The currently include file ordering for rdmavt headers has an
ab/ba include issue the precludes using inlines from rdma_vt.h
in rdmavt_qp.h.
At the heart of the issue is that rdma_vt.h includes rdmavt_qp.h.
Fix the ordering issue by adjusting rdma_vt.h to not require rdmavt_qp.h
and move qp related inlines to rdmavt_qp.h.
Additionally, promote rvt_mmap_info to rdma_vt.h since it is shared
by rdmavt_cq.h and rdmavt_qp.h.

This patch moves hfi1_copy_sge() into rdmavt for sharing with qib.
This patch also moves all the wss_*() functions into rdmavt as
several wss_*() functions are called from hfi1_copy_sge()
When SGE copy mode is adaptive, cacheless copy may be done in some cases
for performance reasons. In those cases, X86 cacheless copy function
is called since the drivers that use rdmavt and may set SGE copy mode
to adaptive are X86 only. For this reason, this patch adds
"depends on X86_64" to rdmavt/Kconfig.

Current implementation allows each qp to have only one send engine. As
such, each qp has only one list to queue prebuilt packets when send engine
resources are not available. To improve performance, it is desired to
support multiple send engines for each qp.
This patch creates the framework to support two send engines
(two legs) for each qp for the TID RDMA protocol, which can be easily
extended to support more send engines. It achieves the goal by creating a
leg specific struct, iowait_work in the iowait struct, to hold the
work_struct and the tx_list as well as a pointer to the parent iowait
struct.
The hfi1_pkt_state now has an additional field to record the current legs
work structure and that is now passed to all egress waiters to determine
the leg that needs to wait via a new iowait helper. The APIs are adjusted
to use the new leg specific struct as required.
Many new and modified helpers are added to support this change.

The post_send() path determines if it should post directly or, schedule
the post for later. The current logic is:
if the swqe ring is empty or (for hfi1) wqe->length <= piothreshold
post the send
else
schedule
This can allow large requests to call the send engine directly. Large
requests can potentially produce a large number of packets prior to
returning to the caller, blocking the caller from posting more requests,
and allowing better parallel processing.
Allow the driver(s) more say in this logic (pass call_send to the driver,
rather than examining a return value).
Update hfi1/qib logic to schedule the send engine if an RC or UC message
is larger than the QP MTU size.

The internal flag IP_BASED_GIDS was added to a field that was being used
to hold the port Info CapabilityMask without considering the effects this
will have. Since most drivers just use the value from the HW MAD it means
IP_BASED_GIDS will also become set on any HW that sets the IBA flag
IsOtherLocalChangesNoticeSupported - which is not intended.
Fix this by keeping port_cap_flags only for the IBA CapabilityMask value
and store unrelated flags externally. Move the bit definitions for this to
ib_mad.h to make it clear what is happening.
To keep the uAPI unchanged define a new set of flags in the uapi header
that are only used by ib_uverbs_query_port_resp.port_cap_flags which match
the current flags supported in rdma-core, and the values exposed by the
current kernel.

The caller to the driver marks GFP_NOIO allocations with help
of memalloc_noio-* calls now. This makes redundant to pass down
to the driver gfp flags, which can be GFP_KERNEL only.
The patch removes the gfp flags argument and updates all driver paths.

According to IBTA spec a QKey violation should not result in a bad qkey
trap being triggered for UD queue pairs. Also since it is a silent error
we do not increment the q_key violation or the dropped packet counters.

Use common header file structs, defines, and accessors
in the drivers. The old declarations are removed.
The repositioning of the includes allows for the removal
of hfi1_message_header and replaces its use with ib_header.
Also corrected are two issues with set_armed_to_active():
- The "packet" parameter is now a pointer as it should have been
- The etype is validated to insure that the header is correct

The dual lock patch moved locking around and missed an issue
with handling irq flags when processing UD loopback
packets. This issue was revealed by smatch.
Fix for both qib and hfi1 to pass the saved flags to the UD request
builder and handle the changes correctly.

This patch adds an additional lock to reduce contention on the s_lock.
This lock is used in post_send() so that the post_send is not
serialized with the send engine and other send related processing.
To do this the s_next_psn is now maintained on post_send() while
post_send() related fields are moved to a new cache line. There is
an s_avail maintained for the post_send() to mitigate trading cache
lines with the send engine. The lock is released/acquired around
releasing the just built packet to the egress mechanism.

Add calls to rcu_read_lock()/rcu_read_unlock() as rvt_lookup_qpn callers
must hold the rcu_read_lock before calling and keep the lock until the
returned qp is no longer in use.
Remove lookaside qp and some qp refcount atomics in the sdma send code
that is redundant with the s_dma_busy refcount, which will also stall
the state processing to the reset state.
Change the qpn hash function to hash_32 which is hash function used
in rvt_lookup_qpn. qpn_hash function would be eliminated in later patches.

This patch removes the post_send and post_one_send from the qib driver.
The "posting" of sends will be done by rdmavt which will walk a WQE and
queue work. This patch will still provide the capability to schedule that
work as well as kick the progress. These are provided to the rdmavt layer.

rc_qacks, and rc_delayed_comp are defined as per CPU variables in rdmavt.

In preparation for moving the queue pair data structure to rdmavt the
members of the driver specific queue pairs which are not common need to be
pushed off to a private driver structure. This structure will be available
in the queue pair once moved to rdmavt as a void pointer. This patch while
not adding a lot of value in and of itself is a prerequisite to move the
queue pair out of the drivers and into rdmavt.
The driver specific, private queue pair data structure should condense as
more of the send side code moves to rdmavt.

providing access to the header files. This is just the beginning of
rdmavt support in qib.

struct qib_mr requires the mr member be the last because struct
qib_mregion contains a dynamic array at the end. The additions
of members should have been placed before this structure as the
comment noted.
Failure to do so was causing random memory corruption. Reproducing
this bug was easy to do by running the client and server of
ib_write_bw -s 8 -n 5 on the same node.
