Replace the open coded offset calculation with the proper helper.
This is an ABI change in that the -1 for a misaligned partition is
properly propagated, which can be considered a bug fix and matches
what is done on the whole device.

Bios queued into BFQ IO scheduler can be associated with a cgroup that
was already offlined. This may then cause insertion of this bfq_group
into a service tree. But this bfq_group will get freed as soon as last
bio associated with it is completed leading to use after free issues for
service tree users. Fix the problem by making sure we always operate on
online bfq_group. If the bfq_group associated with the bio is not
online, we pick the first online parent.

BFQ usage of __bio_blkcg() is a relict from the past. Furthermore if bio
would not be associated with any blkcg, the usage of __bio_blkcg() in
BFQ is prone to races with the task being migrated between cgroups as
__bio_blkcg() calls at different places could return different blkcgs.
Convert BFQ to the new situation where bio->bi_blkg is initialized in
bio_set_dev() and thus practically always valid. This allows us to save
blkcg_gq lookup and noticeably simplify the code.

Track whether bfq_group is still online. We cannot rely on
blkcg_gq->online because that gets cleared only after all policies are
offlined and we need something that gets updated already under
bfqd->lock when we are cleaning up our bfq_group to be able to guarantee
that when we see online bfq_group, it will stay online while we are
holding bfqd->lock lock.

We call bfq_init_rq() from request merging functions where requests we
get should have already gone through bfq_init_rq() during insert and
anyway we want to do anything only if the request is already tracked by
BFQ. So replace calls to bfq_init_rq() with RQ_BFQQ() instead to simply
skip requests untracked by BFQ. We move bfq_init_rq() call in
bfq_insert_request() a bit earlier to cover request merging and thus
can transfer FIFO position in case of a merge.

In bfq_insert_request() we unlock bfqd->lock only to call
trace_block_rq_insert() and then lock bfqd->lock again. This is really
pointless since tracing is disabled if we really care about performance
and even if the tracepoint is enabled, it is a quick call.

When the process is migrated to a different cgroup (or in case of
writeback just starts submitting bios associated with a different
cgroup) bfq_merge_bio() can operate with stale cgroup information in
bic. Thus the bio can be merged to a request from a different cgroup or
it can result in merging of bfqqs for different cgroups or bfqqs of
already dead cgroups and causing possible use-after-free issues. Fix the
problem by updating cgroup information in bfq_merge_bio().

When bfqq is shared by multiple processes it can happen that one of the
processes gets moved to a different cgroup (or just starts submitting IO
for different cgroup). In case that happens we need to split the merged
bfqq as otherwise we will have IO for multiple cgroups in one bfqq and
we will just account IO time to wrong entities etc.
Similarly if the bfqq is scheduled to merge with another bfqq but the
merge didn't happen yet, cancel the merge as it need not be valid
anymore.

It can happen that the parent of a bfqq changes between the moment we
decide two queues are worth to merge (and set bic->stable_merge_bfqq)
and the moment bfq_setup_merge() is called. This can happen e.g. because
the process submitted IO for a different cgroup and thus bfqq got
reparented. It can even happen that the bfqq we are merging with has
parent cgroup that is already offline and going to be destroyed in which
case the merge can lead to use-after-free issues such as:

bfq_setup_cooperator() can mark bic as stably merged even though it
decides to not merge its bfqqs (when bfq_setup_merge() returns NULL).
Make sure to mark bic as stably merged only if we are really going to
merge bfqqs.

Remove the magic autofree semantics and require the callers to explicitly
call bio_init to initialize the bio.
This allows bio_free to catch accidental bio_put calls on bio_init()ed

Refine per-cpu bio alloc cache interfaces so that DM and other block
drivers can properly create and use the cache:
DM uses bioset_init_from_src() to do its final bioset initialization,
so must update bioset_init_from_src() to set BIOSET_PERCPU_CACHE if
%src bioset has a cache.
Also move bio_clear_polled() to include/linux/bio.h to allow users
outside of block core.

Replace the BIO_PERCPU_CACHE bio-internal flag with a REQ_ALLOC_CACHE
one that can be passed to bio_alloc / bio_alloc_bioset, and implement
the percpu cache allocation logic in a helper called from
bio_alloc_bioset. This allows any bio_alloc_bioset user to use the
percpu caches instead of having the functionality tied to struct kiocb.

kernel ulong and compat_ulong_t may not be same width. Use type directly
to eliminate mismatches.

This set of changes removes tracehook.h, moves modification of all of
the ptrace fields inside of siglock to remove races, adds a missing
permission check to ptrace.c
The removal of tracehook.h is quite significant as it has been a major
source of confusion in recent years. Much of that confusion was
around task_work and TIF_NOTIFY_SIGNAL (which I have now decoupled
making the semantics clearer).
For people who don't know tracehook.h is a vestiage of an attempt to
implement uprobes like functionality that was never fully merged, and
was later superseeded by uprobes when uprobes was merged. For many
years now we have been removing what tracehook functionaly a little
bit at a time. To the point where now anything left in tracehook.h is
some weird strange thing that is difficult to understand.

Throttled bios can't be issued after del_gendisk() is done, thus
it's better to cancel them immediately rather than waiting for
throttle is done.
For example, if user thread is throttled with low bps while it's
issuing large io, and the device is deleted. The user thread will
wait for a long time for io to return.

In the whole lifetime of blkcg_gq instance, ->q will be referred, such
as, ->pd_free_fn() is called in blkg_free, and throtl_pd_free() still
may touch the request queue via &tg->service_queue.pending_timer which
is handled by throtl_pending_timer_fn(), so it is reasonable to grab
request queue's refcnt by blkcg_gq instance.
Previously blkcg_exit_queue() is called from blk_release_queue, and it
is hard to avoid the use-after-free. But recently commit 1059699f87 ("block:
move blkcg initialization/destroy into disk allocation/release handler")
is merged to for-5.18/block, it becomes simple to fix the issue by simply
grabbing request queue's refcnt.

In throtl_pending_timer_fn(), request queue is retrieved from throttle
data. And tg's pending timer is deleted synchronously when releasing the
associated blkg, at that time, throttle data may have been freed since
commit 1059699f87 ("block: move blkcg initialization/destroy into disk
allocation/release handler") moves freeing q->td to disk_release() from
blk_release_queue(). So use-after-free on q->td in throtl_pending_timer_fn
can be triggered.
Fixes the issue by:
- do nothing in case that disk is released, when there isn't any bio to
dispatch
- retrieve request queue from blkg instead of throttle data for
non top-level pending timer.

When IO requests are made continuously and the target block device
handles requests faster than request arrival, the request dispatch loop
keeps on repeating to dispatch the arriving requests very long time,
more than a minute. Since the loop runs as a workqueue worker task, the
very long loop duration triggers workqueue watchdog timeout and BUG [1].
To avoid the very long loop duration, break the loop periodically. When
opportunity to dispatch requests still exists, check need_resched(). If
need_resched() returns true, the dispatch loop already consumed its time
slice, then reschedule the dispatch work and break the loop. With heavy
IO load, need_resched() does not return true for 20~30 seconds. To cover
such case, check time spent in the dispatch loop with jiffies. If more
than 1 second is spent, reschedule the dispatch work and break the loop.

blk-iocost and iolatency are cgroup aware rq-qos policies but they didn't
disable merges across different cgroups. This obviously can lead to
accounting and control errors but more importantly to priority inversions -
e.g. an IO which belongs to a higher priority cgroup or IO class may end up
getting throttled incorrectly because it gets merged to an IO issued from a
low priority cgroup.
Fix it by adding blk_cgroup_mergeable() which is called from merge paths and
rejects cross-cgroup and cross-issue_as_root merges.

a647a524a4 ("block: don't call rq_qos_ops->done_bio if the bio isn't
tracked") made bio_endio() skip rq_qos_done_bio() if BIO_TRACKED is not set.
While this fixed a potential oops, it also broke blk-iocost by skipping the
done_bio callback for merged bios.
Before, whether a bio goes through rq_qos_throttle() or rq_qos_merge(),
rq_qos_done_bio() would be called on the bio on completion with BIO_TRACKED
distinguishing the former from the latter. rq_qos_done_bio() is not called
for bios which wenth through rq_qos_merge(). This royally confuses
blk-iocost as the merged bios never finish and are considered perpetually
in-flight.
One reliably reproducible failure mode is an intermediate cgroup geting
stuck active preventing its children from being activated due to the
leaf-only rule, leading to loss of control. The following is from
resctl-bench protection scenario which emulates isolating a web server like
workload from a memory bomb run on an iocost configuration which should
yield a reasonable level of protection.

without any IO control.
Fix it by introducing a new flag BIO_QOS_MERGED to mark merged bios and
calling rq_qos_done_bio() on them too. For consistency and clarity, rename
BIO_TRACKED to BIO_QOS_THROTTLED. The flag checks are moved into
rq_qos_done_bio() so that it's next to the code paths that set the flags.
With the patch applied, the above same benchmark shows:

blkcg_init_queue() may add rq qos structures to request queue, previously
blk_cleanup_queue() calls rq_qos_exit() to release them, but commit
8e141f9eb8 ("block: drain file system I/O on del_gendisk")
moves rq_qos_exit() into del_gendisk(), so memory leak is caused
because queues may not have disk, such as un-present scsi luns, nvme
admin queue, ...
Fixes the issue by adding rq_qos_exit() to blk_cleanup_queue() back.
BTW, v5.18 won't need this patch any more since we move
blkcg_init_queue()/blkcg_exit_queue() into disk allocation/release
handler, and patches have been in for-5.18/block.

We used to sort the plug list if we had multiple queues before dispatching
requests to the IO scheduler. This usually isn't needed, but for certain
workloads that interleave requests to disks, it's a less efficient to
process the plug list one-by-one if everything is interleaved.
Don't sort the list, but skip through it and flush out entries that have
the same target at the same time.

Song reports that a RAID rebuild workload runs much slower recently,
and it is seeing a lot less merging than it did previously. The reason
is that a previous commit reduced the amount of work we do for plug
merging. RAID rebuild interleaves requests between disks, so a last-entry
check in plug merging always misses a merge opportunity since we always
find a different disk than what we are looking for.
Modify the logic such that it's still a one-hit cache, but ensure that
we check enough to find the right target before giving up.

There can't be file system I/O in disk_release(), so move the call to
blk_exit_queue() there, preparing to have the teardown of file system I/O
only functionality in one place, when the gendisk that is needed for it
is torn down.
We still need to freeze queue here since the request is freed after the
bio is completed and passthrough request rely on scheduler tags as well.
The disk can be released before or after queue is cleaned up, and we have
to free the scheduler request pool before blk_cleanup_queue returns,
while the static request pool has to be freed before exiting the
I/O scheduler.

After blk_cleanup_queue() returns, disk may not be released yet, so
probably bio may still be submitted and ->q_usage_counter may be
touched, so far this way seems safe, but not good from API's viewpoint.
Move the release q_usage_counter into blk_queue_release().

The queue's top debugfs dir is removed from blk_release_queue(), so all
hctx's debugfs dirs are removed from there. Given blk_mq_exit_queue()
is only called from blk_cleanup_queue(), it isn't necessary to remove
hctx debugfs from blk_mq_exit_queue().
So remove it from blk_mq_exit_queue().

blkcg works on FS bio level, so it is reasonable to make both blkcg and
gendisk sharing same lifetime. Meantime there won't be any FS IO when
releasing disk, so safe to move blkcg initialization/destroy into disk
allocation/release handler
Long term, we can move blkcg into gendisk completely.

First code becomes more clean by switching to xarray from plain array.
Second use-after-free on q->queue_hw_ctx can be fixed because
queue_for_each_hw_ctx() may be run when updating nr_hw_queues is
in-progress. With this patch, q->hctx_table is defined as xarray, and
this structure will share same lifetime with request queue, so
queue_for_each_hw_ctx() can use q->hctx_table to lookup hctx reliably.

It is inevitable to cause use-after-free on q->queue_hw_ctx between
queue_for_each_hw_ctx() and blk_mq_update_nr_hw_queues(). And converting
to xarray can fix the uaf, meantime code gets cleaner.
Prepare for converting q->queue_hctx_ctx into xarray, one thing is that
xa_for_each() can only accept 'unsigned long' as index, so changes type
of hctx index of queue_for_each_hw_ctx() into 'unsigned long'.

The current code always uses default queue map and hw queue index
for figuring out the numa node for hw queue, this way isn't correct
because blk-mq supports three queue maps, and the correct queue map
should be used for the specified hw queue.

A crash [1] happened to be triggered in conjunction with commit
2d52c58b9c ("block, bfq: honor already-setup queue merges"). The
latter was then reverted by commit ebc69e897e ("Revert "block, bfq:
honor already-setup queue merges""). Yet, the reverted commit was not
the one introducing the bug. In fact, it actually triggered a UAF
introduced by a different commit, and now fixed by commit d29bd41428
("block, bfq: reset last_bfqq_created on group change").
So, there is no point in keeping commit 2d52c58b9c ("block, bfq:
honor already-setup queue merges") out. This commit restores it.

Commit 9d497e2941 ("block: don't protect submit_bio_checks by
q_usage_counter") moved blk_mq_attempt_bio_merge and rq_qos_throttle
calls out of q_usage_counter protection. However, these functions require
q_usage_counter protection. The blk_mq_attempt_bio_merge call without
the protection resulted in blktests block/005 failure with KASAN null-
ptr-deref or use-after-free at bio merge. The rq_qos_throttle call
without the protection caused kernel hang at qos throttle.
To fix the failures, move the blk_mq_attempt_bio_merge and
rq_qos_throttle calls back to q_usage_counter protection.

The NVMe specification defines new data integrity formats beyond the
t10 tuple. Add support for the specification defined CRC64 formats,
assuming the reference tag does not need to be split with the "storage
tag".

In bfq_dispatch_requestfunction, it may have function call:

In this function call, in_serv_queue has beed expired and meet the
conditions to free. In the function bfq_dispatch_request, the address
of in_serv_queue pointing to has been released. For getting the value
of idle_timer_disabled, it will get flags value from the address which
in_serv_queue pointing to, then the problem of use-after-free happens;
Fix the problem by check in_serv_queue == bfqd->in_service_queue, to
get the value of idle_timer_disabled if in_serve_queue is equel to
bfqd->in_service_queue. If the space of in_serv_queue pointing has
been released, this judge will aviod use-after-free problem.
And if in_serv_queue may be expired or finished, the idle_timer_disabled
will be false which would not give effects to bfq_update_dispatch_stats.

Add sysfs files that expose the inline encryption capabilities of
request queues:
/sys/block/$disk/queue/crypto/max_dun_bits
/sys/block/$disk/queue/crypto/modes/$mode
/sys/block/$disk/queue/crypto/num_keyslots
Userspace can use these new files to decide what encryption settings to
use, or whether to use inline encryption at all. This also brings the
crypto capabilities in line with the other queue properties, which are
already discoverable via the queue directory in sysfs.
Design notes:
- Place the new files in a new subdirectory "crypto" to group them
together and to avoid complicating the main "queue" directory. This
also makes it possible to replace "crypto" with a symlink later if
we ever make the blk_crypto_profiles into real kobjects (see below).
- It was necessary to define a new kobject that corresponds to the
crypto subdirectory. For now, this kobject just contains a pointer
to the blk_crypto_profile. Note that multiple queues (and hence
multiple such kobjects) may refer to the same blk_crypto_profile.
An alternative design would more closely match the current kernel
data structures: the blk_crypto_profile could be a kobject itself,
located directly under the host controller device's kobject, while
/sys/block/$disk/queue/crypto would be a symlink to it.
I decided not to do that for now because it would require a lot more
changes, such as no longer embedding blk_crypto_profile in other
structures, and also because I'm not sure we can rule out moving the
crypto capabilities into 'struct queue_limits' in the future. (Even
if multiple queues share the same crypto engine, maybe the supported
data unit sizes could differ due to other queue properties.) It
would also still be possible to switch to that design later without
breaking userspace, by replacing the directory with a symlink.
- Use "max_dun_bits" instead of "max_dun_bytes". Currently, the
kernel internally stores this value in bytes, but that's an
implementation detail. It probably makes more sense to talk about
this value in bits, and choosing bits is more future-proof.
- "modes" is a sub-subdirectory, since there may be multiple supported
crypto modes, sysfs is supposed to have one value per file, and it
makes sense to group all the mode files together.
- Each mode had to be named. The crypto API names like "xts(aes)" are
not appropriate because they don't specify the key size. Therefore,
I assigned new names. The exact names chosen are arbitrary, but
they happen to match the names used in log messages in fs/crypto/.
- The "num_keyslots" file is a bit different from the others in that
it is only useful to know for performance reasons. However, it's
included as it can still be useful. For example, a user might not
want to use inline encryption if there aren't very many keyslots.

Make elv_unregister_queue() a no-op if q->elevator is NULL or is not
registered.
This simplifies the existing callers, as well as the future caller in

As Luis reported, losetup currently doesn't properly create the loop
device without this if the device node already exists because old
scripts created it manually. So default to y for now and remove the
aggressive removal schedule.

When the inflight IOs are slow and no new IOs are issued, we expect
iostat could manifest the IO hang problem. However after
commit 5b18b5a737 ("block: delete part_round_stats and switch to less
precise counting"), io_tick and time_in_queue will not be updated until
the end of IO, and the avgqu-sz and %util columns of iostat will be zero.
Because it has using stat.nsecs accumulation to express time_in_queue
which is not suitable to change, and may %util will express the status
better when io hang occur. To fix io_ticks, we use update_io_ticks and
inflight to update io_ticks when diskstats_show and part_stat_show
been called.

Now that we disable wbt by set WBT_STATE_OFF_DEFAULT in
wbt_disable_default() when switch elevator to bfq. And when
we remove scsi device, wbt will be enabled by wbt_enable_default.

Various block drivers call blk_set_queue_dying to mark a disk as dead due
to surprise removal events, but since commit 8e141f9eb8 that doesn't
work given that the GD_DEAD flag needs to be set to stop I/O.
Replace the driver calls to blk_set_queue_dying with a new (and properly
documented) blk_mark_disk_dead API, and fold blk_set_queue_dying into the
only remaining caller.

When blk_mq_delay_run_hw_queues sets an hctx to run in the future, it can
reset the delay length for an already pending delayed work run_work. This
creates a scenario where multiple hctx may have their queues set to run,
but if one runs first and finds nothing to do, it can reset the delay of
another hctx and stall the other hctx's ability to run requests.
To avoid this I/O stall when an hctx's run_work is already pending,
leave it untouched to run at its current designated time rather than
extending its delay. The work will still run which keeps closed the race
calling blk_mq_delay_run_hw_queues is needed for while also avoiding the
I/O stall.

Add a method to notify the driver that the gendisk is about to be freed.
This allows drivers to tie the lifetime of their private data to that of
the gendisk and thus deal with device removal races without expensive
synchronization and boilerplate code.
A new flag is added so that ->free_disk is only called after a successful
call to add_disk, which significantly simplifies the error handling path
during probing.

We need to throttle split bio in case of IOPS limit even though the
split bio has been marked as BIO_THROTTLED since block layer
accounts split bio actually.
If only throughput throttle is setup, no need to throttle any more
if BIO_THROTTLED is set since we have accounted & considered the
whole bio bytes already.
Add one flag of THROTL_TG_HAS_IOPS_LIMIT for serving this purpose.

Commit 111be88398 ("block-throttle: avoid double charge") marks bio as
BIO_THROTTLED unconditionally if __blk_throtl_bio() is called on this bio,
then this bio won't be called into __blk_throtl_bio() any more. This way
is to avoid double charge in case of bio splitting. It is reasonable for
read/write throughput limit, but not reasonable for IOPS limit because
block layer provides io accounting against split bio.
Chunguang Xu has already observed this issue and fixed it in commit
4f1e9630af ("blk-throtl: optimize IOPS throttle for large IO scenarios").
However, that patch only covers bio splitting in __blk_queue_split(), and
we have other kind of bio splitting, such as bio_split() &
submit_bio_noacct() and other ways.
This patch tries to fix the issue in one generic way by always charging
the bio for iops limit in blk_throtl_bio(). This way is reasonable:
re-submission & fast-cloned bio is charged if it is submitted to same
disk/queue, and BIO_THROTTLED will be cleared if bio->bi_bdev is changed.
This new approach can get much more smooth/stable iops limit compared with
commit 4f1e9630af ("blk-throtl: optimize IOPS throttle for large IO
scenarios") since that commit can't throttle current split bios actually.
Also this way won't cause new double bio iops charge in
blk_throtl_dispatch_work_fn() in which blk_throtl_bio() won't be called
any more.

The code to stack blk-mq drivers is only used by dm-multipath, and
will preferably stay that way. Make it optional and only selected
by device mapper, so that the buildbots more easily catch abuses
like the one that slipped in in the ufs driver in the last merged
window. Another positive side effects is that kernel builds without
device mapper shrink a little bit as well.

Currently, rasdaemon uses the existing tracepoint block_rq_complete
and filters out non-error cases in order to capture block disk errors.
But there are a few problems with this approach:
1. Even kernel trace filter could do the filtering work, there is
still some overhead after we enable this tracepoint.
2. The filter is merely based on errno, which does not align with kernel
logic to check the errors for print_req_error().
3. block_rq_complete only provides dev major and minor to identify
the block device, it is not convenient to use in user-space.
So introduce a new tracepoint block_rq_error just for the error case.
With this patch, rasdaemon could switch to block_rq_error.
Since the new tracepoint has the similar implementation with
block_rq_complete, so move the existing code from TRACE_EVENT
block_rq_complete() into new event class block_rq_completion(). Then add
event for block_rq_complete and block_rq_err respectively from the newly
created event class per the suggestion from Chaitanya Kulkarni.

Since __sbitmap_queue_get_shallow() was introduced in commit c05e667337
("sbitmap: add sbitmap_get_shallow() operation"), it has not been used.
Delete __sbitmap_queue_get_shallow() and rename public
__sbitmap_queue_get_shallow() -> sbitmap_queue_get_shallow() as it is odd
to have public __foo but no foo at all.

__bio_clone_fast should also clone integrity and crypto data, as a clone
without those is incomplete. Right now the only caller that can actually
support crypto and integrity data (dm) does it manually for the one
callchain that supports these, but we better do it properly in the core.
Note that all callers except for the above mentioned one also don't need
to handle failure at all, given that the integrity and crypto clones are
based on mempool allocations that won't fail for sleeping allocations.

Currently, drivers reports BLK_STS_IOERR for devices that are not full
online or being removed. This behavior could cause confusion for users,
as they are not really I/O errors from the device.
Solve this issue with a new state BLK_STS_OFFLINE, which reports "device
offline error" in dmesg instead of "I/O error".
EIO is intentionally kept to not change user visible return value.

Commit 309a62fa3a ("bio-integrity: bio_integrity_advance must update
integrity seed") added code to update the integrity seed value when
advancing a bio. However, it failed to take into account that the
integrity interval might be larger than the 512-byte block layer
sector size. This broke bio splitting on PI devices with 4KB logical
blocks.
The seed value should be advanced by bio_integrity_intervals() and not
the number of sectors.

bio_start_io_acct_time() interface is like bio_start_io_acct() that
allows start_time to be passed in. This gives drivers the ability to
defer starting accounting until after IO is issued (but possibily not

If blk_mq_request_issue_directly() failed from
blk_insert_cloned_request(), the request will be accounted start.
Currently, blk_insert_cloned_request() is only called by dm, and such
request won't be accounted done by dm.
In normal path, io will be accounted start from blk_mq_bio_to_request(),
when the request is allocated, and such io will be accounted done from
__blk_mq_end_request_acct() whether it succeeded or failed. Thus add
blk_account_io_done() to fix the problem.

kobject_init_and_add() takes reference even when it fails.

bio_truncate() clears the buffer outside of last block of bdev, however
current bio_truncate() is using the wrong offset of page. So it can
return the uninitialized data.
This happened when both of truncated/corrupted FS and userspace (via
bdev) are trying to read the last of bdev.

bio_clone_fast() sets the cloned bio to have the same ->bi_bdev as the
source bio. This means that when request-based dm called setup_clone(),
the cloned bio had its ->bi_bdev pointing to the dm device. After Commit
0b6e522cdc ("blk-mq: use ->bi_bdev for I/O accounting")
__blk_account_io_start() started using the request's ->bio->bi_bdev for
I/O accounting, if it was set. This caused IO going to the underlying
devices to use the dm device for their I/O accounting.
Set up the proper ->bi_bdev in blk_rq_prep_clone based on the whole
device bdev for the queue the request is cloned onto.

(which means start == 0). This patch replaces 'next' with 'first' where
things look trivial.

In case of shared tags, there might be more than one hctx which
allocates from the same tags, and each hctx is limited to allocate at
most:
hctx_max_depth = max((bt->sb.depth + users - 1) / users, 4U);
tag idle detection is lazy, and may be delayed for 30sec, so there
could be just one real active hctx(queue) but all others are actually
idle and still accounted as active because of the lazy idle detection.
Then if wake_batch is > hctx_max_depth, driver tag allocation may wait
forever on this real active hctx.
Fix this by recalculating wake_batch when inc or dec active_queues.

Commit cc9c884dd7 ("block: call submit_bio_checks under q_usage_counter")
uses q_usage_counter to protect submit_bio_checks for avoiding IO after
disk is deleted by del_gendisk().
Turns out the protection isn't necessary, because once
blk_mq_freeze_queue_wait() in del_gendisk() returns:
1) all in-flight IO has been done
2) all new IO will be failed in __bio_queue_enter() because
q_usage_counter is dead, and GD_DEAD is set
3) both disk and request queue instance are safe since caller of
submit_bio() guarantees that the disk can't be closed.
Once submit_bio_checks() needn't the protection of q_usage_counter, we can
move submit_bio_checks before calling blk_mq_submit_bio() and
->submit_bio(). With this change, we needn't to throttle queue with
holding one allocated request, then precise driver tag or request won't be
wasted in throttling. Meantime we can unify the bio check for both bio
based and request based driver.

Commit 5fc11eebb4 ("block: open code create_task_io_context in
set_task_ioprio") introduces a needless assignment
'ioc = task->io_context', as the local variable ioc is not further
used before returning.
Even after the further fix, commit a957b61254 ("block: fix error in
handling dead task for ioprio setting"), the assignment still remains
needless.
Drop this needless assignment in set_task_ioprio().
This code smell was identified with 'make clang-analyzer'.

John Garry reported a deadlock that occurs when trying to access a
runtime-suspended SATA device. For obscure reasons, the rescan procedure
causes the link to be hard-reset, which disconnects the device.
The rescan tries to carry out a runtime resume when accessing the device.
scsi_rescan_device() holds the SCSI device lock and won't release it until
it can put commands onto the device's block queue. This can't happen until
the queue is successfully runtime-resumed or the device is unregistered.
But the runtime resume fails because the device is disconnected, and
__scsi_remove_device() can't do the unregistration because it can't get the
device lock.
The best way to resolve this deadlock appears to be to allow the block
queue to start running again even after an unsuccessful runtime resume.
The idea is that the driver or the SCSI error handler will need to be able
to use the queue to resolve the runtime resume failure.
This patch removes the err argument to blk_post_runtime_resume() and makes
the routine act as though the resume was successful always. This fixes the
deadlock.

One device_add is called disk->ev will be freed by disk_release, so we

blk_stat_disable_accounting() is added in commit 68497092bd
("block: make queue stat accounting a reference"), and called in
kyber_exit_sched().
So we have to free q->stats after elevator is unloaded from
blk_exit_queue() in blk_release_queue(). Otherwise kernel panic
is caused.

This is a thin wrapper around bio_add_page(). The main advantage here
is the documentation that folios larger than 2GiB are not supported.
It's not currently possible to allocate folios that large, but if it

The flow in set_task_ioprio can be simplified by simply open coding

If we have a list of requests in our plug list, send it to the driver in
one go, if possible. The driver must set mq_ops->queue_rqs() to support

The batched completions only deal with non-partial requests anyway,
and it doesn't deal with any requests that have errors. Add a completion
handler that assumes it's a full request and that it's all being ended
successfully.

Dexuan reports that he's seeing spikes of very heavy CPU utilization when
running 24 disks and using the 'none' scheduler. This happens off the
sched restart path, because SCSI requires the queue to be restarted async,
and hence we're hammering on mod_delayed_work_on() to ensure that the work
item gets run appropriately.
Avoid hammering on the timer and just use queue_work_on() if no delay
has been specified.

kyber turns on IO statistics when it is loaded on a queue, which means
that even if kyber is then later unloaded, we're still stuck with stats
enabled on the queue.
Change the account enabled from a bool to an int, and pair the enable call
with the equivalent disable call. This ensures that stats gets turned off
again appropriately.

The donation calculation logic assumes that the donor has non-zero
after-donation hweight, so the lowest active hweight a donating cgroup can
have is 2 so that it can donate 1 while keeping the other 1 for itself.
Earlier, we only donated from cgroups with sizable surpluses so this
condition was always true. However, with the precise donation algorithm
implemented, f1de2439ec ("blk-iocost: revamp donation amount
determination") made the donation amount calculation exact enabling even low
hweight cgroups to donate.
This means that in rare occasions, a cgroup with active hweight of 1 can
enter donation calculation triggering the following warning and then a
divide-by-zero oops.

Kashyap reports high CPU usage in blk_mq_queue_tag_busy_iter() and callees
using megaraid SAS RAID card since moving to shared tags [0].
Previously, when shared tags was shared sbitmap, this function was less
than optimum since we would iter through all tags for all hctx's,
yet only ever match upto tagset depth number of rqs.
Since the change to shared tags, things are even less efficient if we have
parallel callers of blk_mq_queue_tag_busy_iter(). This is because in
bt_iter() -> blk_mq_find_and_get_req() there would be more contention on
accessing each request ref and tags->lock since they are now shared among
all HW queues.
Optimise by having separate calls to bt_for_each() for when we're using
shared tags. In this case no longer pass a hctx, as it is no longer
relevant, and teach bt_iter() about this.
Ming suggested something along the lines of this change, apart from a
different implementation.

Typedefs busy_iter_fn and busy_tag_iter_fn are now identical, so delete
busy_iter_fn to reduce duplication.
It would be nicer to delete busy_tag_iter_fn, as the name busy_iter_fn is
less specific.
However busy_tag_iter_fn is used in many different parts of the tree,
unlike busy_iter_fn which is just use in block/, so just take the
straightforward path now, so that we could rename later treewide.

The only user of blk_mq_hw_ctx blk_mq_hw_ctx argument is
blk_mq_rq_inflight().
Function blk_mq_rq_inflight() uses the hctx to find the associated request
queue to match against the request. However this same check is already
done in caller bt_iter(), so drop this check.
With that change there are no more users of busy_iter_fn blk_mq_hw_ctx
argument, so drop the argument.

In case of BLK_MQ_F_BLOCKING, per-hctx srcu is used to protect dispatch
critical area. However, this srcu instance stays at the end of hctx, and
it often takes standalone cacheline, often cold.
Inside srcu_read_lock() and srcu_read_unlock(), WRITE is always done on
the indirect percpu variable which is allocated from heap instead of
being embedded, srcu->srcu_idx is read only in srcu_read_lock(). It
doesn't matter if srcu structure stays in hctx or request queue.
So switch to per-request-queue srcu for protecting dispatch, and this
way simplifies quiesce a lot, not mention quiesce is always done on the
request queue wide.

Remove hctx_lock and hctx_unlock, and add one helper of
blk_mq_run_dispatch_ops() to run code block defined in dispatch_ops
with rcu/srcu read held.
Compared with hctx_lock()/hctx_unlock():
1) remove 2 branch to 1, so we just need to check
(hctx->flags & BLK_MQ_F_BLOCKING) once when running one dispatch_ops
2) srcu_idx needn't to be touched in case of non-blocking
3) might_sleep_if() can be moved to the blocking branch
Also put the added blk_mq_run_dispatch_ops() in private header, so that
the following patch can use it out of blk-mq.c.

refcount_t is not as expensive as it used to be, but it's still more
expensive than the io_uring method of using atomic_t and just checking
for potential over/underflow.
This borrows that same implementation, which in turn is based on the
mm implementation from Linus.

Don't call into generic_file_read_iter() if we know it's O_DIRECT, just
set it up ourselves and call our own handler. This avoids an indirect call
for O_DIRECT.

cgroup.h (therefore swap.h, therefore half of the universe)
includes bpf.h which in turn includes module.h and slab.h.
Since we're about to get rid of that dependency we need
to clean things up.
v2: drop the cpu.h include from cacheinfo.h, it's not necessary
and it makes riscv sensitive to ordering of include files.

When we attempt to merge off the cached request path, we return NULL
if successful. This makes the caller believe that it's should allocate
a new request, and hence we end up with the bio both merged and associated
with a new request. This, predictably, leads to all sorts of crashes.
Pass in a pointer to the bio pointer, and clear it for the merge case.
Then the caller knows that the bio is already queued, and no new requests
need to get allocated.

In __copy_io we know that the newly allocate task_struct does not have
an I/O context yet and is not exiting. So just allocate the I/O context
struct and install it directly. There is no need to lock the task
either as it is just being created.

Commit 7cc4ffc555 ("block, bfq: put reqs of waker and woken in
dispatch list") added a condition to bfq_insert_request() which added
waker's requests directly to dispatch list. The rationale was that
completing waker's IO is needed to get more IO for the current queue.
Although this rationale is valid, there is a hole in it. The waker does
not necessarily serve the IO only for the current queue and maybe it's
current IO is not needed for current queue to make progress. Furthermore
injecting IO like this completely bypasses any service accounting within
bfq and thus we do not properly track how much service is waker's queue
getting or that the waker is actually doing any IO. Depending on the
conditions this can result in the waker getting too much or too few
service.
Consider for example the following job file:

Despite processes have very different IO priorities, they get the same
about of service. The reason is that bfq identifies these processes as
having waker-wakee relationship and once that happens, IO from
fastwriter gets injected during slowwriter's time slice. As a result bfq
is not aware that fastwriter has any IO to do and constantly schedules
only slowwriter's queue. Thus fastwriter is forced to compete with
slowwriter's IO all the time instead of getting its share of time based
on IO priority.
Drop the special injection condition from bfq_insert_request(). As a
result, requests will be tracked and queued in a normal way and on next
dispatch bfq_select_queue() can decide whether the waker's inserted
requests should be injected during the current queue's timeslice or not.

Waker - wakee relationships are important in deciding whether one queue
can preempt the other one. Print information about detected waker-wakee
relationships so that scheduling decisions can be better understood from
block traces.

Currently, when process A starts issuing requests shortly after process
B has completed some IO three times in a row, we decide that B is a
"waker" of A meaning that completing IO of B is needed for A to make
progress and generally stop separating A's and B's IO much. This logic
is useful to avoid unnecessary idling and thus throughput loss for cases
where workload needs to switch e.g. between the process and the
journaling thread doing IO. However the detection heuristic tends to
frequently give false positives when A and B are fighting IO bandwidth
and other processes aren't doing much IO as we are basically deemed to
eventually accumulate three occurences of a situation where one process
starts issuing requests after the other has completed some IO. To reduce
these false positives, cancel the waker detection also if we didn't
accumulate three detected wakeups within given timeout. The rationale is
that if wakeups are really rare, the pointless idling doesn't hurt
throughput that much anyway.
This significantly reduces false waker detection for workload like:

When cgroup IO scheduling is used with BFQ it does not really provide
service differentiation if the cgroup drives a big IO depth. That for
example happens with writeback which asynchronously submits lots of IO
but it can happen with AIO as well. The problem is that if we have two
cgroups that submit IO with different weights, the cgroup with higher
weight properly gets more IO time and is able to dispatch more IO.
However this causes lower weight cgroup to accumulate more requests
inside BFQ and eventually lower weight cgroup consumes most of IO
scheduler tags. At that point higher weight cgroup stops getting better
service as it is mostly blocked waiting for a scheduler tag while its
queues inside BFQ are empty and thus lower weight cgroup gets served.
Check how many requests submitting cgroup has allocated in
bfq_limit_depth() and if it consumes more requests than what would
correspond to its weight limit available depth to 1 so that the cgroup
cannot consume many more requests. With this limitation the higher
weight cgroup gets proper service even with writeback.

Currently we lookup ICQ only after the request is allocated. However BFQ
will want to decide how many scheduler tags it allows a given bfq queue
(effectively a process) to consume based on cgroup weight. So provide a
function blk_mq_sched_get_icq() so that BFQ can lookup ICQ earlier.

function is misleading because it implies that blk_crypto_register()
needs to be paired with blk_crypto_unregister(), which is not the case.

Refactor the request alloction so that blk_mq_get_cached_request tries
to find a cached request first, and the entirely separate and now
self contained blk_mq_get_new_requests allocates one or more requests
if that is not possible.
There is a small change in behavior as submit_bio_checks is called
twice now if a cached request is present but can't be used, but that
is a small price to pay for unwinding this code.

This is essentially never used, yet it's about 1/3rd of the total
queue size. Allocate it when needed, and don't embed it in the queue.
Kill the queue flag for this while at it, since we can just check the
assigned pointer now.

blk_mq_submit_bio has two different plug cases, one that uses full
plugging and a limited plugging one.
The limited plugging case is only used for a corner case that does
not matter in real life:
- no ->commit_rqs (so not NVMe)
- no shared tags (so not SCSI)
- not rotational (so no old disk or floppy driver)
- must have multiple queues (so no eMMC)
Remove the limited merging case and all the related junk to simplify
blk_mq_submit_bio and the functions called from it.

All modern drivers can support extra partitions using the extended
dev_t. In fact except for the ioctl method drivers never even see
partitions in normal operation.
So remove the GENHD_FL_EXT_DEVT and allow extra partitions for all
block devices that do support partitions, and require those that
do not support partitions to explicit disallow them using
GENHD_FL_NO_PART.
