This removes the custom DBG macro in favor of the in-kernel
dev_dbg() macro. Probably a leftover from a time when dev_dbg()
didn't yet exist. Also remove a printk() in favor of dev_err().

EMMC can have two voltage supplies, Vcc and VccQ
which are implemented in the code as consumer
supplies vmmc and vmmc_aux.
If the regulator that supplies vmmc_aux is shared
with other consumers, then sending it to sleep
will disrupt those consumers. However, the
TWL4030-family regulators may have OFF remapped
to SLEEP, in which case 'regulator_disable()'
will put the regulator to sleep only when all
consumers are disabled - which is the desired
behaviour.
This patch adds a platform data field to allow
that option.

This patch moves the setup code for GPIO's and Voltage
Regulators from the board file mmc-twl4030.c to the
driver omap_hsmmc.c. PBIAS and other system control
configuration remains in the board file.
Moving GPIO code to the driver makes the board initialisation
code independent of when GPIO's are defined. That makes the
board initialisation now entirely independent of its original
twl4030 roots.
Moving Voltage Regulator code to the driver allows for further
development of regulator support in the core MMC code. It also
permits the MMC core to be compiled as a module, because the
board code no longer calls MMC core functions.

This patch fixes a bug in the multiblock write tests where the written
data is read back for verifying one block at a time. The tests in
mmc_test assumes that all cards are byte addressable.
This will cause the multi block write tests to fail, leading the user of
the mmc_test driver thinking there is something wrong with the sdhci
driver they are testing.
The start address for the block is calculated as: blocknum * 512. For
block addressable cards the blocknum alone should be used.

In the U300 some hardware bug makes the status flag not come up
signalling a successful write (or anything else, like an error, for
that matter) on write requests. This little quirk makes the writes
work on U300.

This patch abstracts out the CNF area code from tmio_mmc which
is not present in all hardware that can use this driver. This

The main bug was that 'blk_cleanup_queue()' was called while the block
device could still be in use, for example, because the card was removed
while files were still open.
In addition, to be sure that 'mmc_request()' will get called for all new
requests (so it can error them out), the queue is emptied during cleanup.
This is done after the worker thread is stopped to avoid racing with it.
Finally, it is not a device error for this to be happening, so quiet the
(sometimes very many) error messages.

When a card is removed before mmc_blk_probe() has called add_disk(), then
the minor field is uninitialized and has value 0. This caused
mmc_blk_put() to always release devidx 0 even if 0 was still in use. Then
the next mmc_blk_probe() used the first free idx of 0, which oopses in
sysfs, since it is used by another card.

We need to accurately track how many SDIO functions have been initialised
(and keep card->sdio_funcs in sync) so that we don't try to remove more
functions than we initialised if we hit the error path in
mmc_attach_sdio().
Without this patch if we hit the error path in mmc_attach_sdio() we run
the risk of deferencing invalid memory in sdio_remove_func(), leading to a
crash.

This new revision of the IP adds some improvements to the MCI already
present in several Atmel SOC.
Some new registers are added and a particular way of handling DMA
interaction lead to a new sequence in function call which is backward
compatible: On MCI2, we must set the DMAEN bit to enable the DMA
handshaking interface. This must happen before the data transfer command
is sent.
A new function is able to differentiate MCI2 code and is based on
knowledge of processor id (cpu_is_xxx()).

Allow the use of another DMA controller driver in atmel-mci sd/mmc driver.
This adds a generic dma_slave pointer to the mci platform structure where
we can store DMA controller information. In atmel-mci we use information
provided by this structure to initialize the driver (with new helper
functions that are architecture dependant).
This also adds at32/avr32 chip modifications to cope with this new access
method.

Some people run general-purpose distribution kernels on netbooks with
a card that is physically non-removable or logically non-removable
(e.g. used for /home) and cannot be cleanly unmounted during suspend.
Add a module parameter to set whether cards are assumed removable or
non-removable, with the default set by CONFIG_MMC_UNSAFE_RESUME.
In general, it is not possible to tell whether a card present in an MMC
slot after resume is the same that was there before suspend. So there are
two possible behaviours, each of which will cause data loss in some cases:
CONFIG_MMC_UNSAFE_RESUME=n (default): Cards are assumed to be removed
during suspend. Any filesystem on them must be unmounted before suspend;
otherwise, buffered writes will be lost.
CONFIG_MMC_UNSAFE_RESUME=y: Cards are assumed to remain present during
suspend. They must not be swapped during suspend; otherwise, buffered
writes will be flushed to the wrong card.
Currently the choice is made at compile time and this allows that to be
overridden at module load time.

Currently DaVinci EDMA driver supports multiple EDMA channel controller
instances. edma_alloc_channel() api returns a 32 bit value which has the
channel controller number in MSB and the EDMA channel number in LSB. The
variables which store the value returned by edma_alloc_channel() have to
be 32 bit wide now.

Rework the current CIS tuple parsing code, reusing the existing
infrastructure and providing an easy way to add new CISTPL_FUNCE parsers
by TPLFE_TYPE.
Valid known CIS tuples are now silently queued for the SDIO function
driver when not parsed/processed (-EILSEQ) by the SDIO core. Unknown CIS
tuples (-ENOENT) are queued too for the SDIO function driver without
aborting the initialization, but emit a warning in the kernel log.
CISTPL_FUNCE tuples can be "whitelisted" now by adding a matching entry to
the cis_tpl_funce_list table.

The PC Card 8.0 specification (vol. 4, section 3.2.10) says the
TPLLV1_INFO field of the CISTPL_VERS_1 tuple must contain 4 strings. Some
cards don't have all 4 so just parse as many as we can.

Add better support for omitting either the card detect or the write
protect GPIOs if the board does not support it. Add the fields
no_wprotect and no_detect to the platform data which when set indicate the
absence of the respective GPIOs.
Note, this also fixes a minor bug where it tries to free IRQ0 if there is
no detect gpio available.

Fixes for the DMA transfer mode of the driver to try and improve the state
of the code:
- Ensure that dma_complete is set during the end of the command phase
so that transfers do not stall awaiting the completion
- Update the DMA debugging to provide a bit more useful information
such as how many DMA descriptors where not processed and print the
DMA addresses in hexadecimal.
- Fix the DMA channel request code to actually request DMA for the
S3CMCI block instead of whatever '0' signified.
- Add fallback to PIO if we cannot get the DMA channel, as many of the
devices with this block only have a limited number of DMA channels.
- Only try and claim and free the DMA channel if we are trying to use it.
This improves the driver DMA code to the point where it can now identify a
card and read the partition table. However the DMA can still stall when
trying to move data between the host and memory.

Add a selection for the data transfer mode of the s3cmci driver, allowing
for either a configuration or rumtime selection of the use of the DMA or
PIO transfer code.
The PIO only mode is 476 bytes smaller than the driver with both methods
compiled in.

The controller supports SDIO IRQ detection so add support for hardware
assisted SDIO interrupt detection for the SDIO core. This improves the
response time for SDIO interrupts and thus the transfer rate from devices
such as the Marvel 8686.
As a note, it does seem that the controller will miss an IRQ than is held
asserted, so there are some manual checks to see if the SDIO interrupt is
active after a transfer.
Major testing on the S3C2440.

The clear_imask() call should be used to clear the interrupt mask
register, as it may end up clearing the SDIO interrupt bit if this is
enabled.
Change all writes of zero to SDIIMSK register to use clear_imask() ready
for the SDIO updates.

Use the platform id list to match the three different versions of the
hardware block that this driver supports.
This will change the prefix of the console messages produced by this
driver to be prefixed by s3c-mci instead of the hardware block name, such
as s3c2440-mci.

Some manufacturers provide vendor information in non-vendor specific CIS
tuples. For example, Broadcom uses an Extended Function tuple to provide
the MAC address on some of their network cards, as in the case of the
Nintendo Wii WLAN daughter card.
This patch allows passing whitelisted FUNCE tuples unknown to the SDIO
core to a matching SDIO driver instead of rejecting them and failing.

On an OLPC XO-1.5 development board with Via VX855 chipset, the sdhci
controller can take up to 12ms to stabilize its clock, but the current
timeout at which we give up on the controller is 10ms.
The patch increases the timeout delay rather than using a device-specific
quirk -- since we exit the loop when the clock comes up, increasing the
timeout value will only make us mdelay() longer in the errant case of a
device with a clock that is not stabilizing, which it seems worth waiting
a little longer for in general.

Especially for SDIO drivers which may have special conditions/errors to
report, it is a good thing to relay the returned error code back to upper
layers.
This also allows for the rationalization of the resume path where code to
"remove" a no-longer-existing or replaced card was duplicated into the
MMC, SD and SDIO bus drivers.
In the SDIO case, if a function suspend method returns an error, then all
previously suspended functions are resumed and the error returned. An
exception is made for -ENOSYS which the core interprets as "we don't
support suspend so just kick the card out for suspend and return success".
When resuming SDIO cards, the core code only validates the manufacturer
and product IDs to make sure the same kind of card is still present before
invoking functions resume methods. It's the function driver's
responsibility to perform further tests to confirm that the actual same
card is present (same MAC address, etc.) and return an error otherwise.

Currently, all SDIO cards are virtually removed upon a suspend, and
completely reprobed upon a resume. This adds the suspend and resume
methods to the SDIO bus driver so to be able to dispatch those events to
the actual SDIO function drivers for real suspend/resume instead.
All active functions on a card must have a driver with both a suspend and
a resume method though. Failing that, we fall back to the current
behavior of simply "removing" the card when suspending.
When resuming, we make sure the same card is still inserted by comparing
the vendor and product IDs. If there is a mismatch, or if there is simply
no card anymore in the slot, then the previous card is "removed" and the
new card is detected. This is further enhanced with the next patch.

Some time ago, I have send a patch to the mmc_spi subsystem changing the
error codes. This was after a discussion with Pierre about using EINVAL
only for non-recoverable errors. This patch was accepted as

Unfortunately, several weeks later, I realized that this patch has opened
a little can of worms because there are SD cards on the market which
a) claim that they support the switch command
AND
b) refuse to execute this command if operating in SPI mode.
So, such a card would get unusuable in an embedded linux system in SPI
mode, because the init sequence terminates with an error.
This patch adds the missing error codes to the caller of the switch
command and restores the old behaviour to fail gracefully if these
commands can not execute.

Unification of the atmel-mci driver to support the AT91 processors MCI
interface. The atmel-mci driver currently supports the AVR32 and this
patch adds AT91 support.
Add read/write proof selection switch dependent on chip availability of
this feature.
To use this new driver on a at91 the platform driver for your board needs
to be updated.

Normally writes to SDIO function 0 outside the vendor specific CCCR
registers are prohibited.
To support embedded devices that require writes to SDIO function 0 outside
this range (e.g. TI WL127x embedded sdio wifi device),
MMC_QUIRK_LENIENT_FN0 is introduced.
A card quirks field is added to `struct mmc_card' to support non-standard
devices (e.g. embedded sdio devices).

- Get rid of incomprehensible "if { for { if } }" construction for the
exponential divisor calculation. The first if statement isn't correct
at all, since it should check for "host->max_clk / pre_div / 16 >
clock". The error doesn't cause any bugs because the check in the for
loop does the right thing, and so the outer check becomes useless;
- For the linear divisor do the same: a single while statement is more
readable than for + if construction;
- Add dev_dbg() that prints desired and actual clock frequency.

MPC85xx SOCs have normal write-protect state reporting, so we shouldn't
hard-code the quirk.
Instead, look for "sdhci,wp-inverted" property, plus check for
mpc837x_{rdb,mds} machines since older device trees don't specify the new
property.

eSDHC fails to recognize some SDHS cards, throwing timeout errors:
mmc0: error -110 whilst initialising SD card
That's because we calculate timeout value in a wrong way: on eSDHC hosts
the timeout clock is derivied from the SD clock, which is set dynamically.
As David Vrabel suggested, deriving timeout clock from SD clock is a
common scheme, so let's implement DATA_TIMEOUT_USES_SDCLK quirk and use it
for eSDHC hosts.
Also, from now on we don't need esdhc_get_timeout_clock() callback, so
remove it.

SDHCI core tries to write HISPD bit into the host control register, but
the eSDHC controllers don't have that bit, and that causes all sorts of
misbehaviour when using 4-bit mode capable SD cards.

get_min_clock() makes sense only with NONSTANDARD_CLOCK quirk and when
set_clock() callback is specified.
The patch should cause no functional changes, it just makes the code
self-documented and avoids any possible misuse of get_min_clock().

Add support to disconnect the pull-up resistor on CD/DAT[3] (pin 1)
of the card. This may be desired on certain setups of boards,
controllers and embedded sdio devices which do not need the card's
pull-up. As a result, card detection is disabled and power is saved.

Commands like SWITCH (CMD6) send a response and then signal busy while the
operation is completed. These commands are expected to always succeed
(otherwise the response would have indicated an error).
Set an arbitrarily large data timeout value (100ms) for these commands to
ensure that premature timeouts do not occur.

Depending on the manufacturer, there is a small possibility that removing
a card while it is being written to, can render the card permanently
unusable. To prevent that, the card is made inaccessible when the cover
is open.

If an unexpected interrupt occurs while preparing the next request, an
oops can occur.
For example, a new request is setting up DMA for data transfer so
host->data is not NULL. An unexpected transfer complete (TC) interrupt
comes along and the interrupt handler sets host->data to NULL. Oops!
Prevent that by adding a spinlock.

Sometimes the controller unexpectedly produces a TC (transfer complete)
interrupt before the CC (command complete) interrupt for command 6
(SWITCH). This is a problem because the CC interrupt can get mixed up
with the next request. Add a hack for CMD6.

When a card is not in use, the voltage regulator can be put to sleep.
This is an alternative to powering the card off, when powering off is not
safe because the card might be replaced without the driver being aware of
it.
That situation happens if:
- the card is removable i.e. not eMMC
- and there is no card detect
- and there is a cover switch but the cover is open

This patch is preparation for adding context save and restore support.
Keep track of the current power mode so that the context restore function
can avoid restoring the context for a card if the power has been switched
off. If the power is off, the card must be reinitialized anyway which
will re-establish the context.

Power can be saved by powering off cards that are not in use. This is
similar to suspend / resume except it is under the control of the driver,
and does not require any power management support. It can only be used
when the driver can monitor whether the card is removed, otherwise it is
unsafe. This is possible because, unlike suspend, the driver still
receives card detect and / or cover switch interrupts.

omap_mmc_probe lives in .init.text, so using platform_driver_register to
register it is wrong because binding a device after the init memory is
discarded (e.g. via sysfs) results in an oops.
As requested by David Brownell platform_driver_probe is used instead of
moving the probe function to .devinit.text as proposed initially. This
saves some memory, but devices registered after the driver is probed are
not bound (probably there are none) and binding via sysfs isn't possible.

This enables the use of a regulator to power the MMCI/PL180
PrimeCell. The OCR mask is calculated and voltage is set using
the new MMC core functions for discovering voltage ranges
in regulators. The platform translate_vdd function which basically
controls the 4 lines out of the PL180 is disabled if you use a
regulator instead.

This moves the mmci platform data definition struct away from
arch/arm/include/asm/mach/mmc.h into the more proper place among
the other primecells in include/linux/amba/mmci.h and at the same
time renames it to "mmci.h", and also the struct in this file
confusingly named mmc_platform_data has been renamed
mmci_platform_data for clarity.

This makes it possible to pass down the host controller
capabilities for the MMCI driver using the platform data. It
also provides the capabilties for the U300 implementation as an
example, and makes sure the 4bit wide mode is set if this is
requested by the ios() now that we can actually set that
capability for a platform.

This breaks out the clock divider set-up code from the
mmci_set_ios() code and surrounds the two register
writes with a host lock so we don't get collisions if
(in future code) two code paths want to change the
clock divider at the same time as can be the case if
we get something like pre/post- clock frequency change
notifications soonish.

The MMC block needs 3 external datas to work :
- is the MMC card put in "read-only mode" ?
- is a MMC card inserted or removed ?
- enable power towards the MMC card
Several platforms provide these controls through
gpios. Expand the platform_data to request and use these
gpios is set up by board code.

Since commit 8dfd0374be ("MMC core: limit
minimum initialization frequency to 400kHz") MMC core checks for minimum
frequency, and that causes following messages flood when using eSDHC
controllers:
...
mmc0: Minimum clock frequency too high for identification mode
mmc0: Minimum clock frequency too high for identification mode
...
The warnings are legitimate, since if we'd use 133 MHz clocks for standard
SDHCI controllers, we'd not able to scale frequency down to 400 kHz.
But eSDHC controllers have a non-standard SD clock management, so we can
divide clock by 256 * 16, not just 256.
This patch introduces get_min_clock() callback for sdhci core and
implements it for sdhci-of driver, and thus fixes the issue.

Standard data flow for MMC/SD/SDIO cards requires that the mvsdio
controller be set for big endian operation. This is causing problems
with buffers which length is not a multiple of 4 bytes as the last
partial word doesn't get shifted all the way and stored properly in
memory. Let's compensate for this.

Add a new spi_master.flags word listing constraints relevant to that
controller. Define the first constraint bit: a half duplex restriction.
Include that constraint in the OMAP1 MicroWire controller driver.
Have the mmc_spi host be the first customer of this flag. Its coding
relies heavily on full duplex transfers, so it must fail when the
underlying controller driver won't perform them.
(The spi_write_then_read routine could use it too: use the
temporarily-withdrawn full-duplex speedup unless this flag is set, in
which case the existing code applies. Similarly, any spi_master
implementing only SPI_3WIRE should set the flag.)

Some hosts (hardware configurations, or particular SD/MMC slots) may
not support 4-bit bus. For example, on MPC8569E-MDS boards we can
switch between serial (1-bit only) and nibble (4-bit) modes, thought
we have to disable more peripherals to work in 4-bit mode.
Along with some small core changes, this patch modifies sdhci-of
driver, so that now it looks for "sdhci,1-bit-only" property in the
device-tree, and if specified we enable a proper quirk.

In the write recovery routine, the data to get from the card
is allocated from the stack. The DMA mapping documentation says
explicitly stack memory is not mappable by any of the DMA calls.
Change to using kmalloc() to allocate the memory for the result
from the card and then free it once we've finished with the
transaction.

This patch fixes the clock setup in tmio_mmc.
* Incorrect divider setting
* Cruft written to the clock registers (seemingly harmless but Not
Good (tm))
It also eliminates some unnecessary ifs and tidies the loop syntax.
Thanks to Philipp Zabel who discovered the divider issue, commenting
"Except for the SDCLK = HCLK (divider bypassed) case, the clock
setting resulted in double the requested frequency.
The smallest possible frequency (f_max/512) is configured with
a divider setting 0x80, not 0x40."

The Toshiba parts all have a 24 MHz HCLK, but HTC ASIC3 has a 24.576 MHz HCLK
and AMD Imageon w228x's HCLK is 80 MHz. With this patch, the MFD driver
provides the HCLK frequency to tmio_mmc via mfd_cell->driver_data.

This patch changes the reported error code for the responses
to a command from EINVAL to EFAULT/ENOSYS, as EINVAL is reserved
for non-recoverable host errors, and the responses from
the SD/MMC card may be because of recoverable transmission
errors in the command or in the response. Response codes
in SPI mode are NOT protected by a checksum, so don't trust them.

The code is divided in two parts. There is a virtual 'bus' driver
that handles PCI device and registers three new devices one per card

Changes pxamci.c to use the regulator subsystem. Uses the regulator case
CONFIG_REGULATOR is defined and a matching is regulator is provided, or

A pointer to mmc_omap_probe which lives in .init.text is passed to the
core via platform_driver_register and so the kernel might oops if probe
is called after the init code is discarded.
As requested by David Brownell platform_driver_probe is used instead of
moving the probe function to .devinit.text. This saves some memory, but
might have the downside that a device being registered after the call to
mmc_omap_init but before the init sections are discarded will not be
bound anymore to the driver.

With this patch, mmc_rescan can detect the removal of an mmc card and
the insertion of (possibly another) card in the same run. This means
that a card change can be detected without having to call
mmc_detect_change multiple times.
This change generalises the core such that it can be easily used by
hosts which provide a mechanism to detect only the presence of a card
reader cover, which has to be taken off in order to insert a card. Other
hosts ("card detect" or "MMC_CAP_NEEDS_POLL") each receive an event when
a card is removed and when a card is inserted, so it is sufficient for
them if mmc_rescan handles only one event at a time. "Cover detect"
hosts, however, only receive events about the cover status. This means
that between 2 subsequent events, both a card removal and a card
insertion can occur. In this case, the pre-patch version of mmc_rescan
would only detect the removal of the previous card but not the insertion
of the new card.

I found the PrimeCell/AMBA Bus drivers distrusting the resource
passed in as part of the struct amba_device abstraction. This
patch removes all hard coded resource sizes found in the PrimeCell
drivers and move the responsibility of this definition back to
the platform/board device definition, which already exist and
appear to be correct for all in-tree users of these drivers.
We do this using the resource_size() inline function which was
also replicated in the only driver using the resource size, so
that has been changed too. The KMI_SIZE was left in kmi.h in case
someone likes it. Test-compiled against Versatile and Integrator
defconfigs, seems to work but I don't posess these boards and
cannot test them.

Freescale eSDHC controller has the special order for
the HOST version register. that is not same as the other's
registers. The address of HOSTVER in spec is 0xFE, and
we need use the in_be16(0xFE) to access it, not in_be16(0xFC).

Especially with Sandisk SDHC cards, the second SWITCH command was failing
with a timeout and the card was not recognized at all. However if the
system was busy, or debugging was enabled, or a udelay(100) was inserted
before the second SWITCH command in the core code, then the timing was
so that the card started to work.
With some unusual block sizes, the data FIFO status doesn't indicate a
"empty" state right away when the data transfer is done. Queuing
another data transfer in that condition results in a transfer timeout.
The empty FIFO bit eventually get set by itself in less than 50 usecs
when it is not set right away. So let's just poll for that bit before
configuring the controller with a new data transfer.

Empirical evidences show that this is causing far more problems than it
solves when this mode is enabled in the host hardware. Amongst those
cards that are known to be non functional when this bit is set are:
A-Data "Speedy" 2GB SD card
Kodak 512MB SD card
Ativa 1GB MicroSD card
Marvell 8688 (WIFI/Bluetooth) SDIO card
Since those cards do work on other host controllers which do honnor the
hs timing, the issue must be with this particular host hardware.

disable_irq() should wait for all running handlers to complete
before returning. As such, if it's used to disable an interrupt
from that interrupt's handler it will deadlock. This replaces
the dangerous instances with the _nosync() variant which doesn't
have this problem.

Decouple the HSMMC glue from the twl4030 as the only
regulator provider, using the regulator framework instead.
This makes the glue's "mmc-twl4030" name become a complete
misnomer ... this code could probably all migrate into the
HSMMC driver now.
Tested on 3430SDP (SD and low-voltage MMC) and Beagle (SD),
plus some other boards (including Overo) after they were
converted to set up MMC regulators properly.
Eventually all boards should just associate a regulator with
each MMC controller they use. In some cases (Overo MMC2 and
Pandora MMC3, at least) that would be a fixed-voltage regulator
with no real software control. As a temporary hack (pending
regulator-next updates to make the "fixed.c" regulator become
usable) there's a new ocr_mask field for those boards.

Until now we have had a 1:1 mapping between storage device physical
block size and the logical block sized used when addressing the device.
With SATA 4KB drives coming out that will no longer be the case. The
sector size will be 4KB but the logical block size will remain
512-bytes. Hence we need to distinguish between the physical block size
and the logical ditto.
This patch renames hardsect_size to logical_block_size.

The second argument of the probe method points to the amba_id
structure, so it's better passed with the correct type. None of the
current in-tree drivers uses the pointer, so they have only been
checked for a clean compile.

Move all the gpio functions out of <mach/hardware.h> as
this file is for defining the generic IO base addresses
for the kernel IO calls.
Make a new header <mach/gpio-fns.h> to take this and
include it via the chain from <linux/gpio.h> which is
what most of these files should be using (and will be
changed as soon as possible).
Note, this does make minor changes to some drivers but
should not mess up any pending merges.

Till now block layer allowed two separate modes of request execution.
A request is always acquired from the request queue via
elv_next_request(). After that, drivers are free to either dequeue it
or process it without dequeueing. Dequeue allows elv_next_request()
to return the next request so that multiple requests can be in flight.
Executing requests without dequeueing has its merits mostly in
allowing drivers for simpler devices which can't do sg to deal with
segments only without considering request boundary. However, the
benefit this brings is dubious and declining while the cost of the API
ambiguity is increasing. Segment based drivers are usually for very
old or limited devices and as converting to dequeueing model isn't
difficult, it doesn't justify the API overhead it puts on block layer
and its more modern users.
Previous patches converted all block low level drivers to dequeueing
model. This patch completes the API transition by...
* renaming elv_next_request() to blk_peek_request()
* renaming blkdev_dequeue_request() to blk_start_request()
* adding blk_fetch_request() which is combination of peek and start
* disallowing completion of queued (not started) requests
* applying new API to all LLDs
Renamings are for consistency and to break out of tree code so that
it's apparent that out of tree drivers need updating.

With recent cleanups, there is no place where low level driver
directly manipulates request fields. This means that the 'hard'
request fields always equal the !hard fields. Convert all
rq->sectors, nr_sectors and current_nr_sectors references to
accessors.
While at it, drop superflous blk_rq_pos() < 0 test in swim.c.

The S3C24XX DMA API channel configuration registers are being passed
values comprised of register values which makes it hard to move the
API to cover both the S3C24XX and S3C64XX.
These values can be calculated from knowing which device the channel
is connected to, so remove them from the two calls s3c2410_dma_config
and s3c2410_dma_devconfig.

SD/MMC card timeouts can be very high. So avoid busy-waiting,
using the scheduler. Calculate all timeouts in jiffies units,
because this will give us the correct sign when to involve
the scheduler.

Some cards are not able to calculate a valid CRC16 value
for CID and CSD reads (CRC for 512 byte data blocks is OK).
By moving the CRC enable after the read of CID and CSD, these
cards can be used. This patch was tested with a faulty 8 GByte
takeMS Class 6 SDHC card. This patch was suggested by
Pierre Ossman.

Spurious IRQs seen on MMC after 2.6.29. Flush posted write in IRQ
handler.
The interrupt line is released by clearing the error status bits
in the MMCHS_STAT register, which must occur before the interrupt
handler returns to avoid unwanted irqs. Hence the need to flush
the posted write.

Glue between MMC and regulator stacks ... verified with
some OMAP3 boards using adjustable and configured-as-fixed
regulators on several MMC controllers.
These calls are intended to be used by MMC host adapters
using at least one regulator per host. Examples include
slots with regulators supporting multiple voltages and
ones using multiple voltage rails (e.g. DAT4..DAT7 using a
separate supply, or a split rail chip like certain SDIO
WLAN or eMMC solutions).

This patch adds a new driver: sdhci-of. The driver is similar to
the sdhci-pci, it contains common probe code, and controller-specific
ops and quirks.
So far there are only Freescale eSDHC ops and quirks.

FSL eSDHC controllers can support maximum block size up to 4096 bytes,
the MBL (Maximum Block Length) field in the capabilities register
extended by one bit, and is set to 0x3.
But the SDHCI core doesn't support blocks of 4096 bytes, and thus
forces blksz to the lowest value -- 512 bytes. With this patch we can
pin up the blksz to the maximum supported block size, i.e. 2048 bytes.

Small udelay is needed to make eSDHC work in PIO mode. Without
the delay reading causes endless interrupt storm, and writing
corrupts data. The first guess would be that we must wait for
some bit in some register, but I didn't find any reliable bits
that change before and after the delay.

FSL eSDHC hosts have incompatible register map to manage the SDCLK.
This patch adds set_clock callback so that drivers could overwrite
set_clock behaviour.
Similar patch[1] was posted by Ben Dooks, though in Ben's version the
callback is named change_clock, plus the patch has some unrelated bits
that makes the patch difficult to reuse.

Some controllers do not provide clock information in their capabilities
(in the Samsung case, it is because there are multiple clock sources
available to the controller). Add hooks to allow the system to supply
clock information.
p.s.
In the original Ben's patch there was a bug that makes sdhci_add_host()
return -ENODEV even if callbacks were specified. This is fixed now.

This patch adds SDHCI_QUIRK_INVERTED_WRITE_PROTECT quirk. When
specified, the sdhci driver will invert WP state.

This patch adds SDHCI_QUIRK_BROKEN_CARD_DETECTION quirk. When specified,
sdhci driver will set MMC_CAP_NEEDS_POLL MMC host capability, and won't
enable card insert/remove interrupts.
This is needed for hosts with unreliable card detection, such as FSL
eSDHC. The original eSDHC driver was tring to "debounce" card-detection
IRQs by reading present state and disabling particular interrupts. But
with this debouncing scheme I noticed that sometimes we miss card
insertion/removal events.

Some hosts (that is, FSL eSDHC) throw PIO interrupts during DMA
transfers, this causes tons of unneeded interrupts, and thus highly
degraded speed.
This patch modifies the driver so that now we only enable relevant
(DMA or PIO) interrupts during transfers.

Card detection interrupts should be handled separately as they should
not be enabled before mmc_add_host() returns and should be disabled
before calling mmc_remove_host(). The same is for suspend and resume
routines.
sdhci_init() no longer enables card-detection irqs. Instead, two new
functions implemented: sdhci_enable_card_detection() and
sdhci_disable_card_detection().
New sdhci_reinit() call implemented to behave the same way as the old
sdhci_init().
Also, this patch implements and uses few new helpers to manage IRQs in
a more conveinient way, that is:
- sdhci_clear_set_irqs()
- sdhci_unmask_irqs()
- sdhci_mask_irqs()
- SDHCI_INT_ALL_MASK constant
sdhci_enable_sdio_irq() converted to these new helpers, plus the
helpers will be used by the subsequent patches.

Currently the SDHCI driver works with PCI accessors (write{l,b,w} and
read{l,b,w}).
With this patch drivers may change memory accessors, so that we can
support hosts with "weird" IO memory access requirments.
For example, in "FSL eSDHC" SDHCI hardware all registers are 32 bit
width, with big-endian addressing. That is, readb(0x2f) should turn
into readb(0x2c), and readw(0x2c) should be translated to
le16_to_cpu(readw(0x2e)).

Data transfers on third OMAP3 MMC controller don't work
because DMA line numbers are only defined for MMC1 and MMC2.
Fix that and store line numbers in mmc_omap_host structure
to reduce code size.

Some MMC commands result in the card becoming busy after
the response is received. This needs to be specified
for the omap_hsmmc host controller, which is what this
patch does. However, the effect is that some commands
with no data will cause a Transfer Complete (TC) interrupt
in addition to the Command Complete (CC) interrupt.
In order to deal with that, the irq handler has needed
a few changes also.
The benefit of this change is that the omap_hsmmc host
controller driver now waits for the TC interrupt while
the card is busy, so the mmc_block driver needs to poll
the card status just once instead of repeatedly.
i.e. the net result is more sleep and less cpu.
The command sequence for open-ended multi-block write
with DMA is now:
Issue write command CMD25
Receive CC interrupt
Data is sent
Receive TC interrupt (DMA is done)
Issue stop command CMD12
Receive CC interrupt
Card is busy
Receive TC interrupt
Card is now ready for next transfer

Most registers lose its state when the processor wakes up from sleep state.
Thus registers should be initialized, when the processor wakes up. However the
current hsmmc 'resume' function doesn't consider this issue and finally makes
deadlock. So this patch fixes this problem.

Remove code that turns MMC1 power back on after it
has been powered off (when the voltage is 1.8V).
The offending code is not necessary because the
host controller bus voltage is initialized to
3V when probing or resuming. Note that MMC powers up
with the highest voltage available (see mmc_power_up())
which will be 3V also.

Currently we are using an explicit udev rule to trigger loading of the
mmc-block module when an MMC or SD card is detected:
SUBSYSTEM=="mmc", RUN+="/sbin/modprobe -Qba mmc-block"
It makes much more sense for the mmc bus driver and the mmc-block module to
share MODALIAS information so that they are linked automatically.
There is no real information of use in the MMC system at the current time.
All devices inserted require us to load the mmc-block device. Until such
time as useful parameters exist simply reflect the module linkage via
the module alias below:

the work queue. This takes care of cases when delayed_work was scheduled

The s3cmci driver is calling s3c2410_dma_config with incorrect data for
the DCON register. The S3C2410_DCON_HWTRIG is implicit in the channel
configuration and the device selection of S3C2410_DCON_CH0_SDI is
incorrect as the DMA system may not select channel 0.

1. Driver code where pxa_request_dma() is called will most likely
reference DMA registers as well, and it is really unnecessary
to include pxa-regs.h just for this. Move the definitions into
<mach/dma.h> and make relevant drivers include it instead of
<mach/pxa-regs.h>.
2. Introduce DMAC_REGS_VIRT as the virtual address base for these
DMA registers. This allows later processors to re-use the same
IP while registers may start at different I/O address.

The Samsung SDHCI (and FSL eSDHC) controller block seems to fail
to generate an INT_DATA_END after the transfer has completed and
the bus busy state finished.
Changes in e809517f6f to use the
new busy method are the cause of the behaviour change.

The conversion of atmel-mci to dma_request_channel missed the
initialization of the channel dma_slave information. The filter_fn passed
to dma_request_channel is responsible for initializing the channel's
private data. This implementation has the additional benefit of enabling
a generic client-channel data passing mechanism.

Timeouts during a command that has a data phase can result in the next
command issued after the command that failed not being processed, i.e. no
interrupt ever occurs to indicate the command has completed. This failure
can result in a deadlock.
This patch resets the data state machine to clear the error in case of a
command timeout.
Tested on OMAP3430 chip and intensive MMC/SD device removal while
transferring data.

Based on a patch from Tony Lindgren ... after initialization,
never change HCTL.SDVS except for MMC1. The other controller
instances only support 1.8V in that field, although they can
suport other card/SDIO/eMMC/... voltages with level shifting
solutions such as external transceivers.
MMC2 behavior sanity tested on Overo/WLAN, OMAP3430 SDP, and
custom hardware. MMC1 also sanity tested on those platforms
plus Beagle. This also fixes a bug preventing MMC2 (and also
presumably MMC3) from powering down when requested.

Fix the led device naming for the sdhci driver.
The led class documentation defines the led name to have the
form "devicename:colour:function" while not applicable sections
should be left blank.
To comply with the documentation the led device name is changed
from "mmc*" to "mmc*::".

This commit fixes the regression what was added by commit
088a78af97 "s3cmci: Support transfers
which are not multiple of 32 bits."
fifo_free() now returns amount of available space in FIFO buffer in
bytes. But do_pio_write() writes to FIFO 32-bit words. Condition for
return from cycle is (fifo_free() == 0), but when fifo has 1..3 bytes
of free space then this condition will never be true and system hangs.
This patch changes condition in the while() to (fifo_free() > 3).

With the PXA270 MMC hardware, there seems to be an issue of
data corruption on writes where a 4KB data block is offset
by one byte.
If we delay enabling the DMA for writes until after the CMD/RESP
has finished, the problem seems to be fixed.

If ricoh_mmc suspends before sdhci_pci, it will pull the card
out from under the controller, which could leave the system in
a very confused state.
Using suspend_late/resume_early ensures that sdhci_pci suspends first
and resumes second.

alterations/bugfixes related to open drain and HW flow control.
They also add some SDIO registers, I am unsure if these are
in ST HW only or if this is things also added in later ARM
revisions, but they are included in the mmci.h file for
completeness.

This patch adds a MX2/MX3 specific SDHC driver. The hardware is basically
the same as in the MX1, but unlike the MX1 controller the MX2
controller just works as expected. Since the MX1 driver has more
workarounds for bugs than anything else I had no success with supporting
MX1 and MX2 in a sane way in one driver.

Add omap hsmmc controller for 2430 and 34xx.
Note that this controller has different registers compared to
the earlier omap MMC controller, so sharing code currently is
not possible.
Various updates and fixes from linux-omap list have been
merged into this patch.

dma_request_channel provides an exclusive channel, so we no longer need to
pass slave data through dmaengine.

Simply, if a client wants any dmaengine channel then prevent all dmaengine
modules from being removed. Once the clients are done re-enable module
removal.
Why?, beyond reducing complication:
1/ Tracking reference counts per-transaction in an efficient manner, as
is currently done, requires a complicated scheme to avoid cache-line
bouncing effects.
2/ Per-transaction ref-counting gives the false impression that a
dma-driver can be gracefully removed ahead of its user (net, md, or
dma-slave)
3/ None of the in-tree dma-drivers talk to hot pluggable hardware, but
if such an engine were built one day we still would not need to notify
clients of remove events. The driver can simply return NULL to a
->prep() request, something that is much easier for a client to handle.
