Move the generic implementation to <linux/dma-mapping.h> now that all
architectures support it and remove the HAVE_DMA_ATTR Kconfig symbol now
that everyone supports them.

For architectures using dma_map_ops, dma_mmap_coherent() and
dma_get_sgtable() are provided in <asm-generic/dma-mapping-common.h>.
Blackfin does not use dma_map_ops, hence it should implement them as inline
stubs using dma_common_mmap() and dma_common_get_sgtable().

Architectures implement dma_is_consistent() in different ways (some
misinterpret the definition of API in DMA-API.txt). So it hasn't been so
useful for drivers. We have only one user of the API in tree. Unlikely
out-of-tree drivers use the API.
Even if we fix dma_is_consistent() in some architectures, it doesn't look
useful at all. It was invented long ago for some old systems that can't
allocate coherent memory at all. It's better to export only APIs that are
definitely necessary for drivers.
Let's remove this API.

dma_get_cache_alignment returns the minimum DMA alignment. Architectures
defines it as ARCH_DMA_MINALIGN (formally ARCH_KMALLOC_MINALIGN). So we
can unify dma_get_cache_alignment implementations.
Note that some architectures implement dma_get_cache_alignment wrongly.
dma_get_cache_alignment() should return the minimum DMA alignment. So
fully-coherent architectures should return 1. This patch also fixes this
issue.

pci_dma_sync_single was obsoleted long ago.
All the comments are generic, not architecture specific, simply describes
some of the DMA-API (and the same comments are in other files).
Documentation/DMA-API.txt have more detailed descriptions.
This removes the above obsolete and unnecessary DMA API comments. Let's
describe the DMA API in only Documentation/DMA-API.txt.

Bill Gatliff & David Brownell pointed out we were missing some
copyrights, and licensing terms in some of the files in
./arch/blackfin, so this fixes things, and cleans them up.
It also removes:
- verbose GPL text(refer to the top level ./COPYING file)
- file names (you are looking at the file)
- bug url (it's in the ./MAINTAINERS file)
- "or later" on GPL-2, when we did not have that right
It also allows some Blackfin-specific assembly files to be under a BSD
like license (for people to use them outside of Linux).

The recent deprecation of dma_sync_{sg,single} ironically broke Blackfin
systems. This is because we don't define dma_sync_sg_for_cpu at all, so
until the DMA asm-generic conversion/cleanup is done after the next
release, simply stub out the dma_sync_sg_for_{cpu,device} functions.
