James Smart 1d755d6477 scsi: lpfc: Fix ELS field alignments
After seeing some interoperability issues with ADISC, it was determined the
ELS definitions in lpfc were using types that allowed the compiler to add
pad to the structure, causing the structure to no longer be per spec. The
offending structures are ADISC, FAN, and RNID.

This patch implements the simple fix of eliminating the pad by forcing the
compiler to pack the structure. Care was taken to ensure field accesses
won't be by operations that would hit a bad field alignment.

The better solution would be to convert to the uapi fc header definitions,
but the number of changes required to do is rather intrusive so this course
of action was deferred.

This patch adds general RSCN support:

 - The ability to transmit an RSCN to the port on the other end of
   the link (regular port if pt2pt, or fabric controller if fabric).
 - And general recognition of an RSCN ELS when an ELS is received.

James Smart 287aba2592 scsi: lpfc: ls_rjt erroneus FLOGIs
In some link initialization sequences, the fw generates an erroneous FLOGI
payload to the driver without an intervening link bounce.  The driver, when
it sees a 2nd FLOGI without an intervening link bounce, automatically
performs a link bounce. In this, the link bounce causes the situate to
repeat and in a nasty loop of link bounces.

Resolve the issue by validating the FLOGI payload. The erroneous FLOGI will
contain VVL signatures that are not normal. When the driver sees these, it
will simply reject the flogi rather than bouncing the link.  The reject is
consumed within the firmware.

James Smart 7ea92eb458 scsi: lpfc: Implement GID_PT on Nameserver query to support faster failover
The switches seem to respond faster to GID_PT vs GID_FT NameServer
queries.  Add support for GID_PT to be used over GID_FT to enable
faster storage failover detection. Includes addition of new module
parameter to select between GID_PT and GID_FT (GID_FT is default).

James Smart aeb3c8170b Add Fabric assigned WWN support.
Adding support for Fabric assigned WWPN and WWNN.

Firmware sends first FLOGI to fabric with vendor version changes.
On link up driver gets updated service parameter with FAWWN assigned port
name.  Driver sends 2nd FLOGI with updated fawwpn and modifies the
vport->fc_portname in driver.

Note:
Soft wwpn will not be allowed when fawwpn is enabled.

James Smart 895427bd01 scsi: lpfc: NVME Initiator: Base modifications
NVME Initiator: Base modifications

This patch adds base modifications for NVME initiator support.

The base modifications consist of:
- Formal split of SLI3 rings from SLI-4 WQs (sometimes referred to as
  rings as well) as implementation now widely varies between the two.
- Addition of configuration modes:
   SCSI initiator only; NVME initiator only; NVME target only; and
   SCSI and NVME initiator.
   The configuration mode drives overall adapter configuration,
   offloads enabled, and resource splits.
   NVME support is only available on SLI-4 devices and newer fw.
- Implements the following based on configuration mode:
  - Exchange resources are split by protocol; Obviously, if only
     1 mode, then no split occurs. Default is 50/50. module attribute
     allows tuning.
  - Pools and config parameters are separated per-protocol
  - Each protocol has it's own set of queues, but share interrupt
    vectors.
     SCSI:
       SLI3 devices have few queues and the original style of queue
         allocation remains.
       SLI4 devices piggy back on an "io-channel" concept that
         eventually needs to merge with scsi-mq/blk-mq support (it is
	 underway).  For now, the paradigm continues as it existed
	 prior. io channel allocates N msix and N WQs (N=4 default)
	 and either round robins or uses cpu # modulo N for scheduling.
	 A bunch of module parameters allow the configuration to be
	 tuned.
     NVME (initiator):
       Allocates an msix per cpu (or whatever pci_alloc_irq_vectors
         gets)
       Allocates a WQ per cpu, and maps the WQs to msix on a WQ #
         modulo msix vector count basis.
       Module parameters exist to cap/control the config if desired.
  - Each protocol has its own buffer and dma pools.

Adapter Interface fixes and changes

- Modify the macro field from lpfc_init_vpi_vpi to lpfc_init_vfi_vpi
- Add the new CQE_CODE_RECEIVE_V1 CQE Code, add code in the driver to handle
  the new Code the same as the CQE_CODE_RECEIVE code except that there are
  two new checks for this code that will cause the driver to use the new V1
  macros for rq_id and fcf_id.
- Fix a bug in lpfc_prep_seq() where the size out of the first CQE was
  ONLY being used, even though multiple dmabufs make up the sequence,
  each have their own CQE with potentially different sizes.
- Fix bug in lpfc_bsg_ct_unsol_event() where the ulpContext and ulpWord[3]
  fields of the XMIT_SEQUENCE64_CX IOCB were being calculated incorrectly.
- Do physical to logical translation before indexing into the active
  XRI array.
- Populate physical vpi in the iocb data structure.
- Put the current accumulated total in each IOCB in the chain as we are
  walking thru then. The last IOCB in the chain should have the total
  length of the sequence.

Phil Carmody 497888cf69 treewide: fix potentially dangerous trailing ';' in #defined values/expressions
All these are instances of
  #define NAME value;
or
  #define NAME(params_opt) value;

These of course fail to build when used in contexts like
  if(foo $OP NAME)
  while(bar $OP NAME)
and may silently generate the wrong code in contexts such as
  foo = NAME + 1;    /* foo = value; + 1; */
  bar = NAME - 1;    /* bar = value; - 1; */
  baz = NAME & quux; /* baz = value; & quux; */

Initial analysis of the dangers provided by Keith Thompson in that thread.

There are many more instances of more complicated macros having unnecessary
trailing semicolons, but this pile seems to be all of the cases of simple
values suffering from the problem. (Thus things that are likely to be found
in one of the contexts above, more complicated ones aren't.)
