Daniel Wagner 3e3f5a8a0f scsi: csiostor: Fix completion usage
The (re)initializing of the completion object should be done before we
trigger the transfer. Doing this after triggering the hardware opens up
a race window. Without the timeout we would problaly even deadlock. Use
also reinit_completion because we initalize the whole data structure in
csio_scscim_init().

There is only one waiter for the completion, therefore there is no need
to use complete_all(). Let's make that clear by using complete() instead
of complete_all().

The usage pattern of the completion is:

waiter context                          waker context

csio_eh_abort_handler()
  reinit_completion()
  wait_for_completion_timeout()

                                        csio_scsi_err_handler()
                                          complete()

Christoph Hellwig 64d513ac31 scsi: use host wide tags by default
This patch changes the !blk-mq path to the same defaults as the blk-mq
I/O path by always enabling block tagging, and always using host wide
tags.  We've had blk-mq available for a few releases so bugs with
this mode should have been ironed out, and this ensures we get better
coverage of over tagging setup over different configs.

Christoph Hellwig ea6bb33e7f fc: FCP_PTA_SIMPLE is 0
Not need to have an if/else to either assign FCP_PTA_SIMPLE or 0
to a variable.

Btw, it seems we really should factor generating a fcp cmnd from
a scsi_cmnd into a common helper.

Christoph Hellwig db5ed4dfd5 scsi: drop reason argument from ->change_queue_depth
Drop the now unused reason argument from the ->change_queue_depth method.
Also add a return value to scsi_adjust_queue_depth, and rename it to
scsi_change_queue_depth now that it can be used as the default
->change_queue_depth implementation.

Anish Bhatt d7990b0c34 cxgb4i/cxgb4 : Refactor macros to conform to uniform standards
Refactored all macros used in cxgb4i as part of previously started cxgb4 macro
names cleanup. Makes them more uniform and avoids namespace collision.
Minor changes in other drivers where required as some of these macros are used
 by multiple drivers, affected drivers are iw_cxgb4, cxgb4(vf) & csiostor

Christoph Hellwig c8b09f6fb6 scsi: don't set tagging state from scsi_adjust_queue_depth
Remove the tagged argument from scsi_adjust_queue_depth, and just let it
handle the queue depth.  For most drivers those two are fairly separate,
given that most modern drivers don't care about the SCSI "tagged" status
of a command at all, and many old drivers allow queuing of multiple
untagged commands in the driver.

Instead we start out with the ->simple_tags flag set before calling
->slave_configure, which is how all drivers actually looking at
->simple_tags except for one worke anyway.  The one other case looks
broken, but I've kept the behavior as-is for now.

Except for that we only change ->simple_tags from the ->change_queue_type,
and when rejecting a tag message in a single driver, so keeping this
churn out of scsi_adjust_queue_depth is a clear win.

Now that the usage of scsi_adjust_queue_depth is more obvious we can
also remove all the trivial instances in ->slave_alloc or ->slave_configure
that just set it to the cmd_per_lun default.

Christoph Hellwig 2ecb204d07 scsi: always assign block layer tags if enabled
Allow a driver to ask for block layer tags by setting .use_blk_tags in the
host template, in which case it will always see a valid value in
request->tag, similar to the behavior when using blk-mq.  This means even
SCSI "untagged" commands will now have a tag, which is especially useful
when using a host-wide tag map.

Christoph Hellwig 5066863337 scsi: remove abuses of scsi_populate_tag
Unless we want to build a SPI tag message we should just check SCMD_TAGGED
instead of reverse engineering a tag type through the use of
scsi_populate_tag_msg.

Also rename the function to spi_populate_tag_msg, make it behave like the
other spi message helpers, and move it to the spi transport class.

Hariprasad Shenai e2ac962895 cxgb4: Cleanup macros so they follow the same style and look consistent, part 2
Various patches have ended up changing the style of the symbolic macros/register
defines to different style.

As a result, the current kernel.org files are a mix of different macro styles.
Since this macro/register defines is used by different drivers a
few patch series have ended up adding duplicate macro/register define entries
with different styles. This makes these register define/macro files a complete
mess and we want to make them clean and consistent. This patch cleans up a part
of it.

Hannes Reinecke 9cb78c16f5 scsi: use 64-bit LUNs
The SCSI standard defines 64-bit values for LUNs, and large arrays
employing large or hierarchical LUN numbers become more and more
common.

So update the linux SCSI stack to use 64-bit LUN numbers.

Naresh Kumar Inna 49c1241384 [SCSI] csiostor: Retain default adapter configuration in absence of config file.
- Retain firmware defined configuration settings in the absence of
user-provided configuration by eliminating the global RSS and the
PF/VF capabilities mailbox commands.
- Remove S_IRUGO from sysfs parameters that don't have 'show'
functionality.
