Patch series "mm: consolidate definitions of page table accessors", v2.
The low level page table accessors (pXY_index(), pXY_offset()) are
duplicated across all architectures and sometimes more than once. For
instance, we have 31 definition of pgd_offset() for 25 supported
architectures.
Most of these definitions are actually identical and typically it boils
down to, e.g.

These definitions can be shared among 90% of the arches provided
XYZ_SHIFT, PTRS_PER_XYZ and xyz_page_vaddr() are defined.
For architectures that really need a custom version there is always
possibility to override the generic version with the usual ifdefs magic.
These patches introduce include/linux/pgtable.h that replaces
include/asm-generic/pgtable.h and add the definitions of the page table
accessors to the new header.
This patch (of 12):
The linux/mm.h header includes <asm/pgtable.h> to allow inlining of the
functions involving page table manipulations, e.g. pte_alloc() and
pmd_alloc(). So, there is no point to explicitly include <asm/pgtable.h>
in the files that include <linux/mm.h>.
The include statements in such cases are remove with a simple loop:

xenbus_dev_shutdown() is seemingly intended to cause clean shutdown of PV
frontends when a guest is rebooted. Indeed the function waits for a
conpletion which is only set by a call to xenbus_frontend_closed().
This patch removes the shutdown() method from backends and moves
xenbus_dev_shutdown() from xenbus_probe.c into xenbus_probe_frontend.c,
renaming it appropriately and making it static.
NOTE: In the case where the backend is running in a driver domain, the
toolstack should have already terminated any frontends that may be
using it (since Xen does not support re-startable PV driver domains)
so xenbus_dev_shutdown() should never be called.

Today a Xenstore watch event is delivered via a callback function
declared as:
void (*callback)(struct xenbus_watch *,
const char **vec, unsigned int len);
As all watch events only ever come with two parameters (path and token)
changing the prototype to:
void (*callback)(struct xenbus_watch *,
const char *path, const char *token);
is the natural thing to do.
Apply this change and adapt all users.

The xenbus driver has an awful mixture of internally and globally
visible headers: some of the internally used only stuff is defined in
the global header include/xen/xenbus.h while some stuff defined in
internal headers is used by other drivers, too.
Clean this up by moving the externally used symbols to
include/xen/xenbus.h and the symbols used internally only to a new
header drivers/xen/xenbus/xenbus.h replacing xenbus_comms.h and
xenbus_probe.h

System workqueues have been able to handle high level of concurrency
for a long time now and there's no reason to use dedicated workqueues
just to gain concurrency. Replace dedicated xenbus_frontend_wq with the
use of system_wq.
Unlike a dedicated per-cpu workqueue created with create_workqueue(),
system_wq allows multiple work items to overlap executions even on
the same CPU; however, a per-cpu workqueue doesn't have any CPU
locality or global ordering guarantees unless the target CPU is
explicitly specified and the increase of local concurrency shouldn't
make any difference.
In this case, there is only a single work item, increase of concurrency
level by switching to system_wq should not make any difference.

errors.
Replace the uses with standard structure definitions instead. This is
similar to pci and usb device registration.

The user has the option of disabling the platform driver:
00:02.0 Unassigned class [ff80]: XenSource, Inc. Xen Platform Device (rev 01)
which is used to unplug the emulated drivers (IDE, Realtek 8169, etc)
and allow the PV drivers to take over. If the user wishes
to disable that they can set:
xen_platform_pci=0
(in the guest config file)
or
xen_emul_unplug=never
(on the Linux command line)
except it does not work properly. The PV drivers still try to
load and since the Xen platform driver is not run - and it
has not initialized the grant tables, most of the PV drivers
stumble upon:

PV driver check for:
- if running in PV, then it is fine to execute (as that is their
native environment).
- if running in HVM, check if user wanted 'xen_emul_unplug=never',
in which case bail out and don't load any PV drivers.
- if running in HVM, and if PCI device 5853:0001 (xen_platform_pci)
does not exist, then bail out and not load PV drivers.
- (v2) if running in HVM, and if the user wanted 'xen_emul_unplug=ide-disks',
then bail out for all PV devices _except_ the block one.
Ditto for the network one ('nics').
- (v2) if running in HVM, and if the user wanted 'xen_emul_unplug=unnecessary'
then load block PV driver, and also setup the legacy IDE paths.

Only create the delayed resume workqueue if we are running in the same domain

If the xenbus frontend is located in a domain running xenstored, the device
resume is hanging because it is happening before the process resume. This
patch adds extra logic to the resume code to check if we are the domain
running xenstored and delay the resume if needed.

A rather annoying and common case is when booting a PVonHVM guest
and exposing the PV KBD and PV VFB - as broken toolstacks don't
always initialize the backends correctly.
Normally The HVM guest is using the VGA driver and the emulated
keyboard for this (though upstream version of QEMU implements
PV KBD, but still uses a VGA driver). We provide a very basic
two-stage wait mechanism - where we wait for 30 seconds for all
devices, and then for 270 for all them except the two mentioned.
That allows us to wait for the essential devices, like network
or disk for the full 6 minutes.
To trigger this, put this in your guest config:
vfb = [ 'vnc=1, vnclisten=0.0.0.0 ,vncunused=1']
instead of this:
vnc=1
vnclisten="0.0.0.0"

Unfortunately xend creates a bogus console/0 frotend/backend entry pair
on xenstore that console backends cannot properly cope with.
Any guest behavior that is not completely ignoring console/0 is going
to either cause problems with xenconsoled or qemu.
Returning 0 or -ENODEV from xencons_probe is not enough because it is
going to cause the frontend state to become 4 or 6 respectively.
The best possible thing we can do here is just ignore the entry from
xenbus_probe_frontend.

The 'name', 'owner', and 'mod_name' members are redundant with the
identically named fields in the 'driver' sub-structure. Rather than
switching each instance to specify these fields explicitly, introduce
a macro to simplify this.
Eliminate further redundancy by allowing the drvname argument to
DEFINE_XENBUS_DRIVER() to be blank (in which case the first entry from
the ID table will be used for .driver.name).
Also eliminate the questionable xenbus_register_{back,front}end()
wrappers - their sole remaining purpose was the checking of the
'owner' field, proper setting of which shouldn't be an issue anymore
when the macro gets used.

After triggering a crash dump in a HVM guest, the PV backend drivers
will remain in Connected state. When the kdump kernel starts the PV
drivers will skip such devices. As a result, no root device is found and
the vmcore cant be saved.
A similar situation happens after a kexec boot, here the devices will be
in the Closed state.
With this change all frontend devices with state XenbusStateConnected or
XenbusStateClosed will be reset by changing the state file to Closing ->
Closed -> Initializing. This will trigger a disconnect in the backend
drivers. Now the frontend drivers will find the backend drivers in state
Initwait and can connect.

The xenbus bus type uses device_create_file to assign all used device
attributes. However it does not remove them when the device goes away.
This patch uses the dev_attrs field of the bus type to specify default
attributes for all devices.

Use PM_FREEZE, PM_THAW and PM_RESTORE power events for
suspend/resume/checkpoint functionality, instead of PM_SUSPEND
and PM_RESUME. Use of these pm events fixes the Xen Guest hangup
when taking checkpoints. When a suspend event is cancelled
(while taking checkpoints once/continuously), we use PM_THAW
instead of PM_RESUME. PM_RESTORE is used when suspend is not
cancelled. See Documentation/power/devices.txt and linux/pm.h
for more info about freeze, thaw and restore. The sequence of
pm events in a suspend-resume scenario is shown below.
