This adds device tree probing support for the PTP module
adjacent to the ethernet module. It is pretty straight
forward, all resources are in the device tree as they
come to the platform device.

The driver is being passed interrupts, then looking up the
same interrupts as GPIOs a second time to convert them into
interrupts and set properties on them.
This is pointless: the GPIO and irqchip APIs of a GPIO chip
are orthogonal. Just request the interrupts and be done
with it, drop reliance on any GPIO functions or definitions.
Use devres-managed functions and add a small devress quirk
to unregister the clock as well and we can rely on devres
to handle all the resources and cut down a bunch of
boilerplate in the process.

After the recent ixp4xx cleanups, the ptp driver has gained a
build failure in some configurations:

Avoid the last bit of hardcoded constants from platform headers
by turning the ptp driver bit into a platform driver and passing
the IRQ and MMIO address as resources.
This is a bit tricky:
- The interface between the two drivers is now the new
ixp46x_ptp_find() function, replacing the global
ixp46x_phc_index variable. The call is done as late
as possible, in hwtstamp_set(), to ensure that the
ptp device is fully probed.
- As the ptp driver is now called by the network driver, the
link dependency is reversed, which in turn requires a small
Makefile hack
- The GPIO number is still left hardcoded. This is clearly not
great, but it can be addressed later. Note that commit 98ac0cc270
("ARM: ixp4xx: Convert to MULTI_IRQ_HANDLER") changed the
IRQ number to something meaningless. Passing the correct IRQ
in a resource fixes this.
- When the PTP driver is disabled, ethtool .get_ts_info()
now correctly lists only software timestamping regardless
of the hardware.

Most users of ndo_do_ioctl are ethernet drivers that implement
the MII commands SIOCGMIIPHY/SIOCGMIIREG/SIOCSMIIREG, or hardware
timestamping with SIOCSHWTSTAMP/SIOCGHWTSTAMP.
Separate these from the few drivers that use ndo_do_ioctl to
implement SIOCBOND, SIOCBR and SIOCWANDEV commands.
This is a purely cosmetic change intended to help readers find
their way through the implementation.

Generic drivers are unable to use the feature macros from mach/cpu.h

In order to create a proper PCI driver for the IXP4xx
we need to make the old PCI driver and its reliance
on <mach/io.h> optional.
Create a new Kconfig symbol for the legacy PCI driver
IXP4XX_PCI_LEGACY and only activate NEED_MACH_IO_H
for this driver.
A few files need to be adjusted to explicitly include
the <mach/hardware.h> and <mach/cpu.h> headers that
they previously obtained implicitly using <linux/io.h>
that would include <mach/io.h> and in turn include
these two headers.
This breaks our reliance on the old PCI and indirect
PCI support so we can reimplement a proper purely
DT-based driver in the PCI subsystem.

This adds device tree probing to the IXP4xx ethernet
driver.
Add a platform data bool to tell us whether to
register an MDIO bus for the device or not, as well
as the corresponding NPE.
We need to drop the memory region request as part of
this since the OF core will request the memory for the
device.

The former fix only papered over the actual problem: the
ethernet core expects the netdev .dev member to have the
proper DMA masks set, or there will be BUG_ON() triggered
in kernel/dma/mapping.c.
Fix this by simply copying dma_mask and dma_mask_coherent
from the parent device.

Use the netdevice struct device .parent field when calling
dma_pool_create(): the .dma_coherent_mask and .dma_mask
pertains to the bus device on the hardware (platform)
bus in this case, not the struct device inside the network
device. This makes the pool allocation work.

In order to probe this ethernet interface from the device tree
all physical MMIO regions must be passed as resources. Begin
this rewrite by first passing the port base address as a
resource for all platforms using this driver, remap it in
the driver and avoid using any reference of the statically
mapped virtual address in the driver.

The IXP4xx driver was initializing the MDIO bus before even
probing, in the callbacks supposed to be used for setting up
the module itself, and with the side effect of trying to
register the MDIO bus as soon as this module was loaded or
compiled into the kernel whether the device was discovered
or not.
This does not work with multiplatform environments.
To get rid of this: set up the MDIO bus from the probe()
callback and remove it in the remove() callback. Rename
the probe() and remove() calls to reflect the most common
conventions.
Since there is a bit of checking for the ethernet feature
to be present in the MDIO registering function, making the
whole module not even be registered if we can't find an
MDIO bus, we need something similar: register the MDIO
bus when the corresponding ethernet is probed, and
return -EPROBE_DEFER on the other interfaces until this
happens. If no MDIO bus is present on any of the
registered interfaces we will eventually bail out.
None of the platforms I've seen has e.g. MDIO on EthB
and only uses EthC, there is always a Ethernet hardware
on the NPE (B, C) that has the MDIO bus, we just might
have to wait for it.

The ixp46x ptp driver has a somewhat unusual setup, where the ptp
driver and the ethernet driver are in different directories but
access the same registers that are defined a platform specific
header file.
Moving everything into drivers/net/ makes it look more like most
other ptp drivers and allows compile-testing this driver on
other targets.

This adds a minimal support for booting IXP4xx systems
from device tree.
We have to add hacks to the QMGR, NPE and notably also
ethernet and watchdog drivers so that they don't crash
the platform: these drivers are unconditionally starting
to grab regions of statically remapped IO space with no
concern of the device model or other platforms.
We will go in and properly fix these drivers as we go
along but for now this hack gets us to a place where we
can start working on proper device tree support for these
platforms.

With centralized MTU checking, there's nothing productive done by
eth_change_mtu that isn't already done in dev_set_mtu, so mark it as
deprecated and remove all usage of it in the kernel. All callers have been
audited for calls to alloc_etherdev* or ether_setup directly, which means
they all have a valid dev->min_mtu and dev->max_mtu. Now eth_change_mtu
prints out a netdev_warn about being deprecated, for the benefit of
out-of-tree drivers that might be utilizing it.
Of note, dvb_net.c actually had dev->mtu = 4096, while using
eth_change_mtu, meaning that if you ever tried changing it's mtu, you
couldn't set it above 1500 anymore. It's now getting dev->max_mtu also set
to 4096 to remedy that.

This howto made sense in the 1990s when users had to manually configure
ISA cards with jumpers or vendor utilities, but with the implementation
of PCI it became increasingly less and less relevant, to the point where
it has been well over a decade since I last updated it. And there is
no value in anyone else taking over updating it either.
However the references to it continue to spread as boiler plate text
from one Kconfig file into the next. We are not doing end users any
favours by pointing them at this old document, so lets kill it with
fire, once and for all, to hopefully stop any further spread.
No code is changed in this commit, just Kconfig help text.

If both promisc and allmulti are set, promisc should trump allmulti and
disable the MAC filter; otherwise, the interface is not really promisc.
Previously, this code checked IFF_ALLMULTI prior to and without regard for
IFF_PROMISC; if both were set, only multicast and direct unicast traffic
would make it through the filter.

This commit fixes a build error reported by Fengguang, that is
triggered when CONFIG_NETWORK_PHY_TIMESTAMPING is not set:
ERROR: "ptp_classify_raw" [drivers/net/ethernet/oki-semi/pch_gbe/pch_gbe.ko] undefined!
The fix is to introduce its own file for the PTP BPF classifier,
so that PTP_1588_CLOCK and/or NETWORK_PHY_TIMESTAMPING can select
it independently from each other. IXP4xx driver on ARM needs to
select it as well since it does not seem to select PTP_1588_CLOCK
or similar that would pull it in automatically.
This also allows for hiding all of the internals of the BPF PTP
program inside that file, and only exporting relevant API bits
to drivers.
This patch also adds a kdoc documentation of ptp_classify_raw()
API to make it clear that it can return PTP_CLASS_* defines. Also,
the BPF program has been translated into bpf_asm code, so that it
can be more easily read and altered (extensively documented in [1]).
In the kernel tree under tools/net/ we have bpf_asm and bpf_dbg
tools, so the commented program can simply be translated via
`./bpf_asm -c prog` where prog is a file that contains the
commented code. This makes it easily readable/verifiable and when
there's a need to change something, jump offsets etc do not need
to be replaced manually which can be very error prone. Instead,
a newly translated version via bpf_asm can simply replace the old
code. I have checked opcode diffs before/after and it's the very
same filter.

There are currently pch_gbe, cpts, and ixp4xx_eth drivers that open-code
and reimplement a BPF classifier for the PTP protocol. Since all of them
effectively do the very same thing and load the very same PTP/BPF filter,
we can just consolidate that code by introducing ptp_classify_raw() in
the time-stamping core framework which can be used in drivers.
As drivers get initialized after bootstrapping the core networking
subsystem, they can make use of ptp_insns wrapped through
ptp_classify_raw(), which allows to simplify and remove PTP classifier
setup code in drivers.

hwtstamp_ioctl() should validate all fields of hwtstamp_config
before making any changes. Currently it sets the TX configuration
before validating the rx_filter field.
Untested as I don't have a cross-compiler to hand.

The driver core clears the driver data to NULL after device_release
or on probe failure, since commit 0998d06310
(device-core: Ensure drvdata = NULL when no driver is bound).
Thus, it is not needed to manually clear the device driver data to NULL.

The flags argument of the phy_{attach,connect,connect_direct} functions
is then used to assign a struct phy_device dev_flags with its value.
All callers but the tg3 driver pass the flag 0, which results in the
underlying PHY drivers in drivers/net/phy/ not being able to actually
use any of the flags they would set in dev_flags. This patch gets rid of
the flags argument, and passes phydev->dev_flags to the internal PHY
library call phy_attach_direct() such that drivers which actually modify
a phy device dev_flags get the value preserved for use by the underlying
phy driver.
