be7635e728 arch, ftrace: for KASAN put hard/soft IRQ entries into separate sections
KASAN needs to know whether the allocation happens in an IRQ handler.
This lets us strip everything below the IRQ entry point to reduce the
number of unique stack traces needed to be stored.

Move the definition of __irq_entry to <linux/interrupt.h> so that the
users don't need to pull in <linux/ftrace.h>.  Also introduce the
__softirq_entry macro which is similar to __irq_entry, but puts the
corresponding functions to the .softirqentry.text section.

Kirill A. Shutemov 3ed3a4f0dd mm: cleanup *pte_alloc* interfaces
There are few things about *pte_alloc*() helpers worth cleaning up:

 - 'vma' argument is unused, let's drop it;

 - most __pte_alloc() callers do speculative check for pmd_none(),
   before taking ptl: let's introduce pte_alloc() macro which does
   the check.

   The only direct user of __pte_alloc left is userfaultfd, which has
   different expectation about atomicity wrt pmd.

 - pte_alloc_map() and pte_alloc_map_lock() are redefined using
   pte_alloc().

Joonsoo Kim 21c647865a tile: query dynamic DEBUG_PAGEALLOC setting
We can disable debug_pagealloc processing even if the code is compiled
with CONFIG_DEBUG_PAGEALLOC.  This patch changes the code to query
whether it is enabled or not in runtime.

   - Asymmetry

     The hotplug notifier mechanism is asymmetric versus the bringup and
     teardown.  This is mostly caused by the notifier mechanism.

   - Largely undocumented dependencies

     While some notifiers use explicitely defined notifier priorities,
     we have quite some notifiers which use numerical priorities to
     express dependencies without any documentation why.

   - Control processor driven

     Most of the bringup/teardown of a cpu is driven by a control
     processor.  While it is understandable, that preperatory steps,
     like idle thread creation, memory allocation for and initialization
     of essential facilities needs to be done before a cpu can boot,
     there is no reason why everything else must run on a control
     processor.  Before this patch series, bringup looks like this:

       Control CPU                     Booting CPU

       do preparatory steps
       kick cpu into life

                                       do low level init

       sync with booting cpu           sync with control cpu

       bring the rest up

   - All or nothing approach

     There is no way to do partial bringups.  That's something which is
     really desired because we waste e.g.  at boot substantial amount of
     time just busy waiting that the cpu comes to life.  That's stupid
     as we could very well do preparatory steps and the initial IPI for
     other cpus and then go back and do the necessary low level
     synchronization with the freshly booted cpu.

   - Minimal debuggability

     Due to the notifier based design, it's impossible to switch between
     two stages of the bringup/teardown back and forth in order to test
     the correctness.  So in many hotplug notifiers the cancel
     mechanisms are either not existant or completely untested.

   - Notifier [un]registering is tedious

     To [un]register notifiers we need to protect against hotplug at
     every callsite.  There is no mechanism that bringup/teardown
     callbacks are issued on the online cpus, so every caller needs to
     do it itself.  That also includes error rollback.

  What's the new design?

     The base of the new design is a symmetric state machine, where both
     the control processor and the booting/dying cpu execute a well
     defined set of states.  Each state is symmetric in the end, except
     for some well defined exceptions, and the bringup/teardown can be
     stopped and reversed at almost all states.

     So the bringup of a cpu will look like this in the future:

       Control CPU                     Booting CPU

       do preparatory steps
       kick cpu into life

                                       do low level init

       sync with booting cpu           sync with control cpu

                                       bring itself up

     The synchronization step does not require the control cpu to wait.
     That mechanism can be done asynchronously via a worker or some
     other mechanism.

     The teardown can be made very similar, so that the dying cpu cleans
     up and brings itself down.  Cleanups which need to be done after
     the cpu is gone, can be scheduled asynchronously as well.

  There is a long way to this, as we need to refactor the notion when a
  cpu is available.  Today we set the cpu online right after it comes
  out of the low level bringup, which is not really correct.

  The proper mechanism is to set it to available, i.e. cpu local
  threads, like softirqd, hotplug thread etc. can be scheduled on that
  cpu, and once it finished all booting steps, it's set to online, so
  general workloads can be scheduled on it.  The reverse happens on
  teardown.  First thing to do is to forbid scheduling of general
  workloads, then teardown all the per cpu resources and finally shut it
  off completely.

  This patch series implements the basic infrastructure for this at the
  core level.  This includes the following:

   - Basic state machine implementation with well defined states, so
     ordering and prioritization can be expressed.

   - Interfaces to [un]register state callbacks

     This invokes the bringup/teardown callback on all online cpus with
     the proper protection in place and [un]installs the callbacks in
     the state machine array.

     For callbacks which have no particular ordering requirement we have
     a dynamic state space, so that drivers don't have to register an
     explicit hotplug state.

     If a callback fails, the code automatically does a rollback to the
     previous state.

   - Sysfs interface to drive the state machine to a particular step.

     This is only partially functional today.  Full functionality and
     therefor testability will be achieved once we converted all
     existing hotplug notifiers over to the new scheme.

   - Run all CPU_ONLINE/DOWN_PREPARE notifiers on the booting/dying
     processor:

       wait for boot
                                       bring itself up

                                       Signal completion to control cpu

     In a previous step of this work we've done a full tree mechanical
     conversion of all hotplug notifiers to the new scheme.  The balance
     is a net removal of about 4000 lines of code.

     This is not included in this series, as we decided to take a
     different approach.  Instead of mechanically converting everything
     over, we will do a proper overhaul of the usage sites one by one so
     they nicely fit into the symmetric callback scheme.

     I decided to do that after I looked at the ugliness of some of the
     converted sites and figured out that their hotplug mechanism is
     completely buggered anyway.  So there is no point to do a
     mechanical conversion first as we need to go through the usage
     sites one by one again in order to achieve a full symmetric and
     testable behaviour"

Chris Metcalf 77ef8f5177 tile kgdb: fix bug in copy to gdb regs, and optimize memset
David Binderman pointed out that we were doing a full memset()
of the gdb register buffer and then doing a memcpy() to it that
was almost as big.  This commit optimizes that by only doing a
memset() of the registers that are intended to be zero.

While making this change I noticed that we were not copying the
link register (LR, number 55) due to a fencepost error in commit
f419e6f63c ("arch: tile: kernel: kgdb.c: Use memcpy() instead of
pointer copy one by one"), and I've corrected that as well.

Thomas Gleixner fc6d73d674 arch/hotplug: Call into idle with a proper state
Let the non boot cpus call into idle with the corresponding hotplug state, so
the hotplug core can handle the further bringup. That's a first step to
convert the boot side of the hotplugged cpus to do all the synchronization
with the other side through the state machine. For now it'll only start the
hotplug thread and kick the full bringup of the cpu.

 - Some archs do not set 'flags' for children nodes, such as
   "Kernel code".  This patch does not change 'flags' in this

Chris Metcalf 9ce815ed50 tile: fix bug in setting PT_FLAGS_DISABLE_IRQ on kernel entry
This flag value is saved in ptregs and used to decide whether
to disable irqs when returning from the kernel.  Commit 1168df528fe4
("tile: don't assume user privilege is zero") performed a bad
merge from some KVM-enabled code that had not yet been upstreamed.

The only issue with the old code is that we will read the interrupt
mask in more conditions than we need to (e.g., coming from user
space when user space has the Interrupt Critical Section bit set, or
coming from a guest kernel), which is a slow multi-cycle operation.
This change saves those few cycles in the common case.

Chris Metcalf 583b24a210 arch/tile: adopt prepare_exit_to_usermode() model from x86
This change is a prerequisite change for TASK_ISOLATION but also
stands on its own for readability and maintainability.  The existing
tile do_work_pending() was called in a loop from assembly on
the slow path; this change moves the loop into C code as well.
For the x86 version see commit c5c46f59e4 ("x86/entry: Add new,
comprehensible entry and exit handlers written in C").

This change exposes a pre-existing bug on the older tilepro platform;
the singlestep processing is done last, but on tilepro (unlike tilegx)
we enable interrupts while doing that processing, so we could in
theory miss a signal or other asynchronous event.  A future change
could fix this by breaking the singlestep work into a "prepare"
step done in the main loop, and a "trigger" step done after exiting
the loop.  Since this change is intended as purely a restructuring
change, we call out the bug explicitly now, but don't yet fix it.

Chris Metcalf c1b27ab5d6 tile: provide CONFIG_PAGE_SIZE_64KB etc for tilepro
This allows the build system to know that it can't attempt to
configure the Lustre virtual block device, for example, when tilepro
is using 64KB pages (as it does by default).  The tilegx build
already provided those symbols.

Previously we required that the tilepro hypervisor be rebuilt with
a different hardcoded page size in its headers, and then Linux be
rebuilt using the updated hypervisor header.  Now we allow each of
the hypervisor and Linux to be built independently.  We still check
at boot time to ensure that the page size provided by the hypervisor
matches what Linux expects.

The previous patch introduced a flag that specified pages in a VMA should
be placed on the unevictable LRU, but they should not be made present when
the area is created.  This patch adds the ability to set this state via
the new mlock system calls.

We add MLOCK_ONFAULT for mlock2 and MCL_ONFAULT for mlockall.
MLOCK_ONFAULT will set the VM_LOCKONFAULT modifier for VM_LOCKED.
MCL_ONFAULT should be used as a modifier to the two other mlockall flags.
When used with MCL_CURRENT, all current mappings will be marked with
VM_LOCKED | VM_LOCKONFAULT.  When used with MCL_FUTURE, the mm->def_flags
will be marked with VM_LOCKED | VM_LOCKONFAULT.  When used with both
MCL_CURRENT and MCL_FUTURE, all current mappings and mm->def_flags will be
marked with VM_LOCKED | VM_LOCKONFAULT.

Prior to this patch, mlockall() will unconditionally clear the
mm->def_flags any time it is called without MCL_FUTURE.  This behavior is
maintained after adding MCL_ONFAULT.  If a call to mlockall(MCL_FUTURE) is
followed by mlockall(MCL_CURRENT), the mm->def_flags will be cleared and
new VMAs will be unlocked.  This remains true with or without MCL_ONFAULT
in either mlockall() invocation.

munlock() will unconditionally clear both vma flags.  munlockall()
unconditionally clears for VMA flags on all VMAs and in the mm->def_flags
field.

Pull strscpy string copy function implementation from Chris Metcalf.

Chris sent this during the merge window, but I waffled back and forth on
the pull request, which is why it's going in only now.

The new "strscpy()" function is definitely easier to use and more secure
than either strncpy() or strlcpy(), both of which are horrible nasty
interfaces that have serious and irredeemable problems.

strncpy() has a useless return value, and doesn't NUL-terminate an
overlong result.  To make matters worse, it pads a short result with
zeroes, which is a performance disaster if you have big buffers.

strlcpy(), by contrast, is a mis-designed "fix" for strlcpy(), lacking
the insane NUL padding, but having a differently broken return value
which returns the original length of the source string.  Which means
that it will read characters past the count from the source buffer, and
you have to trust the source to be properly terminated.  It also makes
error handling fragile, since the test for overflow is unnecessarily
subtle.

strscpy() avoids both these problems, guaranteeing the NUL termination
(but not excessive padding) if the destination size wasn't zero, and
making the overflow condition very obvious by returning -E2BIG.  It also
doesn't read past the size of the source, and can thus be used for
untrusted source data too.

So why did I waffle about this for so long?

Every time we introduce a new-and-improved interface, people start doing
these interminable series of trivial conversion patches.

And every time that happens, somebody does some silly mistake, and the
conversion patch to the improved interface actually makes things worse.
Because the patch is mindnumbing and trivial, nobody has the attention
span to look at it carefully, and it's usually done over large swatches
of source code which means that not every conversion gets tested.

So I'm pulling the strscpy() support because it *is* a better interface.
But I will refuse to pull mindless conversion patches.  Use this in
places where it makes sense, but don't do trivial patches to fix things
that aren't actually known to be broken.

Peter Zijlstra 62e8a3258b atomic, arch: Audit atomic_{read,set}()
This patch makes sure that atomic_{read,set}() are at least
{READ,WRITE}_ONCE().

We already had the 'requirement' that atomic_read() should use
ACCESS_ONCE(), and most archs had this, but a few were lacking.
All are now converted to use READ_ONCE().

And, by a symmetry and general paranoia argument, upgrade atomic_set()
to use WRITE_ONCE().

Thomas Gleixner bd0b9ac405 genirq: Remove irq argument from irq flow handlers
Most interrupt flow handlers do not use the irq argument. Those few
which use it can retrieve the irq number from the irq descriptor.

Remove the argument.

Christoph Hellwig 452e06af1f dma-mapping: consolidate dma_set_mask
Almost everyone implements dma_set_mask the same way, although some time
that's hidden in ->set_dma_mask methods.

This patch consolidates those into a common implementation that either
calls ->set_dma_mask if present or otherwise uses the default
implementation.  Some architectures used to only call ->set_dma_mask
after the initial checks, and those instance have been fixed to do the
full work.  h8300 implemented dma_set_mask bogusly as a no-ops and has
been fixed.

Unfortunately some architectures overload unrelated semantics like changing
the dma_ops into it so we still need to allow for an architecture override
for now.

Christoph Hellwig ee196371d5 dma-mapping: consolidate dma_supported
Most architectures just call into ->dma_supported, but some also return 1
if the method is not present, or 0 if no dma ops are present (although
that should never happeb). Consolidate this more broad version into
common code.

Also fix h8300 which inorrectly always returned 0, which would have been
a problem if it's dma_set_mask implementation wasn't a similarly buggy
noop.

As a few architectures have much more elaborate implementations, we
still allow for arch overrides.

Christoph Hellwig 1e8937526e dma-mapping: consolidate dma_{alloc,free}_noncoherent
Most architectures do not support non-coherent allocations and either
define dma_{alloc,free}_noncoherent to their coherent versions or stub
them out.

Openrisc uses dma_{alloc,free}_attrs to implement them, and only Mips
implements them directly.

This patch moves the Openrisc version to common code, and handles the
DMA_ATTR_NON_CONSISTENT case in the mips dma_map_ops instance.

Note that actual non-coherent allocations require a dma_cache_sync
implementation, so if non-coherent allocations didn't work on
an architecture before this patch they still won't work after it.

Christoph Hellwig 6894258eda dma-mapping: consolidate dma_{alloc,free}_{attrs,coherent}
Since 2009 we have a nice asm-generic header implementing lots of DMA API
functions for architectures using struct dma_map_ops, but unfortunately
it's still missing a lot of APIs that all architectures still have to
duplicate.

This series consolidates the remaining functions, although we still need
arch opt outs for two of them as a few architectures have very
non-standard implementations.

This patch (of 5):

The coherent DMA allocator works the same over all architectures supporting
dma_map operations.

This patch consolidates them and converges the minor differences:

 - the debug_dma helpers are now called from all architectures, including
   those that were previously missing them
 - dma_alloc_from_coherent and dma_release_from_coherent are now always
   called from the generic alloc/free routines instead of the ops
   dma-mapping-common.h always includes dma-coherent.h to get the defintions
   for them, or the stubs if the architecture doesn't support this feature
 - checks for ->alloc / ->free presence are removed.  There is only one
   magic instead of dma_map_ops without them (mic_dma_ops) and that one
   is x86 only anyway.

Besides that only x86 needs special treatment to replace a default devices
if none is passed and tweak the gfp_flags.  An optional arch hook is provided
for that.

Dave Young 2965faa5e0 kexec: split kexec_load syscall from kexec core code
There are two kexec load syscalls, kexec_load another and kexec_file_load.
 kexec_file_load has been splited as kernel/kexec_file.c.  In this patch I
split kexec_load syscall code to kernel/kexec.c.

And add a new kconfig option KEXEC_CORE, so we can disable kexec_load and
use kexec_file_load only, or vice verse.

The original requirement is from Ted Ts'o, he want kexec kernel signature
being checked with CONFIG_KEXEC_VERIFY_SIG enabled.  But kexec-tools use
kexec_load syscall can bypass the checking.

Vivek Goyal proposed to create a common kconfig option so user can compile
in only one syscall for loading kexec kernel.  KEXEC/KEXEC_FILE selects
KEXEC_CORE so that old config files still work.

Because there's general code need CONFIG_KEXEC_CORE, so I updated all the
architecture Kconfig with a new option KEXEC_CORE, and let KEXEC selects
KEXEC_CORE in arch Kconfig.  Also updated general kernel code with to
kexec_load syscall.

Pull irq updates from Thomas Gleixner:
 "This updated pull request does not contain the last few GIC related
  patches which were reported to cause a regression.  There is a fix
  available, but I let it breed for a couple of days first.

  The irq departement provides:

   - new infrastructure to support non PCI based MSI interrupts
   - a couple of new irq chip drivers
   - the usual pile of fixlets and updates to irq chip drivers
   - preparatory changes for removal of the irq argument from interrupt
     flow handlers
   - preparatory changes to remove IRQF_VALID"
