In PEF enabled systems, some of the resources which were previously
hypervisor privileged are now ultravisor privileged and controlled by
the ultravisor firmware.
This adds FW_FEATURE_ULTRAVISOR to indicate if PEF is enabled.
The host kernel can use FW_FEATURE_ULTRAVISOR, for instance, to skip
accessing resources (e.g. PTCR and LDBAR) in case PEF is enabled.

We had a mid-air collision between two new firmware features, DRMEM_V2
and DRC_INFO, and they ended up with the same value.
No one's actually reported any problems, presumably because the new
firmware that supports both properties is not widely available, and
the two properties tend to be enabled together.
Still if we ever had one enabled but not the other, the bugs that
could result are many and varied. So fix it.

Long ago, only in the lab, there was OPALv1 and OPALv2. Now there is
just OPALv3, with nobody ever expecting anything on pre-OPALv3 to

OPALv2 only ever existed in the lab and didn't escape to the world.
All OPAL systems in the wild are OPALv3.
The probability of there being an OPALv2 system still powered on
anywhere inside IBM is approximately zero, let alone anyone
expecting to run mainline kernels.
So, start to remove references to OPALv2.

Platform events such as partition migration or the new PRRN firmware
feature can cause the NUMA characteristics of a CPU to change, and these
changes will be reflected in the device tree nodes for the affected
CPUs.
This patch registers a handler for Open Firmware device tree updates
and reconfigures the CPU and node maps whenever the associativity
changes. Currently, this is accomplished by marking the affected CPUs in
the cpu_associativity_changes_mask and allowing
arch_update_cpu_topology() to retrieve the new associativity information
using hcall_vphn().
Protecting the NUMA cpu maps from concurrent access during an update
operation will be addressed in a subsequent patch in this series.

The firmware_has_feature() function makes it easy to check for supported
features of the hypervisor. This patch extends the capability of
firmware_has_feature() to include checking for specified bits
in vector 5 of the architecture vector as reported in the device tree.
As part of this the #defines used for the architecture vector are re-defined
such that each option has the index into vector 5 and the feature bit encoded
into it. This makes checking for architecture bits when initiating data
for firmware_has_feature much easier.

Profiling of a page fault scalability microbenchmark shows flush_hash_range
is not calling the batch hpte invalidate hcall (H_BULK_REMOVE).
It turns out we have a duplicate firmware feature for hcall-bulk and the
current setup code stops after finding the first match. This meant we never
batch and always do individual invalidates.
The patch below removes the duplicate and shifts FW_FEATURE_CMO to close
the gap. With the patch applied the single threaded page fault rate improves
from 217169 to 238755 per second on a POWER5 test box, a 10% improvement.
