Just call the block functions directly, don't wrap them
in skd helpers. With only one queueing model enabled, there's
no point in doing that.
Also kill the ->start_time and ->bio from the skd_request_context,
we don't use those anymore.

The skd driver has a selectable rq or bio based queueing model.
For 3.14, we want to turn this into a single blk-mq interface
instead. With the immutable biovecs being merged in 3.13, the
bio model would need patches to even work. So rip it out, with
a conversion pending for blk-mq in the next release.

"elevator: Fix a race in elevator switching and md device initialization"
changed the semantics of elevator_init() in a way that now enforces to hold
the corresponding request queue's sysfs_lock when calling elevator_init()
to fix a race.
The patch did not convert the s390 dasd device driver which is the only
device driver which also calls elevator_init(). So add the missing locking.

This patch fixes a possible Kernel Panic on driver load if
the configuration on the card is messed up or not yet set.
The driver could possible give a 32 bit unsigned all Fs to
the kernel as the device's block size.
Now we only write the block size to the kernel if the
configuration from the card is valid.
Also, driver version is being updated.

This patch fixes a bug in which discards were always
calling pci_unmap_page. Discards should never call the
pci_unmap_page function call because they are never mapped.
This caused a race condition on PowerPC systems when issuing
discards, writes, and reads all at the same time. The
pci_map_page function would eventually map logical address
0 for a read or write. Discards are always assigned a DMA
address of 0 because they are never mapped. So if
pci_map_page mapped address 0 for a DMA and a discard was
"unmapped" then the address would be freed and would cause
an EEH event to occur when Hardware accesses the address.
This was injected/uncovered in commit:

The pci_dma_mapping_error function declares -1 a DMA_ERROR
not 0 like initially thought So before we would never unmap
discards because they were considered NULL.

For a long time, the receiving side has spread "too large" incoming
requests over multiple bios. No need to shrink our max_bio_size
(max_hw_sectors) if the peer is reconfigured to use a different storage.
The problem manifests itself if we are not the top of the device stack
(DRBD is used a LVM PV).
A hardware reconfiguration on the peer may cause the supported
max_bio_size to shrink, and the connection handshake would now
unnecessarily shrink the max_bio_size on the active node.
There is no way to notify upper layers that they have to "re-stack"
their limits. So they won't notice at all, and may keep submitting bios
that are suddenly considered "too large for device".
We already check for compatibility and ignore changes on the peer,
the code only was masked out unless we have a fully established connection.
We just need to allow it a bit earlier during the handshake.
Also consider max_hw_sectors in our merge bvec function, just in case.

Symptoms: disconnect after bitmap exchange due to
bitmap overflow (e:49731075554) while decoding bm RLE packet
In the decoding step of the variable length integer run length encoding
there was potentially an uncatched bitshift by wordsize (variable >> 64).
The result of which is "undefined" :(
(only "sometimes" the result is the desired 0)
Fix: don't do any bit shift magic for shift == 64, just assign.

Online adding of new minors with freshly created meta data
to an resource with an established connection failed, with a
wrong state transition on one side on one side of the new minor.
Freshly created meta-data has a la_size (last agreed size) of 0.
When we online add such devices, the code wrongly got into
the code path for resyncing new storage that was added while
the disk was detached.
Fixed that by making the GREW from ZERO a special case.

Since drbd-8.4.0 it is possible to change the allow-two-primaries
network option while the connection is established.
The sequence code used to partially order packets from the
data socket with packets from the meta-data socket, still assued
that the allow-two-primaries option is constant while the
connection is established.
I.e.
On a node that has the RESOLVE_CONFLICTS bits set, after enabling
allow-two-primaries, when receiving the next data packet it timed out
while waiting for the necessary packets on the data socket to arrive
(wait_for_and_update_peer_seq() function).
Fixed that by always tracking the sequence number, but only waiting
for it if allow-two-primaries is set.

On surprise removal of the device, gendisk, request queue, device index, sysfs
entries, etc are retained as long as device is in use - mounted filesystem,
device opened by an application, etc. The service thread breaks out of the main

free. When there no holders of the device, service thread cleans up the block
and device related stuff and returns.

issued workqueue to prevent an us running out of
mappable addresses on non-HWWD PCIe x8 slots. The
maximum amount that can possible be mapped at one
time now is: 255 dmas X 4 dma channels X 4096 Bytes.

The rsxx driver was not checking the correct value during a
pci_map_page failure. Fixing this also uncovered a
double free if the bio was returned before it was
broken up into indiviadual 4k dmas, that is also
fixed here.

Improve the calculation of required grants to process a request by
using nr_phys_segments instead of always assuming a request is going
to use all posible segments.
nr_phys_segments contains the number of scatter-gather DMA addr+len
pairs, which is basically what we put at every granted page.
for_each_sg iterates over the DMA addr+len pairs and uses a grant
page for each of them.

There's no need to keep the foreign access in a grant if it is not
persistently mapped by the backend. This allows us to free grants that
are not mapped by the backend, thus preventing blkfront from hoarding
all grants.
The main effect of this is that blkfront will only persistently map
the same grants as the backend, and it will always try to use grants
that are already mapped by the backend. Also the number of persistent
grants in blkfront is the same as in blkback (and is controlled by the
value in blkback).

This happens because xgmac_change_mtu modifies dev->mtu before the
network interface is quiesced. And thus there still might be buffers
in use which have a buffer size based on the old MTU.
To fix this I moved the change of dev->mtu after the call to
xgmac_stop.
Another modification is required (in xgmac_stop) to ensure that
xgmac_xmit is really not called anymore (xgmac_tx_complete might wake
up the queue again).
I've tested the fix by switching MTU size every second between 600 and
1500 while network traffic was going on. The test box survived a test
of several hours (until I've stopped it) whereas w/o this fix above
panic occurs after several minutes (at most).

For each RX/TX ring and its CQ, allocation is done on a NUMA node that
corresponds to the core that the data structure should operate on.
The assumption is that the core number is reflected by the ring index.
The affected allocations are the ring/CQ data structures,
the TX/RX info and the shared HW/SW buffer.
For TX rings, each core has rings of all UPs.

Currently all TX/RX rings and completion queues are part of the
netdev priv structure and are allocated statically. This patch
will change the priv to hold only arrays of pointers and therefore
all TX/RX rings and completetion queues will be allocated
dynamically. This is in preparation for NUMA aware allocations.

To guarantee that all unused fields in all FW commands for both inboxes
and outboxes are zeroed out, initialize the mailbox buffer to all zeroes.
This is especially important for SRIOV comm-channel virtual commands
(such as QUERY_FUNC_CAP), where if new fields are added to support new
features, the driver can depend on older kernels passing zeroes in these
fields.
In addition to zeroing out the mailbox buffer at allocation time, all
(now unnecessary) calls to memset by the callers of
mlx4_alloc_cmd_mailbox() are removed.

Now that l2 acceleration ops are in place from the prior patch,
enable ixgbe to take advantage of these operations. Allow it to
allocate queues for a macvlan so that when we transmit a frame,
we can do the switching in hardware inside the ixgbe card, rather
than in software.

Add a operations structure that allows a network interface to export
the fact that it supports package forwarding in hardware between
physical interfaces and other mac layer devices assigned to it (such
as macvlans). This operaions structure can be used by virtual mac
devices to bypass software switching so that forwarding can be done
in hardware more efficiently.

On top of commit 366cddb40 "IB/rdma_cm: TOS <=> UP mapping for IBoE", add
support for case vlan egress map is used.
When the IBoE session is being set over a vlan, inherit the socket priority
to vlan priority mapping which was configured for the vlan device egress map.

If build_skb fails the memory associated with the ring buffer is freed but
the ri->data member is not zeroed in this case. This causes a double-free
of this memory in tg3_free_rings->... path. The patch moves this block after
setting ri->data to NULL.
It would be nice to fix this bug also in stable >= v3.4 trees.

Previously we enabled the upstream PCI-to-PCI bridge only when
"dev->bus->self != NULL". In the case of a VF on a virtual bus, where
"bus->self == NULL", we didn't enable the upstream bridge.
This fixes that by enabling the upstream bridge of the PF corresponding to
the VF.

STI text console (sticon) was broken on 64bit machines with more than
4GB RAM and this lead in some cases to a kernel crash.
Since sticon uses the 32bit STI API it needs to keep pointers to memory
below 4GB. But on a 64bit kernel some memory regions (e.g. the kernel
stack) might be above 4GB which then may crash the kernel in the STI
functions.
Additionally sticon didn't selected the built-in framebuffer fonts by
default. This is now fixed.
On a side-note: Theoretically we could enhance the sticon driver to
use the 64bit STI API. But - beside the fact that some machines don't
provide a 64bit STI ROM - this would just add complexity.

This patch aims to extend round-robin mode with a new option called
packets_per_slave which can have the following values and effects:
0 - choose a random slave
1 (default) - standard round-robin, 1 packet per slave
>1 - round-robin when >1 packets have been transmitted per slave
The allowed values are between 0 and 65535.
This patch also fixes the comment style in bond_xmit_roundrobin().

When decreasing frequency, requested_freq may be less than
freq_target, So requested_freq minus freq_target may be negative,
But reqested_freq's unit is unsigned int, then the negative result
will be one larger interger which may be even higher than
requested_freq.
This patch is to fix such issue. when result becomes negative,
set requested_freq as the min value of policy.

There are two different interfaces for queuing up work items on the
ACPI hotplug workqueue, alloc_acpi_hp_work() used by PCI and PCI host
bridge hotplug code and acpi_os_hotplug_execute() used by the common
ACPI hotplug code and docking stations. They both are somewhat
cumbersome to use and work slightly differently.
The users of alloc_acpi_hp_work() have to submit a work function that
will extract the necessary data items from a struct acpi_hp_work
object allocated by alloc_acpi_hp_work() and then will free that
object, while it would be more straightforward to simply use a work
function with one more argument and let the interface take care of
the execution details.
The users of acpi_os_hotplug_execute() also have to deal with the
fact that it takes only one argument in addition to the work function
pointer, although acpi_os_execute_deferred() actually takes care of
the allocation and freeing of memory, so it would have been able to
pass more arguments to the work function if it hadn't been
constrained by the connection with acpi_os_execute().
Moreover, while alloc_acpi_hp_work() makes GFP_KERNEL memory
allocations, which is correct, because hotplug work items are
always queued up from process context, acpi_os_hotplug_execute()
uses GFP_ATOMIC, as that is needed by acpi_os_execute(). Also,
acpi_os_execute_deferred() queued up by it waits for the ACPI event
workqueues to flush before executing the work function, whereas
alloc_acpi_hp_work() can't do anything similar. That leads to
somewhat arbitrary differences in behavior between various ACPI
hotplug code paths and has to be straightened up.
For this reason, replace both alloc_acpi_hp_work() and
acpi_os_hotplug_execute() with a single interface,
acpi_hotplug_execute(), combining their behavior and being more
friendly to its users than any of the two.

In the atmel driver probe function, the code shows like following:
atmel_nand_probe(...) {
...
err_nand_ioremap:
platform_driver_unregister(&atmel_nand_nfc_driver);
return res;
}
If no nand flash detected, the driver probe function will goto
err_nand_ioremap label.
Then platform_driver_unregister() will be called. It will get the
lock of atmel_nand device since it is parent of nfc_device. The
problem is the lock is already hold by atmel_nand_probe itself.
So system will be in a dead lock.
This patch just simply removed to platform_driver_unregister() call.
When atmel_nand driver is quit the platform_driver_unregister() will
be called in atmel_nand_remove().
[Brian: the NAND platform probe really has no business
registering/unregistering another driver; this fixes the deadlock, but
we should follow up the likely racy behavior here with a better
architecture]

Commit fa180eb448 (PM / Runtime: Idle devices asynchronously after
probe|release) modified __device_release_driver() to call
pm_runtime_put(dev) instead of pm_runtime_put_sync(dev) before
detaching the driver from the device. However, that was a mistake,
because pm_runtime_put(dev) causes rpm_idle() to be queued up and
the driver may be gone already when that function is executed.
That breaks the assumptions the drivers have the right to make
about the core's behavior on the basis of the existing documentation
and actually causes problems to happen, so revert that part of
commit fa180eb448 and restore the previous behavior of
__device_release_driver().

There are too many "chip->ecc" in the nand_scan_tail() which makes the eyes
sore.
This patch uses a local variable "ecc" to replace the "chip->ecc" to
make the code more graceful.
Do the code change with "s/chip->ecc\./ecc->/g" in the nand_scan_tail,
and also change some lines by hand.

Ensure that the error message if we identify a flash we don't know how to
talk to is displayed on the console in order to aid diagnostics. While

"Managed Device Resource" or devm_xx calls takes care of automatic freeing
of the resource in case of:
- failure during driver probe
- failure during resource allocation
- detaching or unloading of driver module (rmmod)

generic frame-work in mtd/nand/nand_bch.c is a wrapper above lib/bch.h which
encapsulates all control information specific to BCH ecc algorithm in software.
Thus this patch:
(1) replace omap specific implementations with equivalent wrapper in nand_bch.c
so that generic code from nand_bch.c is re-used. like;
omap3_correct_data_bch() -> nand_bch_correct_data()

In current implementation omap3_init_bch_tail() is a common function to
define ecc layout for different BCHx ecc schemes.This patch:
(1) removes omap3_init_bch_tail() and defines ecc layout for individual
ecc-schemes along with populating their nand_chip->ecc data in
omap_nand_probe(). This improves the readability and scalability of
code for add new ecc schemes in future.
(2) removes 'struct nand_bbt_descr bb_descrip_flashbased' because default
nand_bbt_descr in nand_bbt.c matches the same (.len=1 for x8 devices).
(3) add the check to see if NAND device has enough OOB/Spare bytes to
store ECC signature of whole page, as defined by ecc-scheme.

current implementation in omap3_init_bch() has some redundant code like:
(1) omap3_init_bch() re-probes the DT-binding to detect presence of ELM h/w
engine on SoC. And based on that it selects implemetation of ecc-scheme.
However, this is already done as part of GPMC DT parsing.
(2) As omap3_init_bch() serves as common function for configuring all types of
BCHx ecc-schemes, so there are multiple levels of redudant if..then..else
checks while populating nand_chip->ecc.
This patch make following changes to OMAP NAND driver:
(1) removes omap3_init_bch(): each ecc-scheme is individually configured in
omap_nand_probe() there by removing redundant if..then..else checks.
(2) adds is_elm_present(): re-probing of ELM device via DT is not required as
it's done in GPMC driver probe. Thus is_elm_present() just initializes ELM
driver with NAND probe data, when ecc-scheme with h/w based error-detection
is used.
(3) separates out configuration of different flavours of "BCH4" and "BCH8"
ecc-schemes as given in below table
(4) conditionally compiles callbacks implementations of ecc.hwctl(),
ecc.calculate(), ecc.correct() to avoid warning of un-used functions.

Remove the compile-time option for FAST_READ, since we have run-time
support for detecting it. This refactors the logic for enabling
fast-read, such that for DT-enabled devices, we honor the
"m25p,fast-read" property but for non-DT devices, we default to using
FAST_READ whenever the flash device supports it.
Normal READ and FAST_READ differ only in the following:
* FAST_READ supports SPI higher clock frequencies [1]
* number of dummy cycles; FAST_READ requires 8 dummy cycles (whereas
READ requires 0) to allow the flash sufficient setup time, even when
running at higher clock speeds
Thus, for flash chips which support FAST_READ, there is otherwise no
limiting reason why we cannot use the FAST_READ opcode instead of READ.
It simply allows the SPI controller to run at higher clock rates. So
theoretically, nobody should be needing the compile-time option anyway.
[1] I have a Spansion S25FL128S datasheet which says:
"The maximum operating clock frequency for the READ command is 50
MHz."
And:
"The maximum operating clock frequency for FAST READ command is 133
MHz."

This patch fixes two memory errors:
1. During a probe failure (in mtd_device_parse_register?) the command
buffer would not be freed.
2. The command buffer's size is determined based on the 'fast_read'
boolean, but the assignment of fast_read is made after this
allocation. Thus, the buffer may be allocated "too small".
To fix the first, just switch to the devres version of kzalloc.
To fix the second, increase MAX_CMD_SIZE unconditionally. It's not worth
saving a byte to fiddle around with the conditions here.
This problem was reported by Yuhang Wang a while back.

In the imx6, all the ready/busy pins are binding togeter.
So we should always check the ready/busy pin of the chip 0.
In the other word, when the CS1 is enabled, we should also check the
ready/busy of chip 0; if we check the ready/busy of chip 1,
we will get the wrong result.

Some nand chip has two DIEs in a single chip, such as Micron MT29F32G08QAA.
Each die has its own chip select pin, so this chip acts as two nand
chips.
If we only scan one chip, we may find that we only get 2G for this chip,
but in actually, this chip's size is 4G.
So scan two chips by default.

as per controller description,
"While programming a NAND flash, status read should never skipped.
Because it may happen that a new command is issued to the NAND Flash,
even when the device has not yet finished processing the previous request.
This may result in unpredictable behaviour."
IFC controller never polls for R/B signal after command send. It just return
control to software. This behaviour may not occur with NAND flash access.
because new commands are sent after polling R/B signal. But it may happen
in scenario where GPCM-ASIC and NAND flash device are working simultaneously.
Update the controller driver to take care of this requirement

Most of the bcm47xx devices use TRX format for storing kernel and some
partition like Squashfs or JFFS2. This is pretty flexible solution, CFE
(the bootloader) just writes (and later boots) TRX at some hardcoded
place and paritions can vary in the size.
However some devices don't use TRX format. Very recently we have
discovered ZTE H218N that has kernel and rootfs partitions at some
"random" places.
This patch allows Linux find a rootfs partition after installing custom
image with a CFE bootloader.

Some bright specification writers decided to write this in the ONFI spec
(from ONFI 3.0, Section 3.1):
"The number of blocks and number of pages per block is not required to
be a power of two. In the case where one of these values is not a
power of two, the corresponding address shall be rounded to an
integral number of bits such that it addresses a range up to the
subsequent power of two value. The host shall not access upper
addresses in a range that is shown as not supported."
This breaks every assumption MTD makes about NAND block/chip-size
dimensions -- they *must* be a power of two!
And of course, an enterprising manufacturer has made use of this lovely
freedom. Exhibit A: Micron MT29F32G08CBADAWP
"- Plane size: 2 planes x 1064 blocks per plane
- Device size: 32Gb: 2128 blockss [sic]"
This quickly hits a BUG() in nand_base.c, since the extra dimensions

This commit replaces the currently hardcoded buffer size, by a
dynamic detection scheme. First a small 256 bytes buffer is allocated
so the device can be detected (using READID and friends commands).
After detection, this buffer is released and a new buffer is allocated
to acommodate the page size plus out-of-band size.

Instead of setting info->dma each time a command is prepared,
we can move it after the DMA buffers are allocated.
This is more clear and it's the proper place to enable this, given
DMA cannot be turned on and off during runtime.

Micron N25Q512A is a spi flash memory with following features:
-64MB size, 1.8V, Mulitple I/O, 4KB Sector erase memory.
-Memory is organised as 1024(64KB) main sectors.
-Each sector is divided into 256 pages.
-Register set/Opcodes are similar to other N25Q family products.

The loop that polls the status register waiting for an operation to complete
foolishly bases the timeout simply on the number of loop iterations that have
ocurred. When I increased the processor clock speed, timeouts started to appear
for long block erasure operations. This patch measures the timeout using
jiffies.

phram was 32-bit limited by design. Machines are growing up, but phram
module is still useful. Update it. The patch is bigger than minimum,
because simple_strtoul() is obsolete.
Tested on MIPS64 and compile-tested for PPC (32 bit).

the new round of OPW applicants, but also there are lots of other people
doing staging tree cleanups these days in order to help get the drivers
into mergable shape.
We also merge, and then revert, the ktap code, as Ingo and the other
perf/ftrace developers feel it should go into the "real" part of the
kernel with only a bit more work, so no need to put it in staging for
now.
All of this has been in linux-next for a while with no reported issues.

get slowly migrated over to the safe versions of the attribute groups
(removing userspace races with the creation of the sysfs files.) Also
in here are some kobject updates, devres expansions, and the first round
of Tejun's sysfs reworking to enable it to be used by other subsystems
as a backend for an in-kernel filesystem.
All of these have been in linux-next for a while with no reported
issues.

It turned out other systems also share the same problem with bug 63111
so I made a patch to catch initial brightness level 0 problem. With that
patch applied, we do not need to place HP 250 G1 in DMI table.

Some firmware doesn't initialize initial backlight level to a proper
value and _BQC will return 0 on first time evaluation. We used to be
able to detect such incorrect value with our code logic, as value 0
normally isn't a valid value in _BCL. But with the introduction of Win8,
firmware begins to fill _BCL with values from 0 to 100, now 0 becomes
a valid value but that value will make user's screen black. This patch
test initial _BQC for value 0, if such a value is returned, do not use
it.

When testing if the firmware's initial value is valid, we should use
the corrected level value instead of the raw value returned from
firmware.

Do not load the Intel pstate driver if the platform firmware
(ACPI BIOS) supports the power management alternatives.
The ACPI BIOS indicates that the OS control mode can be used
if the _PSS (Performance Supported States) is defined in ACPI
table. For the OS control mode, the Intel pstate driver will be
loaded.
HP BIOS has several power management modes (firmware, OS-control and
so on). For the OS control mode in HP BIOS, the Intel p-state driver
will be loaded. When the customer chooses the firmware power
management in HP BIOS, the Intel p-state driver will be ignored.
I put hw_vendor_info vendor_info in case other vendors (Dell, Lenovo...)
have their firmware power management. Vendors should make sure their
firmware power management works properly, and they can go for adding
their vendor info to the variable.
I have verified the patch on HP ProLiant servers. The patch worked
correctly.

According to the ACPI spec (5.0, Section 6.3.5), the "Device
insertion in progress (pending)" (0x80) _OST status code is
reserved for the "Insertion Processing" (0x200) source event
which is "a result of an OSPM action". Specifically, it is not
a notification, so that status code should not be used during
notification processing, which unfortunately is done by
acpi_scan_bus_device_check().
For this reason, drop the ACPI_OST_SC_INSERT_IN_PROGRESS _OST
status evaluation from there (it was a mistake to put it in there
in the first place).

Since _handle_hotplug_event_root() is run from the ACPI hotplug
workqueue, it doesn't need to queue up a work item to eject a PCI
host bridge on the same workqueue. Instead, it can just carry out
the eject by calling acpi_bus_device_eject() directly, so make that
happen.

There is no real reasn why acpi_bus_device_eject() and
acpi_bus_hot_remove_device() should work differently, so rework
acpi_bus_device_eject() so that it can be called internally by
both acpi_bus_hot_remove_device() and acpi_eject_store_work().
Accordingly, rework acpi_hotplug_notify_cb() to queue up the
execution of acpi_bus_hot_remove_device() through
acpi_os_hotplug_execute() on eject request notifications.

It is required to do get_device() on the struct acpi_device in
question before passing it to acpi_bus_hot_remove_device() through
acpi_os_hotplug_execute(), because acpi_bus_hot_remove_device()
calls acpi_scan_hot_remove() that does put_device() on that
object.
The ACPI PCI root removal routine, handle_root_bridge_removal(),
doesn't do that, which may lead to premature freeing of the
device object or to executing put_device() on an object that
has been freed already.
Fix this problem by making handle_root_bridge_removal() use
get_device() as appropriate.

In theory, an ACPI device object may be the parent of another
device object whose hotplug is disabled by user space through its
scan handler. In that case, the eject operation targeting the
parent should fail as though the parent's own hotplug was disabled,
but currently this is not the case, because acpi_scan_hot_remove()
doesn't check the disable/enable hotplug status of the children
of the top-most object passed to it.
To fix this, modify acpi_bus_offline_companions() to return an
error code if hotplug is disabled for the given device object.
[Also change the name of the function to acpi_bus_offline(),
because it is not only about companions any more, and change
the name of acpi_bus_online_companions() accordingly.] Make
acpi_scan_hot_remove() propagate that error to its callers.

ACPI scan handlers should always be attached to struct acpi_device
objects before any ACPI drivers, but there is a window during which
a driver may be attached to a struct acpi_device before checking if
there is a matching scan handler. Namely, that will happen if an
ACPI driver module is loaded during acpi_bus_scan() right after
the first namespace walk is complete and before the given device
is processed by the second namespace walk.
To prevent that from happening, set the match_driver flags of
struct acpi_device objects right before running device_attach()
for them in acpi_bus_device_attach().

Since acpi_pci_slot_init() is now called from acpi_pci_init()
and pci-acpi.h contains its header, remove that header (and the
empty definition of that function for CONFIG_ACPI_PCI_SLOT unset)
from internal.h as it doesn't have to be there any more. That also
avoids a build warning about duplicate function definitions for
CONFIG_ACPI_PCI_SLOT unset.

Certain platforms do not allow writes in the MSI-X BARs to setup or tear
down vector values. To combat against the generic code trying to write to
that and either silently being ignored or crashing due to the pagetables
being marked R/O this patch introduces a platform override.
Note that we keep two separate, non-weak, functions default_mask_msi_irqs()
and default_mask_msix_irqs() for the behavior of the arch_mask_msi_irqs()
and arch_mask_msix_irqs(), as the default behavior is needed by x86 PCI
code.
For Xen, which does not allow the guest to write to MSI-X tables - as the
hypervisor is solely responsible for setting the vector values - we
implement two nops.
This fixes a Xen guest crash when passing a PCI device with MSI-X to the
guest. See the bugzilla for more details.

Ages ago, drivers could return values greater than zero from their probe
function and this would be regarded as success.
But after f3ec4f87d6 ("PCI: change device runtime PM settings for probe
and remove") and 967577b062 ("PCI/PM: Keep runtime PM enabled for unbound
PCI devices"), we set dev->driver to NULL if the driver's probe function
returns a value greater than zero.
__pci_device_probe() treats this as success, and drivers can still mostly
work even with dev->driver == NULL, but PCI power management doesn't work,
and we don't call the driver's remove function on rmmod.
To help catch these driver problems, issue a warning in this case.

The while loop only peeks at the top request in the queue but does
not yet consume it. Since we only handle fs requests, we need to
dequeue and complete all other request command types with error
just in case we would ever receive such an unforeseen request.

We used to use a percpu structure vq_index to record the cpu to queue
mapping, this is suboptimal since it duplicates the work of XPS and
loses all other XPS functionality such as allowing user to configure
their own transmission steering strategy.
So this patch switches to use XPS and suggest a default mapping when
the number of cpus is equal to the number of queues. With XPS support,
there's no need for keeping per-cpu vq_index and .ndo_select_queue(),
so they were removed also.

Commit cc9d4598 'net: mv643xx_eth: use of_phy_connect if phy_node
present' made the call to phy_scan optional, if the DT has a link to
the phy node.
However phy_scan has the side effect of calling phy_addr_set, which
writes the phy MDIO address to the ethernet controller. If phy_addr_set
is not called, and the bootloader has not set the correct address then
the driver will fail to function.

f41f064cf4 ("PCI: Workaround missing pci_set_master in pci drivers") made
pci_enable_bridge() turn on bus mastering if the driver hadn't done so
already. It also added a warning in this case. But there's no reason to
warn about it unless it's actually a problem to enable bus mastering here.
This patch drops the warning because I'm not aware of any such problem.

Ben Herrenschmidt found that commit 928bea9648 ("PCI: Delay enabling
bridges until they're needed") breaks PCI in some powerpc environments.
The reason is that the PCIe port driver will call pci_enable_device() on
the bridge, so the device is enabled, but skips pci_set_master because
pcie_port_auto and no acpi on powerpc.
Because of that, pci_enable_bridge() later on (called as a result of the
child device driver doing pci_enable_device) will see the bridge as
already enabled and will not call pci_set_master() on it.
Fixed by add checking in pci_enable_bridge, and call pci_set_master
if driver skip that.
That will make the code more robot and wade off problem for missing
pci_set_master in drivers.

When pg_init is running no I/O can be submitted to the underlying
devices, as the path priority etc might change. When using queue_io for
this, requests will be piling up within multipath as the block I/O
scheduler just sees a _very fast_ device. All of this queued I/O has to
be resubmitted from within multipathing once pg_init is done.
This approach has the problem that it's virtually impossible to
abort I/O when pg_init is running, and we're adding heavy load
to the devices after pg_init since all of the queued I/O needs to be
resubmitted _before_ any requests can be pulled off of the request queue
and normal operation continues.
This patch will requeue the I/O that triggers the pg_init call, and
return 'busy' when pg_init is in progress. With these changes the block
I/O scheduler will stop submitting I/O during pg_init, resulting in a
quicker path switch and less I/O pressure (and memory consumption) after
pg_init.

The #define for the maximum number of GPIO blocks was retrieved
into pinctrl-at91.c by implicit inclusion of <mach/gpio.h>
from <linux/gpio.h> creating a dependency on machine-local
<mach/gpio.h>. Break the depenency by copying this single
define into the driver.

Commit 2613af0ed1 (virtio_net: migrate mergeable
rx buffers to page frag allocators) try to increase the payload/truesize for
MTU-sized traffic. But this will introduce the extra overhead for GSO packets
received because of the frag list. This commit tries to reduce this issue by
coalesce the possible rx frags when possible during rx. Test result shows the
about 15% improvement on full size GSO packet receiving (and even better than
before commit 2613af0ed1).

Implements resource quota grant decision when resources are requested,
for the following resources: QPs, CQs, SRQs, MPTs, MTTs, vlans, MACs,
and Counters.
When granting a resource, the quota system increases the allocated-count
for that slave.
When the slave later frees the resource, its allocated-count is reduced.
A spinlock is used to protect the integrity of each resource's free-pool counter.
(One slave may be in the process of being granted a resource while another
slave has crashed, initiating cleanup of that slave's resource quotas).

In current kernels, the mlx4 driver running on a VM does not
differentiate between max resource numbers for the HCA and
max quotas -- it simply takes the quota values passed to it
as max-resource values.
However, the driver actually requires the VFs to be aware of
the actual number of resources that the HCA was initialized with,
for QPs, CQs, SRQs and MPTs.
For QPs, CQs and SRQs, the reason is that in completion handling
the driver must know which of the 24 bits are the actual resource
number, and which are "padding" bits.
For MPTs, also, the driver assumes knowledge of the number of MPTs
in the system.
The previous commit fixes the quota logic on the VM for the quota values
passed to it by QUERY_FUNC_CAPS.
For QPs, CQs, SRQs, and MPTs, it takes the max resource numbers
from QUERY_HCA (and not QUERY_FUNC_CAPS). The quotas passed
in QUERY_FUNC_CAPS are used to report max resource number values
in the response to ib_query_device.
However, the Hypervisor driver must consider that VMs
may be running previous kernels, and compatibility must be preserved.
To resolve the incompatibility with previous kernels running on VMs,
we deprecated the quota fields in mlx4_QUERY_FUNC_CAP. In the
deprecated fields, we pass the max-resource values from INIT_HCA
The quota fields are moved to a new location, and the current kernel
driver takes the proper values from that location. There is
also a new flag in dword 0, bit 28 of the mlx4_QUERY_FUNC_CAP mailbox;
if this flag is set, the (VM) driver takes the quota values from the
new location.
VMs running previous kernels will work properly, except that the max resource
numbers reported in ib_query_device for these resources will be
too high. The Hypervisor driver will, however, enforce the quotas
for these VMs.

This is step #1 for implementing SRIOV resource quotas for VFs.
Quotas are implemented per resource type for VFs and the PF, to prevent
any entity from simply grabbing all the resources for itself and leaving
the other entities unable to obtain such resources.
Resources which are allocated using quotas: QPs, CQs, SRQs, MPTs, MTTs, MAC,
VLAN, and Counters.
The quota system works as follows:
Each entity (VF or PF) is given a max number of a given resource (its quota),
and a guaranteed minimum number for each resource (starvation prevention).
For QPs, CQs, SRQs, MPTs and MTTs:
50% of the available quantity for the resource is divided equally among
the PF and all the active VFs (i.e., the number of VFs in the mlx4_core module
parameter "num_vfs"). This 50% represents the "guaranteed minimum" pool.
The other 50% is the "free pool", allocated on a first-come-first-serve basis.
For each VF/PF, resources are first allocated from its "guaranteed-minimum"
pool. When that pool is exhausted, the driver attempts to allocate from
the resource "free-pool".
The quota (i.e., max) for the VFs and the PF is:
The free-pool amount (50% of the real max) + the guaranteed minimum
For MACs:
Guarantee 2 MACs per VF/PF per port. As a result, since we have only
128 MACs per port, reduce the allowable number of VFs from 64 to 63.
Any remaining MACs are put into a free pool.
For VLANs:
For the PF, the per-port quota is 128 and guarantee is 64
(to allow the PF to register at least a VLAN per VF in VST mode).
For the VFs, the per-port quota is 64 and the guarantee is 0.
We assume that VGT VFs are trusted not to abuse the VLAN resource.
For Counters:
For all functions (PF and VFs), the quota is 128 and the guarantee is 0.
In this patch, we define the needed structures, which are added to the
resource-tracker struct. In addition, we do initialization
for the resource quota, and adjust the query_device response to use quotas
rather than resource maxima.
As part of the implementation, we introduce a new field in
mlx4_dev: quotas. This field holds the resource quotas used
to report maxima to the upper layers (ib_core, via query_device).
The HCA maxima of these values are passed to the VFs (via
QUERY_HCA) so that they may continue to use these in handling
QPs, CQs, SRQs and MPTs.

In procedure mlx4_init_mr_table(), slaves should do no processing,
but should return success. This initialization is hypervisor-only.
However, the check for num_mpts being a power-of-2 was performed
before the check to return immediately if the driver is for a slave.
This resulted in spurious failures.
The order of performing the checks is reversed, so that if the
driver is for a slave, no processing is done and success is returned.

In upstream kernels under SRIOV, the vlan register/unregister calls
were NOPs (doing nothing and returning OK). We detect these old
calls from guests (via the comm channel), since previously the
port number in mlx4_register_vlan was passed (improperly) in the
out_param. This has been corrected so that the port number is now
passed in bits 8..15 of the in_modifier field.
For old calls, these bits will be zero, so if the passed port
number is zero, we can still look at the out_param field to see
if it contains a valid port number. If yes, the VM is running
an old driver.
Since for old drivers, the register/unregister_vlan wrappers were
NOPs, we continue this policy -- the reason being that upstream
had an additional bug in eth driver running on guests (where
procedure mlx4_en_vlan_rx_kill_vid() had the following code:

On a VM, mlx4_find_cached_vlan() will always fail, since the
vlan cache is located on the Hypervisor; on guests it is empty.
Therefore, if we allow upstream guests to register vlans, we will
have vlan leakage since the unregister will never be performed.
Leaving vlan reg/unreg for old guest drivers as a NOP is not a
feature regression, since in upstream the register/unregister
vlan wrapper is a NOP.

The functions mlx4_register_vlan, mlx4_unregister_vlan, mlx4_register_mac,
mlx4_unregister_mac all made illegal use of the out_param in multifunc mode
to pass the port number. The firmware spec specifies that the port number
should be passed in bits 8..15 of the input-modifier field for ALLOC_RES and
FREE_RES (sections 20.15.1 and 20.15.2).
For MAC register/unregister, this patch contains workarounds so that guests
running previous kernels continue to work on a new Hypervisor, and guests
running the new kernel will continue to work on old hypervisors.
Vlan registeration capability is still not operational in multifunction mode,
since the vlan wrapper functions are not implemented in this patch.

Commit 2361613206, "of/irq: Refactor interrupt-map parsing" introduced
a potential buffer overflow bug because it doesn't do sufficient range
checking on the input data. This patch adds the appropriate checking and
buffer size adjustments. If the bounds are out of range then warn
loudly. MAX_PHANDLE_ARGS should be sufficient. If it is not then the
value can be increased.

Commit 2361613206, "of/irq: Refactor interrupt-map parsing" introduced
a bug. The irq parsing will fail for some nodes that don't have a reg
property. It is fixed by deferring the check for reg until it is
actually needed. Also adjust the testcase data to catch the bug.

There are MBIM devices out there reporting
dwNtbInMaxSize=2048 dwNtbOutMaxSize=2048
and since the spec require a datagram max size of at least
2048, this means that a full sized datagram will never fit.
Still, sending larger NTBs than the device supports is not
going to help. We do not have any other options than either
a) refusing to bindi, or
b) respect the insanely low value.
Alternative b will at least make these devices work, so go
for it.

Rewriting the "set max datagram" part of dc_ncm_setup to
separate the selection and validatation of the size from
the code which optionally informs the device of this
value. This ensures that we use the correct value
regardless of device support for the get and set commands.
Removing some of the many indent levels while doing this
to make the code more readable.

usbnet use the hard_mtu value for sizing the tx queue and nothing
else. We will be transmitting buffers of up to tx_max size, so
that's the proper value to give usbnet.

optimizing their DMA transfers. This optimization depends on
max sized NTBs not being ZLP terminated. So we are much better
off dropping the padding if we are going to send a ZLP anyway.

Moving the call to cdc_ncm_setup() after the endpoint
setup removes the last remaining reference to ncm_parm
outside cdc_ncm_setup.
Collecting all the ncm_parm based calculations in
cdc_ncm_setup improves readability.

These fields are only used to prevent printing the same speeds
multiple times if we receive multiple identical speed notifications.
The value of these printk's is questionable, and even more so when
we filter out some of the notifications sent us by the firmware. If
we are going to print any of these, then we should print them all.
Removing little used fields is a bonus.
