Linas Vepstas e70ea2634a PCI: rpaphp: Use pcibios_remove_pci_devices() symmetrically
At first blush, the disable_slot() routine does not look
at all like its symmetric with the enable_slot() routine;
as it seems to call a very different set of routines.
However, this is easily fixed: pcibios_remove_pci_devices()
does the right thing.

Rename rpaphp_register_pci_slot() because its easy to confuse
with  rpaphp_register_slot() even though it does something 
completely different. Rename it to rpaphp_enable_slot() because
its almost identical to enbale_slot().

Kenji Kaneshige 5d386e1ac4 pciehp: Event handling rework
The event handler of PCIEHP driver is unnecessarily very complex. In
addition, current event handler can only a fixed number of events at
the same time, and some of events would be lost if several number of
events happened at the same time.

This patch simplify the event handler using 'work queue', and it also
fix the above-mentioned issue.

Greg Kroah-Hartman 823bccfc40 remove "struct subsystem" as it is no longer needed
We need to work on cleaning up the relationship between kobjects, ksets and
ktypes.  The removal of 'struct subsystem' is the first step of this,
especially as it is not really needed at all.

Tim Schmielau cd354f1ae7 [PATCH] remove many unneeded #includes of sched.h
After Al Viro (finally) succeeded in removing the sched.h #include in module.h
recently, it makes sense again to remove other superfluous sched.h includes.
There are quite a lot of files which include it but don't actually need
anything defined in there.  Presumably these includes were once needed for
macros that used to live in sched.h, but moved to other header files in the
course of cleaning it up.

To ease the pain, this time I did not fiddle with any header files and only
removed #includes from .c-files, which tend to cause less trouble.

Compile tested against 2.6.20-rc2 and 2.6.20-rc2-mm2 (with offsets) on alpha,
arm, i386, ia64, mips, powerpc, and x86_64 with allnoconfig, defconfig,
allmodconfig, and allyesconfig as well as a few randconfigs on x86_64 and all
configs in arch/arm/configs on arm.  I also checked that no new warnings were
introduced by the patch (actually, some warnings are removed that were emitted
by unnecessarily included header files).

Arjan van de Ven d54b1fdb1d [PATCH] mark struct file_operations const 5
Many struct file_operations in the kernel can be "const".  Marking them const
moves these to the .rodata section, which avoids false sharing with potential
dirty data.  In addition it'll catch accidental writes at compile time to
these shared resources.

John Keller 3e643e77a9 Altix: Add ACPI SSDT PCI device support (hotplug)
Support for dynamic loading and unloading of ACPI SSDT tables upon slot
hotplugs and unplugs.

On SN platforms, we now represent every populated root bus slot with a single
ACPI SSDT table containing info for every device and PPB attached to the slot.
 These SSDTs are generated by the prom at initial boot and hotplug time.  The
info in these SSDT tables is used by the SN kernel IO "fixup" code (which is
called at boot and hotplug time).

On hotplugs (i.e.  enable_slot()), if running with an ACPI capable prom,
attempt to obtain a new ACPI SSDT table for the slot being hotplugged.  If
successful, add the table to the ACPI namespace (acpi_load_table()) and then
walk the new devices and add them to the ACPI infrastructure (acpi_bus_add()).

On hot unplugs (i.e.  disable_slot()), if running with an ACPI capable prom,
attempt to remove the SSDT table associated with the slot from the ACPI
namespace (acpi_unload_table_id()) and infastructure (acpi_bus_trim()).

 A bug was fixed where the sgi hotplug driver was removing
 the slot's SSDT table from the ACPI namespace a bit too early in
 disable_slot(). Also, we now call acpi_bus_start() subsequent
 to acpi_bus_add().

Kenji Kaneshige 9d167dc367 pciehp: remove unnecessary pci_disable_msi
This patch fixes the problem that "irq XX: nobody cared" kernel oops
is reported when pciehp is once rmmoded and insmoded again. The cause
of this problem is pciehp driver calls pci_disable_msi() at controller
release time, even though it must be done by PCI Express Port Bus
driver. This patch removes unnecessary pci_disable_msi() call from
pciehp driver.

Rolf Eike Beer ac9e989187 PCI: Change memory allocation for acpiphp slots
Change memory allocation for acpiphp slots

Change the "struct slot" that acpiphp uses for managing it's slots to
directly contain the memory for the needed struct hotplug_slot_info and
the slot's name. This way we need only two memory allocations per slot
instead of four.

While we are at it: make_slot_name() is just a wrapper around snprintf()
knowing the right arguments to call it. Since the function makes just one
function call and is only called from one place I inlined it by hand.

Finally this fixes a possible bug waiting for someone to hit it. There were
two unused local variables in acpiphp_register_hotplug_slot(). gcc did not
find them because they were used in memory allocations with sizeof(*var).
They had the same types as the target of the allocation, but nevertheless
this was just weird.

This patch fixes the problem that system will panic if multiple power
on/off operations are issued to the same slot in parallel. This
problem can be easily reproduced by commands below.

The cause is lack of locking for enable/disable operations. This patch
fixes this problem.

David Howells 7d12e780e0 IRQ: Maintain regs pointer globally rather than passing to IRQ handlers
Maintain a per-CPU global "struct pt_regs *" variable which can be used instead
of passing regs around manually through all ~1800 interrupt handlers in the
Linux kernel.

The regs pointer is used in few places, but it potentially costs both stack
space and code to pass it around.  On the FRV arch, removing the regs parameter
from all the genirq function results in a 20% speed up of the IRQ exit path
(ie: from leaving timer_interrupt() to leaving do_IRQ()).

Where appropriate, an arch may override the generic storage facility and do
something different with the variable.  On FRV, for instance, the address is
maintained in GR28 at all times inside the kernel as part of general exception
handling.

Having looked over the code, it appears that the parameter may be handed down
through up to twenty or so layers of functions.  Consider a USB character
device attached to a USB hub, attached to a USB controller that posts its
interrupts through a cascaded auxiliary interrupt controller.  A character
device driver may want to pass regs to the sysrq handler through the input
layer which adds another few layers of parameter passing.

I've build this code with allyesconfig for x86_64 and i386.  I've runtested the
main part of the code on FRV and i386, though I can't test most of the drivers.
I've also done partial conversion for powerpc and MIPS - these at least compile
with minimal configurations.

This will affect all archs.  Mostly the changes should be relatively easy.
Take do_IRQ(), store the regs pointer at the beginning, saving the old one:

	struct pt_regs *old_regs = set_irq_regs(regs);

And put the old one back at the end:

	set_irq_regs(old_regs);

Don't pass regs through to generic_handle_irq() or __do_IRQ().

In timer_interrupt(), this sort of change will be necessary:

	-	update_process_times(user_mode(regs));
	-	profile_tick(CPU_PROFILING, regs);
	+	update_process_times(user_mode(get_irq_regs()));
	+	profile_tick(CPU_PROFILING);

I'd like to move update_process_times()'s use of get_irq_regs() into itself,
except that i386, alone of the archs, uses something other than user_mode().

Some notes on the interrupt handling in the drivers:

 (*) input_dev() is now gone entirely.  The regs pointer is no longer stored in
     the input_dev struct.

 (*) finish_unlinks() in drivers/usb/host/ohci-q.c needs checking.  It does
     something different depending on whether it's been supplied with a regs
     pointer or not.

 (*) Various IRQ handler function pointers have been moved to type
     irq_handler_t.

Theodore Ts'o 8e18e2941c [PATCH] inode_diet: Replace inode.u.generic_ip with inode.i_private
The following patches reduce the size of the VFS inode structure by 28 bytes
on a UP x86.  (It would be more on an x86_64 system).  This is a 10% reduction
in the inode size on a UP kernel that is configured in a production mode
(i.e., with no spinlock or other debugging functions enabled; if you want to
save memory taken up by in-core inodes, the first thing you should do is
disable the debugging options; they are responsible for a huge amount of bloat
in the VFS inode structure).

This patch:

The filesystem or device-specific pointer in the inode is inside a union,
which is pretty pointless given that all 30+ users of this field have been
using the void pointer.  Get rid of the union and rename it to i_private, with
a comment to explain who is allowed to use the void pointer.  This is just a
cleanup, but it allows us to reuse the union 'u' for something something where
the union will actually be used.

Currently acpiphp calls pci_enable_device() against all

Satoru Takeuchi 0dad3510ee acpiphp: stop bus device before acpi_bus_trim
Contrary to PCI bridge hot-add, we need to follow the sequence below
for PCI bridge hot-removal.

  (1) Stop devices (detach drivers, remove from the global list, etc.)
  (2) Unbind ACPI node from the devices (remove the _PRT entries)
  (3) Remove devices (remove from the device list, etc.)

This patch fixes acpiphp driver to follow above sequence for P2P
bridge hot-removal.

Randy.Dunlap 995982ca79 sysfs_remove_bin_file: no return value, dump_stack on error
Make sysfs_remove_bin_file() void.  If it detects an error,
printk the file name and call dump_stack().

sysfs_hash_and_remove() now returns an error code indicating
its success or failure so that sysfs_remove_bin_file() can
know success/failure.

Convert the only driver that checked the return value of
sysfs_remove_bin_file().

Here is a patch against the CPCI hotplug core to fix up PCI resource
assignment such that things will actually work when a hot inserted
device is enabled.  I mentioned this patch to you way back in April at
ELC, but am only now out from under things enough to clean it up and
submit it.  I've basically cribbed the corresponding code from
shpchp_pci.c, so there are no big surprises.  If it's still possible, I
wouldn't mind this going into 2.6.18, but it wouldn't be the end of the
world if it went into 2.6.19.

Andrew Morton 135c294fa3 [PATCH] don't select CONFIG_HOTPLUG
It's useful to be able to turn off CONFIG_HOTPLUG for compile-coverage testing
and for section-checking coverage.  But a few things go and select
CONFIG_HOTPLUG, making it a royal PITA to turn the thing off.

It's only turnable offable if CONFIG_EMBEDDED anyway.  So let's make those
things depend on HOTPLUG, not select it.

Kristen Accardi 95b38b3f45 ACPIPHP: prevent duplicate slot numbers when no _SUN
Dock bridges generally do not implement _SUN, yet show up as ejectable
slots.  If you have more than one ejectable slot that does not implement
SUN, with the current code you will get duplicate slot numbers.  So, if
there is no _SUN, use the current count of the number of slots found
instead.

Current SHPCHP driver doesn't take care of RsvdP/RsvdZ[*] bits in
controller SERR-INT register. This might cause unpredicable
results. This patch fixes this bug.

[*] RsvdP and RsvdZ are defined in SHPC spec as follows:

    RsvdP - Reserved and Preserved. Register bits of this type are
    reserved for future use as R/W bits. The value read is
    undefined. Writes are ignored. Software must follow These rules
    when accessing RsvdP bits:

	- Software must ignore RsvdP bits when testing values read
          from these registers.
	- Software must not depend on RsvdP bit's ability to retain
          information when written
	- Software must always write back the value read in the RsvdP
	  bits when writing one of these registers.

    RsvdZ - Reserved and Zero. Register bits of this type are reserved
    for future use as R/WC bits. The value read is undefined. Writes
    are ignored. Software must follow these rules when accessing RsvdZ
    bits:

Current SHPCHP driver doesn't take care of RsvdP/RsvdZ[*] bits
in logical slot registers. This might cause unpredicable results. This
patch fixes this bug.

[*] RsvdP and RsvdZ are defined in SHPC spec as follows:

    RsvdP - Reserved and Preserved. Register bits of this type are
    reserved for future use as R/W bits. The value read is
    undefined. Writes are ignored. Software must follow These rules
    when accessing RsvdP bits:

	- Software must ignore RsvdP bits when testing values read
          from these registers.
	- Software must not depend on RsvdP bit's ability to retain
          information when written
	- Software must always write back the value read in the RsvdP
	  bits when writing one of these registers.

    RsvdZ - Reserved and Zero. Register bits of this type are reserved
    for future use as R/WC bits. The value read is undefined. Writes

  notify handler for bridge. Because, the role of the
  handlers are not same. The hot-remove case is ditto.

John Rose e6ad00576f [PATCH] PCI: rpaphp: remove init error condition
The init function for the RPA PCI Hotplug driver returns -ENODEV in the
case that no hotplug-capable slots are detected in the system.  This is
bad, since hot-capable slots can be added after boot to a purely virtual
POWER partition.  This is also bad because DLPAR I/O operations depend
on the rpaphp module.

Change the rpaphp init module to return success for the case of
partitions that own no hotplug-capable slots at boot.  Such slots can be
dynamically added after boot.

Kristen Accardi 783c49fc50 [PATCH] PCI Hotplug: add common acpi functions to core
shpchprm_acpi.c and pciehprm_acpi.c are nearly identical.  In addition,
there are functions in both these files that are also in acpiphp_glue.c.
This patch will remove duplicate functions from shpchp, pciehp, and
acpiphp and move this functionality to pci_hotplug, as it is not
hardware specific.  Get rid of shpchprm* and pciehprm* files since they
are no longer needed.  shpchprm_nonacpi.c and pciehprm_nonacpi.c are
identical, as well as shpchprm_legacy.c and can be replaced with a
macro.

This patch also changes acpiphp to use the common hpp code.

Kristen Accardi 20416ea540 [PATCH] acpiphp: add dock event handling
These patches add generic dock event handling to acpiphp.  If there are
pci devices that need to be inserted/removed after the dock event, the
event notification will be handed down to the normal pci hotplug event
handler in acpiphp so that new bridges/devices can be enumerated.

Because some dock stations do not have pci bridges or pci devices that
need to be inserted after a dock, acpiphp will remain loaded to handle
dock events even if no hotpluggable pci slots are discovered.

You probably need to have the pci=assign-busses kernel parameter enabled
to use these patches, and you may not allow ibm_acpi to handle docking
notifications and use this patch.

This patch incorporates feedback provided by many.

Kenji Kaneshige f7391f5325 [PATCH] shpchp: event handling rework
The event handler of SHPCHP driver is unnecessarily very complex. In
addition, current event handler can only a fixed number of events at
the same time, and some of events would be lost if several number of
events happened at the same time.

This patch simplify the event handler by using 'work queue', and it
also fix the above-mentioned issue.

Kenji Kaneshige e4e73041ec [PATCH] shpchp - Fix incorrect return value of interrupt handler
Current SHPCHP driver has a bug in its interrupt handler which cause
"IRQ #: nobody cared" oops. This problem can be reproduced easily by
the following operation.

    # cd /sys/bus/pci/slots/<slot#>
    # while true; do echo 1 > attention ; done &

The reason is that when command complete interrupt is raised, current
SHPCHP driver's interrupt handler returns IRQ_NONE regardless of if
the interrupt is handled or not.

This patch fixes this issue.

This patch fixes the AMD POGO errata on the hotplug controller where the
platform will lock up or reboot if PERR/SERR generation is enabled and a
slot is sent an enable command.  This fix disables PERR/SERR generation
before a slot is sent the enable command by first saving related
registers, turning off SERR/PERR generation, enabling the slot, then
restoring the registers.

linas 3c0c644188 [PATCH] PCI Hotplug: PCI panic on dlpar add (add pci slot to running partition)
Removing and then adding a PCI slot to a running partition results in
a kernel panic. The current code attempts to add iospace for an entire
root bus, which is inappropriate, and silently fails.  When a pci device
tries to use the iospace, a page fault is taken, as the iospace had not
been mapped, and of course the page fault cannot be resolved.

This only occurs for PCI adapters using pio, which may be why it hadn't
been seen earlier (this seems to have been broken for a while).
This patch has survived testing of dozens of slot add and removes.

This patch disables power fault, MRL sensor and presence detection
interrupts when a PCIe slot is powered-off and enables those
interrupts when it is powered-on again. This is necessary to prevent
the associated events from causing an endless cycle of interrupts
due to the power-fault bit, which stays set till power is restored
to the slot.

Current SHPCHP driver uses msleep_interruptible() function to wait for
a command completion event. But I think this would cause an unnecessary
long wait until timeout, if command completion interrupt came before
task state was changed to TASK_INTERRUPTIBLE. This patch fixes this
issue. With this patch, command completion becomes faster as follows:

Rajesh Shah 5a49f2036a [PATCH] PCI Express Hotplug: clear sticky power-fault bit
Per the PCI Express spec, the power-fault-detected bit in the
slot status register can be set anytime hardware detects a power
fault, regardless of whether the slot has a device populated in
it or not. This bit is sticky and must be explicitly cleared.
This patch is needed to allow hot-add after such a power fault
has been detected.

This patch further tweaks how we request control of hotplug
controller hardware from BIOS. We first search the ACPI namespace
corresponding to a specific hotplug controller looking for an
_OSC or OSHP method. On failure, we successively move to the
ACPI parent object, till we hit the highest level host bridge
in the hierarchy. This allows for different types of BIOS's
which place the _OSC/OSHP methods at various places in the acpi
namespace, while still not encroaching on the namespace of
some other root level host bridge.

This patch also introduces a new load time option (pciehp_force)
that allows us to bypass all _OSC/OSHP checking. Not supporting
these methods seems to be be the most common ACPI firmware problem
we've run into. This will still _not_ allow the pciehp driver to
work correctly if the BIOS really doesn't support pciehp (i.e. if
it doesn't generate a hotplug interrupt). Use this option with
caution.  Some BIOS's may deliberately not build any _OSC/OSHP
methods to make sure it retains control the hotplug hardware.
Using the pciehp_force parameter for such systems can lead to
two separate entities trying to control the same hardware.

all host bridges. This allows us to gain control of each hotplug
controller individually, even if BIOS fails to give us control of
some other hotplug controller in the system.

system BIOS to the hotplug driver, and to run the _HPP
method to get hotplug device parameters like cache line size,
latency timer and SERR/PERR enable from BIOS.

Note that one of the side effects of this patch is that pciehp
does not automatically enable the hot-added device or its DMA
bus mastering capability now. It expects the device driver to
do that. This may break some drivers and we will have to fix
them as they are reported.

-- The crash was due to the PHB not having a pci_dn structure yet,

underneath it, 17 times so far, as of this writing.

Tim Schmielau 4e57b68178 [PATCH] fix missing includes
I recently picked up my older work to remove unnecessary #includes of
sched.h, starting from a patch by Dave Jones to not include sched.h
from module.h. This reduces the number of indirect includes of sched.h
by ~300. Another ~400 pointless direct includes can be removed after
this disentangling (patch to follow later).
However, quite a few indirect includes need to be fixed up for this.

In order to feed the patches through -mm with as little disturbance as
possible, I've split out the fixes I accumulated up to now (complete for
i386 and x86_64, more archs to follow later) and post them before the real
patch.  This way this large part of the patch is kept simple with only
adding #includes, and all hunks are independent of each other.  So if any
hunk rejects or gets in the way of other patches, just drop it.  My scripts
will pick it up again in the next round.

The shpc driver registers its probe function for all pci-pci
bridges in the system. Not all of them will be shpc capable, so
look for this capability early in the probe function and return

did some initialization work on all bridges before detecting
that shpc is not supported and unwinds the work it's already done
in that case.

Rajesh Shah 7c56075e60 [PATCH] PCI: Fix PCI bus mastering enable problem in pciehp
Martin Franc reported that the pciehp driver was not enabling bus
master capability on his hot-plugged card. pciehprm_enable_card()
was updating the PCI command register only if _HPP indicated a
value for SERR or PERR that was different from the current setting.
I don't have hardware that reproduces this problem, but Martin
reports that this patch fixes the problem for him.

Paul Mackerras 1635317fac [PATCH] Separate pci bits out of struct device_node
This patch pulls the PCI-related junk out of struct device_node and
puts it in a separate structure, struct pci_dn.  The device_node now
just has a void * pointer in it, which points to a struct pci_dn for
nodes that represent PCI devices.  It could potentially be used in
future for device-specific data for other sorts of devices, such as
virtual I/O devices.

These fixes were suggested by pcihpd-discuss, but were dropped in the
initial checkin of the code.  These fixes include cleaning up the
hotplug driver sysfs filename, and some minor code cleanups.  The driver
also requires at least PROM 4.30, not 4.20.

John Rose 56d8456b06 [PATCH] PCI Hotplug: rpaphp: Purify hotplug
Currently rpaphp registers the following bus types as hotplug slots:
1) Actual PCI Hotplug slots
2) Embedded/Internal PCI slots
3) PCI Host Bridges

The second and third bus types are not actually direct parents of
removable adapters.  As such, the rpaphp has special case code to fake
results for attributes like power, adapter status, etc.  This patch
removes types 2 and 3 from the rpaphp module.

This patch also changes the DLPAR module so that slots can be
DLPAR-added/removed without having been designated as hotplug-capable.

John Rose 9c209c919d [PATCH] PCI Hotplug: rpaphp: Change slot pci reference
The slot structure in the rpaphp module currently references the PCI
contents of the slot using the PCI device of the parent bridge.  This
is unnecessary, since the module is actually interested in the
subordinate bus of the bridge.  The dependency on a PCI bridge device
also prohibits the module from registering hotplug slots that have a
root bridge as a parent, since root bridges on PPC64 don't have PCI
devices.

This patch changes struct slot to reference the PCI subsystem using a
pci_bus rather than a pci_dev.

Currently, rpaphp registers Virtual I/O slots as hotplug slots.  The
only purpose of this registration is to ensure that the VIO subsystem
is notified of new VIO buses during DLPAR adds.  Similarly, rpaphp
notifies the VIO subsystem when a VIO bus is DLPAR-removed.  The rpaphp
module has special case code to fake results for attributes like power,
adapter status, etc.

The VIO register/unregister functions could just as easily be made from
the DLPAR module.  This patch moves the VIO registration calls to the
DLPAR module, and removes the VIO fluff from rpaphp altogether.

Keith Moore 4002307d2b [PATCH] cpqphp: fix oops during unload without probe
drivers/pci/hotplug/cpqphp_core.c calls cpqphp_event_start_thread()
in one_time_init(), which is called whenever the hardware is probed.
Unfortunately, cpqphp_event_stop_thread() is *always* called when
the module is unloaded. If the hardware is never probed, then
cpqphp_event_stop_thread() tries to manipulate a couple of
uninitialized mutexes.

Rajesh Shah 8d50e332c8 [PATCH] acpi hotplug: decouple slot power state changes from physical hotplug
Current acpiphp code does not distinguish between the physical presence and
power state of a device/slot.  That is, if a device has to be disabled, it
also tries to physically ejects the device.  This patch decouples power state
from physical presence.  You can now echo to the corresponding sysfs power
control file to repeatedly enable and disable a device without having to
physically re-insert it.

Rajesh Shah 364d5094a4 [PATCH] acpi hotplug: clean up notify handlers on acpiphp unload
A root bridge may not have directly attached hotpluggable slots under it.
Instead, it may have p2p bridges with slots under it.  In this case, we need
to clean up the p2p bridges and slots properly too.  Patch below applies on
top of the original patch, and fixes this problem.  Without this, acpiphp
leaves behind notify handlers on module unload, and subsequent module load
attempts don't work properly too.  Patch was tested on an ia64 Tiger4 box.

Rajesh Shah 42f49a6ae5 [PATCH] acpi hotplug: convert acpiphp to use generic resource code
This patch converts acpiphp to use the generic PCI resource assignment code.
It's quite large, but most of it is deleting the acpiphp_pci and acpiphp_res
files.  It's tested on an hp Integrity rx8620 (which won't work without this
patch).  Testers with other hardware welcomed.

Here is my third attempt at a patch to further update the CompactPCI
hotplug driver infrastructure to address the pci_enable_device issue
discussed on the list as well as a few other issues I discovered during
some more testing.  This version addresses a few more issues pointed out
by Prarit Bhargava.  Changes include:
- cpci_enable_device and its recursive calling of pci_enable_device on
  new devices removed.
- Use list_rwsem to avoid slot status change races between disable_slot
  and check_slots.
- Fixed oopsing in cpci_hp_unregister_bus caused by calling list_del on
  a slot after calling pci_hp_deregister.
- Removed kfree calls in cleanup_slots since release_slot will have
  done it already.
- Reworked init_slots a bit to fix latch and adapter file updating on
  subsequent calls to cpci_hp_start.
- Improved sanity checking in cpci_hp_register_controller.
- Now shut things down correctly in cpci_hotplug_exit.
- Switch to pci_get_slot instead of deprecated pci_find_slot.
- A bunch of CodingStyle fixes.

[PATCH] CPCI: update

I have finally done some work to update the CompactPCI hotplug driver to
fix some of the outstanding issues in 2.6:
- Added adapter and latch status ops so that those files will get created
  by the current PCI hotplug core.  This used to not be required, but
  seems to be now after some of the sysfs rework in the core.
- Replaced slot list spinlock with a r/w semaphore to avoid any potential
  issues with sleeping.  This quiets all of the runtime warnings.
- Reworked interrupt driven hot extraction handling to remove need for a
  polling operator for ENUM# status.  There are a lot of boards that only
  have an interrupt driven by ENUM#, so this lowers the bar to entry.
- Replaced pci_visit_dev usage with better use of the PCI core functions.
  The new code is functionally equivalent to the previous code, but the
  use of pci_enable_device on insert needs to be investigated further, as
  I need to do some more testing to see if it is still necessary.

here is the patch that fixes the bug introduced by my previous patch which
already went into 2.6.12-rc2 and is likely to cause trouble is someone hits
one the else case here by accident.

Using the &= operation before the if statement destroys the information the
if asks for so we always go into the else branch.

Initial git repository build. I'm not bothering with the full history,
even though we have it. We can create a separate "historical" git
archive of that later if we want to, and in the meantime it's about
3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.
