Jonathan Brassow 3a0f9aaee0 dm raid: round region_size to power of two
If the user does not supply a bitmap region_size to the dm raid target,
a reasonable size is computed automatically.  If this is not a power of 2,
the md code will report an error later.

This patch catches the problem early and rounds the region_size to the
next power of two.

Mike Snitzer 018debea8d dm thin: emit ignore_discard in status when discards disabled
If "ignore_discard" is specified when creating the thin pool device then
discard support is disabled for that device.  The pool device's status
should reflect this fact rather than stating "no_discard_passdown"
(which implies discards are enabled but passdown is disabled).

Joe Thornber e3cbf94513 dm persistent data: fix nested btree deletion
When deleting nested btrees, the code forgets to delete the innermost
btree.  The thin-metadata code serendipitously compensates for this by
claiming there is one extra layer in the tree.

This patch corrects both problems.

Joe Thornber e8088073c9 dm thin: fix race between simultaneous io and discards to same block
There is a race when discard bios and non-discard bios are issued
simultaneously to the same block.

Discard support is expensive for all thin devices precisely because you
have to be careful to quiesce the area you're discarding.  DM thin must
handle this conflicting IO pattern (simultaneous non-discard vs discard)
even though a sane application shouldn't be issuing such IO.

The race manifests as follows:

1. A non-discard bio is mapped in thin_bio_map.
   This doesn't lock out parallel activity to the same block.

2. A discard bio is issued to the same block as the non-discard bio.

3. The discard bio is locked in a dm_bio_prison_cell in process_discard
   to lock out parallel activity against the same block.

4. The non-discard bio's mapping continues and its all_io_entry is
   incremented so the bio is accounted for in the thin pool's all_io_ds
   which is a dm_deferred_set used to track time locality of non-discard IO.

5. The non-discard bio is finally locked in a dm_bio_prison_cell in
   process_bio.

The race can result in deadlock, leaving the block layer hanging waiting
for completion of a discard bio that never completes, e.g.:

The thinp-test-suite's test_discard_random_sectors reliably hits this
deadlock on fast SSD storage.

The fix for this race is that the all_io_entry for a bio must be
incremented whilst the dm_bio_prison_cell is held for the bio's
associated virtual and physical blocks.  That cell locking wasn't
occurring early enough in thin_bio_map.  This patch fixes this.

Care is taken to always call the new function inc_all_io_entry() with
the relevant cells locked, but they are generally unlocked before
calling issue() to try to avoid holding the cells locked across
generic_submit_request.

Also, now that thin_bio_map may lock bios in a cell, process_bio() is no
longer the only thread that will do so.  Because of this we must be sure
to use cell_defer_except() to release all non-holder entries, that
were added by the other thread, because they must be deferred.

This patch depends on "dm thin: replace dm_cell_release_singleton with
cell_defer_except".

Joe Thornber b7ca9c9273 dm thin: replace dm_cell_release_singleton with cell_defer_except
Change existing users of the function dm_cell_release_singleton to share
cell_defer_except instead, and then remove the now-unused function.

Everywhere that calls dm_cell_release_singleton, the bio in question
is the holder of the cell.

If there are no non-holder entries in the cell then cell_defer_except
behaves exactly like dm_cell_release_singleton.  Conversely, if there
*are* non-holder entries then dm_cell_release_singleton must not be used
because those entries would need to be deferred.

Consequently, it is safe to replace use of dm_cell_release_singleton
with cell_defer_except.

This patch is a pre-requisite for "dm thin: fix race between
simultaneous io and discards to same block".

Mike Snitzer c1a94672a8 dm: disable WRITE SAME
WRITE SAME bios are not yet handled correctly by device-mapper so
disable their use on device-mapper devices by setting
max_write_same_sectors to zero.

As an example, a ciphertext device is incompatible because the data
gets changed according to the location at which it written and so the
dm crypt target cannot support it.

Alasdair G Kergon e910d7ebec dm ioctl: prevent unsafe change to dm_ioctl data_size
Abort dm ioctl processing if userspace changes the data_size parameter
after we validated it but before we finished copying the data buffer
from userspace.

The dm ioctl parameters are processed in the following sequence:
 1. ctl_ioctl() calls copy_params();
 2. copy_params() makes a first copy of the fixed-sized portion of the
    userspace parameters into the local variable "tmp";
 3. copy_params() then validates tmp.data_size and allocates a new
    structure big enough to hold the complete data and copies the whole
    userspace buffer there;
 4. ctl_ioctl() reads userspace data the second time and copies the whole
    buffer into the pointer "param";
 5. ctl_ioctl() reads param->data_size without any validation and stores it
    in the variable "input_param_size";
 6. "input_param_size" is further used as the authoritative size of the
    kernel buffer.

The problem is that userspace code could change the contents of user
memory between steps 2 and 4.  In particular, the data_size parameter
can be changed to an invalid value after the kernel has validated it.
This lets userspace force the kernel to access invalid kernel memory.

The fix is to ensure that the size has not changed at step 4.

This patch shouldn't have a security impact because CAP_SYS_ADMIN is
required to run this code, but it should be fixed anyway.

This patch fixes a compilation failure on sparc32 by renaming struct node.

struct node is already defined in include/linux/node.h. On sparc32, it
happens to be included through other dependencies and persistent-data
doesn't compile because of conflicting declarations.

Jesper Juhl e5d85b9ac3 [media] rc: Fix double free in gpio_ir_recv_probe()
At the 'err_request_irq' label, rc_unregister_device(rcdev) frees its
argument. So when we fall through to the 'err_gpio_request' label
further down and call rc_free_device(rcdev) then that's a double free.
Fix that by moving 'rcdev = NULL' from after the call to
rc_free_device() to after rc_unregister_device(). That fixes the
problem since rc_free_device() just does nothing if passed NULL and
there's no further use of 'rcdev' after the call to rc_free_device()
so it's not needed there.

Lad, Prabhakar caff80c35f [media] davinci: vpbe: pass different platform names to handle different ip's
The vpbe driver can handle different platforms DM644X, DM36X and
DM355. To differentiate between this platforms venc_type/vpbe_type
was passed as part of platform data which was incorrect. The correct
way to differentiate to handle this case is by passing different
platform names.
This patch creates platform_device_id[] array supporting different
platforms and assigns id_table to the platform driver, and finally
in the probe gets the actual device by using platform_get_device_id()
and gets the appropriate driver data for that platform.
Taking this approach will also make the DT transition easier.

downscaling facility. The driver supports both continuous and

Manjunath Hadli da43b6ccad [media] davinci: vpfe: dm365: add IPIPE support for media controller driver
Add the IPIPE subdevice to the DM365 vpfe driver. The IPIPE is the
major sub IP in the DM365 capture VPFE hardware and implements
black clamping, color space conversion, edge enhancements etc.

Manjunath Hadli 622897da67 [media] davinci: vpfe: add v4l2 video driver support
Add a generic video driver functionality to be used by all the vpfe
drivers for davinci SoCs. The functionality includes all the
standard v4l2 interfaces including streaming. The video node
interface can be used both as an input and output node for both
continuous and single shot modes. Also supports dv_presets to include
HD modes, wth support for both user pointer IO and mmap. The buffering
mechanism is based on videobuf2 interface.

Add the vpfe capture driver which implements media controller
interface. The driver supports the DM365 sub ip units for capture
namely - ISIF, IPIPE, IPIPEIF, Resizer. This file represents the main
driver which does isr registration, v4l2 device registration,
media registration and platform driver registrations.
It calls the appropriate subdevs from here to create subdevices
and media entities.

Hans de Goede 18fa0d36ad [media] gspca_sonixb: Properly wait between i2c writes
We must wait for the previous i2c write to complete before starting a new
one. Sofar we were getting away with this, but it seems that some parts
of the usb-subsystem has been sped up making us go to fast :)
This fixes streaming on sn9c103 based cams not working with an
"i2c_w error" error.

Hans de Goede 7a29ee2e37 [media] stk-webcam: Add an upside down dmi table, and add the Asus G1 to it
The stk-webcam module is inserted upside-down in some webcams, so we
need to de hflip and vflip by default on some models.
Note that this patch inverts the value of the controls as reported by
the control API in this case so that for the user they still make sense
(iow not doing any flipping from the ctrl API pov results in an upright
image).

Sylwester Nawrocki 6319d6a002 [media] fimc-lite: Add ISP FIFO output support
Add second source media pad for the FIFO data output to FIMC-IS
and implement subdev s_stream op for configurations where FIMC-LITE
is used as a glue logic between FIMC-IS and MIPI-CSIS or an image
sensor. The second source media pad will be linked to the FIMC-LITE
video node.
For proper configuration the attached image sensor/video encoder
properties are needed, like video bus type, signal polarities, etc.
For this purpose there is a small routine added that walks the
pipeline and returns the sensor subdev.

Setting FIMC_REG_CITRGFMT_FLIP_X_MIRROR bit causes X-axis image
flip (vertical flip) and thus it corresponds to V4L2_CID_VFLIP.
Likewise, setting FIMC_REG_CITRGFMT_FLIP_Y_MIRROR bit causes Y-axis
image flip (horizontal flip) and thus it corresponds to V4L2_CID_HFLIP.
Currently the driver does X-axis flip when V4L2_CID_HFLIP is set and
Y-axis flip for V4L2_CID_VFLIP. Fix this incorrect assignment by setting
proper FIMC_REG_CITRGFMT register bits for ctx->hflip and ctx->vflip.

Robin Holt 891348ca0f SGI-XP: handle non-fatal traps
We found a user code which was raising a divide-by-zero trap.  That trap
would lead to XPC connections between system-partitions being torn down
due to the die_chain notifier callouts it received.

This also revealed a different issue where multiple callers into
xpc_die_deactivate() would all attempt to do the disconnect in parallel
which would sometimes lock up but often overwhelm the console on very
large machines as each would print at least one line of output at the
end of the deactivate.

I reviewed all the users of the die_chain notifier and changed the code
to ignore the notifier callouts for reasons which will not actually lead
to a system to continue on to call die().

Andrew Morton 5abe257af8 revert "rtc: recycle id when unloading a rtc driver"
Revert commit 2830a6d201.

We already perform the ida_simple_remove() in rtc_device_release(),
which is an appropriate place.  Commit 2830a6d20 ("rtc: recycle id when
unloading a rtc driver") caused the kernel to emit

	ida_remove called for id=0 which is not allocated.

warnings when rtc_device_release() tries to release an alread-released
ID.

Let's restore things to their previous state and then work out why
Vincent's kernel wasn't calling rtc_device_release() - presumably a bug
in a specific sub-driver.

Zhenzhong Duan 9f9c9cbb60 drivers/firmware/dmi_scan.c: fetch dmi version from SMBIOS if it exists
The right dmi version is in SMBIOS if it's zero in DMI region

This issue was originally found from an oracle bug.
One customer noticed system UUID doesn't match between dmidecode & uek2.

So we need to get dmi version to distinguish.  If version is 0.0, the
real version is taken from the SMBIOS version.  This is part of original
kernel comment in code.

Woodhouse, David 6491d4d028 intel-iommu: Free old page tables before creating superpage
The dma_pte_free_pagetable() function will only free a page table page
if it is asked to free the *entire* 2MiB range that it covers. So if a
page table page was used for one or more small mappings, it's likely to
end up still present in the page tables... but with no valid PTEs.

This was fine when we'd only be repopulating it with 4KiB PTEs anyway
but the same virtual address range can end up being reused for a
*large-page* mapping. And in that case were were trying to insert the
large page into the second-level page table, and getting a complaint
from the sanity check in __domain_mapping() because there was already a
corresponding entry. This was *relatively* harmless; it led to a memory
leak of the old page table page, but no other ill-effects.

Fix it by calling dma_pte_clear_range (hopefully redundant) and
dma_pte_free_pagetable() before setting up the new large page.

Guennadi Liakhovetski cea4c9e46c [media] media: soc-camera: remove superfluous JPEG checking
Explicit checks for the JPEG pixel format in soc_mbus_bytes_per_line() and
soc_mbus_image_size() are superfluous, because also without them these
functions will perform correctly. The former will return 0 based on
packing == SOC_MBUS_PACKING_VARIABLE and the latter will simply multiply
the user-provided line length by the image height to obtain a frame buffer
size estimate. The original version of the "media: soc_camera: don't clear
pix->sizeimage in JPEG mode" patch was correct and my amendment, adding
these two checks was superfluous.

Guennadi Liakhovetski 05efa71bdc [media] media: add a VEU MEM2MEM format conversion and scaling driver
Video Engine Unit (VEU) is an IP block, found in multiple SuperH and ARM-
based sh-mobile and r-mobile SoCs, capable of processing video data. It
can perform colour-space conversion, scaling and several filtering
transformations. This patch adds an initial implementation of a mem2mem
V4L2 driver for VEU. So far only conversion from NV12 to RGB565 is
supported. Further functionality shall be added in the future.
This driver is based on a VEU vidix driver by Magnus Damm.

Sends ofld_connect_wr on same ctrlq. It needs to go  on the same control txq as

Retries on active open replies with EADDRINUSE.

Uses active open fw wr only if active filter region is set.

Adds stat for ofld_connect_wr failures.

This patch also adds debugfs file to show endpoints.

Vipul Pandya 1cab775c3e RDMA/cxgb4: Fix LE hash collision bug for passive open connection
It establishes passive open connection through firmware work request. Passive
open connection will go through this path as now instead of listening server we
create a server filter which will redirect the incoming SYN packet to the
offload queue. After this driver tries to establish the connection using
firmware work request.

Vipul Pandya 5be78ee924 RDMA/cxgb4: Fix LE hash collision bug for active open connection
It enables establishing active open connection using fw_ofld_connection work
request when cpl_act_open_rpl says TCAM full error which may be because
of LE hash collision. Current support is only for IPv4 active open connections.

Sets ntuple bits in active open requests. For T4 firmware greater than 1.4.10.0
ntuple bits are required to be set.

Adds nocong and enable_ecn module parameter options.

We never really clarified if unmap could be done in atomic context.
But since mapping might require sleeping, this implies mutex in use
to synchronize mapping/unmapping, so unmap could sleep as well.  Add
a might_sleep() to clarify this.

Gregory CLEMENT bc1008cf7d gpio/mvebu-gpio: Make mvebu-gpio depend on OF_CONFIG
When building without device tree support (for the mv78x00 SoCs for
example), the build failed because of_irq_count is undeclared. However
mvebu-gpio is not designed to build without device tree support. So
make it depends on OF_CONFIG, remove the #ifdef OF_CONFIG line and the
platform_device_id.

Tested on RD-78x00-mASA, DB-78460-BP, DB-88F6710-BP-DDR3,
DB-MV784MP-GP, Mirabox and OpenBlocks AX3.

Fix regression introduced by 85b144f860
"drm/ttm: call ttm_bo_cleanup_refs with reservation and lru lock held, v3"

Slowpath ttm_bo_cleanup_refs_and_unlock accidentally tried to increase
refcount on &bo->sync_obj instead of bo->sync_obj.

The compiler didn't complain since sync_obj_ref takes a void pointer,
so it was still valid c.

This could result in lockups, memory corruptions, and warnings like

Tomas Winkler d692170037 watchdog: mei: avoid oops in watchdog unregister code path
With commit c7d3df3 "mei: use internal watchdog device registration
tracking" will crash the kernel on shutdown path on systems
where ME watchdog is not present.
Since the watchdog was never initialized in such case
the WDOG_UNREGISTERED bit is never set and the system
crashes on access to uninitialized variables down the path.

To solve the issue we query for NULL on watchdog driver driver_data
to check whether the device is registered. This is handled in the
driver and doesn't depend on watchdog core internals.

The current sp5100_tco driver only supports SP5100/SB7x0 chipset, doesn't
support SB8x0 chipset, because current sp5100_tco driver doesn't know that the
offset address for watchdog timer was changed from SB8x0 chipset.

The offset address of SP5100 and SB7x0 chipsets are as follows, quote from the
AMD SB700/710/750 Register Reference Guide (Page 164) and the AMD SP5100
Register Reference Guide (Page 166).

address (for example, 0xbafe00) from wrong offset address, and the following

With this patch, sp5100_tco driver supports SB8x0 chipset, and can avoid
iomem resource conflict. The processing of this patch is as follows.

 Step 1) Attempt to get the watchdog base address from indirect I/O (0xCD6
         and 0xCD7).
  - Go to the step 7 if obtained address hasn't conflicted with other
    resource. But, currently, the address (0xfec000f0) conflicts with the
    IOAPIC MMIO address, and the following message is logged.

       SP5100 TCO timer: mmio address 0xfec000f0 already in use

    0xfec000f0 is recommended by AMD BIOS Developer's Guide. So, go to the
    next step.

 Step 2) Attempt to get the SBResource_MMIO base address from AcpiMmioEN (for
         SB8x0,  PM_Reg:24h) or SBResource_MMIO (SP5100/SB7x0, PCI_Reg:9Ch)
         register.
  - Go to the step 7 if these register has enabled by BIOS, and obtained
    address hasn't conflicted with other resource.
  - If above condition isn't true, go to the next step.

 Step 3) Attempt to get the free MMIO address from allocate_resource().
  - Go to the step 7 if these register has enabled by BIOS, and obtained
    address hasn't conflicted with other resource.
  - Driver initialization has failed if obtained address has conflicted
    with other resource, and no 'force_addr' parameter is specified.

 Step 4) Use the specified address If 'force_addr' parameter is specified.
  - allocate_resource() function may fail, when the PCI bridge device occupies
    iomem resource from 0xf0000000 to 0xffffffff. To handle such a case,
    I added 'force_addr' parameter to sp5100_tco driver. With 'force_addr'
    parameter, sp5100_tco driver directly can assign MMIO address for watchdog
    timer from free iomem region. Note that It's dangerous to specify wrong
    address in the 'force_addr' parameter.

    indirect I/O (0xCD6 and 0xCD7).

 Step 7) Enable and setup the watchdog timer

This patch has worked fine on my test environment (ASUS M4A89GTD-PRO/USB3 and
DL165G7). therefore I believe that it's no problem to re-program the MMIO
address for watchdog timer to chipset during disabled watchdog. However,
I'm not sure about it, because I don't know much about chipset programming.

Convert omap_wdt to new watchdog core. On OMAP boards, there are usually
multiple watchdogs. Since the new watchdog core supports multiple
watchdogs, all watchdog drivers used on OMAP should be converted.

The legacy watchdog device node is still created, so this should not
break existing users.

With device tree support in place, we should not use IMX_HAVE_PLATFORM_IMX2_WDT
as a dependency for selecting the imx2_wdt driver.

Karicheri, Muralidharan 5235f57a6f davinci_wdt: preparation for switch to common clock framework
As a first step towards migrating davinci platforms to use common clock
framework, replace all instances of clk_enable() with clk_prepare_enable()
and clk_disable() with clk_disable_unprepare(). Until the platform is
switched to use the CONFIG_HAVE_CLK_PREPARE Kconfig variable, this just
adds a might_sleep() call and would work without any issues.

This will make it easy later to switch to common clk based implementation
of clk driver from DaVinci specific driver.

IT8721 and IT8728 support Intel PECI temperature reporting. Each sensor
can be programmed to display the temperature reported on the PECI interface.

If configured for Intel PECI, the driver reported the wrong sensor type for
the respective thermal sensor. Fix the code to correctly report it as
"Intel PECI (6)".

A deeper analysis of the code shows that these are false positives, as

write is never attempted with i >= 3. However this shows that the code
isn't very robust and future changes could easily introduce a buffer
overflow. So let's add a safety check to prevent that and make smatch
happy.

some fixups are needed to work around BIOS bugs, in particular when

Lennert Buytenhek 4945106d21 ksz884x: fix receive polling race condition
The ksz884x driver does receive processing in a custom tasklet, and
seems to be assuming that since it takes its private interface spinlock
with spin_lock_irq(), it won't be running concurrently with its own
interrupt handler, as it cannot be preempted by it, but since its
interrupt handler doesn't do any locking whatsoever, the receive
processing tasklet and interrupt handler can end up running concurrently
on different CPUs.

As a result of this, the ksz884x receive path ends up locking up fairly
easily, when the receive processing tasklet's reenabling of receive
interrupts (due to it being done with polling the receive ring) races
with the interrupt handler's disabling of receive interrupts (due to a
new receive interrupt coming in) resulting in the receive interrupt
being masked but the receive processing tasklet not being scheduled.

Fix this by making the ksz884x interrupt handler take its private
interface spinlock.  This requires upgrading the spin_lock() in the
transmit cleanup tasklet to a spin_lock_irq(), as otherwise the IRQ
handler can preempt transmit cleanup and deadlock the system, but
with those two changes, no more receive lockups have been observed.

Jack Morgenstein 3c439b5586 mlx4_core: Allow choosing flow steering mode
Device managed flow steering will be enabled only under administrator
directive provided through setting the existing module parameter
log_num_mgm_entry_size to -1 (if the device actually supports flow
steering).  If flow steering isn't requested or not available, the
driver will use the value of log_num_mgm_entry_size and B0 steering.

Jack Morgenstein 7b8157bedc mlx4_core: Adjustments to Flow Steering activation logic for SR-IOV
Separate flow steering capability detection from the decision to activate.

For the master (and for native), detect the flow steering capability
in mlx4_dev_cap, but activate the appropriate steering type in a new
function choose_flow_steering() based on detected data.

For VFs, activate flow steering based on what was actually activated
by the master, where that info is obtained via QUERY_HCA. This fixes
the current VF detection which is wrongly based on QUERY_DEV_CAP.

Also, for SR-IOV mode, if flow steering may be activated, do so only
if the max number of QPs per rule is sufficient to satisfy one
subscription per VF.  If not, fall back to B0 mode. This is needed to
serve registrations done by L2 network drivers such as mlx4_en and
IPoIB when the network stack attempts to join to multicast groups such
as all-hosts or the IPoIB broadcast group.

Since VFs may be mapped to VMs which aren't trusted entities, flow
steering rules attached through the wrapper on behalf of VFs must be
checked to make sure that the specified QP number is assigned to that
VF.  Also, make sure to keep the QP busy till the end of the operation
from the resource tracker point of view.

Vipul Pandya dca4faeb81 cxgb4: Add LE hash collision bug fix path in LLD driver
It supports establishing passive open connection through firmware filter work
request. Passive open connection will go through this path as now instead of
listening server we create a server filter which will redirect the incoming SYN
packet to the offload queue.

It divides filter region into regular filters and server filter portion. It
introduces new server filter region which will be exclusively used for creating
server filters. This region will not overlap with regular filter region.

It provides new API cxgb4_alloc_sftid in LLD for getting stid in case of LE
hash collision path. This new stid will be used to open server filter in the
filter region.

Vipul Pandya f2b7e78dbc cxgb4: Add T4 filter support
The T4 architecture is capable of filtering ingress packets at line rate
using the rule in TCAM. If packet hits a rule in the TCAM then it can be either
dropped or passed to the receive queues based on a rule settings.

This patch adds framework for managing filters and to use T4's filter
capabilities. It constructs a Firmware Filter Work Request which writes the
filter at a specified index to get the work done. It hosts shadow copy of
ingress filter entry to check field size limitations and save memory in the
case where the filter table is large.

Roland Dreier b13912bbb4 IPoIB: Call skb_dst_drop() once skb is enqueued for sending
Currently, IPoIB delays collecting send completions for TX packets in
order to batch work more efficiently.  It does skb_orphan() right after
queuing the packets so that destructors run early, to avoid problems
like holding socket send buffers for too long (since we might not
collect a send completion until a long time after the packet is
actually sent).

However, IPoIB clears IFF_XMIT_DST_RELEASE because it actually looks
at skb_dst() to update the PMTU when it gets a too-long packet.  This
means that the packets sitting in the TX ring with uncollected send
completions are holding a reference on the dst.  We've seen this lead
to pathological behavior with respect to route and neighbour GC.  The
easy fix for this is to call skb_dst_drop() when we call skb_orphan().

Also, give packets sent via connected mode (CM) the same skb_orphan()
/ skb_dst_drop() treatment that packets sent via datagram mode get.

renamed of_find_matching_node() to of_find_matching_node_and_match() and
created a new static inline of_find_matching_node() wrapper around the
new name. However, the change neglected to change the EXPORT_SYMBOL()
reference causing build errors for modules.

This patch fixes the EXPORT_SYMBOL() statement. Discovered on a PowerPC
Efika build with the mpc52xx_uart driver being built as a module.

Shawn Guo c1e37ea287 net: fec: forbid FEC_PTP on SoCs that do not support
Beside imx6q, the kernel built from imx_v6_v7_defconfig is also
supposed to be running on other IMX SoCs that do not have the PTP
block.  Before fec driver gets fixed to run-time detect target hardware
rather than conditional compiling with #ifdef CONFIG_FEC_PTP, let's
give it a quick fix in Kconfig to forbid FEC_PTP on those IMX SoCs that
do not support PTP.

Sathya Perla d23e946cb6 be2net: fix wrong frag_idx reported by RX CQ
The RX CQ can report completions with invalid frag_idx when the RXQ that
was *previously* using it, was not cleaned up properly. This hits
a BUG_ON() in be2net.

When completion coalescing is enabled on a CQ, an explicit CQ-notify
(with rearm) is needed for each compl, to flush partially coalesced CQ
entries that are pending DMA.

In be_close(), this fix now notifies CQ for each compl, waits explicitly
for the flush compl to arrive and complains if it doesn't arrive.

Also renaming be_crit_error() to be_hw_error() as it's the more
appropriate name and to convey that we don't wait for the flush compl
only when a HW error has occurred.

This patch fixes the checkpatch error and warning as below:

  WARNING: space prohibited between function name and open parenthesis '('
  ERROR: trailing statements should be on next line

Also, long comments are fixed for the preferred style and unnecessary
lines are removed.
