ESE (Extent Space Efficient) volumes are thin-provisioned and therefore
space is only occupied with real data. In order to make previously used
space available for re-allocation again, discard support is enabled for
ESE volumes allowing the DASD driver to release said space.

Userspace tools might have the need to release space for Extent Space
Efficient (ESE) volumes when working with such a device.
Provide the necessarry interface for such a task by implementing a new
ioctl BIODASDRAS. The ioctl uses the format_data_t data structure for
data input:

There is dasd_sleep_on() and dasd_sleep_on_interruptible() to start CCW
requests uninterruptible and interruptible. However, there is only
dasd_sleep_on_queue() to start requests from CCW queues uninterruptible.
Add dasd_sleep_on_queue_interruptible() to provide a way to start
requests from CCW queues interruptible. _dasd_sleep_on_queue() already
provides this functionality.

A dynamic formatting is issued whenever a write request returns with
either a No Record Found error (Command Mode), Incorrect Length error
(Transport Mode), or File Protected error (Transport Mode). All three
cases mean that the tracks in question haven't been initialized in a
desired format yet.
The part of the volume that was tried to be written on is then formatted
and the original request is re-queued.
As the formatting will happen during normal I/O operations, it is quite
likely that there won't be any memory available to build the respective
request. Another two pages of memory are allocated per volume
specifically for the dynamic formatting.
The dasd_eckd_build_format() function is extended to make sure that the
original startdev is reused. Also, all formatting and format check
functions use the new memory pool exclusively now to reduce complexity.
Read operations will always return zero data when unformatted areas are
read.

In order to work with Extent Space Efficient (ESE) volumes, certain
viable information about those volumes and the corresponding extent
pool (such as extent size, configured space, allocated space, etc.) can
be provided.
Use the CCW commands Volume Storage Query and Logical Configuration
Query to receive detailed information about ESE volumes and the extent
pool respectively. These information are made accessible via internal
functions for subsequent users, and via sysfs attributes for userpsace
usage.
The new sysfs attributes reside in separate directories called capacity
and extent_pool.

The disk layout and volume information of a DASD reside in the first two
tracks of cylinder 0. When a DASD is set online, currently the first
three tracks are read and analysed to confirm an expected layout.
For CDL (Compatible Disk Layout) only count area data of the first track
is evaluated and checked against expected key and data lengths. For LDL
(Linux Disk Layout) the first and third track is evaluated. However,
an LDL formatted volume is expected to be in the same format across all
tracks. Checking the third track therefore doesn't have any more value
than checking any other track at random.
Now, an Extent Space Efficient (ESE) DASD is initialised by only
formatting the first two tracks, as those tracks always contain all
information necessarry.
Checking the third track on an ESE volume will therefore most likely
fail with a record not found error, as the third track will be empty.
This in turn leads to the device being recognised with a volume size of
0. Attempts to write volume information on the first two tracks then
fail with "no space left on device" errors.
Initialising the first three tracks for an ESE volume is not a viable
solution, because the third track is already a regular track and could
contain user data. With that there is potential for data corruption.
Instead, always only analyse the first two tracks, as it is sufficiant
for both CDL and LDL, and allow ESE volumes to be recognised as well.

Pankaj reports that starting with commit ad428cdb52 "dax: Check the
end of the block-device capacity with dax_direct_access()" device-mapper
no longer allows dax operation. This results from the stricter checks in
__bdev_dax_supported() that validate that the start and end of a
block-device map to the same 'pagemap' instance.
Teach the dax-core and device-mapper to validate the 'pagemap' on a
per-target basis. This is accomplished by refactoring the
bdev_dax_supported() internals into generic_fsdax_supported() which
takes a sector range to validate. Consequently generic_fsdax_supported()
is suitable to be used in a device-mapper ->iterate_devices() callback.
A new ->dax_supported() operation is added to allow composite devices to
split and route upper-level bdev_dax_supported() requests.

The DASD driver incorrectly limits the maximum number of blocks of ECKD
DASD volumes to 32 bit numbers. Volumes with a capacity greater than
2^32-1 blocks are incorrectly recognized as smaller volumes.
This results in the following volume capacity limits depending on the
formatted block size:

The same problem occurs when a volume with more than 17895697 cylinders

The dasd_eckd_restore_device() function calls dasd_generic_read_dev_chars
with a temporary buffer on the stack. With CONFIG_VMAP_STACK=y this is
a vmalloc address but dasd_generic_restore_device() uses the address of
the buffer as I/O address. Circumvent this by using the already allocated
cqr->data buffer for the RDC data.

Update device_add_disk() to take an 'groups' argument so that

Fix a panic that occurs for a device that got an error in
dasd_eckd_check_characteristics() during online processing.
For example the read configuration data command may have failed.
If this error occurs the device is not being set online and the earlier
invoked steps during online processing are rolled back. Therefore
dasd_eckd_uncheck_device() is called which needs a valid private
structure. But this pointer is not valid if
dasd_eckd_check_characteristics() has failed.
Check for a valid device->private pointer to prevent a panic.

dcssblk_direct_access() needs to check the validity of pointers kaddr
and pfn for NULL assignment. If anyone equals to NULL, it doesn't need
to calculate the value.
If either of them is equal to NULL, that is to say callers may
have no need for kaddr or pfn, so this patch is prepared for allowing
them to pass in NULL instead of having to pass in a pointer or local
variable that they then just throw away.

Reduce the default values for the number of hardware queues and queue depth
to significantly reduce the memory footprint of a DASD device.
The memory consumption per DASD device reduces from approximately 40MB to
approximately 1.5MB.
This is necessary to build systems with a large number of DASD devices and
a reasonable amount of memory.
Performance measurements showed that good performance results are possible
with the new default values even on systems with lots of CPUs and lots of
alias devices.

Let the block layer allocate per request data to store
struct dasd_ccw_req. We still need extra preallocated
memory for usage by ccw programs (which vary in length)
and for requests which don't originate from the block
layer.

However, the memcpy_mcsafe() facility is available through the pmem
block driver. In order to safely handle media errors, via the DAX

The BLK_EH_NOT_HANDLED implies nothing happen, but very often that

Provide __dasd_cancel_req that is called with the ccw device lock
held to simplify the locking in dasd_times_out. Also this removes
the following sparse warning:
context imbalance in 'dasd_times_out' - different lock contexts for basic block
Note: with this change dasd_schedule_device_bh is now called (via
dasd_cancel_req) with the ccw device lock held. But is is already
the case for other codepaths.

Similar to the ->copy_from_iter() operation, a platform may want to
deploy an architecture or device specific routine for handling reads
from a dax_device like /dev/pmemX. On x86 this routine will point to a
machine check safe version of copy_to_iter(). For now, add the plumbing
to device-mapper and the dax core.

Dasd uses completion_data from struct request to store per request
private data - this is problematic since this member is part of a
union which is also used by IO schedulers.
Let the block layer maintain space for per request data behind each
struct request.

Half of the branch up to commit d2c997c0f1 ("fs, dax: use
page->mapping to warn...") have been in -next for several releases.
The of_pmem driver and the address range scrub rework were late
arrivals, and the dax work was scaled back at the last moment.
The of_pmem driver missed a previous merge window due to an oversight.
A sense of obligation to rectify that miss is why it is included for
4.17. It has acks from PowerPC folks. Stephen reported a build failure
that only occurs when merging it with your latest tree, for now I have
fixed that up by disabling modular builds of of_pmem. A test merge
with your tree has received a build success report from the 0day robot
over 156 configs.
An initial version of the ARS rework was submitted before the merge
window. It is self contained to libnvdimm, a net code reduction, and
passing all unit tests.
The filesystem-dax changes are based on the wait_var_event()
functionality from tip/sched/core. However, late review feedback
showed that those changes regressed truncate performance to a large
degree. The branch was rewound to drop the truncate behavior change
and now only includes preparation patches and cleanups (with full acks
and reviews). The finalization of this dax-dma-vs-trnucate work will
need to wait for 4.18.

In support of allowing device-mapper to compile out idle/dead code when
there are no dax providers in the system, introduce the DAX_DRIVER
symbol. This is selected by all leaf drivers that device-mapper might be
layered on top. This allows device-mapper to conditionally 'select DAX'
only when a provider is present.

Internal DASD device driver I/O such as query host access count or
path verification is started using the _sleep_on() function.
To mark a request as started or ended the callback_data is set to either
DASD_SLEEPON_START_TAG or DASD_SLEEPON_END_TAG.
In cases where the request has to be stopped unconditionally the status is
set to DASD_SLEEPON_END_TAG as well which leads to immediate clearing of
the request.
But the request might still be on a device request queue for normal
operation which might lead to a panic because of a BUG() statement in
__dasd_device_process_final_queue() or a list corruption of the device
request queue.
Fix by removing the setting of DASD_SLEEPON_END_TAG in the
dasd_cancel_req() and dasd_generic_requeue_all_requests() functions and
ensure that the request is not deleted in the requeue function.
Trigger the device tasklet in the requeue function and let the normal
processing cleanup the request.

The next patch from Al will sort out the final differences, and we
should be all done.

Pull poll annotations from Al Viro:
"This introduces a __bitwise type for POLL### bitmap, and propagates
the annotations through the tree. Most of that stuff is as simple as
'make ->poll() instances return __poll_t and do the same to local
variables used to hold the future return value'.
Some of the obvious brainos found in process are fixed (e.g. POLLIN
misspelled as POLL_IN). At that point the amount of sparse warnings is
low and most of them are for genuine bugs - e.g. ->poll() instance
deciding to return -EINVAL instead of a bitmap. I hadn't touched those
in this series - it's large enough as it is.
Another problem it has caught was eventpoll() ABI mess; select.c and
eventpoll.c assumed that corresponding POLL### and EPOLL### were
equal. That's true for some, but not all of them - EPOLL### are
arch-independent, but POLL### are not.
The last commit in this series separates userland POLL### values from
the (now arch-independent) kernel-side ones, converting between them
in the few places where they are copied to/from userland. AFAICS, this
is the least disruptive fix preserving poll(2) ABI and making epoll()
work on all architectures.
As it is, it's simply broken on sparc - try to give it EPOLLWRNORM and
it will trigger only on what would've triggered EPOLLWRBAND on other
architectures. EPOLLWRBAND and EPOLLRDHUP, OTOH, are never triggered
at all on sparc. With this patch they should work consistently on all
architectures"

In dasd_term_IO() ccw_device_clear() is called and the return code is
checked afterwards. Though, the return codes -EIO and -EBUSY will never
be returned and can therefore be removed from the check.
In dasd_start_IO() the return code of either ccw_device_tm_start() or
ccw_device_start() is checked. However, neither of them returns
-ETIMEDOUT. Remove that check as well.

If a dax buffer from a device that does not map pages is passed to
read(2) or write(2) as a target for direct-I/O it triggers SIGBUS. If
gdb attempts to examine the contents of a dax buffer from a device that
does not map pages it triggers SIGBUS. If fork(2) is called on a process
with a dax mapping from a device that does not map pages it triggers
SIGBUS. 'struct page' is required otherwise several kernel code paths
break in surprising ways. Disable filesystem-dax on devices that do not
map pages.
In addition to needing pfn_to_page() to be valid we also require devmap
pages. We need this to detect dax pages in the get_user_pages_fast()
path and so that we can stop managing the VM_MIXEDMAP flag. For DAX
drivers that have not supported get_user_pages() to date we allow them
to opt-in to supporting DAX with the CONFIG_FS_DAX_LIMITED configuration
option which requires ->direct_access() to return pfn_t_special() pfns.
This leaves DAX support in brd disabled and scheduled for removal.
Note that when the initial dax support was being merged a few years back
there was concern that struct page was unsuitable for use with next
generation persistent memory devices. The theoretical concern was that
struct page access, being such a hotly used data structure in the
kernel, would lead to media wear out. While that was a reasonable
conservative starting position it has not held true in practice. We have
long since committed to using devm_memremap_pages() to support higher
order kernel functionality that needs get_user_pages() and
pfn_to_page().

In support of removing the VM_MIXEDMAP indication from DAX VMAs,
introduce pfn_t_special() for drivers to indicate that _PAGE_SPECIAL
should be used for DAX ptes. This also helps identify drivers like
dccssblk that only want to use DAX in a read-only fashion without
get_user_pages() support.
Ideally we could delete axonram and dcssblk DAX support, but if we need
to keep it better make it explicit that axonram and dcssblk only support
a sub-set of DAX due to missing _PAGE_DEVMAP support.

We store per path and per device configuration data to identify the
path or device correctly. The per path configuration data might get
mixed up if the original request gets into error recovery and is
started with a random path mask.
This would lead to a wrong identification of a path in case of a CUIR
event for example.
Fix by copying the path mask from the original request to the error
recovery request in case it is a path verification request.

Prevent that a prefix flag is set based on invalid configuration data.
The validity.verify_base flag should only be set for alias devices.
Usually the unit address type is either one of base, PAV alias or
HyperPAV alias. But in cases where the unit address type is not set or
any other value the validity.verify_base flag might be set as well.
This would lead to follow on errors.
Explicitly check for alias devices and set the validity flag only for
them.

getnstimeofday() and timespec are deprecated since they can
overflow on 32-bit architectures. This simply changes to the
explicitly typed timespec64 version that doesn't have that
problem.
It would be nice to also convert to monotonic timestamps
and call ktime_get_ts64() rather than ktime_get_real_ts64(),
but that would be a user-visible change.

This mechanically converts all remaining cases of ancient open-coded timer
setup with the old setup_timer() API, which is the first step in timer
conversions. This has no behavioral changes, since it ultimately just
changes the order of assignment to fields of struct timer_list when
finding variations of:

Many source files in the tree are missing licensing information, which
makes it harder for compliance tools to determine the correct license.
By default all files without license information are under the default
license of the kernel, which is GPL version 2.
Update the files which contain no license information with the 'GPL-2.0'
SPDX license identifier. The SPDX identifier is a legally binding
shorthand, which can be used instead of the full boiler plate text.
This patch is based on work done by Thomas Gleixner and Kate Stewart and
Philippe Ombredanne.
How this work was done:
Patches were generated and checked against linux-4.14-rc6 for a subset of
the use cases:
- file had no licensing information it it.
- file was a */uapi/* one with no licensing information in it,
- file was a */uapi/* one with existing licensing information,
Further patches will be generated in subsequent months to fix up cases
where non-standard license headers were used, and references to license
had to be inferred by heuristics based on keywords.
The analysis to determine which SPDX License Identifier to be applied to
a file was done in a spreadsheet of side by side results from of the
output of two independent scanners (ScanCode & Windriver) producing SPDX
tag:value files created by Philippe Ombredanne. Philippe prepared the
base worksheet, and did an initial spot review of a few 1000 files.
The 4.13 kernel was the starting point of the analysis with 60,537 files
assessed. Kate Stewart did a file by file comparison of the scanner
results in the spreadsheet to determine which SPDX license identifier(s)
to be applied to the file. She confirmed any determination that was not
immediately clear with lawyers working with the Linux Foundation.
Criteria used to select files for SPDX license identifier tagging was:
- Files considered eligible had to be source code files.
- Make and config files were included as candidates if they contained >5
lines of source
- File already had some variant of a license header in it (even if <5
lines).
All documentation files were explicitly excluded.
The following heuristics were used to determine which SPDX license
identifiers to apply.
- when both scanners couldn't find any license traces, file was
considered to have no license information in it, and the top level
COPYING file license applied.
For non */uapi/* files that summary was:

- when the two scanners agreed on the detected license(s), that became
the concluded license(s).
- when there was disagreement between the two scanners (one detected a
license but the other didn't, or they both detected different
licenses) a manual inspection of the file occurred.
- In most cases a manual inspection of the information in the file
resulted in a clear resolution of the license that should apply (and
which scanner probably needed to revisit its heuristics).
- When it was not immediately clear, the license identifier was
confirmed with lawyers working with the Linux Foundation.
- If there was any question as to the appropriate license identifier,
the file was flagged for further research and to be revisited later
in time.
In total, over 70 hours of logged manual review was done on the
spreadsheet to determine the SPDX license identifiers to apply to the
source files by Kate, Philippe, Thomas and, in some cases, confirmation
by lawyers working with the Linux Foundation.
Kate also obtained a third independent scan of the 4.13 code base from
FOSSology, and compared selected files where the other two scanners
disagreed against that SPDX file, to see if there was new insights. The
Windriver scanner is based on an older version of FOSSology in part, so
they are related.
Thomas did random spot checks in about 500 files from the spreadsheets
for the uapi headers and agreed with SPDX license identifier in the
files he inspected. For the non-uapi files Thomas did random spot checks
in about 15000 files.
In initial set of patches against 4.14-rc6, 3 files were found to have
copy/paste license identifier errors, and have been fixed to reflect the
correct identifier.
Additionally Philippe spent 10 hours this week doing a detailed manual
inspection and review of the 12,461 patched files from the initial patch
version early this week with:
- a full scancode scan run, collecting the matched texts, detected
license ids and scores
- reviewing anything where there was a license detected (about 500+
files) to ensure that the applied SPDX license was correct
- reviewing anything where there was no detection but the patch license
was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied
SPDX license was correct
This produced a worksheet with 20 files needing minor correction. This
worksheet was then exported into 3 different .csv files for the
different types of files to be modified.
These .csv files were then reviewed by Greg. Thomas wrote a script to
parse the csv files and add the proper SPDX tag to the file, in the
format that the file expected. This script was further refined by Greg
based on the output to detect more types of files automatically and to
distinguish between header and source .c files (which need different
comment types.) Finally Greg ran the script using the .csv files to
generate the patches.

debug_event currently truncates the data if used with a size larger than
the buf_size of the debug feature. For lots of callers of this function,
wrappers have been implemented that loop until all data is handled.
Move that functionality into debug_event_common and get rid of the wrappers.

The z/VM hypervisor provides virtual disks (VDISK) which are backed by
main memory of the hypervisor. Those devices are seen as DASD FBA disks
within the Linux guest.
Whenever data is written to such a device, memory is allocated
on-the-fly by z/VM accordingly. This memory, however, is not being freed
if data on the device is deleted by the guest OS.
In order to make memory usable after deletion again, add discard support
to the FBA discipline.
While at it, update comments regarding the DASD_FEATURE_* flags.

This way we don't need a block_device structure to submit I/O. The
block_device has different life time rules from the gendisk and
request_queue and is usually only available when the block device node
is open. Other callers need to explicitly create one (e.g. the lightnvm
passthrough code, or the new nvme multipathing code).
For the actual I/O path all that we need is the gendisk, which exists
once per block device. But given that the block layer also does
partition remapping we additionally need a partition index, which is
used for said remapping in generic_make_request.
Note that all the block drivers generally want request_queue or
sometimes the gendisk, so this removes a layer of indirection all
over the stack.

Unsigned long long and unsigned long were different in size for 31-bit.
For 64-bit the size for both datatypes is 8 Bytes and since the support
for 31-bit is long gone we can clean up a little and change everything
to unsigned long.
Change get_phys_clock() along the way to accept unsigned long as well so
that the DASD code can be consistent.

Since commit caf7df1227 ("block: remove the errors field from struct request")
rq->errors can't be (mis)used by block device drivers to store the error
condition for usage during async completion. Because of that I simply used
async completion only for the non-error paths.
This patch places the error within the private data of struct request and
uses async completion for all paths again.

The Prefix CCW is not mandatory and raw I/O can also be issued without
it. Check whether the Prefix CCW is supported and if not use the
combination of Define Extent and Locate Record Extended instead.
While at it, sort the variable declarations, replace the gotos with
early exits, and remove an error check at the end which is irrelevant.
Also, remove the XRC check as it is not relevant for raw I/O.

We already have define_extent() that prepares necessary data for the
Define Extent CCW. The exact same thing is done in prefix_LRE().
Remove the duplicate code and move commands that were only used in
combination with the Prefix command to define_extent(). One of these
commands needs the blocksize to be specified. Add the blksize parameter
to define_extent() to account for that.
In addition, the check_XRC() function can be made more generic. Do this
and remove the Prefix-specific check_XRC_on_prefix() function.
Furthermore, prefix_LRE() uses fill_LRE_data() to prepare Locate Record
Extended data. Rename the function to fit the scheme better and make it
usable outside of the Prefix context by adding the corresponding CCW
command.

If a device is offline it can still be set to read-only via the bus id
through sysfs. Only the read-only feature flag for the ccw_device is
then set. If the device is online the corresponding block device needs
to be set to read-only as well (via set_disk_ro()).
The check whether there is a device to do so, however, happens after the
feature flag was set. This leads to an unnecessary "no such device"
error in the offline case.
This bug was introduced by commit 7571cb1c8e3cc ("s390/dasd: Make use of
dasd_set_feature() more often"). Fix this by simply returning count if
no device is available.

blk_queue_split() is always called with the last arg being q->bio_split,
where 'q' is the first arg.
Also blk_queue_split() sometimes uses the passed-in 'bs' and sometimes uses
q->bio_split.
This is inconsistent and unnecessary. Remove the last arg and always use
q->bio_split inside blk_queue_split()

The safe offline processing may hang forever because it waits for I/O
which can not be started because of the offline flag that prevents new
I/O from being started.
Allow I/O to be started during safe offline processing because in this
special case we take care that the queues are empty before throwing away
the device.

The safe offline processing needs, as well as the normal offline
processing, to be locked against multiple parallel executions. But it
should be able to be overtaken by a normal offline processing to make sure
that the device does not wait forever for outstanding I/O if the user
wants to.
Unfortunately the parallel processing of safe offline and normal offline
might lead to a race situation where both threads report successful
execution to the CIO layer which in turn tries to deregister the kobject
of the device twice. This leads to a
refcount_t: underflow; use-after-free.
error and the device is not able to be set online again afterwards without
a reboot.
Correct the locking of the safe offline processing by doing the following:
- Use the cdev lock to secure all set and test operations to the
device flags.
- Two safe offline processes are locked against each other using
the DASD_FLAG_SAFE_OFFLINE and DASD_FLAG_SAFE_OFFLINE_RUNNING
device flags.
The differentiation between offline triggered and offline running
is needed since the normal offline attribute is owned by CIO and
we have to pass over control in between.
- The dasd_generic_set_offline process handles the offline
processing. It is locked against parallel execution using the
DASD_FLAG_OFFLINE.
- Only a running safe offline should be able to be overtaken by a
single normal offline. This is ensured by clearing the
DASD_FLAG_SAFE_OFFLINE_RUNNING flag when a normal offline
overtakes. So this can only happen ones.
- The safe offline just aborts in this case doing nothing and
the normal offline processing finishes as usual.

We have two flags, DASD_FLAG_DEVICE_RO and DASD_FEATURE_READONLY, that
tell us whether a device is read-only. DASD_FLAG_DEVICE_RO is set when a
device is attached as read-only to z/VM and DASD_FEATURE_READONLY is set
when either the corresponding kernel parameter is configured, or the
read-only state is changed via sysfs.
This is valuable information in any case. However, only the feature flag
is being checked at the moment when we display the current state.
Fix this by checking both flags.

Currently we use nornal Linux errno values in the block layer, and while
we accept any error a few have overloaded magic meanings. This patch
instead introduces a new blk_status_t value that holds block layer specific
status codes and explicitly explains their meaning. Helpers to convert from
and to the previous special meanings are provided for now, but I suspect
we want to get rid of them in the long run - those drivers that have a
errno input (e.g. networking) usually get errnos that don't know about
the special block layer overloads, and similarly returning them to userspace
will usually return somethings that strictly speaking isn't correct
for file system operations, but that's left as an exercise for later.
For now the set of errors is a very limited set that closely corresponds
to the previous overloaded errno values, but there is some low hanging
fruite to improve it.
blk_status_t (ab)uses the sparse __bitwise annotations to allow for sparse
typechecking, so that we can easily catch places passing the wrong values.

Setup a dax_dev to have the same lifetime as the dcssblk block device
and add a ->direct_access() method that is equivalent to
dcssblk_direct_access(). Once fs/dax.c has been converted to use
dax_operations the old dcssblk_direct_access() will be removed.

On some z/VM systems the query host access command is not supported for
temp disks, though the corresponding feature code is set.
This does not have any impact beside that the information is not available.
Suppress the full blown command reject error messages to not confuse the
user. The error is still logged in the s390dbf.

Since commit dd22f551 "block: Change direct_access calling convention",
the device size calculation in dcssblk_direct_access() is off-by-one.
This results in bdev_direct_access() always returning -ENXIO because the
returned value is not page aligned.
Fix this by adding 1 to the dev_sz calculation.

If safe offline is called for a DASD alias device a null pointer is passed
to fsync_bdev. So check for existence of the blockdevice before calling
fsync_bdev.
Should not be a real world problem since safe offline for an alias device
does not make sense and fsync_bdev can deal with a NULL pointer which it
gets after successful NULL pointer dereferencing on s390.

The function dasd_busid() still uses simple_strtoul() to convert a
string to an integer value. This function is obsolete for quite some
time already and should be replaced.
The whole parameter parsing semantic still relies somewhat on the fact,
that simple_strtoul() parses a string containing literals without
complains and just returns the parsed integer value plus the residual
string. kstrtoint(), however, would return -EINVAL in such a case.
Since we want to get rid of simple_strtoul() and now have a nice dasd[]
containing only single elements, we can clean up and simplify a few
things.
Replace simple_strtoul() with kstrtouint(), improve and simplify the
overall parameter parsing by the following:
- instead of residual strings return proper error codes
- remove dasd_parse_next_element() and decide directly what sort of
element is being parsed
- if we parse a device or a range of devices, split that element into
separate bits with a new function
- remove warning about invalid ending as it doesn't apply anymore
- annotate all parsing functions and data that can be freed after
initialisation with __init and __initdata respectively
- clean up bits and pieces while at it

When the DASD driver is built into the kernel, the entire comma
separated parameter list is stored as one single element in the dasd[]
array, opposed to the module build where each element is stored
separately in dasd[].
There is no point in doing so. Therefore, store each part of the list as
single elements in dasd[] as well when built into the kernel.
Also, create a define for the maximum of 256 parameters.

With this feature, the DASD device driver more robustly handles DASDs
that are attached via multiple channel paths and are subject to
constant Interface-Control-Checks (IFCCs) and Channel-Control-Checks
(CCCs) or loss of High-Performance-FICON (HPF) functionality on one or
more of these paths.
If a channel path does not work correctly, it is removed from normal
operation as long as other channel paths are available. All extended
error recovery states can be queried and reset via user space
interfaces.

The function dasd_ro_store() calls set_disk_ro() to set the device in
question read-only. Since set_disk_ro() might sleep, we can't call it
while holding a lock. However, we also can't simply check if the device,
block, and gdp references are valid before we call set_disk_ro() because
an offline processing might have been started in the meanwhile which
will destroy those references.
In order to reliably call set_disk_ro() we have to ensure several
things:
- Still check validity of the mentioned references but additionally
check if offline processing is running and bail out accordingly. Also,
do this while holding the device lock.
- To ensure that the block device is still safe after the lock, increase
the open_count while still holding the device lock.

The reference to a device in question may get lost when the extended
error reporting (EER) attribute is being enabled/disabled while the
device is set offline at the same time. This is due to missing
refcounting and incorrect locking. Fix this by the following:
- In dasd_eer_store() get the device directly and handle the refcount
accordingly.
- Move the lock in dasd_eer_enable() up so we can ensure safe
processing.
- Check if the device is being set offline and return with -EBUSY if so.
- While at it, change the return code from -EPERM to -EMEDIUMTYPE as
suggested by a FIXME, since that is what we're actually checking.

Before we set a device offline, the open_count for the block device is
checked and certain flags are checked and set as well.
However, this is all done without holding any lock. Potentially, if the
open_count was checked but the DASD_FLAG_OFFLINE wasn't set yet, a
different process might want to increase the open_count depending on
whether DASD_FLAG_OFFLINE is set or not in the meanwhile.
This is quite racy and can lead to the loss of the device for that
process and subsequently lead to a panic.
Fix this by checking the open_count and setting the offline flags while
holding the ccwdev lock.

When setting certain attributes, we actually set the according feature
flag. Do this by using dasd_set_feature() at a few occurrences and
remove duplicate code.
In dasd_set_feature() dasd_find_busid() is used to retrieve the devmap
for the device in question. Combined with the change above, this would
require the device to be set online at least once so that a devmap is
being created. Change that by using dasd_devmap_from_cdev() instead,
which uses dasd_find_busid() first and will create a devmap accordingly
if there is none yet.

Currently the block queue value max_segments is set to -1L, which
is then implicitly casted to unsigned short in blk_queue_max_segments.
This results in 65535 (64k) max_segments.
Even though the resulting value is correct, setting it implicitly using
-1L is rather confusing. Set the value explicitly using the USHRT_MAX
macro instead.

If the DASD device gets blocked for any reason, e.g. because it is reserved
somewhere, the host_access_count sysfs entry or the host_access_list
debugfs entry may sleep forever. Make it interruptible so that userspace
can use ^C to abort the operation.

A DASD device consists of the device itself and a discipline with a
corresponding private structure. These fields are set up during online
processing right after the device is created and before it is processed by
the state machine and made available for I/O.
During offline processing the discipline pointer and the private data gets
freed within the state machine and without protection of the existing
reference count. This might lead to a kernel panic because a function might
have taken a device reference and accesses the discipline pointer and/or
private data of the device while this is already freed.
Fix by freeing the discipline pointer and the private data after ensuring
that there is no reference to the device left.

Internal I/O is processed by the _sleep_on_function which might wait for a
device to get operational. During offline processing this will never happen
and therefore the refcount of the device will not drop to zero and the
offline processing blocks as well.
Fix by letting requests fail in the _sleep_on function during offline
processing. No further handling of the requests is necessary since this is
internal I/O and the device is thrown away afterwards.

On LPAR the read message buffer command should be executed on the path
it was received on otherwise there is a chance that the CUIR assignment
might be faulty and the wrong channel path is set online/offline.
Fix by setting the path mask accordingly.
On z/VM we might not be able to do I/O on this path but there it does
not matter on which path the read message buffer command is executed.
Therefor implement a retry with an open path mask.

When a device is in a status where CIO has killed all I/O by itself the
interrupt for a clear request may not contain an irb to determine the
clear function. Instead it contains an error pointer -EIO.
This was ignored by the DASD int_handler leading to a hanging device
waiting for a clear interrupt.
Handle -EIO error pointer correctly for requests that are clear pending and
treat the clear as successful.

Currently, presence of direct_access() in block_device_operations
indicates support of DAX on its block device. Because
block_device_operations is instantiated with 'const', this DAX
capablity may not be enabled conditinally.
In preparation for supporting DAX to device-mapper devices, add
QUEUE_FLAG_DAX to request_queue flags to advertise their DAX
support. This will allow to set the DAX capability based on how
mapped device is composed.

It is possible to specify a user offset for the TOD clock, e.g. +2 hours.
The TOD clock will carry this offset even if the clock is synchronized
with STP. This makes the time stamps acquired with get_sync_clock()
useless as another LPAR migth use a different TOD offset.
Use the PTFF instrution to get the TOD epoch difference and subtract
it from the TOD clock value to get a physical timestamp. As the epoch
difference contains the sync check delta as well the LPAR offset value
to the physical clock needs to be refreshed after each clock
synchronization.

- Until now, dax has been disabled if media errors were found on
any device. This enables the use of DAX in the presence of these
errors by making all sector-aligned zeroing go through the driver.
- The driver (already) has the ability to clear errors on writes that
are sent through the block layer using 'DSMs' defined in ACPI 6.1.
Other misc changes:
- When mounting DAX filesystems, check to make sure the partition
is page aligned. This is a requirement for DAX, and previously, we
allowed such unaligned mounts to succeed, but subsequent reads/writes
would fail.
- Misc/cleanup fixes from Jan that remove unused code from DAX related to
zeroing, writeback, and some size checks.

dcssblk_remove_store() holds the dcssblk_devices_sem semaphore while
calling device_unregister(), which in turn tries to acquire the kernfs
kn->dev_map rwsem for the device sysfs subtree. The same rwsem is also
acquired when using the per-device sysfs attributes in the device sub-tree,
and the attribute handlers then also acquire the dcssblk_devices_sem.
This can lead to a deadlock when removing a DCSS while concurrently
reading from / writing to one of its sysfs attributes. The following

Reorder lcu and device lock to get rid of the error-prone trylock
mechanism.
The locking order is lcu lock -> device lock.
This protects against changes to the lcu device lists and enables us
to iterate over the devices, take the cdev lock and make changes to
the device structures.
The complicated part is the summary unit check handler that gets an
interrupt on one device of the lcu that leads to structural changes of
the whole lcu itself. This work needs to be done even if devices on
the lcu disappear. So a device independent worker is used.
The old approach tried to update some lcu structures and set up the
lcu worker in the interrupt context with the device lock held.
But this forced the lock order "cdev lock -> lcu lock" that made it
hard to have the lcu lock held and iterate over all devices and change
them.
The new approach is to schedule a device specific worker that gets
out of the interrupt context and rid of the device lock for summary
unit checks. This worker is able to take the lcu lock and schedule the
lcu worker that updates all devices. The time between interrupt and
worker execution is no problem because the devices in the lcu reject
all I/O in this time with an appropriate error. The dasd driver can
deal with this situation and re-drive the I/O later on.

Currently, dasd_format is calling the format logic of a DASD discipline
with PAV enabled. If that fails with an error code of -EAGAIN the value
of retries is decremented and the discipline function is called with PAV
turned off.
The loop is supposed to try this up to 255 times until success. However,
-EAGAIN can only occur once here and therefore the loop will never reach
the 255 retries.
So, replace the unnecessarily complicated loop logic and simply try again
without PAV enabled in case of an -EAGAIN error.

git commit 1ec2772e0c ("s390/diag: add a statistic for diagnose
calls") added function calls to gather diagnose statistics.
In case of the dasd diag driver the function call was added between a
register asm statement which initialized register r2 and the inline
assembly itself. The function call clobbers the contents of register
r2 and therefore the diag 0x250 call behaves in a more or less random
way.
Fix this by extracting the function call into a separate function like
we do everywhere else.

The correct lock order for LCU lock and cdev lock is to take the cdev
lock first and afterwards the LCU lock. This is caused by the fact
that LCU functions are called in an interrupt context with the cdev
lock implicitly hold by CIO.
To assure the right locking order but also be able to iterate over
devices in a LCU introduce a trylock block that can be called with
the device lock for one device hold and then takes the LCU lock and
try to lock all devices accounted to this LCU. Afterwards all devices
and the LCU itself are locked.

Commit ca369d51b ("sd: Fix device-imposed transfer length limits")
introduced a new queue limit max_dev_sectors which limits the maximum
sectors for requests. The default value leads to small dasd requests
and therefor to a performance drop.
Set the max_dev_sectors value to the same value as the max_hw_sectors
to use the maximum available request size for DASD devices.

The channel checks the specified length and the provided amount of
data for CCWs and provides an incorrect length error if the size does
not match. Under z/VM with simulation activated the length may get
changed. Having the suppress length indication bit set is stated as
good CCW coding practice and avoids errors under z/VM.

For the purpose of communicating the optional presence of a 'struct
page' for the pfn returned from ->direct_access(), introduce a type that
encapsulates a page-frame-number plus flags. These flags contain the
historical "page_link" encoding for a scatterlist entry, but can also
denote "device memory". Where "device memory" is a set of pfns that are
not part of the kernel's linear mapping by default, but are accessed via
the same memory controller as ram.
The motivation for this new type is large capacity persistent memory
that needs struct page entries in the 'memmap' to support 3rd party DMA
(i.e. O_DIRECT I/O with a persistent memory source/target). However,
we also need it in support of maintaining a list of mapped inodes which
need to be unmapped at driver teardown or freeze_bdev() time.

Enabling failfast should let request fail immediately if either an
error occurred or the device gets disconnected.
For disconnected devices new requests are not fetches from the block
queue and therefore failfast is not triggered.
Fix by letting the DASD driver fetch requests for disconnected devices
with failfast active.

Path verification is either done via dasd_eckd_read_conf() which is
triggered during online processing and resume or via
do_path_verification_work() which is triggered after path events.
The dasd_eckd_read_conf() version added paths unconditionally and did
not check if the path mask was empty. This led to devices having the
disconnected stop flag set but a valid path mask. So they where not
working although they had paths validated successfully. After a resume
this state could even not be solved with additional paths added.
Fix by checking for an empty path mask in dasd_eckd_read_conf() and
clearing the device stop bits for a newly added channel path.

For a valid PAV assignment the DASD driver needs to notice possibly
changed configuration data. Thus the failing of read configuration
data should also fail the device restore to prevent invalid PAV
assignment. The failed device may get restored after additional paths
get available later on.
If the restore fails after the device was added to the lcu alias
handling it needs to be removed from the alias handling before exiting
the restore function.

The configuration data is stored per path and also the first valid
configuration data per device. When dasd_eckd_read_conf is called
again after a path got lost the device configuration data is cleared
but possibly not the per path configuration data. This might lead to a
double free when the lost path gets operational again.
Fix by clearing all per path configuration data when the first valid
configuration data is received and stored.

A summary unit check occurs when the lcu updates the PAV configuration
e.g. base PAV assignment or PAV mode at all. This requires the reset
of the drivers internal pavgroups. Therefore the alias devices are
flushed and moved via a temporary list to the active_devices list
where they are not associated with a pavgroup. In conjunction with
updates to the base device the pavgroup may be removed since both
base_list and alias_list are empty. Unfortunately during alias flush
and move to the active_device list from alias_list the pavgroup
pointer is not deleted in the device private structure. This leads to
a list del_corruption if another lcu_update tries to move the device
in the non existent pavgroup.
Fix by removing the pavgroup pointer after the alias device was moved
to the active_devices list.

We were able to reduce the CPU overhead of big paging scenarios
when announcing our paging disks as non-rotational.
Almost all dasd devices are implemented in storage servers with
cache, raid, striping and lots of magic. There is no point in
optimizing the disk schedulers and swap code for a single platter
moving arm rotational disks. Given the complexity of the setup
and the fact that this change is mostly to disable the additional
overhead in swap code, lets keep the other functionality unchanged
and do not disable the this device as entropy source - unlike other
non-rotational devices.

persistence guarantees for kernel operations on a DAX mapping.
- Convert the ACPI NFIT 'BLK' driver to map the block apertures as
cacheable to improve performance.
- Miscellaneous updates and fixes to libnvdimm including support for
issuing "address range scrub" commands, clarifying the optimal
'sector size' of pmem devices, a clarification of the usage of the
ACPI '_STA' (status) property for DIMM devices, and other minor
fixes"

Pull core block updates from Jens Axboe:
"This first core part of the block IO changes contains:
- Cleanup of the bio IO error signaling from Christoph. We used to
rely on the uptodate bit and passing around of an error, now we
store the error in the bio itself.
- Improvement of the above from myself, by shrinking the bio size
down again to fit in two cachelines on x86-64.
- Revert of the max_hw_sectors cap removal from a revision again,
from Jeff Moyer. This caused performance regressions in various
tests. Reinstate the limit, bump it to a more reasonable size
instead.
- Make /sys/block/<dev>/queue/discard_max_bytes writeable, by me.
Most devices have huge trim limits, which can cause nasty latencies
when deleting files. Enable the admin to configure the size down.
We will look into having a more sane default instead of UINT_MAX
sectors.
- Improvement of the SGP gaps logic from Keith Busch.
- Enable the block core to handle arbitrarily sized bios, which
enables a nice simplification of bio_add_page() (which is an IO hot
path). From Kent.
- Improvements to the partition io stats accounting, making it
faster. From Ming Lei.
- Also from Ming Lei, a basic fixup for overflow of the sysfs pending
file in blk-mq, as well as a fix for a blk-mq timeout race
condition.
- Ming Lin has been carrying Kents above mentioned patches forward
for a while, and testing them. Ming also did a few fixes around
that.
- Sasha Levin found and fixed a use-after-free problem introduced by
the bio->bi_error changes from Christoph.

None of the implementations currently use it. The common
bdev_direct_access() entry point handles all the size checks before
calling ->direct_access().

Update the annotation for the kaddr pointer returned by direct_access()
so that it is a __pmem pointer. This is consistent with the PMEM driver
and with how this direct_access() pointer is used in the DAX code.

The way the block layer is currently written, it goes to great lengths
to avoid having to split bios; upper layer code (such as bio_add_page())
checks what the underlying device can handle and tries to always create
bios that don't need to be split.
But this approach becomes unwieldy and eventually breaks down with
stacked devices and devices with dynamic limits, and it adds a lot of
complexity. If the block layer could split bios as needed, we could
eliminate a lot of complexity elsewhere - particularly in stacked
drivers. Code that creates bios can then create whatever size bios are
convenient, and more importantly stacked drivers don't have to deal with
both their own bio size limitations and the limitations of the
(potentially multiple) devices underneath them. In the future this will
let us delete merge_bvec_fn and a bunch of other code.
We do this by adding calls to blk_queue_split() to the various
make_request functions that need it - a few can already handle arbitrary
size bios. Note that we add the call _after_ any call to
blk_queue_bounce(); this means that blk_queue_split() and
blk_recalc_rq_segments() don't need to be concerned with bouncing
affecting segment merging.
Some make_request_fn() callbacks were simple enough to audit and verify
they don't need blk_queue_split() calls. The skipped ones are:
* nfhd_make_request (arch/m68k/emu/nfblock.c)
* axon_ram_make_request (arch/powerpc/sysdev/axonram.c)
* simdisk_make_request (arch/xtensa/platforms/iss/simdisk.c)
* brd_make_request (ramdisk - drivers/block/brd.c)
* mtip_submit_request (drivers/block/mtip32xx/mtip32xx.c)
* loop_make_request
* null_queue_bio
* bcache's make_request fns
Some others are almost certainly safe to remove now, but will be left
for future patches.
