Currently the driver validates command codes received from the
application. COMMON_SET_FEATURES is not currently being approved.
Add definition of the missing command and allow it to be issued by
applications.

Change vocabulary of 0373 log msg from "error" to "cmpl" The current
language of the 0373 message contains the word "error" which caused a
number of customers to inquire about the "error" and if it should be a
concern. It isn't an error, it's simply an io completion status.
Revise the message to replace the word "error" with "cmpl" for completion.

When the kdump kernel shuts down lpfc calls flush_work_queue on an
interrupt to schedule the cq handler. When there is only one CPU active on
the kdump kernel, it is possible for the work_on to get scheduled on a
non-active CPU causing it to never be scheduled.
When in the kdump environment, per-CPU affinity of cq's to cpus is not
necessary. In those cases, use a general queue_work rather than a
queue_work_on().

When vports are deleted, it is observed that there is memory/kthread
leakage as the vport isn't fully being released.
There is a shost reference taken in scsi_add_host_dma that is not released
during scsi_remove_host. It was noticed that other drivers resolve this by
doing a scsi_host_put after calling scsi_remove_host.
The vport_delete routine is taking two references one that corresponds to
an access to the scsi_host in the vport_delete routine and another that is
released after the adapter mailbox command completes that destroys the VPI
that corresponds to the vport.
Remove one of the references taken such that the second reference that is
put will complete the missing scsi_add_host_dma reference and the shost
will be terminated.

Call traces have been observed running different tests that involve aborts
and setting the rrq active flag. The lpfc_set_rrq_active routine is doing
a mempool_alloc under the soft_irq processing level. When the mempool needs
to get a new buffer from the free pool and has to wait for memory to become
free it will check the flags passed in on the alloc and dump the stack if
the thread is running in interrupt context.
Replace the GFP_KERNEL flag with GFP_ATOMIC so that the memory allocation
will not attempt to sleep if there is no mem available.

When using DUMP on SLI3 to read VPD and Port status data (config region
23), the adapter is overruning the kmalloc'd buffer causing havoc on other
consumers of the allocation pools.
Rework the loops processing the dump data and validate/size memory lengths
before performing bcopy.

During driver unload/reload testing, the NVMe initiator would not
re-establish connectivity to NVMe controllers on reload.
The failing NVMe array supports concurrent FCP and NVMe operation via
different nport_id's. The array was repeatedly sending an ADISC every 2
seconds after PLOGI completed and while NVMe subsystems were executing
discovery. The target would continue this state for roughly 45 seconds.
The driver's current behavior on ADISC receipt is to validate a the ADISC
vs the device and issue a RESUME_RPI to restore transmission. The receipt
of the ADISC effectively caused a driver to take actions similar to a
logout and login for the remote port, causing the deregistration of the
nvme rport and a subsequent re-registration. This caused a constant reset
and re-connect of the NVMe controller while this 45s window occurred. There
was no need for the state changes as ADISC does not change login state.
This patch corrects this behavior by validating if the remoteport is
already logged in (MAPPED) and when true, avoids the call to set the ndlp
state to MAPPED, which triggers the unreg/re-reg. Thus ADISC does not
change the login state of the node.

Coverity reported the following error:
Assigned value that is never used may represent unnecessary computation.
The rc variable was initially assigned a value but in several cases, when
an error case is detected, it is reassigned a new value. The initial value
had little use.
In code-reviewing this routine, it could use some cleanup:
- Setting the initialization value to -ENODEV is a much better choice and
lessens code in the routine.
- The wasn't tracking logic errors vs no error and mailbox failure.
Better to resolve by adding a status to track the mailbox failure
and merge it with the logic error when the routine returns.

Commit cdb42becdd ("scsi: lpfc: Replace io_channels for nvme and fcp with
general hdw_queues per cpu") has introduced static checker warnings for
potential null dereferences in 'lpfc_sli4_hba_unset()' and commit 1ffdd2c044
("scsi: lpfc: resolve static checker warning in lpfc_sli4_hba_unset") has
tried to fix it. However, yet another potential null dereference is
remaining. This commit fixes it.
This bug was discovered and resolved using Coverity Static Analysis
Security Testing (SAST) by Synopsys, Inc.

The axchg structure is a structure allocated early in the
lpfc_nvme_unsol_ls_handler() to represent the newly received exchange.
Upon error, the out_fail path in the routine unconditionally frees the
pointer, yet subsequently passes the pointer to the abort routine.
Additionally, the abort routine, lpfc_nvme_unsol_ls_issue_abort(), also
has a failure path that will attempt to delete the pointer on error.
Fix these errors by:
- Removing the unconditional free so that it stays valid if passed
to the abort routine.
- Revise the abort routine to not free the pointer. Instead, return
a success/failure status. Note: if success, the later completion of
the abort frees the structure.
- Back in the unsol_ls_handler() error path, if the abort routine was
skipped (thus no possible reference) or the abort routine returned
error, free the pointer.

Additional testing encountered null pointers that weren't fully qualified
in lpfc_nvmet_xmt_ls_abort_cmp() and lpfc_nvmet_unsol_issue_abort().
The same error was detected and reported by static checker reporting:

In order to create or activate a new node, lpfc_els_unsol_buffer() invokes
lpfc_nlp_init() or lpfc_enable_node() or lpfc_nlp_get(), all of them will
return a reference of the specified lpfc_nodelist object to "ndlp" with
increased refcnt.
When lpfc_els_unsol_buffer() returns, local variable "ndlp" becomes
invalid, so the refcount should be decreased to keep refcount balanced.
The reference counting issue happens in one exception handling path of
lpfc_els_unsol_buffer(). When "ndlp" in DEV_LOSS, the function forgets to
decrease the refcnt increased by lpfc_nlp_init() or lpfc_enable_node() or
lpfc_nlp_get(), causing a refcnt leak.
Fix this issue by calling lpfc_nlp_put() when "ndlp" in DEV_LOSS.

As the nvmet layer does not have the concept of a remoteport object, which
can be used to identify the entity on the other end of the fabric that is
to receive an LS, the hosthandle was introduced. The driver passes the
hosthandle, a value representative of the remote port, with a ls request
receive. The LS request will create the association. The transport will
remember the hosthandle for the association, and if there is a need to
initiate a LS request to the remote port for the association, the
hosthandle will be used. When the driver loses connectivity with the
remote port, it needs to notify the transport that the hosthandle is no
longer valid, allowing the transport to terminate associations related to
the hosthandle.
This patch adds support to the driver for the hosthandle. The driver will
use the ndlp pointer of the remote port for the hosthandle in calls to
nvmet_fc_rcv_ls_req(). The discovery engine is updated to invalidate the
hosthandle whenever connectivity with the remote port is lost.

Now that common helpers exist, add the ability to receive NVME LS requests
to the driver. New requests will be delivered to the transport by
nvme_fc_rcv_ls_req().
In order to complete the LS, add support for Send LS Response and send
LS response completion handling to the driver.

Send LS Abort support is needed when Send LS Request is supported.
Currently, the ability to abort an NVME LS request is limited to the nvme
(host) side of the driver. In preparation of both the nvme and nvmet sides
supporting Send LS Abort, rework the existing ls_req abort routines such
that there is common code that can be used by both sides.
While refactoring it was seen the logic in the abort routine was incorrect.
It attempted to abort all NVME LS's on the indicated port. As such, the
routine was reworked to abort only the NVME LS request that was specified.

In preparation for supporting both intiator mode and target mode
receiving NVME LS's, commonize the existing NVME LS request receive
handling found in the base driver and in the nvmet side.
Using the original lpfc_nvmet_unsol_ls_event() and
lpfc_nvme_unsol_ls_buffer() routines as a templates, commonize the
reception of an NVME LS request. The common routine will validate the LS
request, that it was received from a logged-in node, and allocate a
lpfc_async_xchg_ctx that is used to manage the LS request. The role of
the port is then inspected to determine which handler is to receive the
LS - nvme or nvmet. As such, the nvmet handler is tied back in. A handler
is created in nvme and is stubbed out.

To support FC-NVME-2 support (actually FC-NVME (rev 1) with Ammendment 1),
both the nvme (host) and nvmet (controller/target) sides will need to be
able to receive LS requests. Currently, this support is in the nvmet side
only. To prepare for both sides supporting LS receive, rename
lpfc_nvmet_rcv_ctx to lpfc_async_xchg_ctx and commonize the definition.

A lot of files in lpfc include nvme headers, building up relationships that
require a file to change for its headers when there is no other change
necessary. It would be better to localize the nvme headers.
There is also no need for separate nvme (initiator) and nvmet (tgt)
header files.
Refactor the inclusion of nvme headers so that all nvme items are
included by lpfc_nvme.h
Merge lpfc_nvmet.h into lpfc_nvme.h so that there is a single header used
by both the nvme and nvmet sides. This prepares for structure sharing
between the two roles. Prep to add shared function prototypes for upcoming
shared routines.

The current LLDD api has:
nvme-fc: contains api for transport to do LS requests (and aborts of
them). However, there is no interface for reception of LS's and sending
responses for them.
nvmet-fc: contains api for transport to do reception of LS's and sending
of responses for them. However, there is no interface for doing LS
requests.
Revise the api's so that both nvme-fc and nvmet-fc can send LS's, as well
as receiving LS's and sending their responses.
Change name of the rcv_ls_req struct to better reflect generic use as
a context to used to send an ls rsp. Specifically:
nvmefc_tgt_ls_req -> nvmefc_ls_rsp
nvmefc_tgt_ls_req.nvmet_fc_private -> nvmefc_ls_rsp.nvme_fc_private
Change nvmet_fc_rcv_ls_req() calling sequence to provide handle that
can be used by transport in later LS request sequences for an association.

In an audit of lockdep calls in the driver, there are multiple lockdep
checks in successive calling layers. E.g. a routine checks, and then calls
a lower routine that also checks, and so on. Calling sequences result in
many redundant checks.
Refine the code to remove lower-level lockdep checks. Update comments on
the lock, correcting a few places where lock object in comment was
incorrect.

By default, the driver attempts to allocate a hdwq per logical cpu in order
to provide good cpu affinity. Some systems have extremely high cpu counts
and this can significantly raise memory consumption.
In testing on x86 platforms (non-AMD) it is found that sharing of a hdwq by
a physical cpu and its HT cpu can occur with little performance
degredation. By sharing, the hdwq count can be halved, significantly
reducing the memory overhead.
Change the default behavior of the driver on non-AMD x86 platforms to
share a hdwq by the cpu and its HT cpu.

Implementation of a previous patch added a condition to an if check that
always end up with the if test being true. Execution of the else clause was
inadvertently negated. The additional condition check was incorrect and
unnecessary after the other modifications had been done in that patch.
Remove the check from the if series.

The lldd rebinds the ndlp with rport during a nvme rport registration (via
nvme_fc_register_remoteport). If rport & ndlp pointers are same as the
previous one, the lldd will re-use the ndlp and rport association without
re-initialization. This assumption is incorrect. The lldd should be
ignorant of whether the returned rport pointer is new or not, and should
always assume it is new.
Remove the re-binding code, always assumes that rport pointer received from
transport is a new pointer.

A previous change introduced the atomic use of queue_claimed flag for eq's
and cq's. The code works fine, but the clearing of the queue_claimed flag
is not atomic.
Change queue_claimed = 0 into xchg(&queue_claimed, 0) to be consistent for
change under atomicity.

The original patch was to resolve the lldd being able to be unloaded
while being used to talk to the boot device of the system. However, the
end result of the original patch is that any driver unload while a nvme
controller is live via the lldd is now being prohibited. Given the module
reference, the module teardown routine can't be called, thus there's no
way, other than manual actions to terminate the controllers.

The default lun queue depth by the driver has been 30 for many years.
However, this value, when used with more recent hardware, has actually
throttled some tests that concentrate io on a lun.
Increase the default lun queue depth to 64.
Queue full handling, reported by the target, remains in effect and
unchanged.

During code review, identified dss feature that was a prototype only and
was never productized in SLI3. They shouldn't be there and prevents reuse
of the command areas.
Remove any code in the driver to deal with dss, including code to deal with
fips, which is associated with the dss feature.

Currently driver ktime stats, measuring code paths, is NVME-specific.
Convert the stats routines such that the code paths are generic, providing
status for NVME and SCSI. Added ktime stat calls in SCSI queuecommand and

The cpu io statistics were capped by a hard define limit of 128. This
effectively was a max number of CPUs, not an actual CPU count, nor actual
CPU numbers which can be even larger than both of those values. This made
stats off/misleading and on large CPU count systems, wrong.
Fix the stats so that all CPUs can have a stats struct. Fix the looping
such that it loops by hdwq, finds CPUs that used the hdwq, and sum the
stats, then display.

The AER interfaces to clear error status registers were a confusing mess:
- pci_cleanup_aer_uncorrect_error_status() cleared non-fatal errors
from the Uncorrectable Error Status register.
- pci_aer_clear_fatal_status() cleared fatal errors from the
Uncorrectable Error Status register.
- pci_cleanup_aer_error_status_regs() cleared the Root Error Status
register (for Root Ports), the Uncorrectable Error Status register,
and the Correctable Error Status register.
Rename them to make them consistent:

During target side fault injections, it is possible to hit the
NLP_WAIT_FOR_UNREG case in lpfc_nvme_remoteport_delete. A prior commit
fixed a rebind and delete race condition, but called lpfc_nlp_put
unconditionally. This triggered a deletion and the crash.
Fix by movng nlp_put to inside the NLP_WAIT_FOR_UNREG case, where the nlp
will be being unregistered/removed. Leave the reference if the flag isn't
set.

The lpfc_sli4_wq_release() routine iterates for each interim value when
updating the wq consuemr index. This wastes cycles and possibly confuses
things as thevalue itterates (and the modulo logic is being applied).
There's no reason for this. Just set it to the value from the hw.

Injecting EEH on a 32GB card is causing kernel oops
The pci error handler is doing an IO flush and the offline code is also
doing an IO flush. When the 1st flush is complete the hdwq is destroyed
(freed), yet the second flush accesses the hdwq and crashes.
Added a check in lpfc_sli4_fush_io_rings to check both the HBA_IOQ_FLUSH
flag and the hdwq pointer to see if it is already set and not already
freed.

SCSI layer sends driver IOs with more s/g segments than driver can handle.
This results in "Too many sg segments from dma_map_sg. Config 64, seg_cnt
219" error messages from the lpfc_scsi_prep_dma_buf_s3() routine.
The was due to use the driver using individual templates for pport and
vport, host reset enabled or not, nvme vs scsi, etc. In the end, there was
a combination for a vport that didn't match the pport.
Rather than enumerating more templates and more discretionary assignments,
revert to a base template that is copied to a template specific to the
pport/vport. Then, based on role, attributes and sli type, modify the
fields that are different for that port. Added a log message to
lpfc_create_port to validate values.

In lpfc_nvmet_prep_fcp_wqe() the line "rsp->sg_cnt = 0" is modifying the
transport's data structure. This may result in the transport believing the
s/g list was already freed, thus may not unmap/free it properly. Lpfc
driver should not modify the transport data structure.
The zeroing of the sg_cnt is to avoid use of the transport's sgl in a
subsequent loop where the driver builds the necessary requests for the
adapter firmware to complete the IO.
Change LLDD to use a local copy of the transport sg_cnt when building
requests to be passed to the adapter fw.

Unloading the driver resulted in a call to del_timer_sync for the
cpuhp_poll_timer. However the call to setup the timer had never been made,
so the timer structures used by lockdep checking were not initialized.
Unconditionally call setup_timer for the cpuhp_poll_timer during driver
initialization. Calls to start the timer remain "as needed".

When processing the completion of a "Reg Rpi" login mailbox command in
lpfc_sli_def_mbox_cmpl, a call may be made to lpfc_unreg_login. The vpi is
extracted from the completing mailbox context and passed as an input for
the next. However, the vpi stored in the mailbox command context is an
absolute vpi, which for SLI4 represents both base + offset. When used with
a non-zero base component, (function id > 0) this results in an
out-of-range access beyond the allocated phba->vpi_ids array.
Fix by subtracting the function's base value to get an accurate vpi number.

This patch modifies lpfc to register for Link Integrity events via the use
of an RDF ELS and to perform Link Integrity FPIN logging.
Specifically, the driver was modified to:
- Format and issue the RDF ELS immediately following SCR registration.
This registers the ability of the driver to receive FPIN ELS.
- Adds decoding of the FPIN els into the received descriptors, with
logging of the Link Integrity event information. After decoding, the ELS
is delivered to the scsi fc transport to be delivered to any user-space
applications.
- To aid in logging, simple helpers were added to create enum to name
string lookup functions that utilize the initialization helpers from the
fc_els.h header.
- Note: base header definitions for the ELS's don't populate the
descriptor payloads. As such, lpfc creates it's own version of the
structures, using the base definitions (mostly headers) and additionally
declaring the descriptors that will complete the population of the ELS.

The current code does some odd +1 over maximum xri count checks and
requires that the lun_queue_count can't be bigger than maximum xri count
divided by 8. These items are bogus.
Clean the code up to cap lun_queue_count to maximum xri count.

There was report of an odd "Fix me..." log message, which was tracked down
to the lpfc_els_rcv_rps() routine. This was in handling of a very old and
obsolete ELS - Read Port Status. The RPS ELS was defined in FC-LS-1, but
deprecated in FC-LS-2, and removed from all later FC-LS revisions. It was
replaced by the Read Diagnostic Parameters (RDP) ELS and the Link Error
Status Block descriptor.
There should be no support for the RSP ELS. Remove support from driver.

Coverity reported a memory corruption error for the fdmi attributes
routines:
CID 15768 [Memory Corruption] Out-of-bounds access on FDMI
Sloppy coding of the fmdi structures. In both the lpfc_fdmi_attr_def and
lpfc_fdmi_reg_port_list structures, a field was placed at the start of
payload that may have variable content. The field was given an arbitrary
type (uint32_t). The code then uses the field name to derive an address,
which it used in things such as memset and memcpy. The memset sizes or
memcpy lengths were larger than the arbitrary type, thus coverity reported
an error.
Fix by replacing the arbitrary fields with the real field structures
describing the payload.

When performing reset testing, the eq's list for related hwqs was getting
corrupted. In cases where there is not a 1:1 eq to hwq, the eq is
shared. The eq maintains a list of hwqs utilizing it in case of cpu
offlining and polling. During the reset, the hwqs are being torn down so
they can be recreated. The recreation was getting confused by seeing a
non-null eq assignment on the eq and the eq list became corrupt.
Correct by clearing the hdwq eq assignment when the hwq is cleaned up.

When driver is set to enable bb credit recovery, the switch displayed the
setting as inactive. If the link bounces, it switches to Active.
During link up processing, the driver currently does a MBX_READ_SPARAM
followed by a MBX_CONFIG_LINK. These mbox commands are queued to be
executed, one at a time and the completion is processed by the worker
thread. Since the MBX_READ_SPARAM is done BEFORE the MBX_CONFIG_LINK, the
BB_SC_N bit is never set the the returned values. BB Credit recovery status
only gets set after the driver requests the feature in CONFIG_LINK, which
is done after the link up. Thus the ordering of READ_SPARAM needs to follow
the CONFIG_LINK.
Fix by reordering so that READ_SPARAM is done after CONFIG_LINK. Added a
HBA_DEFER_FLOGI flag so that any FLOGI handling waits until after the
READ_SPARAM is done so that the proper BB credit value is set in the FLOGI
payload.

If a call to lpfc_get_cmd_rsp_buf_per_hdwq returns NULL (memory allocation
failure), a previously allocated lpfc_io_buf resource is leaked.
Fix by releasing the lpfc_io_buf resource in the failure path.

The driver is occasionally seeing the following SLI Port error, requiring
reset and reinit:
Port Status Event: ... error 1=0x52004a01, error 2=0x218
The failure means an RQ timeout. That is, the adapter had received
asynchronous receive frames, ran out of buffer slots to place the frames,
and the driver did not replenish the buffer slots before a timeout
occurred. The driver should not be so slow in replenishing buffers that a
timeout can occur.
When the driver received all the frames of a sequence, it allocates an IOCB
to put the frames in. In a situation where there was no IOCB available for
the frame of a sequence, the RQ buffer corresponding to the first frame of
the sequence was not returned to the FW. Eventually, with enough traffic
encountering the situation, the timeout occurred.
Fix by releasing the buffer back to firmware whenever there is no IOCB for
the first frame.

This series concludes the work I did for linux-5.5 on the compat_ioctl()
cleanup, killing off fs/compat_ioctl.c and block/compat_ioctl.c by moving
everything into drivers.
Overall this would be a reduction both in complexity and line count, but
as I'm also adding documentation the overall number of lines increases
in the end.
My plan was originally to keep the SCSI and block parts separate.
This did not work easily because of interdependencies: I cannot
do the final SCSI cleanup in a good way without first addressing the
CDROM ioctls, so this is one series that I hope could be merged through
either the block or the scsi git trees, or possibly both if you can
pull in the same branch.
The series comes in these steps:
1. clean up the sg v3 interface as suggested by Linus. I have
talked about this with Doug Gilbert as well, and he would
rebase his sg v4 patches on top of "compat: scsi: sg: fix v3
compat read/write interface"
2. Actually moving handlers out of block/compat_ioctl.c and
block/scsi_ioctl.c into drivers, mixed in with cleanup
patches
3. Document how to do this right. I keep getting asked about this,
and it helps to point to some documentation file.
The branch is based on another one that fixes a couple of bugs found
during the creation of this series.

This series concludes the work I did for linux-5.5 on the compat_ioctl()
cleanup, killing off fs/compat_ioctl.c and block/compat_ioctl.c by moving
everything into drivers.
Overall this would be a reduction both in complexity and line count, but
as I'm also adding documentation the overall number of lines increases
in the end.
My plan was originally to keep the SCSI and block parts separate.
This did not work easily because of interdependencies: I cannot
do the final SCSI cleanup in a good way without first addressing the
CDROM ioctls, so this is one series that I hope could be merged through
either the block or the scsi git trees, or possibly both if you can
pull in the same branch.
The series comes in these steps:
1. clean up the sg v3 interface as suggested by Linus. I have
talked about this with Doug Gilbert as well, and he would
rebase his sg v4 patches on top of "compat: scsi: sg: fix v3
compat read/write interface"
2. Actually moving handlers out of block/compat_ioctl.c and
block/scsi_ioctl.c into drivers, mixed in with cleanup
patches
3. Document how to do this right. I keep getting asked about this,
and it helps to point to some documentation file.

When running Cisco-MDS diagnostics which perform driver-level frame loop
back, the switch is reporting errors. Diagnostic has a limit on latency
that is not being met by the driver.
The requirement of Latency frames is that they should be responded back by
the host with a maximum delay of few hundreds of microseconds. If the
switch doesn't get response frames within this time frame, it fails the
test.
Test is failing as the lpfc-wq workqueue was overwhelmed by the packet rate
and in some cases, the work element yielded to other kernel elements.
To resolve, reduce the outstanding load allowed by the adapter. This
ensures the driver spends a reasonable amount of time doing loopback and
can do so such that latency values can be met. Load is managed by reducing
the number of receive buffers posted such that the link can be
backpressured to reduce load.

When unattaching, the driver did not unmap the DPP bar. This caused the
next load of the driver, which attempts to enable wc, to not work correctly
and wc to be disabled due to an address mapping overlap.
Fix by unmapping on unattach.

The order of the flags/checks for adapters where FC-AL is supported
erroneously excluded lpe35000 adapter models. Also noted that the G7 flags
for Loop and Persistent topology are incorrect. They should follow the
rules as G6.
Rework the logic to enable LPe35000 FC-AL support. Collapse G7 support
logic to the same rules as G6.

there are link bounce events triggered via issue_lip
Dynamic FW logging had logic that prematurely breaks from the buffer
filling loop.
Fix the check for buffer overrun by looking before copying and restricting
copy length to the remaining buffer. When copying, ensure space for NULL
character is left in the buffer. While in the routine - ensure the buffer
is cleared before adding elements.

There are reports of multiple ports on the same system displaying different
hostnames in fabric FDMI displays.
Currently, the driver registers the hostname at initialization and obtains
the hostname via init_utsname()->nodename queried at the time the FC link
comes up. Unfortunately, if the machine hostname is updated after
initialization, such as via DHCP or admin command, the value registered
initially will be incorrect.
Fix by having the driver save the hostname that was registered with FDMI.
The driver then runs a heartbeat action that will check the hostname. If
the name changes, reregister the FMDI data.
The hostname is used in RSNN_NN, FDMI RPA and FDMI RHBA.

When the WriteObject mailbox response has change_status set to is 0x2
(Firmware Reset) or 0x04 (Port Migration Reset), the CSF field should also
be checked to see if a fw reset is sufficient to enable all new features in
the updated firmware image. If not, a fw reset would start the new
firmware, but with a feature level equal to existing firmware. To enable
the new features, a chip reset/pci slot reset would be required.
Check the CSF bit when change_status is 0x2 or 0x4 to know whether to
perform a pci bus reset or fw reset.

This patch reworks the fdmi symbolic node name data for the following two
issues:
- Correcting extraneous periods following the DV and HN fdmi data fields.
- Avoiding buffer overflow issues when formatting the data.
The fix to the fist issue is to just remove the characters.
The fix to the second issue has all data being staged in temporary storage
before being moved to the real buffer.

NVMe device re-discovery does not complete. Dev_loss_tmo messages seen on
initiator after recovery from a link disturbance.
The failing case is the following:
When the driver (as a NVME target) receives a PLOGI, the driver initiates
an "unreg rpi" mailbox command. While the mailbox command is in progress,
the driver requests that an ACC be sent to the initiator. The target's ACC
is received by the initiator and the initiator then transmits a PLOGI. The
driver receives the PLOGI prior to receiving the completion for the PLOGI
response WQE that sent the ACC. (Different delivery sources from the hw so
the race is very possible). Given the PLOGI is prior to the ACC completion
(signifying PLOGI exchange complete), the driver LS_RJT's the PRLI. The
"unreg rpi" mailbox then completes. Since PRLI has been received, the
driver transmits a PLOGI to restart discovery, which the initiator then
ACC's. If the driver processes the (re)PLOGI ACC prior to the completing
the handling for the earlier ACC it sent the intiators original PLOGI,
there is no state change for completion of the (re)PLOGI. The ndlp remains
in "PLOGI Sent" and the initiator continues sending PRLI's which are
rejected by the target until timeout or retry is reached.
Fix by: When in target mode, defer sending an ACC for the received PLOGI
until unreg RPI completes.

In nvme-fc: it's possible to have connected active controllers
and as no references are taken on the LLDD, the LLDD can be
unloaded. The controller would enter a reconnect state and as
long as the LLDD resumed within the reconnect timeout, the
controller would resume. But if a namespace on the controller
is the root device, allowing the driver to unload can be problematic.
To reload the driver, it may require new io to the boot device,
and as it's no longer connected we get into a catch-22 that
eventually fails, and the system locks up.
Fix this issue by taking a module reference for every connected
controller (which is what the core layer did to the transport
module). Reference is cleared when the controller is removed.

Currently the lpfc driver sizes its cpu_map array based on
num_possible_cpus(). However, that can be a value that is less than the
highest cpu id bit that is set. As such, if a thread runs on a cpu with a
larger cpu id, or for_each_possible_cpu() is used, the driver could index
off the end of the array and return garbage or GPF.
The driver maintains its own internal copy of the "num_possible" cpu value
and sizes arrays by it.
Fix by setting the driver's value to the value of the last cpu id bit set
in the possible_mask - plus 1. Thus cpu_map will be sized to allow access
by any cpu id possible.

Looking at the recent conversion from smp_processor_id() to
raw_smp_processor_id(), realized that the allocation should be based on the
cpu the hdwq is bound to, not the executing cpu.

Driver is setting the initiator nvme template with a max hw queues value of
the present cpu count which is odd. It should be registering the number of
hdwq queues (queues created on the adapter).
Change to set nvme tempate, in all cases, to the number of hardware queues.

Currently, cpu_map[cpu#]->hdwq is left to equal LPFC_VECTOR_MAP_EMPTY for
not present CPUs. If a CPU is dynamically hot-added, it is possible we may
crash due to not assigning an allocated hdwq.
Correct by assigning a hdwq at initialization for all not-present CPUs.

Some adapters support the ability to hold multiple adapter dumps on the
adapter flash. Some adapters default to enabling this feature while others
default to single-dump.

The current driver attempts to allocate an interrupt vector per cpu using
the systems managed IRQ allocator (flag PCI_IRQ_AFFINITY). The system IRQ
allocator will either provide the per-cpu vector, or return fewer
vectors. When fewer vectors, they are evenly spread between the numa nodes
on the system. When run on an AMD architecture, if interrupts occur to a
cpu that is not in the same numa node as the adapter generating the
interrupt, there are extreme costs and overheads in performance. Thus, if
1:1 vector allocation is used, or the "balanced" vectors in the other numa
nodes, performance can be hit significantly.
A much more performant model is to allocate interrupts only on the cpus
that are in the numa node where the adapter resides. I/O completion is
still performed by the cpu where the I/O was generated. Unfortunately,
there is no flag to request the managed IRQ subsystem allocate vectors only
for the CPUs in the numa node as the adapter.
On AMD architecture, revert the irq allocation to the normal style
(non-managed) and then use irq_set_affinity_hint() to set the cpu
affinity and disable user-space rebalancing.
Tie the support into CPU offline/online. If the cpu being offlined owns a
vector, the vector is re-affinitized to one of the other CPUs on the same
numa node. If there are no more CPUs on the numa node, the vector has all
affinity removed and lets the system determine where it's serviced.
Similarly, when the cpu that owned a vector comes online, the vector is
reaffinitized to the cpu.

The recent affinitization didn't address cpu offlining/onlining. If an
interrupt vector is shared and the low order cpu owning the vector is
offlined, as interrupts are managed, the vector is taken offline. This
causes the other CPUs sharing the vector will hang as they can't get io
completions.
Correct by registering callbacks with the system for Offline/Online
events. When a cpu is taken offline, its eq, which is tied to an interrupt
vector is found. If the cpu is the "owner" of the vector and if the
eq/vector is shared by other CPUs, the eq is placed into a polled mode.
Additionally, code paths that perform io submission on the "sharing CPUs"
will check the eq state and poll for completion after submission of new io
to a wq that uses the eq.
Similarly, when a cpu comes back online and owns an offlined vector, the eq
is taken out of polled mode and rearmed to start driving interrupts for eq.

During heavy RCN activity and log_verbose = 0 we see these messages:

This is due to delayed RSCN activity.
Correct by avoiding the timeout thus the messages by restarting the
discovery timeout whenever an rscn is received.
Filter PRLI responses such that severity depends on whether expected for
the configuration or not. For example, PRLI errors on a fabric will be
informational (they are expected), but Point-to-Point errors are not
necessarily expected so they are raised to an error level.

When reading sysfs nvme_info file while a remote port leaves and comes
back, a NULL pointer is encountered. The issue is due to ndlp list
corruption as the the nvme_info_show does not use the same lock as the rest
of the code.
Correct by removing the rcu_xxx_lock calls and replace by the host_lock and
phba->hbaLock spinlocks that are used by the rest of the driver. Given
we're called from sysfs, we are safe to use _irq rather than _irqsave.

The driver today is reading service parameters from the firmware and then
overwriting the firmware-provided values with values of its own. There are
some switch features that require preliminary FLOGI's that are
switch-specific and done prior to the actual fabric FLOGI for traffic. The
fw will perform those FLOGIs and will revise the service parameters for the
features configured. As the driver later overwrites those values with its
own values, it misconfigures things like BBSCN use by doing so.
Correct by eliminating the driver-overwrite of firmware values. The driver
correctly re-reads the service parameters after each link up to obtain the
latest values from firmware.

If the driver receives a login that is later then LOGO'd by the remote port
(aka ndlp), the driver, upon the completion of the LOGO ACC transmission,
will logout the node and unregister the rpi that is being used for the
node. As part of the unreg, the node's rpi value is replaced by the
LPFC_RPI_ALLOC_ERROR value. If the port is subsequently offlined, the
offline walks the nodes and ensures they are logged out, which possibly
entails unreg'ing their rpi values. This path does not validate the node's
rpi value, thus doesn't detect that it has been unreg'd already. The
replaced rpi value is then used when accessing the rpi bitmask array which
tracks active rpi values. As the LPFC_RPI_ALLOC_ERROR value is not a valid
index for the bitmask, it may fault the system.
Revise the rpi release code to detect when the rpi value is the replaced
RPI_ALLOC_ERROR value and ignore further release steps.

When debugging a recent discovery customer problem it was very hard to tell
what was happening with the existing discovery log messages. To fully debug
the issue additional log messages were necessary.

In the past, the lpe32000 models, based their main support being for 32G,
and as FC-AL is not supported in the FC standards past 8G, did not support
FC-AL operation.
This patch adds private-loop FC-AL support for the LPE32000 adapters
when a link is 8G or below. To avoid conditions where link rate may
change, which would cause non-connectivity to the AL device, FC-AL
mode must become a persistent setting and the link kept at a speed
supporting FC-AL.
The patch:
- Adds a pls attribute indicating whether the adapter properly supports
FC-AL.
- Adds support for the adapter to indicate that topology should be fixed
and the topology types to be configured.
- Adds a pt attribute to report the persistent topology if present.

Add two new macros to aid in message logging:
Both macros print a message if the corresponding lpfc verbosity setting is
set or the kernel log level is WARNING or more critical.
One macro is for use with a phba structure, the other with a vport
structure.

Currently, the FW logging facility is a load/boot time parameter which
requires the driver to be unloaded/reloaded or the system rebooted in order
to change its configuration.
Convert the logging facility to allow dynamic enablement and configuration.
Specifically:
- Convert the feature so that it can be enabled dynamically via an
attribute. Additionally, the size of the buffer can be configured
dynamically.
- Add locks around states that now may be changing.
- Tie the feature into debugfs so that the logs can be read at any time.

The existing "auto eq delay" mechanism was sometimes skipping over an EQ,
not ramping the coalescing down under light load fast enough, and in other
cases never kicked in as cpu sharing by multiple vectors didn't quite add
up right.
Tweak the interrupt mechanism such that:
- Add a flag to the EQ to force checking for colaescing values when being
serviced in the interrupt handler. The flag will be set by any CQ bound
to the EQ whenever the number of CQ elements process in a single scan
meets or exceeds the hardware queue notify level. E.g. there's a
significant number of completions happening.
- In the heartbeat work item that checks coalescing:
- Replace the structure that was counting the number of EQs that
interrupted on a single cpu with a new structure that looks at the EQ
to see whether EQ currently has a coalescing value (thus it should be
re-evaluate) or was marked by the new flag indicating heavy
completions.
- When a cpu, which may be servicing multiple vectors, had at least 1 EQ
that should be checked, a new coalescing delay is calculated based on
the number of interrupts that occurred on the cpu.
- The new coalescing value is then applied to the EQs that had
interrupted on the cpu.

Lower IOps performance with write operations. Perf tool shows lock
contention in dma_pool_alloc and dma_pool_free related to the
txrdy_payload_pool.
The allocations are for dma buffers for XFER_RDY's, which actually are not
needed for the FCP_TRECEIVE command as the command contents are used by the
adapter to generate the IU.
Remove the allocations and the associated buffer pool. Rather than leaving
NULLs in buffer pointer locations, set command and sgl to indicate skipped
SGLE indexes.

In lpfc_abort_handler, the lock acquire order is hbalock (irqsave),
buf_lock (irq) and ring_lock (irq). The issue is that in two places the
locks are released out of order - the buf_lock and the hbalock - resulting
in the cpu preemption/lock flags getting restored out of order and
deadlocking the cpu.
Fix the unlock order by fully releasing the hbalocks as well.

Crash was caused by a bad ndlp address passed to I/O indicated by the XRI
aborted CQE. The address was not NULL so the routine deferenced the ndlp
ptr. The bad ndlp also caused the lpfc_sli4_io_xri_aborted to call an
erroneous io handler. Root cause for the bad ndlp was an lpfc_ncmd that
was aborted, put on the abort_io list, completed, taken off the abort_io
list, sent to lpfc_release_nvme_buf where it was put back on the abort_io
list because the lpfc_ncmd->flags setting LPFC_SBUF_XBUSY was not cleared
on the final completion.
Rework the exchange busy handling to ensure the flags are properly set for
both scsi and nvme.

When operating in private loop mode, PLOGI exchanges are racing and the
driver tries to abort it's PLOGI. But the PLOGI abort ends up terminating
the login with the other end causing the other end to abort its PLOGI as
well. Discovery never fully completes.
Fix by disabling the PLOGI abort when private loop and letting the state
machine play out.

When the adapter FW is administratively set to RO mode, a FW update
triggered by the driver's sysfs attribute will fail. Currently, the
driver's logging mechanism does not properly parse the adapter return codes
and print a meaningful message. This oversight prevents quick diagnosis in
the field.
Parse the adapter return codes for Write_Object and write an appropriate
message to the system console.

Currently, lpfc_nvmet_mrq is always scaled back to the min(lpfc_nvmet_mrq,
lpfc_irq_chann). There's no reason to reduce it to the number of interrupt
vectors. Rather, it should be scaled down based on the number of hardware
queues for the system (if lower than max of 16).
Change scaling to use hardware queue count rather than interrupt vector
count.

The queue pointer might not be valid. The rest of the code checks the
pointer before accessing it. lpfc_sli4_process_missed_mbox_completions is

The BUILD_NVME define never got defined anywhere, causing NVMe commands to
be treated as SCSI commands when freeing the buffers. This was causing a
stuck discovery and a horrible crash in lpfc_set_rrq_active() later on.

T10 PI support on SLI-4-based FCoE adapters is not supported. A prior
commit in the 12.4.0.0 stream added device recognition that would prevent
T10 PI enablement. However, it didn't contain a complete device list. Thus
some SLI-4 FCoE adapters still had T10 PI enabled.
Fix by expanding the device list that identifies FCoE devices.

In lpfc_release_io_buf, an lpfc_io_buf is returned to the 'available' pool
before any associated sgl or cmd and rsp buffers are returned via their
respective 'put' routines. If xri rebalancing occurs and an lpfc_io_buf
structure is reused quickly, there may be a race condition between release
of old and association of new resources.
Re-ordered lpfc_release_io_buf to release sgl and cmd/rsp
buffer lists before releasing the lpfc_io_buf structure for re-use.

Many of the sgl-per-hdwq paths are locking with spin_lock_irq() and
spin_unlock_irq() and may unwittingly raising irq when it shouldn't. Hard
deadlocks were seen around lpfc_scsi_prep_cmnd().
Fix by converting the locks to irqsave/irqrestore.

After study, it was determined there was a double free of a CT iocb during
execution of lpfc_offline_prep and lpfc_offline. The prep routine issued
an abort for some CT iocbs, but the aborts did not complete fast enough for
a subsequent routine that waits for completion. Thus the driver proceeded
to lpfc_offline, which releases any pending iocbs. Unfortunately, the
completions for the aborts were then received which re-released the ct
iocbs.
Turns out the issue for why the aborts didn't complete fast enough was not
their time on the wire/in the adapter. It was the lpfc_work_done routine,
which requires the adapter state to be UP before it calls
lpfc_sli_handle_slow_ring_event() to process the completions. The issue is
the prep routine takes the link down as part of it's processing.
To fix, the following was performed:
- Prevent the offline routine from releasing iocbs that have had aborts
issued on them. Defer to the abort completions. Also means the driver
fully waits for the completions. Given this change, the recognition of
"driver-generated" status which then releases the iocb is no longer
valid. As such, the change made in the commit 296012285c is reverted.
As recognition of "driver-generated" status is no longer valid, this
patch reverts the changes made in
commit 296012285c ("scsi: lpfc: Fix leak of ELS completions on adapter reset")
- Modify lpfc_work_done to allow slow path completions so that the abort
completions aren't ignored.
- Updated the fdmi path to recognize a CT request that fails due to the
port being unusable. This stops FDMI retries. FDMI will be restarted on
next link up.

Scenarios were seen where a host hung when the system booted or the host
was very slow in booting. The link would not come up and no luns were
visible to the host.
After investigation, this was found to be due to the introduction of a new
ACQE that adapter may generate to report a adapter hw warning. The ACQE was
delivered to the driver very early in adapter initialization, when the
driver did not expect command completion. As part of handling this
unexpected interrupt the an EQEs are consumed and discarded and the EQ
rearmed. The issue is the CQ that cause the EQE and thus the interrupt was
not processed and the CQ was left unarmed. Meaning it would no longer
generate a new interrupt condition. Subsequent mailbox commands used to
initialize the adapter use the same CQ, and as there was no completion
interrupt generated, the driver never saw the mailbox commands complete and
it would wait long command timeouts.
Fix by having the early flush routine also process the related CQ and rearm
the CQ.

When the port, running as a nvme target, receives an ABTS, it submits
commands to the adapter to Abort i/o outstanding in the adapter. The Abort
command formatting routine left a command field set to zero, which
instructs the adapter to generate an ABTS on the wire as part of cleaning
up the I/O. This is common operation for an initiator, but not for a
target.
Fix the driver to check whether an ABTS had been received for the I/O, and
if so, change the Abort command formatting so that the ABTS generation is
disabled (IA=1). No need to ABTS it when the other side already has.
Also refactored the code such that there is a single routine being used for
nvme or nvmet ABORT requests, and IA is an argument.

An issue was seen discovering all SCSI Luns when a target device undergoes
link bounce.
The driver currently does not qualify the FC4 support on the target.
Therefore it will send a SCSI PRLI and an NVMe PRLI. The expectation is
that the target will reject the PRLI if it is not supported. If a PRLI
times out, the driver will retry. The driver will not proceed with the
device until both SCSI and NVMe PRLIs are resolved. In the failure case,
the device is FCP only and does not respond to the NVMe PRLI, thus
initiating the wait/retry loop in the driver. During that time, a RSCN is
received (device bounced) causing the driver to issue a GID_FT. The GID_FT
response comes back before the PRLI mess is resolved and it prematurely
cancels the PRLI retry logic and leaves the device in a STE_PRLI_ISSUE
state. Discovery with the target never completes or resets.
Fix by resetting the node state back to STE_NPR_NODE when GID_FT completes,
thereby restarting the discovery process for the node.

Faults are seen with RIP of lpfc_scsi_cmd_iocb_cmpl(). The failure is when
lpfc_update_status is being called as part of the completion. After
debugging, it was seen the issue was the shost pointer that the driver
derived from the scsi cmd. The crash showed the cmd->device pointer being
bogus, which is likely as the scsi devices were offlined prior. The bogus
device pointer caused subsequent pointers derived from the location,
specifically the vport, to be bogus.
Fix by adjusting the calling sequence to pass in the vport rather than
having to derive it from the cmd structure.

Symptoms were seen of the driver not having valid data for mailbox
commands. After debugging, the following sequence was found:
The driver maintains a port-wide pointer of the mailbox command that is
currently in execution. Once finished, the port-wide pointer is cleared
(done in lpfc_sli4_mq_release()). The next mailbox command issued will set
the next pointer and so on.
The mailbox response data is only copied if there is a valid port-wide
pointer.
In the failing case, it was seen that a new mailbox command was being
attempted in parallel with the completion. The parallel path was seeing
the mailbox no long in use (flag check under lock) and thus set the port
pointer. The completion path had cleared the active flag under lock, but
had not touched the port pointer. The port pointer is cleared after the
lock is released. In this case, the completion path cleared the just-set
value by the parallel path.
Fix by making the calls that clear mbox state/port pointer while under
lock. Also slightly cleaned up the error path.

When target-side fault injections are made, the driver isn't reconnecting
to the remote port. The driver is logging "2753" error messages which
state:
"PLOGI failure DID:1B2400 Status:x3/xf0240008"
The failures status is indicating a Illegal field error, which points to
the Temporary RPI field being used for the ELS. This error typically means
the driver used an RPI that was already registered (shouldn't be registered
if using it in this context).
Study has found that if the driver were in discovery attempts and
encountered an error, it wouldn't flag the temporary rpi in error. Yet the
rpi was released for reallocation in these error paths and another ELS
could allocate the rpi. In the failure situation a retry was done on an ELS
that had encountered an error, and as the rpi wasn't marked in error, the
ELS reused the rpi it originally allocated. But that rpi had been allocated
by a different ELS issued after the original error and before the retry
attempt. The different ELS had succeeded and the RPI was registered.
Fix by marking the rpi state for the node to be in error, aka as needing
reallocation, upon an error in the els processing. Error state marking is
always done prior to release back to the internal rpi free list, which the
driver wasn't doing in cases prior.
Also enhanced some of the logging to help in the next case of problem
troubleshooting.

A prior use-after-free mailbox fix solved it's problem by null'ing a ndlp
pointer. However, further testing has shown that this change causes a
later state change to occasionally be skipped, which results in a reference
count never being decremented thus the rpi is never released, which causes
a vport delete to never succeed.
Revise the fix in the prior patch to no longer null the ndlp. Instead the
RELEASE_RPI flag is set which will drive the release of the rpi.
Given the new code was added at a deep indentation level, refactor the code
block using a new routine that avoids the indentation issues.

The nvme-fc transport may call to abort an io on controller reset. If the
driver is out of resources to issue an abort command, it just gives up and
does nothing. The transport expects the lldd to always be able to terminate
an io it has issued. At that point, the controller hangs waiting for
aborted ios to be returned. Note: flaged by "6136" and "6176" error
messages.
Root issue was the adapter mis-allocated the number resources it allocated
for command entries for the adapter.
Convert the driver to allocate command resources based on the number of
xris supported by the FC port - 1 resource for the original command and 1
resource for the abort request.

After exchanging PLOGI on an SLI-3 adapter, the PRLI exchange failed. Link
trace showed the port was assigned a non-zero n_port_id, but didn't use the
address on the PRLI. The assigned address is set on the port by the
CONFIG_LINK mailbox command. The driver responded to the PRLI before the
mailbox command completed. Thus the PRLI response used the old n_port_id.
Defer the PRLI response until CONFIG_LINK completes.

A recent patch unconditionally marks the hba as in error as part of
resetting the adapter. The driver flow that called the adapter reset was a
recovery path, which expects the adapter to not be in an error state in
order to finish the recovery. Given the new error state being set, the
recovery fails and the adapter is left in limbo.
Revise the adapter reset routine so that it will only mark the adapter in
error if it was unable to reset the adapter.

The 12.4.0.0 patch that merged WQ/CQ pairs into single per-cpu pair
contained a bug: a local variable was set to the queue pair by index. This
should have allowed the local variable to be natively used. Instead, the
code reused the index relative to the local variable, obtaining a random
pointer value that when used eventually faulted the system
Convert offending code to use local variable.

Currently, each hardware queue, typically allocated per-cpu, consists of a
WQ/CQ pair per protocol. Meaning if both SCSI and NVMe are supported 2
WQ/CQ pairs will exist for the hardware queue. Separate queues are
unnecessary. The current implementation wastes memory backing the 2nd set
of queues, and the use of double the SLI-4 WQ/CQ's means less hardware
queues can be supported which means there may not always be enough to have
a pair per cpu. If there is only 1 pair per cpu, more cpu's may get their
own WQ/CQ.
Rework the implementation to use a single WQ/CQ pair by both protocols.

FC-NVMe-2 added support for sequence level error recovery in the FC-NVME
protocol. This allows for the detection of errors and lost frames and
immediate retransmission of data to avoid exchange termination, which
escalates into NVMeoFC connection and association failures. A significant
RAS improvement.
The driver is modified to indicate support for SLER in the NVMe PRLI is
issues and to check for support in the PRLI response. When both sides
support it, the driver will set a bit in the WQE to enable the recovery
behavior on the exchange. The adapter will take care of all detection and
retransmission.

Typical SLI-4 hardware supports up to 2 4KB pages to be registered per XRI
to contain the exchanges Scatter/Gather List. This caps the number of SGL
elements that can be in the SGL. There are not extensions to extend the
list out of the 2 pages.
The G7 hardware adds a SGE type that allows the SGL to be vectored to a
different scatter/gather list segment. And that segment can contain a SGE
to go to another segment and so on. The initial segment must still be
pre-registered for the XRI, but it can be a much smaller amount (256Bytes)
as it can now be dynamically grown. This much smaller allocation can
handle the SG list for most normal I/O, and the dynamic aspect allows it to
support many MB's if needed.
The implementation creates a pool which contains "segments" and which is
initially sized to hold the initial small segment per xri. If an I/O
requires additional segments, they are allocated from the pool. If the
pool has no more segments, the pool is grown based on what is now
needed. After the I/O completes, the additional segments are returned to
the pool for use by other I/Os. Once allocated, the additional segments are
not released under the assumption of "if needed once, it will be needed
again". Pools are kept on a per-hardware queue basis, which is typically
1:1 per cpu, but may be shared by multiple cpus.
The switch to the smaller initial allocation significantly reduces the
memory footprint of the driver (which only grows if large ios are
issued). Based on the several K of XRIs for the adapter, the 8KB->256B
reduction can conserve 32MBs or more.
It has been observed with per-cpu resource pools that allocating a resource
on CPU A, may be put back on CPU B. While the get routines are distributed
evenly, only a limited subset of CPUs may be handling the put routines.
This can put a strain on the lpfc_put_cmd_rsp_buf_per_cpu routine because
all the resources are being put on a limited subset of CPUs.

Added code to support driver loopback with MDS Diagnostics. This style of
diagnostics passes frames from the fabric to the driver who then echo them
back out the link. SEND_FRAME WQEs are used to transmit the frames. Added
the SOF and EOF field location definitions for use by SEND_FRAME.
Also ensure that enable_mds_diags is a RW parameter.

The driver is allowing the user to change lpfc_enable_bg while loading the
driver against a FCoE adapter. This is not supported.
No check is made for the adapter type when applying the blockguard
enablement value.
Fix by verifying the adapter type before setting the enablement flag.

GetTrunkInfo is displaying an incorrect link speed when the link is a trunk
and the link has gone down. The driver is not clearing the logical speed
as part of the link down transition.
Fix by setting the logical speed to UNKNOWN SPEED when the link goes down.

Max Frame Size value is shown as 34816 in fdmishow from Switch.
The driver uses bbRcvSize in common service param which is obtained from
the READ_SPARM mailbox command. The bbRcvSize field which is displayed is a
three nibble field but the driver is printing a full four nibbles.
Fix by masking off the upper nibble.

The scsi transport fc bsg interface does not expect the bsg_job_done()
callback to be done if the bsg request call returns failure. Several of the
HST_VENDOR cases in the driver unconditionally call bsg_job_done()
regardless of the returning value.
Fix the code to only call bsg_job_done() if the call to lpfc_bsg_request()
will return success.

If an unsolicited ABTS was received, the driver looks up the exchange it
references. It it does various searches looking for the exchange
context. When one is eventually matched and it is associated with an XRI
context, the driver sends an ABORT WQE to terminate the exchange. Current
code looks at whether the transport had taken action on the XRI yet or not
(no action if set to LPFC_NVMET_STE_RCV; action if non-LPFC_NVMET_STE_RCV).
Based on action or not one of two (sol vs unsol) issue abort routines are
called. The unsol version cheats and transmits a sequence containing an
ABTS with no interaction with the adapter. The sol version issues an Abort
WQE and lets the adapter manage whether the ABTS is sent to not.
The issue is the unsol version is sending ABTS unconditionally for the
exchange that received the ABTS. It's unnecessary.
Remove the conditional and just call the adapter command-based routine to
let the adapter manage the ABTS.

As part of firmware download, the adapter is reset. On the adapter the
reset causes the function to stop and all outstanding io is terminated
(without responses). The reset path then starts teardown of the adapter,
starting with deregistration of the remote ports with the nvme-fc
transport. The local port is then deregistered and the driver waits for
local port deregistration. This never finishes.
The remote port deregistrations terminated the nvme controllers, causing
them to send aborts for all the outstanding io. The aborts were serviced in
the driver, but stalled due to its state. The nvme layer then stops to
reclaim it's outstanding io before continuing. The io must be returned
before the reset on the controller is deemed complete and the controller
delete performed. The remote port deregistration won't complete until all
the controllers are terminated. And the local port deregistration won't
complete until all controllers and remote ports are terminated. Thus things
hang.
The issue is the reset which stopped the adapter also stopped all the
responses that would drive i/o completions, and the aborts were also
stopped that stopped i/o completions. The driver, when resetting the
adapter like this, needs to be generating the completions as part of the
adapter reset so that I/O complete (in error), and any aborts are not
queued.
Fix by adding flush routines whenever the adapter port has been reset or
discovered in error. The flush routines will generate the completions for
the scsi and nvme outstanding io. The abort ios, if waiting, will be caught
and flushed as well.

This issue is specific to SLI-3 adapters, specifically when DIF is used.
Once seen, this message floods the logs:
9064 BLKGRD: lpfc_scsi_prep_dma_buf_s3: Too many sg segments from
dma_map_sg
The driver, upon detecting an error such as too many elements in an sglist,
misrepresents the error by treating it as a temporary resource issue by
returning MLQUEUE_HOST_BUSY. In these cases, no retry will fix it and it
should have been a hard error. The repeated retry was causing the spamming
of the log.
As for the initial reason of why an I/O encountered this issue at all is
not clear as parameters set by the driver should have avoided this. The
dm multipath maintainer has been notified of the issue.
Fix by changing the return code for the dma mapping routines to indicate
cases that are not retryable and return DID_ERROR on those cases.

If the adapter encounters a condition which causes the adapter to fail
(driver must detect the failure) simultaneously to a request to the driver
to reset the adapter (such as a host_reset), the reset path will be racing
with the asynchronously-detect adapter failure path. In the failing
situation, one path has started to tear down the adapter data structures
(io_wq's) while the other path has initiated a repeat of the teardown and
is in the lpfc_sli_flush_xxx_rings path and attempting to access the
just-freed data structures.
Fix by the following:
- In cases where an adapter failure is detected, rather than explicitly
calling offline_eratt() to start the teardown, change the adapter state
and let the later calls of posted work to the slowpath thread invoke the
adapter recovery. In essence, this means all requests to reset are
serialized on the slowpath thread.
- Clean up the routine that restarts the adapter. If there is a failure
from brdreset, don't immediately error and leave things in a partial
state. Instead, ensure the adapter state is set and finish the teardown
of structures before returning.
- If in the scsi host reset handler and the board fails to reset and
restart (which can be due to parallel reset/recovery paths), instead of
hard failing and explicitly calling offline_eratt() (which gets into the
redundant path), just fail out and let the asynchronous path resolve the
adapter state.

During cable pull testing a deadlock was seen between lpfc_nlp_counters()
vs lpfc_mbox_process_link_up() vs lpfc_work_list_done(). They are all
waiting on the shost->host_lock.
Issue is all of these cases raise irq when taking out the lock but use
spin_unlock_irq() when unlocking. The unlock path is will unconditionally
re-enable interrupts in cases where irq state should be preserved. The
re-enablement allowed the other paths to execute which then causes the
deadlock.
Fix by converting the lock/unlock to irqsave/irqrestore.

In a test with high nvme remote port counts connected via a multi-hop FC
switch config where switches were systematically reset (e.g. fabric
partitioning and re-establishment), the nvme remote ports would switch
addresses based on the switch reconfiguration events. The driver would get
into a situation where the nvme port changed address, PLOGI and PRLI would
succeed nvme transport registration occurred, but subsequent LS requests by
the nvme subsystem failed due to a bad ndlp state and connectivity to the
device failed.
The driver hit a race condition on multiple devices that address swapped
simultaneously. In cases where the driver notices the remote port structure
came back as the same value as previously (meaning a nvme_rport structure
was re-enabled and did not go through devloss_tmo/connect_tmo_failures on
all controllers) the driver would unconditionally exit assuming the ndlp
information was correct. But, if the ndlp's had been swapped, the ndlp had
stale port state information, which when used by the LS request commands,
would fail the commands.
Fix by checking whether a node swap had occurred, and only exit if no ndlp
swap had occurred.

In situations where zoning is not being used, thus NVMe initiators see
other NVMe initiators as well as NVMe targets, a link bounce on an
initiator will cause the NVMe initiators to spew "6169" State Error
messages.
The driver is not qualifying whether the remote port is a NVMe targer or
not before calling the lpfc_nvme_rescan_port(), which validates the role
and prints the message if its only an NVMe initiator.
Fix by the following:
- Before calling lpfc_nvme_rescan_port() ensure that the node is a NVMe
storage target or a NVMe discovery controller.
- Clean up implementation of lpfc_nvme_rescan_port. remoteport pointer
will always be NULL if a NVMe initiator only. But, grabbing of
remoteport pointer should be done under lock to coincide with the
registering of the remote port with the fc transport.

On an SLI-3 adapter which does not support NVMe, but with the driver global
attribute to enable nvme on any adapter if it does support NVMe
(e.g. module parameter lpfc_enable_fc4_type=3), the SGL and total SGE
values are being munged by the protocol enablement when it shouldn't be.
Correct by changing the location of where the NVME sgl information is being
applied, which will avoid any SLI-3-based adapter.

If admin changes the devloss_tmo on an rport via the fc_remote_port rport
dev_loss_tmo attribute, the value is on set on scsi stack. The change is
not propagated to NVMe.
The set routine in the lldd lacks the call to
nvme_fc_set_remoteport_devloss() to set the value.
Fix by adding the call to the lldd set routine.

In tests with remote ports contantly logging out/logging coupled with
occassional local link bounce, if a remote port is disocnnected for longer
than devloss_tmo and then subsequently reconnected, eventually the test
will fail to login with the remote port and remote port connectivity is
lost.
When devloss_tmo expires, the driver does not free the node struct until
the port or npiv instances is being deleted. The node is left allocated but
the state set to UNUSED. If the node was in the process of logging in when
the local link drop occurred, meaning the RPI was allocated for the node in
order to send the ELS, but not yet registered which comes after successful
login, the node is moved to the NPR state, and if devloss expires, to
UNUSED state. If the remote port comes back, the node associated with it
is restarted and this path happens to allocate a new RPI and overwrites the
prior RPI value. In the cases where the port was logged in and loggs out,
the path did release the RPI but did not set the node rpi value. In the
cases where the remote port never finished logging in, the path never did
the call to release the rpi. In this latter case, when the node is
subsequently restore, the new rpi allocation overwrites the rpi that was
not released, and the rpi is now leaked. Eventually the port will run out
of RPI resources to log into new remote ports.
Fix by following changes:
- When an rpi is released, do so under locks and ensure the node rpi value
is set to a non-allocated value (LPFC_RPI_ALLOC_ERROR). Note:
refactored to a small service routine to avoid indentation issues.
- When re-enabling a node, check the rpi value to determine if a new
allocation is necessary. If already set, use the prior rpi.
Enhanced logging to help in the future.

If a remote port is removed and remains removed for devloss_tmo, if an RSCN
is subsequently received indicating the presence of the remte port, the
driver does not login to and rediscovery the remote port.
Currently, in order to for a port to be rediscovered post an RSCN, the node
state must be NPR to reflect not logged in. When devloss expires, the node
state is marked UNUSED. When an RSCN occurs, the nodes referenced by the
RSCN will have a NPR_2B_DISC flag set, but the re-login will only be
attempted if the node is in NPR_NODE state. Thus the node is skipped over.
Fix by recognizing the NPR_2B_DISC and UNUSED and transition the node back
to NPR state to allow the re-login to take place.

If an admin updates lpfc's devloss_tmo sysfs attribute, the kernel will
oops.
Coding of a loop allowed a new value (rport) to be set/checked for null
followed by an older value (remoteport) checked for null to allow progress
where the new value, even though null, will be referenced.
Rework the logic to validate and prevent any reference to the null ptr.

It's possible for the driver to initiate an FLOGI and before it completes,
another link down/up transition occurs requiring a new FLOGI. Currently,
nothing is done to abort/noop the older FLOGI request to the adapter, so if
this transition occurs and the FLOGI completion is received after the link
down/up transition, the driver may erroneously act on the older FLOGI. In
most cases, the adapter properly terminates/fails the FLOGI, but there is a
timing condition where the FLOGI may complete on the wire prior to the
transition, but the response may not be seen/processed by the driver before
the driver sees the link transition.
Fix by having the link down handler in the driver run through any
outstanding ELS's and change the completion handler of the ELS so that it
will be no-op'd and released.

When tearing down the adapter for a reset, online/offline, or driver
unload, the queue free routine would hit a GPF oops. This only occurs on
conditions where the number of hardware queues created is fewer than the
number of cpus in the system. In this condition cpus share a hardware
queue. And of course, it's the 2nd cpu that shares a hardware that
attempted to free it a second time and hit the oops.
Fix by reworking the cpu to hardware queue mapping such that:
Assignment of hardware queues to cpus occur in two passes:
first pass: is first time assignment of a hardware queue to a cpu.
This will set the LPFC_CPU_FIRST_IRQ flag for the cpu.
second pass: for cpus that did not get a hardware queue they will
be assigned one from a primary cpu (one set in first pass).
Deletion of hardware queues is driven by cpu itteration, and queues
will only be deleted if the LPFC_CPU_FIRST_IRQ flag is set.
Also contains a few small cleanup fixes and a little better logging.

irq. But, the path is already under the hbalock which raised irq so it's
unnecessary.

lpfc_nvme_register_port hit a null prev_ndlp pointer in a test with lots of
target ports swapping addresses. The oldport value was stale, thus it's
ndlp (prev_ndlp set to it) was used.
Fix by moving oldrport pointer checks, and if used prev_ndlp pointer
assignment, to be done while the lock is held.

The driver is inadvertently trying to issue an INIT_VPI mailbox command on
an SLI-3 driver. The command is specific to SLI-4. When the call is made to
send the command, if on an SLI-3 adapter, an array pointer is NULL and the
driver will oops.
Fix by restricting the command to SLI-4 adapters only.

If a target issues an ADISC to the port and the target is a NVME target,
the driver is inadvertantly invalidating the login and marking the remote
port as logged out. Communication with the target is lost.
Revise the ADISC check so that FCP or NVME targets will be marked valid at
the end of ADISC processing. Enhance logging to recognize condition
better.

Some remote ports may be slow in registering their GID_FT protocol
information with the fabric. If the remote port is an initiator, it may
send PLOGI to the port before the GID_FT logic is complete. Meaning, after
accepting the PLOGI, when the driver may see no response to the GID_FT that
is issued after the login to determine the protocols supported so that
proper PRLI's may be transmit. If the driver has no fc4 information, it
currently stops and the remote port is not discovered.
Fix by issuing a LOGO when there is no GID_FT information. The LOGO
completion handling will attempt to re-login if the nport_id is still
present.

In cases of remote-port-side cable pull/replug, there happens to be a
target that upon replug will send the port a PLOGI, a PRLI, and a LOGO.
When this sequence is received by the driver, the PLOGI accepted and a
GFT_ID is issued to find the protocol support for the remote port. While
the GFT_ID is outstanding, a LOGO is received. The driver logs the remote
port out and unregisters the RPI and schedules a new PLOGI transmission.
However, the GFT_ID was not terminated. When it completed, the driver
attempted to transition the remote port to PRLI transmission, which cancels
the PLOGI scheduling. The PRLI transmit attempt is rejected by the adapter
as the remote port is not logged in. No retry is attempted as it's expected
the logout is noted and the supposedly scheduled PLOGI should address the
state. As there is no PLOGI, the remote port does not get re-discovered.
Fix by aborting the outstanding GFT_ID if the related remote port is logged
out.
Ensure a PRLI transmit attempt only occurs if the remote port is logging
in. This avoids the incorrect attempt while logged out.

If the adapter is reset while there are outstanding ELS's, subsequent
reinitialization of the adapter will fail as it has not recovered all of
the io contexts relative to the ELS's.
If an ELS timed out or otherwise failed and an the ELS was attempted to be
aborted (which changes the ELS completion context), in causes where the
driver generates completions for the outstanding IO as the adapter would
not due to being reset, the driver released only the ELS context and failed
to release the abort context. When the adapter went to reinit, as it had
not received all of the contexts, it failed to reinit.
Fix by having the ELS completion handler identify the driver-generated
completion status and release the abort context.

Unusually high IO latency can be observed with little IO in progress. The
latency may remain high regardless of amount of IO and can only be cleared
by forcing lpfc_fcp_imax values to non-zero and then back to zero.
The driver's eq_delay mechanism that scales the interrupt coalescing based
on io completion load failed to reduce or turn off coalescing when load
decreased. Specifically, if no io completed on a cpu within an eq_delay
polling window, the eq delay processing was skipped and no change was made
to the coalescing values. This left the coalescing values set when they
were no longer applicable.
Fix by always clearing the percpu counters for each time period and always
run the eq_delay calculations if an eq has a non-zero coalescing value.

After seeing some interoperability issues with ADISC, it was determined the
ELS definitions in lpfc were using types that allowed the compiler to add
pad to the structure, causing the structure to no longer be per spec. The
offending structures are ADISC, FAN, and RNID.
This patch implements the simple fix of eliminating the pad by forcing the
compiler to pack the structure. Care was taken to ensure field accesses
won't be by operations that would hit a bad field alignment.
The better solution would be to convert to the uapi fc header definitions,
but the number of changes required to do is rather intrusive so this course
of action was deferred.

When connected to a high number of remote ports, the driver is encountering
PLOGI errors. The errors are due to adapter detected failures indicating
illegal field values.
Turns out the driver was prematurely clearing an RPI bitmask before waiting
for an UNREG_RPI mailbox completion. This allowed the RPI to be reused
before it was actually available.
Fix by clearing RPI bitmask only after UNREG_RPI mailbox completion.

scsi-mq operation inherently performs pre-allocation of resources for
blk-mq request queues. Even though the kdump environment reduces the
configuration to a single CPU, thus 1 hardware queue, which helps
significantly, the resources are still rather large due to the per request
allocations. blk-mq pre-allocations can be over 4KB per request. With
adapter can_queue values in the 4k or 8k range, this can easily be 32MBs
before any other driver memory is factored in. Driver SGL DMA buffer
allocation can be up to 8KB per request as well adding an additional
64MB. Totals are well over 100MB for a single shost. Given kdump memory
auto-sizing utilities don't accommodate this amount of memory well, it's
very possible for kdump to fail due to lack of memory.
Fix by having the driver recognize that it is booting within a kdump
context and reduce the number of requests it will support to a more
reasonable value.

When SCSI-MQ is enabled, the SCSI-MQ layers will do pre-allocation of MQ
resources based on shost values set by the driver. In newer cases of the
driver, which attempts to set nr_hw_queues to the cpu count, the
multipliers become excessive, with a single shost having SCSI-MQ
pre-allocation reaching into the multiple GBytes range. NPIV, which
creates additional shosts, only multiply this overhead. On lower-memory
systems, this can exhaust system memory very quickly, resulting in a system
crash or failures in the driver or elsewhere due to low memory conditions.
After testing several scenarios, the situation can be mitigated by limiting
the value set in shost->nr_hw_queues to 4. Although the shost values were
changed, the driver still had per-cpu hardware queues of its own that
allowed parallelization per-cpu. Testing revealed that even with the
smallish number for nr_hw_queues for SCSI-MQ, performance levels remained
near maximum with the within-driver affiinitization.
A module parameter was created to allow the value set for the nr_hw_queues
to be tunable.

When a configurations runs with a single cpu (such as a kdump kernel),
which causes the driver to request a single vector, when the driver
subsequently requests an irq affinity mask, the mask comes back null. The
driver currently does nothing in this scenario, which leaves mappings to
hardware queues incomplete and crashes the system.
Fix by recognizing the null mask and assigning the vector to the first cpu
in the system.

The RISC-V architecture has a register named the "Supervisor Exception
Program Counter", or "sepc". This abbreviation triggers checkpatch.pl's
misspelling detector, resulting in noise in the checkpatch output. The
risk that this noise could cause more useful warnings to be missed seems
to outweigh the harm of an occasional misspelling of "spec". Thus drop
the "sepc" entry from the misspelling list.

The lpfc_debug_dump_all_queues() function repeatedly calls into
lpfc_debug_dump_qe() which has a temporary 128 byte buffer. This was fine
before the introduction of CONFIG_GCC_PLUGIN_STRUCTLEAK_VERBOSE because
each instance could occupy the same stack slot. However, now they each get
their own copy, which leads to a huge increase in stack usage as seen from
the compiler warning:

Avoid this by not marking lpfc_debug_dump_qe() as inline so the compiler
can choose to emit a static version of this function when it's needed or
otherwise silently drop it. As an added benefit, not inlining multiple
copies of this function means we save several kilobytes of .text section,
reducing the file size from 47kb to 43.
It is somewhat unusual to have a function that is static but not inline in
a header file, but this does not cause problems here because it is only
used by other inline functions. It would however seem reasonable to move
all the lpfc_debug_dump_* functions into lpfc_debugfs.c and not mark them
inline as a later cleanup.

This topic branch covers a fundamental change in how our sg lists are
allocated to make mq more efficient by reducing the size of the
preallocated sg list. This necessitates a large number of driver
changes because the previous guarantee that if a driver specified
SG_ALL as the size of its scatter list, it would get a non-chained
list and didn't need to bother with scatterlist iterators is now
broken and every driver *must* use scatterlist iterators.
This was broken out as a separate topic because we need to convert all
the drivers before pulling the trigger and unconverted drivers kept
being found, necessitating a rebase.

The big merge conflict this time around is the SPDX licence tags.

accurate than the one in the tree, so the resolution is to take our
version for all the SPDX conflicts"
Note on the SPDX license tag conversion conflicts: the SCSI tree had
done its own SPDX conversion, which in some cases conflicted with the
treewide ones done by Thomas & co.
In almost all cases, the conflicts were purely syntactic: the SCSI tree
used the old-style SPDX tags ("GPL-2.0" and "GPL-2.0+") while the
treewide conversion had used the new-style ones ("GPL-2.0-only" and
"GPL-2.0-or-later").
In these cases I picked the new-style one.
In a few cases, the SPDX conversion was actually different, though. As
explained by James above, and in more detail in a pre-pull-request
thread:
"The other problem is actually substantive: In the libsas code Luben
Tuikov originally specified gpl 2.0 only by dint of stating:
* This file is licensed under GPLv2.
In all the libsas files, but then muddied the water by quoting GPLv2
verbatim (which includes the or later than language). So for these
files Christoph did the conversion to v2 only SPDX tags and Thomas
converted to v2 or later tags"
So in those cases, where the spdx tag substantially mattered, I took the
SCSI tree conversion of it, but then also took the opportunity to turn
the old-style "GPL-2.0" into a new-style "GPL-2.0-only" tag.
Similarly, when there were whitespace differences or other differences
to the comments around the copyright notices, I took the version from
the SCSI tree as being the more specific conversion.
Finally, in the spdx conversions that had no conflicts (because the
treewide ones hadn't been done for those files), I just took the SCSI
tree version as-is, even if it was old-style. The old-style conversions
are perfectly valid, even if the "-only" and "-or-later" versions are
perhaps more descriptive.

To support scenarios which aren't bound to nvmetcli add port scenarios,
which is currently where the nvmet_fc transport invokes the discovery
event callbacks, a syfs attribute is added to lpfc which can be written
to cause an RSCN to be generated for the nport.

This patch updates RSCN receive processing to check for the remote
port being an NVME port, and if so, invoke the nvme_fc callback to
rescan the remote port. The rescan will generate a discovery udev
event.

This patch adds support for the nvmet discovery op. When the callback
routine is called, the driver will call the routine to generate an RSCN
to the port on the other end of the link.

Unlike the legacy I/O path, scsi-mq preallocates a large array to hold
the scatterlist for each request. This static allocation can consume
substantial amounts of memory on modern controllers which support a
large number of concurrently outstanding requests.
To facilitate a switch to a smaller static allocation combined with a
dynamic allocation for requests that need it, we need to make sure all
SCSI drivers handle chained scatterlists correctly.
Convert remaining drivers that directly dereference the scatterlist
array to using the iterator functions.

Crashes in scsi_queue_rq or in dma_unmap_direct_sg during BFS when lpfc has
lpfc_enable_bg=1.
lpfc is setting DIX and prot sg after scsi_add_host_with_dma() has been
called. The scsi_host_set_prot() and scsi_host_set_guard() routines need to
be called before scsi_add_host_with_dma().
Revise the calling sequence to set the protection/guard data before calling
scsi_add_host_with_dma().

FDMI protocol support registration was not accurately showing nvme
support. The fcponly-path clears the parameter object.
Move the code out of the fcponly code path. Fix the FDMI registration data
to properly check for nvme support. Commonize the manner in which the fdmi
routines set protocol support.

Issuing a LUN reset was resulting in a command failure which then escalated
to a host reset.
The FCP-4 spec allows fcp_rsp_len field to specify the number of valid
bytes of FCP_RSP_INFO, and the value could be 4 or 8. The driver is
allowing only a value of 8, thus it failed the command.
Revise the driver to allow 4 or 8.

While fixing the resources per socket, realized the driver was not using
hardware queues (up to 1 per cpu) if there were fewer interrupt
vectors. The driver was only using the hardware queue assigned to the cpu
with the vector.
Rework the affinity map check to use the additional hardware queue elements
that had been allocated. If the cpu count exceeds the hardware queue count
- share, but choose what is shared with by: hyperthread peer, core peer,
socket peer, or finally similar cpu in a different socket.

The driver was coded expecting enough hardware queues and interrupt vectors
such that at least there was one per socket. In the case where there were
fewer than sockets, cpus were left unassigned thus null pointers.
Rework the affinity mappings. Map settings for the cpu's that are in the
irq cpu mask. For each cpu not in the mask, map to another cpu that does
have a mask. Choice of the "other" cpu will attempt to map to the same cpu
but differing hyperthread, or cpu within in same core, or cpu within same
socket, or finally cpu in the base socket.

Invalid logical speed is displayed for trunk enabled ports when all ports
are down. Also noted that link speed is incorrectly reported for the units
when links are up.
Current code is returning the logical link speed from the last event from
the adapter. In cases where the last link went down, the link speed in the
event was not valid - meaning that although the links where down the field
had a bogus value.
Rework the event handling to qualify the trunk link state before using the
event speed data.
Also correct units on other areas where the logical link speed was taken
from a link event.

The driver unconditionally says fw doesn't support nvme when in
truth it was a driver parameter settings that disabled nvme support.
Rework the code validating nvme support to accurately report what
condition is disabling nvme support. Save state on whether nvme
fw supports nvme in case sysfs attributes change dynamically.

There is a race condition with the abort handler declaring a waitq
item on it's stack, followed by a timeout in the abort handler that
has it give up on the abort return to its caller. When the io is
finally aborted and its completion handler called, it references
the waitq element that the abort_handler set up, which is no longer
valid resulting in a deadlock.
Fix by clearing the waitq reference, under lock, when the abort
handler timeout gives up. Have the completion handler validate the
waitq before referencing it.

When queued work is executed posting a new command to the transport
the driver is reporting a null buffer.
The driver had received an ABTS which matched a command that had
been scheduled for delivery to the transport. The driver proceeded
to cancel the command, but the work item was never cancelled.
Fix by cancelling the queued work item. Also turns out the ABTS
response was not properly sending a BA_ACC, so set the flag to
send the ACC.

Use-after-free memory overwrite detected. Problem reported
by Ewan Milne at Red Hat after running lpfc target with additional
memory checking enabled.
Race condition when lpfc_nvmet_xmt_ls_rsp_cmp frees the ctxp
memory in interrupt context before lpfc_nvmet_xmt_ls_rsp
clears a field in the ctxp after successfully issuing the wqe.
Remove the unnecessary ctxp write after reposting the rq buffer. The
ctxp->rqb_buffer field is not checked in LS handling after the wqe
is submitted.

Under heavy load the target stops responding, the drivers aborts
timeout and we start recovery by logging out of the target, but
the target is never logged into again.
In a point-to-point scenario, there were battling PLOGI's. When we
received a PLOGI request after having sent one, the driver cancels
the processing of the original plogi. However, the completion path
of the remaining plogi was coded to skip the reg_rpi that should
be happening on the 2nd plogi.
Correct by adding a simple pt2pt check such that the 2nd plogi isn't
skipped and the reg_login occurs.

The driver currently is relying on firmware to match ABTSs to existing
exchanges. This works fine as long as an exchange has been assigned to the
io and work posted to it. However, for unmapped frames (rxid=0xFFFF), the
driver has yet to assign an xri. The driver was blindly saying it couldn't
match the ABTS and sending the BA_xxx. However, the command frame may have
been in queues waiting on xri's before posting to the nvmet_fc layer. When
xri's became available, the command frame would still be pushed to the
transport and that io would execute, even though the io had been killed by
ABTS. The initiator, seeing the io ABTS'd, would reuse the exchange for a
different io which would be received on the target and pushed up. If the
"zombie" io then came back down and started transmitting, the initiator
would match the oxid and accept erroneous data. Bad things happened.
Add tracking of active exchanges in the target to allow matching of a
received ABTS against active or pending IO requests. If the ABTS is matched
to a pending or active IO, the drive initiates cleanup and conditionally
notifies the transport.

Currently the driver is notified of new command frame receipt by CQEs. As
part of the CQE processing, the driver upcalls the nvmet_fc transport to
deliver the command. nvmet_fc, as part of receiving the command builds out
a context for it, where one of the first steps is to allocate memory for
the io.
When running with tests that do large ios (1MB), it was found on some
systems, the total number of outstanding I/O's, at 1MB per, completely
consumed the system's memory. Thus additional ios were getting blocked in
the memory allocator. Given that this blocked the lpfc thread processing
CQEs, there were lots of other commands that were received and which are
then held up, and given CQEs are serially processed, the aggregate delays
for an IO waiting behind the others became cummulative - enough so that the
initiator hit timeouts for the ios.
The basic fix is to avoid the direct upcall and instead schedule a work
item for each io as it is received. This allows the cq processing to
complete very quickly, and each io can then run or block on it's own.
However, this general solution hurts latency when there are few ios. As
such, implemented the fix such that the driver watches how many CQEs it has
processed sequentially in one run. As long as the count is below a
threshold, the direct nvmet_fc upcall will be made. Only when the count is
exceeded will it revert to work scheduling.
Given that debug of this showed a surprisingly long delay in cq processing,
the io timer stats were updated to better reflect the processing of the
different points.

After receiving an unsolicited ABTS (meaning rxid is 0xFFFF), the driver
used the oxid from the initiator to match against a local xri which may
have been allocated for the io. The xri would be the rxid - it's an invalid
check resulting in the command not being matched or erroneously matched.
Change the lookup to use the oxid and the SID to match against received
IO's original values.
