For MPI Endpoint/Mcpu, driver should double buffer data buffer/SGLs.
This is normally copied from host to internal memory of IOC by DMA
engine of PCI device. Since the interface to DMA from host to mCPU is
not present for Mcpu/MPI Endpoint device, driver does double copy of
those buffers directly to the mCPU memory region via BAR0 region.
Introduced API to calculate and return BAR0 mapped host buffer's
physical and virtual address for the provided smid.

This patch finishes all outstanding SCSI IO commands (but not other commands,
e.g., task management) in the shutdown and unload paths.
It first waits for the commands to complete (this is done after setting
'ioc->remove_host = 1 ', which prevents new commands to be queued) then it
flushes commands that might still be running.
This avoids triggering error handling (e.g., abort command) for all commands
possibly completed by the adapter after interrupts disabled.

This patch adds checks for 'ioc->remove_host' in the SCSI error handlers, so
not to access pointers/resources potentially freed in the PCI shutdown/module
unload path. The error handlers may be invoked after shutdown/unload,
depending on other components.
This problem was observed with kexec on a system with a mpt3sas based adapter
and an infiniband adapter which takes long enough to shutdown:
The mpt3sas driver finished shutting down / disabled interrupt handling, thus
some commands have not finished and timed out.
Since the system was still running (waiting for the infiniband adapter to
shutdown), the scsi error handler for task abort of mpt3sas was invoked, and
hit an oops -- either in scsih_abort() because 'ioc->scsi_lookup' was NULL
without commit dbec4c9040 ("scsi: mpt3sas: lockless command submission"), or
later up in scsih_host_reset() (with or without that commit), because it
eventually called mpt3sas_base_get_iocstate().
After the above commit, the oops in scsih_abort() does not occur anymore
(_scsih_scsi_lookup_find_by_scmd() is no longer called), but that commit is
too big and out of the scope of linux-stable, where this patch might help, so
still go for the changes.
Also, this might help to prevent similar errors in the future, in case code
changes and possibly tries to access freed stuff.
Note the fix in scsih_host_reset() is still important anyway.

should be all done.

cpu_msix_table is allocated to store online cpus, but pci_irq_get_affinity
may return cpu_possible_mask which is then used to access cpu_msix_table.
That causes bad user experience. Fix limits access to only online cpus,
I've also added an additional test to protect from an unlikely change in
cpu_online_mask.

Performance improvement using block layer tag.
Curent driver gets scsiio tracker and free smid from link list and array
based tracking managed by driver. Accessing list in main io path is
performance pentaly because of protection using spinlock
"scsi_lookup_lock".
In this patch:
1. Driver removes all link list access from main io path and
use scmd->request->tag to get free smid.
2. Instead of holding 'struct scsiio_tracker' in its own pool
driver can embed it into the scsi command.
Driver provides cmd_size in scsi_host_template, so that struct
scsiio_tracker is preallocated by scsi mid layer for each scsi command.

ioctl passthrough commands require a SCSIIO smid, but cannot easily
integrate with the block layer. But the driver already has reserved some
SCSIIO smids and we're only ever allowing one ioctl command at a time we
can use the first reserved smid for ioctl commands.

1. In IO path, setting of "ATA command pending" flag early before device
removal, invalid device handle etc., checks causes any new commands
to be always returned with SAM_STAT_BUSY and when the driver removes
the drive the SML issues SYNC Cache command and that command is
always returned with SAM_STAT_BUSY and thus making SYNC Cache command
to requeued.
2. If the driver gets an ATA PT command for a SATA drive then the driver
set "ATA command pending" flag in device specific data structure not
to allow any further commands until the ATA PT command is completed.
However, after setting the flag if the driver decides to return the
command back to upper layers without actually issuing to the firmware
(i.e., returns from qcmd failure return paths) then the corresponding
flag is not cleared and this prevents the driver from sending any new
commands to the drive.
This patch fixes above two issues by setting of "ATA command pending"
flag after checking for whether device deleted, invalid device handle,
device busy with task management. And by setting "ATA command pending"
flag to false in all of the qcmd failure return paths after setting the
flag.

After taking a closer look, I found that the problem is that the new
code mixes up pointers and dma_addr_t values unnecessarily.
This changes it to use the correct types consistently, which lets us get
rid of a lot of type casts in the process. I'm also renaming some
variables to avoid confusion between physical and dma address spaces
that are often distinct.

* The controller firmware sends separate events for NVMe devices and
PCIe switches similar to existing SAS events.
* NVMe device detection, addition and removal are reported by the
firmware through PCIe Topology Change list events.
* The PCIe device state change events are sent when the firmware
detects any abnormal conditions with a NVMe device or switch.
* The enumeration event are sent when the firmware starts PCIe device
enumeration and stops.
* This patch has the code change to handle the events and add/remove
NVMe devices in driver's inventory.

* Mpt3sas driver uses the NVMe Encapsulated Request message to send an
NVMe command to an NVMe device attached to the IOC.
* Normal I/O commands like reads and writes are passed to the
controller as SCSI commands and the controller has the ability to
translate the commands to NVMe equivalent.
* This encapsulated NVMe command is used by applications to send
direct NVMe commands to NVMe drives.

* Added support for translating the SGLs associated with incoming
commands either to IEE SGL or NVMe PRPs for NVMe devices.
* The hardware translation of IEEE SGL to NVMe PRPs has limitations
and if a command cannot be translated by hardware then it will go to
firmware and the firmware needs to translate it. This will have a
performance impact. To avoid that, the driver proactively checks
whether the translation will be done in hardware or not. If not, then
driver translates.

a) During scan, pcie devices are probed and added to SML to drivers

like handle, target_id etc.
c) slave_destroy & target_destroy are called when driver unregisters
or removes device. Also frees allocated resources and info.

Many source files in the tree are missing licensing information, which
makes it harder for compliance tools to determine the correct license.
By default all files without license information are under the default
license of the kernel, which is GPL version 2.
Update the files which contain no license information with the 'GPL-2.0'
SPDX license identifier. The SPDX identifier is a legally binding
shorthand, which can be used instead of the full boiler plate text.
This patch is based on work done by Thomas Gleixner and Kate Stewart and
Philippe Ombredanne.
How this work was done:
Patches were generated and checked against linux-4.14-rc6 for a subset of
the use cases:
- file had no licensing information it it.
- file was a */uapi/* one with no licensing information in it,
- file was a */uapi/* one with existing licensing information,
Further patches will be generated in subsequent months to fix up cases
where non-standard license headers were used, and references to license
had to be inferred by heuristics based on keywords.
The analysis to determine which SPDX License Identifier to be applied to
a file was done in a spreadsheet of side by side results from of the
output of two independent scanners (ScanCode & Windriver) producing SPDX
tag:value files created by Philippe Ombredanne. Philippe prepared the
base worksheet, and did an initial spot review of a few 1000 files.
The 4.13 kernel was the starting point of the analysis with 60,537 files
assessed. Kate Stewart did a file by file comparison of the scanner
results in the spreadsheet to determine which SPDX license identifier(s)
to be applied to the file. She confirmed any determination that was not
immediately clear with lawyers working with the Linux Foundation.
Criteria used to select files for SPDX license identifier tagging was:
- Files considered eligible had to be source code files.
- Make and config files were included as candidates if they contained >5
lines of source
- File already had some variant of a license header in it (even if <5
lines).
All documentation files were explicitly excluded.
The following heuristics were used to determine which SPDX license
identifiers to apply.
- when both scanners couldn't find any license traces, file was
considered to have no license information in it, and the top level
COPYING file license applied.
For non */uapi/* files that summary was:

and that resulted in the third patch in this series.
- when the two scanners agreed on the detected license(s), that became
the concluded license(s).
- when there was disagreement between the two scanners (one detected a
license but the other didn't, or they both detected different
licenses) a manual inspection of the file occurred.
- In most cases a manual inspection of the information in the file
resulted in a clear resolution of the license that should apply (and
which scanner probably needed to revisit its heuristics).
- When it was not immediately clear, the license identifier was
confirmed with lawyers working with the Linux Foundation.
- If there was any question as to the appropriate license identifier,
the file was flagged for further research and to be revisited later
in time.
In total, over 70 hours of logged manual review was done on the
spreadsheet to determine the SPDX license identifiers to apply to the
source files by Kate, Philippe, Thomas and, in some cases, confirmation
by lawyers working with the Linux Foundation.
Kate also obtained a third independent scan of the 4.13 code base from
FOSSology, and compared selected files where the other two scanners
disagreed against that SPDX file, to see if there was new insights. The
Windriver scanner is based on an older version of FOSSology in part, so
they are related.
Thomas did random spot checks in about 500 files from the spreadsheets
for the uapi headers and agreed with SPDX license identifier in the
files he inspected. For the non-uapi files Thomas did random spot checks
in about 15000 files.
In initial set of patches against 4.14-rc6, 3 files were found to have
copy/paste license identifier errors, and have been fixed to reflect the
correct identifier.
Additionally Philippe spent 10 hours this week doing a detailed manual
inspection and review of the 12,461 patched files from the initial patch
version early this week with:
- a full scancode scan run, collecting the matched texts, detected
license ids and scores
- reviewing anything where there was a license detected (about 500+
files) to ensure that the applied SPDX license was correct
- reviewing anything where there was no detection but the patch license
was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied
SPDX license was correct
This produced a worksheet with 20 files needing minor correction. This
worksheet was then exported into 3 different .csv files for the
different types of files to be modified.
These .csv files were then reviewed by Greg. Thomas wrote a script to
parse the csv files and add the proper SPDX tag to the file, in the
format that the file expected. This script was further refined by Greg
based on the output to detect more types of files automatically and to
distinguish between header and source .c files (which need different
comment types.) Finally Greg ran the script using the .csv files to
generate the patches.

Enclosure handles are not updated after host reset. As a result, driver
device structure is holding previously assigned enclosure handle which
is different from the enclosure handle populated in the corresponding
device page.

to current value by referring the enclosure handles from corresponding
device pages

Whenever an I/O for a RAID volume fails with IOCStatus
MPI2_IOCSTATUS_SCSI_IOC_TERMINATED and SCSIStatus equal to
(MPI2_SCSI_STATE_TERMINATED | MPI2_SCSI_STATE_NO_SCSI_STATUS) then
return the I/O to SCSI midlayer with "DID_RESET" (i.e. retry the IO
infinite times) set in the host byte.
Previously, the driver was completing the I/O with "DID_SOFT_ERROR"
which causes the I/O to be quickly retried. However, firmware needed
more time and hence I/Os were failing.

For Dev Handles whose value is less than HBA's phys count number, driver
would return HBA's SAS address value. As a result, for a Virtual SES
device the driver was returning the HBA's SAS address. Updated the
driver to return Virtual SES' SAS address.

To reduce the memory footprint of the driver in the kdump kernel, we
apply the following settings when reset_devices is set:
1. Use single MSI-x vector.
2. Disable RDPQ mode.
3. Set sg_table_size to 32 by default.
4) Set SCSI IO Queue depth to 200.

While removing Expander devices, we are removing expander device entry
from the list before freeing its child devices. While freeing child
device we are finding its parent device node as NULL and therefore we
are not freeing the child device's allocated data structures. Updated
the driver to remove the expander device from the list only after
freeing all its child devices.

Earlier Active Cable Exception event with reason code "Cable Degraded
(0x02))" was added only for Active Cable. Now this event is extended to
Passive cable too. Re-arranged display message accordingly.
Also added Cable Exception Event event for SAS3008 & SAS3108 HBAs
(i.e. MPI 2.5 spec supporting HBAs). Previously, this event was enabled
only for MPI 2.6 spec supporting HBA devices.

Simplify the SMP passthrough code by switching it to the generic bsg-lib
helpers that abstract away the details of the request code, and gets
drivers out of seeing struct scsi_request.
For the libsas host SMP code there is a small behavior difference in
that we now always clear the residual len for successful commands,
similar to the three other SMP handler implementations. Given that
there is no partial command handling in the host SMP handler this should
not matter in practice.

Instead of passing a "wait" argument to scsi_internal_device_block(),
split this function into a function that waits and a function that

Due to relaxed ordering requirements on multiple architectures, drivers
are required to use wmb/rmb/mb combinations when they need to guarantee
observability between the memory and the HW.
The mpt3sas driver is already using wmb() for this purpose. However, it
issues a writel following wmb(). writel() function on arm/arm64
arhictectures have an embedded wmb() call inside.
This results in unnecessary performance loss and code duplication.
writel already guarantees ordering for both cpu and bus. we don't need
additional wmb()

Move scsi_remove_host call into sas_remove_host and remove it from SAS
HBA drivers, so we don't mess up the ordering. This solves an issue with
double deleting sysfs entries that was introduced by the change of sysfs

Commit 669f044170 ("scsi: srp_transport: Move queuecommand() wait code
to SCSI core") can make scsi_internal_device_block() sleep. However,
the mpt3sas driver can call this function from an interrupt
handler. Hence add a second argument to scsi_internal_device_block()
that restores the old behavior of this function for the mpt3sas handler.

Commit <f2e767bb5d6e> ("mpt3sas: Force request partial completion
alignment") was not considering the case of commands not operating on
logical block size units (e.g. REQ_OP_ZONE_REPORT and its 64B aligned
partial replies). In this case, forcing alignment of resid to the device
logical block size can break the command result, e.g. in the case of
REQ_OP_ZONE_REPORT, the exact number of zone reported by the device.
Move the partial completion alignement check of mpt3sas to a generic
implementation in sd_done(). The check is added within the default
section of the initial req_op() switch case so that the report and reset
zone commands are ignored. In addition, as sd_done() is not called for
passthrough requests, resid corrections are not done as intended by the
initial mpt3sas patch.

The firmware or device, possibly under a heavy I/O load, can return on a
partial unaligned boundary. Scsi-ml expects these requests to be
completed on an alignment boundary. Scsi-ml blindly requeues the I/O
without checking the alignment boundary of the I/O request for the
remaining bytes. This leads to errors, since devices cannot perform
non-aligned read/write operations.
This patch fixes the issue in the driver. It aligns unaligned
completions of FS requests, by truncating them to the nearest alignment
boundary.

Due existence of loop in the IO path our HBA will receive heavy IOs and
also as driver is not updating the Reply Post Host Index frequently, So
there will be a high chance that our Firmware unable to find any free
entry in the Reply Post Descriptor Queue (i.e. Queue overflow occurs)
and can observe 0x2100 firmware fault. So to fix this, we have defined
a thresh hold value. After continuously processing this thresh hold
number of reply descriptors driver will update the Reply Descriptor Host
Index so that this thresh hold number of reply descriptors entries will
be freed and these entries will be available for firmware and we won't
observe this Firmware fault. We have defined this threshold value as
1/3rd of the hba queue depth.

Driver processes the event MPI26_EVENT_ACTIVE_CABLE_DEGRADED when a
cable is present and is running at a degraded speed (below the SAS3 12
Gb/s rate). Prints added to inform the user that the cable is not

mpt3sas has a firmware failure where it can only handle one pass through
ATA command at a time. If another comes in, contrary to the SAT
standard, it will hang until the first one completes (causing long
commands like secure erase to timeout). The original fix was to block
the device when an ATA command came in, but this caused a regression
with

So fix the original fix of the secure erase timeout by properly
returning SAM_STAT_BUSY like the SAT recommends. The original patch

This patch adds support for request iopriority handling in the mpt3sas
layer. This works only when a ATA device is behind the SATL. The ATA
device also has to indicate that it supports command priorities in the
identify information that is pulled from the SATL.

While issuing any ATA passthrough command to firmware the driver will
block the device. But it will unblock the device only if the I/O
completes through the ISR path. If a controller reset occurs before
command completion the device will remain in blocked state.
Make sure we unblock the device following a controller reset if an ATA
passthrough command was queued.

This is a work around for a bug with LSI Fusion MPT SAS2 when perfoming
secure erase. Due to the very long time the operation takes, commands
issued during the erase will time out and will trigger execution of the
abort hook. Even though the abort hook is called for the specific
command which timed out, this leads to entire device halt
(scsi_state terminated) and premature termination of the secure erase.
Set device state to busy while ATA passthrough commands are in progress.

An UNMAP command on a PI formatted device will leave the Logical Block
Application Tag and Logical Block Reference Tag as all F's (for those LBAs
that are unmapped). To avoid IO errors if those LBAs are subsequently read
before they are written with valid tag fields, the MPI SCSI IO requests
need to set the EEDPFlags element EEDP Escape Mode field, Bits [7:6]
appropriately. A value of 2 should be set to disable all PI checks if the
Logical Block Application Tag is 0xFFFF for PI types 1 and 2. A value
of 3 should be set to disable all PI checks if the Logical Block
Application Tag is 0xFFFF and the Logical Block Reference Tag is
0xFFFFFFFF for PI type 3.

For SAS35 devices MSIX vectors are inceased to 128 from 96. To support this
Reply post host index register count is increased to 16. Also variable
msix96_vector is replaced with combined_reply_queue and variable
combined_reply_index_count is added to set different values for SAS3 and
SAS35 devices.

When device missing event arrives, device_remove_in_progress bit will be
set and hence driver has to stop sending IOCTL commands.Now the check has
been added in IOCTL path to test device_remove_in_progress bit is set, if
so then IOCTL will be failed printing failure message.

No. of MSIX vectors supported = min (Total no. of CPU cores,
MSIX vectors supported by card)
when RDPQ is disabled "max_msix_vectors" module parameter which was
declared as global was set to '8' and hence if there are more than one card
in system among which if RDPQ disabled card is enumerated first then only 8
MSIX vectors was getting enabled for all the cards(including RDPQ enabled
card,which can support more than 8 MSIX vectors).
Used local variable instead of global variable ,if RDPQ is disabled this
local variable is set to '8' else it is set to "max_msix_vectors" (by
default this is set to -1, whose value can be set by user during driver
load time).So now regardless of whether RDPQ disabled card is enumerated
first or RDPQ enabled card is enumerated first , MSIX vectors enabled
depends on the cards capability.

That check should be for the line sas_target_priv_data->raid_device =
raid_device;
Due to above hunk, we are not initializing raid_device's starget for
raid volumes, and so during raid disk deletion driver is not calling
scsi_remove_target() API as driver observes starget field of
raid_device's structure as NULL.
