Current vexpress smp init code detects whether to override the
default smp ops with MCPM smp ops by matching the "cci-400"
compatible string, in that MCPM requires control over CCI ports
to manage low-power states entry/exit.
The "cci-400" compatible string check is a necessary but not
sufficient condition for MCPM to work, because the cci-400
can be made visible to the kernel, but firmware can nonetheless
disable non-secure CCI ports control, while still allowing PMU
access; if booted in non-secure world, the kernel would still
blindly override smp operations with MCPM operations, resulting
in kernel faults when the CCI ports programming interface is
accessed from non-secure world.
This means that the "cci-400" compatible string check would
result in a false positive in systems that eg boot in HYP mode,
where CCI ports non-secure access is explicitly not allowed,
and it is reported in the respective device tree nodes with
CCI ports marked as disabled.
Refactor the smp operations initialization to make sure that
the kernel is actually allowed to take control over CCI ports
(by enabling MCPM smp operations) before overriding default
vexpress smp operations.

Now, with the CLCD DT support available, there is no
more reason to keep the non-DT support for V2P-CA9.
Removed, together with "some" supporting code. It was
necessary to make PLAT_VERSATILE_SCHED_CLOCK optional
and selected by the machines still interested in it.

As all cores must be properly described in the Device Tree,
there is no point in getting their numbers from SCU on
A5/A9 platforms. This significantly simplifies the code,
removing the need for flat-tree scanning and early static
mapping.

arm_dt_init_cpu_maps parses the device tree, validates and sets the
cpu_possible_mask appropriately. It is unnecessary to do another DT
parse to get the number of cpus, use num_possible_cpus instead.
This patch also removes setting cpu_present_mask as platforms should
only re-initialize it in smp_prepare_cpus() if present != possible.

In preparation of moving gic code to drivers/irqchip, remove the direct
platform dependencies on gic_raise_softirq. Move the setup of
smp_cross_call into the gic code and use arch_send_wakeup_ipi_mask
function to trigger wake-up IPIs.

This patch starts using all the configuration infrastructure.
- generic GPIO library is forced now
- sysreg GPIOs are used as MMC CD and WP information sources;
thanks to this MMCI auxiliary data is not longer necessary
- DVI muxer and mode control is removed from non-DT V2P-CA9 code
as this is now handled by the vexpress-dvi driver
- clock generators control is removed as is being handled by the
common clock driver now
- the sysreg and sysctl control is now delegated to the
appropriate drivers and all related code was removed
- NOR Flash set_vpp function has been removed as the control
bit used does _not_ control its VPP line, but the #WP signal
instead (which is de facto unusable in case of Linux MTD
drivers); this also allowed the remove its DT auxiliary
data
The non-DT code defines only minimal required number of
the config devices. Device Trees are updated to make use
of all new features.

Scan flatten device looking for A5/A9 SCU node and initialize
it using base address in "reg" property. If nothing is found,
assume that there is no special SCU initialization required
and initialize CPUs basing on numbers of "cpu" type devices
in "cpus" node of the Device Tree.
All this happens only if the board was booted with FDT,
otherwise ct_desc callbacks are used.

This patch gets rid of the MMIO_P2V and __MMIO_P2V macros,
defining constant virtual base for motherboard and tile
peripherals instead.
Additionally, in preparation for the new motherboard memory
map, the motherboard peripherals are using base pointers
calculated in runtime, instead of compile-time calculated
values.

The current Versatile Express BSP defines the MACHINE_START macro
in the core tile code.
This patch moves this into the generic board code and introduces a
method for determining the current tile at runtime, allowing the
Kernel to have support for multiple tiles compiled in. Tile-specific
functions are executed via a descriptor struct containing the correct
implementations for the current tile.

There is a subtle race in the CPU hotplug code, where a CPU which has
been offlined can online itself before being requested, which results
in things going astray on the next online/offline cycle.
What happens in the normal online/offline/online cycle is:
CPU0 CPU3
requests boot of CPU3
pen_release = 3
flush cache line
checks pen_release, reads 3
starts boot
pen_release = -1
... requests CPU3 offline ...
... dies ...
checks pen_release, reads -1
requests boot of CPU3
pen_release = 3
flush cache line
checks pen_release, reads 3
starts boot
pen_release = -1
However, as the write of -1 of pen_release is not fully flushed back to
memory, and the checking of pen_release is done with caches disabled,
this allows CPU3 the opportunity to read the old value of pen_release:
CPU0 CPU3
requests boot of CPU3
pen_release = 3
flush cache line
checks pen_release, reads 3
starts boot
pen_release = -1
... requests CPU3 offline ...
... dies ...
checks pen_release, reads 3
starts boot

Fix this by grouping the write of pen_release along with its cache line
flushing code to ensure that any update to pen_release is always pushed
out to physical memory.

There is a certain amount of smp_prepare_cpus() which doesn't belong
in the platform support code - that is, code which is invariant to the
SMP implementation. Move this code into arch/arm/kernel/smp.c, and
add a platform_ prefix to the original function.

scu_get_core_count() never returns zero cores, so we don't need to
check and correct if ncores is zero.
Tegra was missing the check against NR_CPUS, leading to a potential
bitfield overflow if this becomes the case.

We don't need to re-pass the base address for the CPU interfaces to the
GIC for secondary CPUs, as it will never be different from the boot CPU
- and even if it was, we'd overwrite the boot CPU's base address.
Get rid of this argument, and rename to gic_secondary_init().

This allows us to use smp_cross_call() to trigger a number of different
software generated interrupts, rather than combining them all on one
SGI. Recover the SGI number via do_IPI.
