Martin Schwidefsky 50d7280d43 [S390] fault handler performance optimization.
Slim down the do_exception function to handle only the fast path of a
fault and move the exceptional cases into a new function. That slightly
increases the performance of the fault handling.

Martin Schwidefsky 7ecb344ae8 [S390] Improve notify_page_fault implementation.
notify_page_fault does a preempt_disable/preempt_enable for each
fault generated by a kernel access to user space. If kprobes
is not active that is unnecessary since the interrupts are not
reenabled yet. To play safe repeat the kprobe_running check after
preempt_disable().

Martin Schwidefsky b11b533427 [S390] Improve address space mode selection.
Introduce user_mode to replace the two variables switch_amode and
s390_noexec. There are three valid combinations of the old values:
  1) switch_amode == 0 && s390_noexec == 0
  2) switch_amode == 1 && s390_noexec == 0
  3) switch_amode == 1 && s390_noexec == 1
They get replaced by
  1) user_mode == HOME_SPACE_MODE
  2) user_mode == PRIMARY_SPACE_MODE
  3) user_mode == SECONDARY_SPACE_MODE
The new kernel parameter user_mode=[primary,secondary,home] lets
you choose the address space mode the user space processes should
use. In addition the CONFIG_S390_SWITCH_AMODE config option
is removed.

Martin Schwidefsky 61365e132e [S390] Improve address space check.
A data access in access-register mode always is a user mode access,
the code to inspect the access-registers can be removed. The second
change is to use a different test to check for no-execute fault.
The third change is to pass the translation exception identification
as parameter, in theory the trans_exc_code in the lowcore could have
been overwritten by the time the call to check_space from do_no_context
is done.

When the resume image is copied back and a write access happen to an

Ingo Molnar cdd6c482c9 perf: Do the big rename: Performance Counters -> Performance Events
Bye-bye Performance Counters, welcome Performance Events!

In the past few months the perfcounters subsystem has grown out its
initial role of counting hardware events, and has become (and is
becoming) a much broader generic event enumeration, reporting, logging,
monitoring, analysis facility.

Naming its core object 'perf_counter' and naming the subsystem
'perfcounters' has become more and more of a misnomer. With pending
code like hw-breakpoints support the 'counter' name is less and
less appropriate.

All in one, we've decided to rename the subsystem to 'performance
events' and to propagate this rename through all fields, variables
and API names. (in an ABI compatible fashion)

The word 'event' is also a bit shorter than 'counter' - which makes
it slightly more convenient to write/handle as well.

Thanks goes to Stephane Eranian who first observed this misnomer and
suggested a rename.

User-space tooling and ABI compatibility is not affected - this patch
should be function-invariant. (Also, defconfigs were not touched to
keep the size down.)

This patch has been generated via the following script:

  FILES=$(find * -type f | grep -vE 'oprofile|[^K]config')

  sed -i \
    -e 's/PERF_EVENT_/PERF_RECORD_/g' \
    -e 's/PERF_COUNTER/PERF_EVENT/g' \
    -e 's/perf_counter/perf_event/g' \
    -e 's/nb_counters/nb_events/g' \
    -e 's/swcounter/swevent/g' \
    -e 's/tpcounter_event/tp_event/g' \
    $FILES

  for N in $(find . -name perf_counter.[ch]); do
    M=$(echo $N | sed 's/perf_counter/perf_event/g')
    mv $N $M
  done

  FILES=$(find . -name perf_event.*)

  sed -i \
    -e 's/COUNTER_MASK/REG_MASK/g' \
    -e 's/COUNTER/EVENT/g' \
    -e 's/\<event\>/event_id/g' \
    -e 's/counter/event/g' \
    -e 's/Counter/Event/g' \
    $FILES

... to keep it as correct as possible. This script can also be
used by anyone who has pending perfcounters patches - it converts
a Linux kernel tree over to the new naming. We tried to time this
change to the point in time where the amount of pending patches
is the smallest: the end of the merge window.

Namespace clashes were fixed up in a preparatory patch - and some
stylistic fallout will be fixed up in a subsequent patch.

( NOTE: 'counters' are still the proper terminology when we deal
with hardware registers - and these sed scripts are a bit
over-eager in renaming them. I've undone some of that, but
in case there's something left where 'counter' would be
better than 'event' we can undo that on an individual basis
instead of touching an otherwise nicely automated patch. )

Martin Schwidefsky 50aa98bad0 [S390] fix recursive locking on page_table_lock
Suzuki Poulose reported the following recursive locking bug on s390:

Here is the stack trace : (see Appendix I for more info)

  [<0000000000406ed6>] _spin_lock+0x52/0x94
  [<0000000000103bde>] crst_table_free+0x14e/0x1a4
  [<00000000001ba684>] __pmd_alloc+0x114/0x1ec
  [<00000000001be8d0>] handle_mm_fault+0x2cc/0xb80
  [<0000000000407d62>] do_dat_exception+0x2b6/0x3a0
  [<0000000000114f8c>] sysc_return+0x0/0x8
  [<00000200001642b2>] 0x200001642b2

The page_table_lock is already acquired in __pmd_alloc (mm/memory.c) and
it tries to populate the pud/pgd with a new pmd allocated. If another
thread populates it before we get a chance, we free the pmd using
pmd_free().

On s390x, pmd_free(even pud_free ) is #defined to crst_table_free(),
which acquires the page_table_lock to protect the crst_table index updates.

Hence this ends up in a recursive locking of the page_table_lock.

The solution suggested by Dave Hansen is to use a new spin lock in the mmu
context to protect the access to the crst_list and the pgtable_list.

Martin Schwidefsky f481bfafd3 [S390] make page table walking more robust
Make page table walking on s390 more robust. The current code requires
that the pgd/pud/pmd/pte loop is only done for address ranges that are
below the end address of the last vma of the address space. But this
is not always true, e.g. the generic page table walker does not guarantee
this. Change TASK_SIZE/TASK_SIZE_OF to reflect the current size of the
address space. This makes the generic page table walker happy but it
breaks the upgrade of a 3 level page table to a 4 level page table.

Gary Hade c04fc586c1 mm: show node to memory section relationship with symlinks in sysfs
Show node to memory section relationship with symlinks in sysfs

Add /sys/devices/system/node/nodeX/memoryY symlinks for all
the memory sections located on nodeX. For example:
/sys/devices/system/node/node1/memory135 -> ../../memory/memory135
indicates that memory section 135 resides on node1.

Also revises documentation to cover this change as well as updating
Documentation/ABI/testing/sysfs-devices-memory to include descriptions
of memory hotremove files 'phys_device', 'phys_index', and 'state'
that were previously not described there.

In addition to it always being a good policy to provide users with
the maximum possible amount of physical location information for
resources that can be hot-added and/or hot-removed, the following
are some (but likely not all) of the user benefits provided by
this change.
Immediate:
  - Provides information needed to determine the specific node
on which a defective DIMM is located. This will reduce system
downtime when the node or defective DIMM is swapped out.
  - Prevents unintended onlining of a memory section that was
previously offlined due to a defective DIMM. This could happen
during node hot-add when the user or node hot-add assist script
onlines _all_ offlined sections due to user or script inability
to identify the specific memory sections located on the hot-added
node. The consequences of reintroducing the defective memory
    could be ugly.
  - Provides information needed to vary the amount and distribution
of memory on specific nodes for testing or debugging purposes.
Future:
  - Will provide information needed to identify the memory
sections that need to be offlined prior to physical removal
    of a specific node.

Symlink creation during boot was tested on 2-node x86_64, 2-node
ppc64, and 2-node ia64 systems. Symlink creation during physical
memory hot-add tested on a 2-node x86_64 system.

Christian Borntraeger 250cf776f7 [S390] pgtables: Fix race in enable_sie vs. page table ops
The current enable_sie code sets the mm->context.pgstes bit to tell
dup_mm that the new mm should have extended page tables. This bit is also
used by the s390 specific page table primitives to decide about the page
table layout - which means context.pgstes has two meanings. This can cause
any kind of bugs. For example - e.g. shrink_zone can call
ptep_clear_flush_young while enable_sie is running. ptep_clear_flush_young
will test for context.pgstes. Since enable_sie changed that value of the old
struct mm without changing the page table layout ptep_clear_flush_young will
do the wrong thing.
The solution is to split pgstes into two bits
- one for the allocation
- one for the current state

Andi Kleen a551643895 hugetlb: modular state for hugetlb page size
The goal of this patchset is to support multiple hugetlb page sizes. This
is achieved by introducing a new struct hstate structure, which
encapsulates the important hugetlb state and constants (eg. huge page
size, number of huge pages currently allocated, etc).

The hstate structure is then passed around the code which requires these
fields, they will do the right thing regardless of the exact hstate they
are operating on.

This patch adds the hstate structure, with a single global instance of it
(default_hstate), and does the basic work of converting hugetlb to use the
hstate.

Future patches will add more hstate structures to allow for different
hugetlbfs mounts to have different page sizes.

Christian Borntraeger 74b6b522ec KVM: s390: fix locking order problem in enable_sie
There are potential locking problem in enable_sie. We take the task_lock
and the mmap_sem. As exit_mm uses the same locks vice versa, this triggers
a lockdep warning.
The second problem is that dup_mm and mmput might sleep, so we must not
hold the task_lock at that moment.

The solution is to dup the mm unconditional and use the task_lock before and
afterwards to check if we can use the new mm. dup_mm and mmput are called
outside the task_lock, but we run update_mm while holding the task_lock,
protection us against ptrace.

Calling the generic vmemmap_alloc_block() function to get initialized

Carsten Otte 402b08622d s390: KVM preparation: provide hook to enable pgstes in user pagetable
The SIE instruction on s390 uses the 2nd half of the page table page to
virtualize the storage keys of a guest. This patch offers the s390_enable_sie
function, which reorganizes the page tables of a single-threaded process to
reserve space in the page table:
s390_enable_sie makes sure that the process is single threaded and then uses
dup_mm to create a new mm with reorganized page tables. The old mm is freed
and the process has now a page status extended field after every page table.

Code that wants to exploit pgstes should SELECT CONFIG_PGSTE.

This patch has a small common code hit, namely making dup_mm non-static.

Edit (Carsten): I've modified Martin's patch, following Jeremy Fitzhardinge's
review feedback. Now we do have the prototype for dup_mm in
include/linux/sched.h. Following Martin's suggestion, s390_enable_sie() does now
call task_lock() to prevent race against ptrace modification of mm_users.

Martin Schwidefsky 2f569afd9c CONFIG_HIGHPTE vs. sub-page page tables.
Background: I've implemented 1K/2K page tables for s390. These sub-page
page tables are required to properly support the s390 virtualization
instruction with KVM. The SIE instruction requires that the page tables
have 256 page table entries (pte) followed by 256 page status table entries
(pgste). The pgstes are only required if the process is using the SIE
instruction. The pgstes are updated by the hardware and by the hypervisor
for a number of reasons, one of them is dirty and reference bit tracking.
To avoid wasting memory the standard pte table allocation should return
1K/2K (31/64 bit) and 2K/4K if the process is using SIE.

Problem: Page size on s390 is 4K, page table size is 1K or 2K. That means
the s390 version for pte_alloc_one cannot return a pointer to a struct
page. Trouble is that with the CONFIG_HIGHPTE feature on x86 pte_alloc_one
cannot return a pointer to a pte either, since that would require more than
32 bit for the return value of pte_alloc_one (and the pte * would not be
accessible since its not kmapped).

Solution: The only solution I found to this dilemma is a new typedef: a
pgtable_t. For s390 pgtable_t will be a (pte *) - to be introduced with a
later patch. For everybody else it will be a (struct page *). The
additional problem with the initialization of the ptl lock and the
NR_PAGETABLE accounting is solved with a constructor pgtable_page_ctor and
a destructor pgtable_page_dtor. The page table allocation and free
functions need to call these two whenever a page table page is allocated or
freed. pmd_populate will get a pgtable_t instead of a struct page pointer.
To get the pgtable_t back from a pmd entry that has been installed with
pmd_populate a new function pmd_pgtable is added. It replaces the pmd_page
call in free_pte_range and apply_to_pte_range.

Christian Borntraeger a2fd64d6aa [S390] vmemmap: allocate struct pages before 1:1 mapping
We have seen an oops in an OOM situation, where show_mem tried to
access the struct page of a dcss segment. The vmemmap code has
already created the 1:1 mapping but failed allocating the struct
pages. In the OOM case, show_mem now walks the memory. It uses
pfn_valid to detect if it may access the struct page. In the case
described above, the mapping was established and pfn_valid returned
true. As the struct pages were not allocated, the kernel oopsed.

We have to ensure that we have created the struct pages, before we
add a mapping pointing to the pages.

compare the task structure with 'init_pid_ns.child_reaper', which is
initialized during boot to the /sbin/init process and never changes.

Changelog:

	2.6.22-rc4-mm2-pidns1:
	- Use 'init_pid_ns.child_reaper' to determine if a given task is the
global init (/sbin/init) process. This would improve performance
and remove dependence on the task_pid().

	2.6.21-mm2-pidns2:

	- [Sukadev Bhattiprolu] Changed is_container_init() calls in {powerpc,
ppc,avr32}/traps.c for the _exception() call to is_global_init().
This way, we kill only the cgroup if the cgroup's init has a
bug rather than force a kernel panic.

Will Schmidt dcca2bde4f During VM oom condition, kill all threads in process group
We have had complaints where a threaded application is left in a bad state
after one of it's threads is killed when we hit a VM: out_of_memory
condition.

Killing just one of the process threads can leave the application in a bad
state, whereas killing the entire process group would allow for the
application to restart, or be otherwise handled, and makes it very obvious
that something has gone wrong.

This change allows the entire process group to be taken down, rather
than just the one thread.

Christoph Hellwig 1eeb66a1bb move die notifier handling to common code
This patch moves the die notifier handling to common code. Previous
various architectures had exactly the same code for it. Note that the new
code is compiled unconditionally, this should be understood as an appel to
the other architecture maintainer to implement support for it aswell (aka
sprinkling a notify_die or two in the proper place)

arm had a notifiy_die that did something totally different, I renamed it to
arm_notify_die as part of the patch and made it static to the file it's
declared and used at. avr32 used to pass slightly less information through
this interface and I brought it into line with the other architectures.

Replaced check_user_space() + __check_access_register with the new

(kernel in home space, user in primary/secondary space).
Secondly the user process can switch to the accress register mode if

an arbitrary value to the access registers. If any other value than
0 for primary space or 1 for secondary space is loaded and memory
is accessed using the base register related to the access register,
the program should be terminated with a SIGSEGV. To achieve that the
DUALD pointer in the DUCT and the PSALD pointer in the PASTE need

Eric W. Biederman 0b4d414714 [PATCH] sysctl: remove insert_at_head from register_sysctl
The semantic effect of insert_at_head is that it would allow new registered
sysctl entries to override existing sysctl entries of the same name. Which is
pain for caching and the proc interface never implemented.

I have done an audit and discovered that none of the current users of
register_sysctl care as (excpet for directories) they do not register
duplicate sysctl entries.

So this patch simply removes the support for overriding existing entries in
the sys_sysctl interface since no one uses it or cares and it makes future
enhancments harder.

So we'll just need to replace NO_IOPORT with NO_IOMEM in Kconfig and

BTW, there's an annoying bit of junk in there - IO_SPACE_LIMIT. We

Gerald Schaefer c1821c2e97 [S390] noexec protection
This provides a noexec protection on s390 hardware. Our hardware does
not have any bits left in the pte for a hw noexec bit, so this is a
different approach using shadow page tables and a special addressing
mode that allows separate address spaces for code and data.

As a special feature of our "secondary-space" addressing mode, separate
page tables can be specified for the translation of data addresses
(storage operands) and instruction addresses. The shadow page table is
used for the instruction addresses and the standard page table for the
data addresses.
The shadow page table is linked to the standard page table by a pointer
in page->lru.next of the struct page corresponding to the page that
contains the standard page table (since page->private is not really
private with the pte_lock and the page table pages are not in the LRU
list).
Depending on the software bits of a pte, it is either inserted into
both page tables or just into the standard (data) page table. Pages of
a vma that does not have the VM_EXEC bit set get mapped only in the
data address space. Any try to execute code on such a page will cause a
page translation exception. The standard reaction to this is a SIGSEGV
with two exceptions: the two system call opcodes 0x0a77 (sys_sigreturn)
and 0x0aad (sys_rt_sigreturn) are allowed. They are stored by the
kernel to the signal stack frame. Unfortunately, the signal return
mechanism cannot be modified to use an SA_RESTORER because the
exception unwinding code depends on the system call opcode stored
behind the signal stack frame.

This feature requires that user space is executed in secondary-space
mode and the kernel in home-space mode, which means that the addressing
modes need to be switched and that the noexec protection only works
for user space.
After switching the addressing modes, we cannot use the mvcp/mvcs
instructions anymore to copy between kernel and user space. A new
mvcos instruction has been added to the z9 EC/BC hardware which allows
to copy between arbitrary address spaces, but on older hardware the
page tables need to be walked manually.

Heiko Carstens f4eb07c17d [S390] Virtual memmap for s390.
Virtual memmap support for s390. Inspired by the ia64 implementation.

Unlike ia64 we need a mechanism which allows us to dynamically attach
shared memory regions.
These memory regions are accessed via the dcss device driver. dcss
implements the 'direct_access' operation, which requires struct pages
for every single shared page.
Therefore this implementation provides an interface to attach/detach
shared memory:

int add_shared_memory(unsigned long start, unsigned long size);
int remove_shared_memory(unsigned long start, unsigned long size);

The purpose of the add_shared_memory function is to add the given
memory range to the 1:1 mapping and to make sure that the
corresponding range in the vmemmap is backed with physical pages.
It also initialises the new struct pages.

remove_shared_memory in turn only invalidates the page table
entries in the 1:1 mapping. The page tables and the memory used for
struct pages in the vmemmap are currently not freed. They will be
reused when the next segment will be attached.
Given that the maximum size of a shared memory region is 2GB and
in addition all regions must reside below 2GB this is not too much of
a restriction, but there is room for improvement.

Heiko Carstens bcc8bcb1f0 [S390] revert add_active_range() usage patch.
Commit 7676bef9c1 breaks DCSS support on
s390. DCSS needs initialized struct pages to work. With the usage of
add_active_range() only the struct pages for physically present pages
are initialized.
This could be fixed if the DCSS driver would initiliaze the struct pages
itself, but this doesn't work too. This is because the mem_map array
does not include holes after the last present memory area and therefore
there is nothing that could be initialized.
To fix this and to avoid some dirty hacks revert this patch for now.
Will be added later when we move to a virtual mem_map.

Sukadev Bhattiprolu f400e198b2 [PATCH] pidspace: is_init()
This is an updated version of Eric Biederman's is_init() patch.
(http://lkml.org/lkml/2006/2/6/280). It applies cleanly to 2.6.18-rc3 and
replaces a few more instances of ->pid == 1 with is_init().

Further, is_init() checks pid and thus removes dependency on Eric's other
patches for now.

Eric's original description:

There are a lot of places in the kernel where we test for init
because we give it special properties. Most significantly init
must not die. This results in code all over the kernel test
	->pid == 1.

Introduce is_init to capture this case.

With multiple pid spaces for all of the cases affected we are
looking for only the first process on the system, not some other
process that has pid == 1.

Martin Schwidefsky d4b6899678 [PATCH] s390: remove pagex support
Remove pagex pseudo page fault code. It does not work together with the
system call speedup that makes the complete system call path enabled for
interrupts. To make pagex and the syscall speedup code work together we would
have to add code to the program check handler to do a critical section cleanup
like the asynchronous interrupt code. This would make program checks slower.
Not what we want.

Newer versions of z/VM have the improved pfault pseudo page fault interface.
This replaces the old pagex interface and does not have the problem. So its
better to just rip out the pagex code.

Hugh Dickins 872fec16d9 [PATCH] mm: init_mm without ptlock
First step in pushing down the page_table_lock. init_mm.page_table_lock has
been used throughout the architectures (usually for ioremap): not to serialize
kernel address space allocation (that's usually vmlist_lock), but because
pud_alloc,pmd_alloc,pte_alloc_kernel expect caller holds it.

Reverse that: don't lock or unlock init_mm.page_table_lock in any of the
architectures; instead rely on pud_alloc,pmd_alloc,pte_alloc_kernel to take
and drop it when allocating a new one, to check lest a racing task already
did. Similarly no page_table_lock in vmalloc's map_vm_area.

Some temporary ugliness in __pud_alloc and __pmd_alloc: since they also handle
user mms, which are converted only by a later patch, for now they have to lock
differently according to whether or not it's init_mm.

If sources get muddled, there's a danger that an arch source taking
init_mm.page_table_lock will be mixed with common source also taking it (or
neither take it). So break the rules and make another change, which should
break the build for such a mismatch: remove the redundant mm arg from
pte_alloc_kernel (ppc64 scrapped its distinct ioremap_mm in 2.6.13).

Exceptions: arm26 used pte_alloc_kernel on user mm, now pte_alloc_map; ia64
used pte_alloc_map on init_mm, now pte_alloc_kernel; parisc had bad args to
pmd_alloc and pte_alloc_kernel in unused USE_HPPA_IOREMAP code; ppc64
map_io_page forgot to unlock on failure; ppc mmu_mapin_ram and ppc64 im_free
took page_table_lock for no good reason.

Martin Schwidefsky b6d09449d5 [PATCH] s390: pfault interrupt race
There is a race in pfault_interrupt. That function gets called two times for
each pfault notification. Once with a subcode of 0 to indicate that a real
page is not available and once with a subcode of 0x80 to indicate that the
page is present again.

Since the two external interrupts can be delivered on two different cpus the
order in which the two calls are made is unpredictable. It is possible that
the subcode 0x80 interrupt is completed before the subcode 0x00 interrupt has
done the wake_up() call.

To avoid calling wake_up() on an already removed task structure proper task
structure reference counting is needed. Increase the reference counter in the
subcode 0x00 interrupt before setting pfault_wait to zero and return the
reference after the wake_up call.
