Julian Wiedmann 7eee12b44e s390/qdio: lock device while installing IRQ handler
During setup, qdio takes control of the presented ccw device and replaces
the device's IRQ handler with its own. To avoid any interference with
conccurent activity on the device, this should be done while holding the
device's lock.

Greg Kroah-Hartman 724117b77b s390: cio: add SPDX identifiers to the remaining files
It's good to have SPDX identifiers in all files to make it easier to
audit the kernel tree for correct licenses.

Update the drivers/s390/cio/ files with the correct SPDX license
identifier based on the license text in the file itself.  The SPDX
identifier is a legally binding shorthand, which can be used instead of
the full boiler plate text.

This work is based on a script and data from Thomas Gleixner, Philippe
Ombredanne, and Kate Stewart.

Heiko Carstens 5a79859ae0 s390: remove 31 bit support
Remove the 31 bit support in order to reduce maintenance cost and
effectively remove dead code. Since a couple of years there is no
distribution left that comes with a 31 bit kernel.

The 31 bit kernel also has been broken since more than a year before
anybody noticed. In addition I added a removal warning to the kernel
shown at ipl for 5 minutes: a960062e58 ("s390: add 31 bit warning
message") which let everybody know about the plan to remove 31 bit
code. We didn't get any response.

Given that the last 31 bit only machine was introduced in 1999 let's
remove the code.
Anybody with 31 bit user space code can still use the compat mode.

Sebastian Ott 5245c924c2 s390/qdio: add helpers to manage qdio buffers
Users of qdio buffers employ different strategies to manage these
buffers. The qeth driver uses huge contiguous buffers which leads
to high order allocations with all their downsides.

This patch provides helpers to allocate, free, and reset arrays of
qdio buffers using non contiguous pages.

Jan Glauber 6726a807c3 s390/qdio: rename the misleading PCI flag of qdio devices
For each detected qdio device a line like to following is printed:
qdio: 0.0.4102 OSA on SC 1045 using AI:1 QEBSM:0 PCI:1 TDD:1 SIGA: W AP

The PCI flag is misleading as this stands for "program controlled interrupt".
Rename it to PRI "program requested interrupt" which is more accurate and
does not interfere with another popular piece of technology.

Leave the pci string in the code since changing that would result in a huge
patch.

Remove or replace BUG/BUG_ON where possible and convert WARN_ON
to WARN_ON_ONCE if they can occur freqeuently as pointed out by:

Checks have been removed if:
- the error condition leads to a hardware error which gets logged
  and in most cases stops the device
- the error condition is a null pointer access
- the error condition is just pointless or already handled at
  another location

Heiko Carstens a53c8fab3f s390/comments: unify copyright messages and remove file names
Remove the file name from the comment at top of many files. In most
cases the file name was wrong anyway, so it's rather pointless.

Also unify the IBM copyright statement. We did have a lot of sightly
different statements and wanted to change them one after another
whenever a file gets touched. However that never happened. Instead
people start to take the old/"wrong" statements to use as a template
for new files.
So unify all of them in one go.

Steffen Maier e58b0d902f [S390] qdio: fix kernel panic for zfcp 31-bit
The queue_start_poll function pointer field in struct qdio_initialize
had to change its type and become a vector of function pointers to
support asynchronous delivery of storage blocks so rename the field to
make the type change explicit and ensure no other user of qdio tries
to use the field the old way. During setting up the qdio queues, only
dereference vector elements if the vector is actually allocated.

Swen Schillig dfe5bb5061 [SCSI] qdio: base support for hardware data router with zfcp
FICON Express8S supports hardware data router, which requires an
adapted qdio request format.
This part 1/2 provides the qdio base required for exploitation in
zfcp.

Jan Glauber 3d6c76ff32 [S390] qdio: outbound tasklet scan threshold
Introduce a scan treshold for the qdio outbound queues. By setting the
threshold the driver can tell qdio after how much used SBALs qdio
should schedule the outbound tasklet that scans the queue for finished
SBALs. The threshold is specific by the drivers because a
Hipersockets device is much faster in utilizing outbound buffers than a
ZFCP or OSA device.

The default values after how many used SBALs the tasklet should run are:

OSA:          > 31 SBALs
Hipersockets: > 7 SBALs
zfcp:         > 55 SBALs

Jan Glauber d36deae750 qdio: extend API to allow polling
Extend the qdio API to allow polling in the upper-layer driver. This
is needed by qeth to use NAPI.

To use the new interface the upper-layer driver must specify the
queue_start_poll(). This callback is used to signal the upper-layer
driver that is has initiative and must process the inbound queue by
calling qdio_get_next_buffers(). If the upper-layer driver wants to
stop polling it calls qdio_start_irq().

Since adapter interrupts are not completely stoppable qdio implements
a software bit QDIO_QUEUE_IRQS_DISABLED to safely disable interrupts for an
input queue.

The old interface is preserved and will be used as is by zfcp.

Jan Glauber 432ac5e04b [S390] qdio: optimize cache line usage of struct qdio_irq
Remove a memset hack that relied on the internal layout of the
qdio_irq struct and move the per device statistics data into an own
cache line to avoid cache line bashing between the inbound and the
outbound queue tasklets. Also reduce the number of allocated queues
from 32 to 4 which is the current maximum. That saves a cache line
in struct qdio_irq.

Heiko Carstens d1bf85902c [S390] cio: fix storage key handling
Some parts of cio do not shift PAGE_DEFAULT_KEY correctly and end up
with an incorrect key in their data structures.
Since the default key is zero this doesn't really matter. However if
somebody would use key-controlled protection for debugging purposes
it would be quite helpful if all of this would work as expected.

Also remove a stale declaration.

* Don't write debug feature log entries for sl, slsb and sbal since these
  elements can be located from the qdio_q pointer which is also logged.
* Convert WARN_ON for wrong alignment of sbal to BUG_ON.
* Remove WARN_ON's for wrong alignment of q / qib / slib since these
  alignments should be guaranteed by kmem_cache_alloc alignment /
  struct aligned attribute / __get_free_page.

Jan Glauber d303b6fd85 [S390] qdio: report SIGA errors directly
Errors from SIGA instructions are stored in the per queue qdio_error
and reported back when the queue handler is called. That opens a race
when multiple error conditions occur simultanously.

Report SIGA errors immediately in the return value of do_QDIO so the
upper layer can react and SIGA errors no longer interfere with other
errors.

Move the SIGA error handling in qeth from the outbound handler to
qeth_flush_buffers.

Jan Glauber 7a0b4cbc7d [S390] qdio: fix error reporting for hipersockets
Hipersocket connections can encounter temporary busy conditions.
In case of the busy bit set we retry the SIGA operation immediatelly.
If the busy condition still persists after 100 ms we fail and report
the error to the upper layer. The second stage retry logic is removed.
In case of ongoing busy conditions the upper layer needs to reset the
connection.

The reporting of a SIGA error is now done synchronously to allow the
network driver to requeue the buffers. Also no error trace is created
for the temporary SIGA errors so the error message view is not flooded.

Jan Glauber 50f769df1c [S390] qdio: improve inbound buffer acknowledgement
- Use automatic acknowledgement of incoming buffers in QEBSM mode
- Move ACK for non-QEBSM mode always to the newest buffer to prevent
  a race with qdio_stop_polling
- Remove the polling spinlock, the upper layer drivers return new buffers
  in the same code path and could not run in parallel
- Don't flood the error log in case of no-target-buffer-empty
- In handle_inbound we check if we would overwrite an ACK'ed buffer, if so
  advance the pointer to the oldest ACK'ed buffer so we don't overwrite an
  empty buffer in qdio_stop_polling

Fix the two section mismatch warnings below.
This fixes two real bugs since the code which has __exit annotations
may already be gone when it is called.

WARNING: vmlinux.o(.init.text+0x1cc4a): Section mismatch in reference from the function init_QDIO() to the function .exit.text:qdio_setup_exit()
The function __init init_QDIO() references
a function __exit qdio_setup_exit().
This is often seen when error handling in the init function
uses functionality in the exit path.
The fix is often to remove the __exit annotation of
qdio_setup_exit() so it may be used outside an exit section.
