Get rid of the last occurances of _v1 suffixes, and move the version
number right after the "uac" string. Now things are consitent again.

For both UAC1 and UAC2, interrupt endpoint messages are now parsed with
structs rather that with anonymous buffer array accesses.
For UAC2, only CUR interrupt notifications are supported for now.
snd_usb_mixer_status_complete() was renamed to
snd_usb_mixer_interrupt().
Fixed one indentation flaw on the way.

USB Audio Class v2.0 compliant devices have different descriptors and a
different way of setting/getting min/max/res/cur properties. This patch
adds support for them.

Introduce a number of new structs for mixer, selector, feature and
processing units and some static inline helpers to access fields which
have dynamic offsets. Use them in mixer.c to parse the descriptors. This
is necessary for the upcoming audio v2 parsers.

Use the definitions from linux/usb/audio.h all over the ALSA USB audio
driver and add some missing definitions there as well.
Use the endpoint attribute macros from linux/usb/ch9 and remove the own
things from sound/usb/usbaudio.h.
Now things are also nicely prefixed which makes understanding the code
easier.

This patch adds some definitions for audio class v2.
Unfortunately, the UNIT types PROCESSING_UNIT and EXTENSION_UNIT have
different numerical representations in both standards, so there is need
for a _V1 add-on now. usbmixer.c is changed accordingly.

In preparation of support for v2.0 audio class, use the structs from
linux/usb/audio.h and add some new ones to describe the fields that are
actually parsed by the descriptor decoders.
Also, factor out code from usb_create_streams(). This makes it easier to
adopt the new iteration logic needed for v2.0.

include/linux/usb/audio.h is exported to userspace,
so part of this file that is for internal kernel
usage need to be guarded with ifdef __KERNEL__.
This way make headers_install will stript it out.

linux/usb/audio.h is a public header file that includes definitions
exported to userspace. To avoid namespace clashes, prefix all macro
definitions with UAC_. Existing macros and structures prefixed with
USB_AC_ and USB_AS_ are renamed for consistency.

For the header files in include/linux/usb, add missing multiple
inclusion protection and standardize what's already there. The
apparent standards:
* macro name of __LINUX_USB_headerfile_H
* inclusion protection placed after leading comment block
* macro name added as a comment on the final #endif
* any obvious trivial whitespace cleanup associated with the above

This driver is glue between the USB gadget interface
and the ALSA MIDI interface. It allows us to appear
as a MIDI Streaming device to a host system on the
other end of a USB cable.
This includes linux/usb/audio.h and linux/usb/midi.h
containing definitions from the relevant USB specifications
for USB audio and USB MIDI devices.
The following changes have been made since the first RFC
posting:
* Bug fixes to endpoint handling.
* Workaround for USB_REQ_SET_CONFIGURATION handling,
not understood yet.
* Added SND and SND_RAWMIDI dependencies in Kconfig.
* Moved usb_audio.h and usb_midi.h to usb/*.h
* Added module parameters for ALSA card index and id.
* Added module parameters for USB descriptor IDs and strings.
* Removed some unneeded stuff inherited from zero.c, more to go.
* Provide DECLARE_* macros for the variable-length structs.
* Use kmalloc instead of usb_ep_alloc_buffer.
* Limit source to 80 columns.
* Return actual error code instead of -ENOMEM in a few places.
