Convert pgtable code so pte_val()/pXd_val() aren't used as lvalue
anymore. This allows in later step to convert pte_val()/pXd_val() to
functions, which in turn makes it impossible to use these macros to
modify page table entries like they have been used before.
Therefore a construct like this:
pte_val(*pte) = __pa(addr) | prot;
which would directly write into a page table, isn't possible anymore
with the last step of this series.

There is a confusion with regard to the source address of
memcpy_real() and calling functions. While the declared
type for a source assumes a virtual address, in fact it
always called with physical address of the source.
This confusion led to bugs in copy_oldmem_kernel() and
copy_oldmem_user() functions, where __pa() macro applied
mistakenly to physical addresses. It does not lead to a
real issue, since virtual and physical addresses are
currently the same.
Fix both the bugs and memcpy_real() prototype by making
type of source address consistent to the function name
and the way it actually used.

Due to historical reasons memcpy_absolute() and friend functions
misuse the notion of physical vs virtual addresses difference.
Note: this does not fix a bug currently, since virtual and physical
addresses are identical.

Since commit 4064b98270 ("mm: allow VM_FAULT_RETRY for multiple
times") allowed VM_FAULT_RETRY for multiple times, the
FAULT_FLAG_ALLOW_RETRY bit of fault_flag will not be changed in the page
fault path, so the following check is no longer needed:

When CONFIG_DEBUG_VM is defined check that pending remove
and tracking nibbles (bits 31-24 of the page refcount) are
cleared. Should the earlier stages of the page lifespan
have a race or logical error, such check could help in
exposing the issue.

Explicitly encode immediate value of pending remove nibble
(bits 31-28) and tracking nibble (bits 27-24) of the page
refcount whenever these nibbles are tested or changed, for
better readability. Also, add some comments describing how
the fragments are handled.

There is a race on concurrent 2KB-pgtables release paths when
both upper and lower halves of the containing parent page are
freed, one via page_table_free_rcu() + __tlb_remove_table(),
and the other via page_table_free(). The race might lead to a
corruption as result of remove of list item in page_table_free()
concurrently with __free_page() in __tlb_remove_table().
Let's assume first the lower and next the upper 2KB-pgtables are
freed from a page. Since both halves of the page are allocated
the tracking byte (bits 24-31 of the page _refcount) has value
of 0x03 initially:

The problem is page_table_free() releases the page as result of
lower nibble unset and __tlb_remove_table() observing zero too
early. With this update page_table_free() will use the similar
logic as page_table_free_rcu() + __tlb_remove_table(), and mark
the fragment as pending for removal in the upper nibble until
after the list_del().
In other words, the parent page is considered as unreferenced and
safe to release only when the lower nibble is cleared already and
unsetting a bit in upper nibble results in that nibble turned zero.

The SCLP early buffer is used only during kernel initialization and can be
freed afterwards. The only way to ensure that it is not released while
being in use, is to release it in free_initmem().

After adding the missing __va()/__pa() calls to the base asce
functions there are even more casts in the code than before. Make the
code more readable by passing and using pointers to page tables,
instead of using unsigned values for the same purpose.
This allows to get rid of nearly all casts within the code.

The base asce functions create/free page tables open-coded to make
sure that the returned asce and page tables do not make use of any
enhanced DAT features like e.g. large pages. This is required for some
I/O functions that use an asce, like e.g. some service call requests.
Handling of virtual vs physical addresses is missing; therefore add
that now.
Note: this currently doesn't fix a real bug, since virtual addresses
are indentical to physical ones.

diag10_range() expects a pfn, however the current cmm code is shifting
a virtual address, instead of a physical address by PAGE_SHIFT bits,
which would give a wrong result in case if V!=R.
Use virt_to_pfn() to fix this.
Note: this currently doesn't fix a real bug, since virtual addresses
are indentical to physical ones.

The page table dumper walks page table tables without using standard
page table primitives in order to also dump broken entries. However it
currently does not translate physical to virtual addresses before
dereferencing them. Therefore add this missing translation.

"While looking at some issues related to the exit path in the kernel I
found several instances where the code is not using the existing
abstractions properly.
This set of changes introduces force_fatal_sig a way of sending a
signal and not allowing it to be caught, and corrects the misuse of
the existing abstractions that I found.
A lot of the misuse of the existing abstractions are silly things such
as doing something after calling a no return function, rolling BUG by
hand, doing more work than necessary to terminate a kernel thread, or
calling do_exit(SIGKILL) instead of calling force_sig(SIGKILL).
In the review a deficiency in force_fatal_sig and force_sig_seccomp
where ptrace or sigaction could prevent the delivery of the signal was
found. I have added a change that adds SA_IMMUTABLE to change that
makes it impossible to interrupt the delivery of those signals, and
allows backporting to fix force_sig_seccomp

Introduce variants of the convert and destroy page functions that also
clear the PG_arch_1 bit used to mark them as secure pages.
The PG_arch_1 flag is always allowed to overindicate; using the new
functions introduced here allows to reduce the extent of overindication
and thus improve performance.
These new functions can only be called on pages for which a reference
is already being held.

The Breaking-Event-Address-Register (BEAR) stores the address of the
last breaking event instruction. Breaking events are usually instructions
that change the program flow - for example branches, and instructions
that modify the address in the PSW like lpswe. This is useful for debugging
wild branches, because one could easily figure out where the wild branch
was originating from.
What is problematic is that lpswe is considered a breaking event, and
therefore overwrites BEAR on kernel exit. The BEAR enhancement facility
adds new instructions that allow to save/restore BEAR and also an lpswey
instruction that doesn't cause a breaking event. So we can save BEAR on
kernel entry and restore it on exit to user space.

Instructions IPTE, IDTE and CRDTE accept Page-Table Origin
as one of the arguments, but instead the pgtable virtual
address is passed. Fix that and also update the crdte()

We already optimize get_guest_storage_key() to assume that if we don't have
a PTE table and don't have a huge page mapped that the storage key is 0.
Similarly, optimize reset_guest_reference_bit() to simply do nothing if
there is no PTE table and no huge page mapped.

There are multiple things broken about our storage key handling
functions:
1. We should not walk/touch page tables outside of VMA boundaries when
holding only the mmap sem in read mode. Evil user space can modify the
VMA layout just before this function runs and e.g., trigger races with
page table removal code since commit dd2283f260 ("mm: mmap: zap pages
with read mmap_sem in munmap"). gfn_to_hva() will only translate using
KVM memory regions, but won't validate the VMA.
2. We should not allocate page tables outside of VMA boundaries: if
evil user space decides to map hugetlbfs to these ranges, bad things
will happen because we suddenly have PTE or PMD page tables where we
shouldn't have them.
3. We don't handle large PUDs that might suddenly appeared inside our page
table hierarchy.
Don't manually allocate page tables, properly validate that we have VMA and
bail out on pud_large().
All callers of page table handling functions, except
get_guest_storage_key(), call fixup_user_fault() in case they
receive an -EFAULT and retry; this will allocate the necessary page tables
if required.
To keep get_guest_storage_key() working as expected and not requiring
kvm_s390_get_skeys() to call fixup_user_fault() distinguish between
"there is simply no page table or huge page yet and the key is assumed
to be 0" and "this is a fault to be reported".
Although commit 637ff9efe5 ("s390/mm: Add huge pmd storage key handling")
introduced most of the affected code, it was actually already broken
before when using get_locked_pte() without any VMA checks.
Note: Ever since commit 637ff9efe5 ("s390/mm: Add huge pmd storage key
handling") we can no longer set a guest storage key (for example from
QEMU during VM live migration) without actually resolving a fault.
Although we would have created most page tables, we would choke on the
!pmd_present(), requiring a call to fixup_user_fault(). I would
have thought that this is problematic in combination with postcopy life
migration ... but nobody noticed and this patch doesn't change the
situation. So maybe it's just fine.

returns. Mark die __noreturn so that no one expects die to return.
Remove the do_exit calls after die as they will never be reached.

This reverts commit 9857a17f20.
That commit was completely broken, and I should have caught on to it
earlier. But happily, the kernel test robot noticed the breakage fairly
quickly.
The breakage is because "try_get_page()" is about avoiding the page
reference count overflow case, but is otherwise the exact same as a
plain "get_page()".
In contrast, "try_get_compound_head()" is an entirely different beast,
and uses __page_cache_add_speculative() because it's not just about the
page reference count, but also about possibly racing with the underlying
page going away.
So all the commentary about how
"try_get_page() has fallen a little behind in terms of maintenance,
try_get_compound_head() handles speculative page references more
thoroughly"
was just completely wrong: yes, try_get_compound_head() handles
speculative page references, but the point is that try_get_page() does
not, and must not.
So there's no lack of maintainance - there are fundamentally different
semantics.
A speculative page reference would be entirely wrong in "get_page()",
and it's entirely wrong in "try_get_page()". It's not about
speculation, it's purely about "uhhuh, you can't get this page because
you've tried to increment the reference count too much already".
The reason the kernel test robot noticed this bug was that it hit the
VM_BUG_ON() in __page_cache_add_speculative(), which is all about
verifying that the context of any speculative page access is correct.
But since that isn't what try_get_page() is all about, the VM_BUG_ON()
tests things that are not correct to test for try_get_page().

There are only two try_get_page() callsites, so just call

In case of splitting to 4k mapping the early exit in walk_pte_level()
must only be taken iff flags is equal to SET_MEMORY_4K.
Currently the early exit is taken if the flag is set, and also others
might be set. This may lead to the situation that a mapping is split
but other changes are not done, like e.g. setting pages to R/W.
There is currently no such caller, but there might be in the future.

The dma section name is confusing, since the code which resides within
that section has nothing to do with direct memory access. Instead the
limitation is that the code has to run in 31 bit addressing mode, and
therefore has to reside below 2GB. So the name was chosen since
ZONE_DMA is the same region.
To reduce confusion rename the section to amode31, which hopefully
describes better what this is about.
Note: this will also change vmcoreinfo strings
- SDMA=... gets renamed to SAMODE31=...
- EDMA=... gets renamed to EAMODE31=...

The functions get_online_cpus() and put_online_cpus() have been
deprecated during the CPU hotplug rework. They map directly to
cpus_read_lock() and cpus_read_unlock().
Replace deprecated CPU-hotplug functions with the official version.
The behavior remains unchanged.

The new boot data struct shall replace global variables INITRD_START and
INITRD_SIZE. It is initialized in the decompressor and passed
to the decompressed kernel. In comparison to the old solution, this one
doesn't access data at fixed physical addresses which will become important
when the decompressor becomes relocatable.

Since commit 903cd0f315 ("swiotlb: Use is_swiotlb_force_bounce for
swiotlb data bouncing") if code sets swiotlb_force it needs to do so
before the swiotlb is initialised. Otherwise
io_tlb_default_mem->force_bounce will not get set to true, and devices
that use (the default) swiotlb will not bounce despite switolb_force
having the value of SWIOTLB_FORCE.
Let us restore swiotlb functionality for PV by fulfilling this new
requirement.
This change addresses what turned out to be a fragility in
commit 64e1f0c531 ("s390/mm: force swiotlb for protected
virtualization"), which ain't exactly broken in its original context,
but could give us some more headache if people backport the broken
change and forget this fix.

Turns out that the bit 61 in the TEID is not always 1 and if that's
the case the address space ID and the address are
unpredictable. Without an address and its address space ID we can't
export memory and hence we can only send a SIGSEGV to the process or
panic the kernel depending on who caused the exception.
Unfortunately bit 61 is only reliable if we have the "misc" UV feature
bit.

Patch series "Add support for SVM atomics in Nouveau", v11.
Introduction
============
Some devices have features such as atomic PTE bits that can be used to
implement atomic access to system memory. To support atomic operations to
a shared virtual memory page such a device needs access to that page which
is exclusive of the CPU. This series introduces a mechanism to
temporarily unmap pages granting exclusive access to a device.
These changes are required to support OpenCL atomic operations in Nouveau
to shared virtual memory (SVM) regions allocated with the
CL_MEM_SVM_ATOMICS clSVMAlloc flag. A more complete description of the

Implementation
==============
Exclusive device access is implemented by adding a new swap entry type
(SWAP_DEVICE_EXCLUSIVE) which is similar to a migration entry. The main
difference is that on fault the original entry is immediately restored by
the fault handler instead of waiting.
Restoring the entry triggers calls to MMU notifers which allows a device
driver to revoke the atomic access permission from the GPU prior to the
CPU finalising the entry.
Patches
=======
Patches 1 & 2 refactor existing migration and device private entry
functions.
Patches 3 & 4 rework try_to_unmap_one() by splitting out unrelated
functionality into separate functions - try_to_migrate_one() and
try_to_munlock_one().
Patch 5 renames some existing code but does not introduce functionality.
Patch 6 is a small clean-up to swap entry handling in copy_pte_range().
Patch 7 contains the bulk of the implementation for device exclusive
memory.
Patch 8 contains some additions to the HMM selftests to ensure everything
works as expected.
Patch 9 is a cleanup for the Nouveau SVM implementation.
Patch 10 contains the implementation of atomic access for the Nouveau
driver.
Testing
=======
This has been tested with upstream Mesa 21.1.0 and a simple OpenCL program
which checks that GPU atomic accesses to system memory are atomic.
Without this series the test fails as there is no way of write-protecting
the page mapping which results in the device clobbering CPU writes. For

Further testing has been performed by adding support for testing exclusive
access to the hmm-tests kselftests.
This patch (of 10):
Remove multiple similar inline functions for dealing with different types
of special swap entries.
Both migration and device private swap entries use the swap offset to
store a pfn. Instead of multiple inline functions to obtain a struct page
for each swap entry type use a common function pfn_swap_entry_to_page().
Also open-code the various entry_to_pfn() functions as this results is
shorter code that is easier to understand.

Currently there are two separate places where kernel memory layout has
to be known and adjusted:
1. early kasan setup.
2. paging setup later.
Those 2 places had to be kept in sync and adjusted to reflect peculiar
technical details of one another. With additional factors which influence
kernel memory layout like ultravisor secure storage limit, complexity
of keeping two things in sync grew up even more.
Besides that if we look forward towards creating identity mapping and
enabling DAT before jumping into uncompressed kernel - that would also
require full knowledge of and control over kernel memory layout.
So, de-duplicate and move kernel memory layout setup logic into
the decompressor.

Add conditional trap handlers similar to conditional system calls
(COND_SYSCALL), to reduce the number of ifdefs.
Trap handlers which may or may not exist depending on config options
are supposed to have a COND_TRAP entry, which redirects to
default_trap_handler() for non-existent trap handlers during link
time.
This allows to get rid of the secure execution trap handlers for the
!PGSTE case.

This patch converts s390 to use the generic entry infrastructure from
kernel/entry/*.
There are a few special things on s390:
- PIF_PER_TRAP is moved to TIF_PER_TRAP as the generic code doesn't
know about our PIF flags in exit_to_user_mode_loop().
- The old code had several ways to restart syscalls:
a) PIF_SYSCALL_RESTART, which was only set during execve to force a
restart after upgrading a process (usually qemu-kvm) to pgste page
table extensions.
b) PIF_SYSCALL, which is set by do_signal() to indicate that the
current syscall should be restarted. This is changed so that
do_signal() now also uses PIF_SYSCALL_RESTART. Continuing to use
PIF_SYSCALL doesn't work with the generic code, and changing it
to PIF_SYSCALL_RESTART makes PIF_SYSCALL and PIF_SYSCALL_RESTART
more unique.
- On s390 calling sys_sigreturn or sys_rt_sigreturn is implemented by
executing a svc instruction on the process stack which causes a fault.
While handling that fault the fault code sets PIF_SYSCALL to hand over
processing to the syscall code on exit to usermode.
The patch introduces PIF_SYSCALL_RET_SET, which is set if ptrace sets
a return value for a syscall. The s390x ptrace ABI uses r2 both for the
syscall number and return value, so ptrace cannot set the syscall number +
return value at the same time. The flag makes handling that a bit easier.
do_syscall() will just skip executing the syscall if PIF_SYSCALL_RET_SET
is set.
CONFIG_DEBUG_ASCE was removd in favour of the generic CONFIG_DEBUG_ENTRY.
CR1/7/13 will be checked both on kernel entry and exit to contain the
correct asces.

Create a region 3 page table which contains only invalid entries, and
use that via "s390_invalid_asce" instead of the kernel ASCE whenever
there is either
- no user address space available, e.g. during early startup
- as an intermediate ASCE when address spaces are switched
This makes sure that user space accesses in such situations are
guaranteed to fail.

Remove set_fs support from s390. With doing this rework address space
handling and simplify it. As a result address spaces are now setup
like this:

To achieve this the getcpu vdso syscall is removed in order to avoid
secondary address mode and a separate vdso address space in for user
space. The getcpu vdso syscall will be implemented differently with a
subsequent patch.
The kernel accesses user space always via secondary address space.
This happens in different ways:
- with mvcos in home space mode and directly read/write to secondary
address space
- with mvcs/mvcp in primary space mode and copy from primary space to
secondary space or vice versa
- with e.g. cs in secondary space mode and access secondary space
Switching translation modes happens with sacf before and after
instructions which access user space, like before.
Lazy handling of control register reloading is removed in the hope to
make everything simpler, but at the cost of making kernel entry and
exit a bit slower. That is: on kernel entry the primary asce is always
changed to contain the kernel asce, and on kernel exit the primary
asce is changed again so it contains the user asce.
In kernel mode there is only one exception to the primary asce: when
kvm guests are executed the primary asce contains the gmap asce (which
describes the guest address space). The primary asce is reset to
kernel asce whenever kvm guest execution is interrupted, so that this
doesn't has to be taken into account for any user space accesses.

Currently we have to consider too many different values which
in the end only affect identity mapping size. These are:
1. max_physmem_end - end of physical memory online or standby.
Always <= end of the last online memory block (get_mem_detect_end()).
2. CONFIG_MAX_PHYSMEM_BITS - the maximum size of physical memory the
kernel is able to support.
3. "mem=" kernel command line option which limits physical memory usage.
4. OLDMEM_BASE which is a kdump memory limit when the kernel is executed as
crash kernel.
5. "hsa" size which is a memory limit when the kernel is executed during
zfcp/nvme dump.
Through out kernel startup and run we juggle all those values at once
but that does not bring any amusement, only confusion and complexity.
Unify all those values to a single one we should really care, that is
our identity mapping size.

We can only have protected guest pages after a successful set secure
parameters call as only then the UV allows imports and unpacks.
By moving the test we can now also check for it in s390_reset_acc()
and do an early return if it is 0.

Compiling the kernel with Kasan disables automatic 3-level vs 4-level
kernel space paging selection, because the shadow memory offset has
to be known at compile time and there is no such offset which would be
acceptable for both 3 and 4-level paging. Instead S390_4_LEVEL_PAGING
option was introduced which allowed to pick how many paging levels to
use under Kasan.
With the introduction of protected virtualization, kernel memory layout
may be affected due to ultravisor secure storage limit. This adds
additional complexity into how memory layout would look like in
combination with Kasan predefined shadow memory offsets. To simplify
this make Kasan 4-level paging default and remove Kasan 3-level paging
support.

Currently the kernel crashes in Kasan instrumentation code if
CONFIG_KASAN_S390_4_LEVEL_PAGING is used on protected virtualization
capable machine where the ultravisor imposes addressing limitations on
the host and those limitations are lower then KASAN_SHADOW_OFFSET.
The problem is that Kasan has to know in advance where vmalloc/modules
areas would be. With protected virtualization enabled vmalloc/modules
areas are moved down to the ultravisor secure storage limit while kasan
still expects them at the very end of 4-level paging address space.
To fix that make Kasan recognize when protected virtualization is enabled
and predefine vmalloc/modules areas position which are compliant with
ultravisor secure storage limit.
Kasan shadow itself stays in place and might reside above that ultravisor
secure storage limit.
One slight difference compaired to a kernel without Kasan enabled is that
vmalloc/modules areas position is not reverted to default if ultravisor
initialization fails. It would still be below the ultravisor secure
storage limit.

Kasan configuration options and size of physical memory present could
affect kernel memory layout. In particular vmemmap, vmalloc and modules
might come before kasan shadow or after it. To make ptdump correctly
output markers in the right order markers have to be sorted.
To preserve the original order of markers with the same start address
avoid using sort() from lib/sort.c (which is not stable sorting algorithm)
and sort markers in place.

We don't need to export pages if we destroy the VM configuration
afterwards anyway. Instead we can destroy the page which will zero it
and then make it accessible to the host.
Destroying is about twice as fast as the export.

ARCH_HAS_DEBUG_WX feature support brought attention to the fact that
currently initial kasan shadow memory mapped without noexec flag. So fix that.
Temporary initial identity mapping is still created without noexec, but
it is replaced by properly set up paging later.

Checks the whole kernel address space for W+X mappings. Note that
currently the first lowcore page unfortunately has to be mapped
W+X. Therefore this not reported as an insecure mapping.
For the very same reason the wording is also different to other
architectures if the test passes:
On s390 it is "no unexpected W+X pages found" instead of
"no W+X pages found".
