Sagi Grimberg <sagig@dev.mellanox.co.il> points out that a steady
stream of CQ events could starve other work because of the boundless
loop pooling in rpcrdma_{send,recv}_poll().
Instead of a (potentially infinite) while loop, return after
collecting a budgeted number of completions.

The current CQ handler uses the ib_wc.opcode field to distinguish
between event types. However, the contents of that field are not
reliable if the completion status is not IB_WC_SUCCESS.
When an error completion occurs on a send event, the CQ handler
schedules a tasklet with something that is not a struct rpcrdma_rep.
This is never correct behavior, and sometimes it results in a panic.
To resolve this issue, split the completion queue into a send CQ and
a receive CQ. The send CQ handler now handles only struct rpcrdma_mw
wr_id's, and the receive CQ handler now handles only struct
rpcrdma_rep wr_id's.

The MEMWINDOWS and MEMWINDOWS_ASYNC memory registration modes were
intended as stop-gap modes before the introduction of FRMR. They
are now considered obsolete.
MEMWINDOWS_ASYNC is also considered unsafe because it can leave
client memory registered and exposed for an indeterminant time after
each I/O.
At this point, the MEMWINDOWS modes add needless complexity, so

Two memory region leaks were found during testing:
1. rpcrdma_buffer_create: While allocating RPCRDMA_FRMR's
ib_alloc_fast_reg_mr is called and then ib_alloc_fast_reg_page_list is
called. If ib_alloc_fast_reg_page_list returns an error it bails out of
the routine dropping the last ib_alloc_fast_reg_mr frmr region creating a
memory leak. Added code to dereg the last frmr if
ib_alloc_fast_reg_page_list fails.
2. rpcrdma_buffer_destroy: While cleaning up, the routine will only free
the MR's on the rb_mws list if there are rb_send_bufs present. However, in
rpcrdma_buffer_create while the rb_mws list is being built if one of the MR
allocation requests fail after some MR's have been allocated on the rb_mws
list the routine never gets to create any rb_send_bufs but instead jumps to
the rpcrdma_buffer_destroy routine which will never free the MR's on rb_mws
list because the rb_send_bufs were never created. This leaks all the MR's
on the rb_mws list that were created prior to one of the MR allocations
failing.
Issue(2) was seen during testing. Our adapter had a finite number of MR's
available and we created enough connections to where we saw an MR
allocation failure on our Nth NFS connection request. After the kernel
cleaned up the resources it had allocated for the Nth connection we noticed
that FMR's had been leaked due to the coding error described above.
Issue(1) was seen during a code review while debugging issue(2).

An NFS/RDMA client's source port is meaningless for RDMA transports.
The transport layer typically sets the source port value on the
connection to a random ephemeral port.
Currently, NFS server administrators must specify the "insecure"
export option to enable clients to access exports via RDMA.
But this means NFS clients can access such an export via IP using an
ephemeral port, which may not be desirable.
This patch eliminates the need to specify the "insecure" export
option to allow NFS/RDMA clients access to an export.

For my reproducer, kernel log shows:
NFS: server cheating in read reply: count 4096 > recvd 0
rpcrdma_inline_fixup() is zeroing the xdr_stream::page_len field,
and xdr_align_pages() is now returning that value to the READ XDR
decoder function.
That field is set up by xdr_inline_pages() by the READ XDR encoder
function. As far as I can tell, it is supposed to be left alone
after that, as it describes the dimensions of the reply xdr_stream,
not the contents of that stream.

This patch enhances the IB core support for Memory Windows (MWs).
MWs allow an application to have better/flexible control over remote
access to memory.
Two types of MWs are supported, with the second type having two flavors:
Type 1 - associated with PD only
Type 2A - associated with QPN only
Type 2B - associated with PD and QPN
Applications can allocate a MW once, and then repeatedly bind the MW
to different ranges in MRs that are associated to the same PD. Type 1
windows are bound through a verb, while type 2 windows are bound by
posting a work request.
The 32-bit memory key is composed of a 24-bit index and an 8-bit
key. The key is changed with each bind, thus allowing more control
over the peer's use of the memory key.
The changes introduced are the following:
* add memory window type enum and a corresponding parameter to ib_alloc_mw.
* type 2 memory window bind work request support.
* create a struct that contains the common part of the bind verb struct
ibv_mw_bind and the bind work request into a single struct.
* add the ib_inc_rkey helper function to advance the tag part of an rkey.
Consumer interface details:
* new device capability flags IB_DEVICE_MEM_WINDOW_TYPE_2A and
IB_DEVICE_MEM_WINDOW_TYPE_2B are added to indicate device support
for these features.
Devices can set either IB_DEVICE_MEM_WINDOW_TYPE_2A or
IB_DEVICE_MEM_WINDOW_TYPE_2B if it supports type 2A or type 2B
memory windows. It can set neither to indicate it doesn't support
type 2 windows at all.
* modify existing provides and consumers code to the new param of
ib_alloc_mw and the ib_mw_bind_info structure

Commit 43cedbf0e8 (SUNRPC: Ensure that
we grab the XPRT_LOCK before calling xprt_alloc_slot) is causing
hangs in the case of NFS over UDP mounts.
Since neither the UDP or the RDMA transport mechanism use dynamic slot
allocation, we can skip grabbing the socket lock for those transports.
Add a new rpc_xprt_op to allow switching between the TCP and UDP/RDMA
case.
Note that the NFSv4.1 back channel assigns the slot directly
through rpc_run_bc_task, so we can ignore that case.

rpciod is trying to allocate memory for a new socket to talk to the
server. The VM ends up calling ->releasepage to get more memory, and it
tries to do a blocking commit. That commit can't succeed however without
a connected socket, so we deadlock.
Fix this by setting PF_FSTRANS on the workqueue task prior to doing the
socket allocation, and having nfs_release_page check for that flag when
deciding whether to do a commit call. Also, set PF_FSTRANS
unconditionally in rpc_async_schedule since that function can also do
allocations sometimes.

Pull nfsd changes from Bruce Fields:
Highlights:
- Benny Halevy and Tigran Mkrtchyan implemented some more 4.1 features,
moving us closer to a complete 4.1 implementation.
- Bernd Schubert fixed a long-standing problem with readdir cookies on
ext2/3/4.
- Jeff Layton performed a long-overdue overhaul of the server reboot
recovery code which will allow us to deprecate the current code (a
rather unusual user of the vfs), and give us some needed flexibility
for further improvements.
- Like the client, we now support numeric uid's and gid's in the
auth_sys case, allowing easier upgrades from NFSv2/v3 to v4.x.
Plus miscellaneous bugfixes and cleanup.
Thanks to everyone!
There are also some delegation fixes waiting on vfs review that I
suppose will have to wait for 3.5. With that done I think we'll finally
turn off the "EXPERIMENTAL" dependency for v4 (though that's mostly
symbolic as it's been on by default in distro's for a while).
And the list of 4.1 todo's should be achievable for 3.5 as well:
http://wiki.linux-nfs.org/wiki/index.php/Server_4.0_and_4.1_issues
though we may still want a bit more experience with it before turning it
on by default.

When the rpc_memreg_strategy is 5, FRMR are used to map RPC data.
This mode uses an FRMR to map the RPC data, then invalidates
(i.e. unregisers) the data in xprt_rdma_free. These FRMR are used
across connections on the same mount, i.e. if the connection goes
away on an idle timeout and reconnects later, the FRMR are not
destroyed and recreated.
This creates a problem for transport errors because the WR that
invalidate an FRMR may be flushed (i.e. fail) leaving the
FRMR valid. When the FRMR is later used to map an RPC it will fail,
tearing down the transport and starting over. Over time, more and
more of the FRMR pool end up in the wrong state resulting in
seemingly random disconnects.
This fix keeps track of the FRMR state explicitly by setting it's
state based on the successful completion of a reg/inv WR. If the FRMR
is ever used and found to be in the wrong state, an invalidate WR
is prepended, re-syncing the FRMR state and avoiding the connection loss.

The RPCRDMA marshalling logic assumed that xdr->page_base was an
offset into the first page of xdr->page_list. It is in fact an
offset into the xdr->page_list itself, that is, it selects the
first page in the page_list and the offset into that page.
The symptom depended in part on the rpc_memreg_strategy, if it was
FRMR, or some other one-shot mapping mode, the connection would get
torn down on a base and bounds error. When the badly marshalled RPC
was retransmitted it would reconnect, get the error, and tear down the
connection again in a loop forever. This resulted in a hung-mount. For
the other modes, it would result in silent data corruption. This bug is
most easily reproduced by writing more data than the filesystem
has space for.
This fix corrects the page_base assumption and otherwise simplifies
the iov mapping logic.

svc_xprt_received must be called when ->xpo_recvfrom has finished
receiving a message, so that the XPT_BUSY flag will be cleared and
if necessary, requeued for further work.
This call is currently made in each ->xpo_recvfrom function, often
from multiple different points. In each case it is the earliest point
on a particular path where it is known that the protection provided by
XPT_BUSY is no longer needed.
However there are (still) some error paths which do not call
svc_xprt_received, and requiring each ->xpo_recvfrom to make the call
does not encourage robustness.
So: move the svc_xprt_received call to be made just after the
call to ->xpo_recvfrom(), and move it of the various ->xpo_recvfrom
methods.
This means that it may not be called at the earliest possible instant,
but this is unlikely to be a measurable performance issue.
Note that there are still other calls to svc_xprt_received as it is
also needed when an xprt is newly created.

percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files. percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.
percpu.h -> slab.h dependency is about to be removed. Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability. As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.
http://userweb.kernel.org/~tj/misc/slabh-sweep.py
The script does the followings.
* Scan files for gfp and slab usages and update includes such that
only the necessary includes are there. ie. if only gfp is used,
gfp.h, if slab is used, slab.h.
* When the script inserts a new include, it looks at the include
blocks and try to put the new include such that its order conforms
to its surrounding. It's put in the include block which contains
core kernel includes, in the same order that the rest are ordered -
alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
doesn't seem to be any matching order.
* If the script can't find a place to put a new include (mostly
because the file doesn't have fitting include block), it prints out
an error message indicating which .h file needs to be added to the
file.
The conversion was done in the following steps.
1. The initial automatic conversion of all .c files updated slightly
over 4000 files, deleting around 700 includes and adding ~480 gfp.h
and ~3000 slab.h inclusions. The script emitted errors for ~400
files.
2. Each error was manually checked. Some didn't need the inclusion,
some needed manual addition while adding it to implementation .h or
embedding .c file was more appropriate for others. This step added
inclusions to around 150 files.
3. The script was run again and the output was compared to the edits
from #2 to make sure no file was left behind.
4. Several build tests were done and a couple of problems were fixed.
e.g. lib/decompress_*.c used malloc/free() wrappers around slab
APIs requiring slab.h to be added manually.
5. The script was run on all .h files but without automatically
editing them as sprinkling gfp.h and slab.h inclusions around .h
files could easily lead to inclusion dependency hell. Most gfp.h
inclusion directives were ignored as stuff from gfp.h was usually
wildly available and often used in preprocessor macros. Each
slab.h inclusion directive was examined and added manually as
necessary.
6. percpu.h was updated not to include slab.h.
7. Build test were done on the following configurations and failures
were fixed. CONFIG_GCOV_KERNEL was turned off for all tests (as my
distributed build env didn't work with gcov compiles) and a few
more options had to be turned off depending on archs to make things
build (like ipr on powerpc/64 which failed due to missing writeq).
* x86 and x86_64 UP and SMP allmodconfig and a custom test config.
* powerpc and powerpc64 SMP allmodconfig
* sparc and sparc64 SMP allmodconfig
* ia64 SMP allmodconfig
* s390 SMP allmodconfig
* alpha SMP allmodconfig
* um on x86_64 SMP allmodconfig
8. percpu.h modifications were reverted so that it could be applied as
a separate patch and serve as bisection point.
Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

RPC universal address generation is currently done in several places:
rpcb_clnt.c, nfs4proc.c xprtsock.c, and xprtrdma.c. Remove the
redundant cases that convert a socket address to a universal
address. The nfs4proc.c case takes a pre-formatted presentation
address string, not a socket address, so we'll leave that one.
Because the new uaddr constructor uses the recently introduced
rpc_ntop(), it now supports proper "::" shorthanding for IPv6
addresses. This allows the kernel to register properly formed
universal addresses with the local rpcbind service, in _all_ cases.
The kernel can now also send properly formed universal addresses in
RPCB_GETADDR requests, and support link-local properly when
encoding and decoding IPv6 addresses.

This logic sets the connection parameter that configures the local device
and informs the remote peer how many concurrent incoming RDMA_READ
requests are supported. The original logic didn't really do what was
intended for two reasons:
- The max number supported by the device is typically smaller than
any one factor in the calculation used, and
- The field in the connection parameter structure where the value is
stored is a u8 and always overflows for the default settings.
So what really happens is the value requested for responder resources
is the left over 8 bits from the "desired value". If the desired value
happened to be a multiple of 256, the result was zero and it wouldn't
connect at all.
Given the above and the fact that max_requests is almost always larger
than the max responder resources supported by the adapter, this patch
simplifies this logic and simply requests the max supported by the device,
subject to a reasonable limit.

RPCRDMA requests that specify a read-list are fetched with RDMA_READ. Using
an FRMR to map the data sink improves NFSRDMA security on transports that
place the RDMA_READ data sink LKEY on the wire because the valid lifetime
of the MR is only the duration of the RDMA_READ. The LKEY is invalidated
when the last RDMA_READ WR completes.
Mapping the data sink also allows for very large amounts to data to be
fetched with a single WR, so if the client is also using FRMR, the entire
RPC read-list can be fetched with a single WR.

RDMA_READ completions are kept on a separate queue from the general
I/O request queue. Since a separate lock is used to protect the RDMA_READ
completion queue, a race exists between the dto_tasklet and the
svc_rdma_recvfrom thread where the dto_tasklet sets the XPT_DATA
bit and adds I/O to the read-completion queue. Concurrently, the
recvfrom thread checks the generic queue, finds it empty and resets
the XPT_DATA bit. A subsequent svc_xprt_enqueue will fail to enqueue
the transport for I/O and cause the transport to "stall".
The fix is to protect both lists with the same lock and set the XPT_DATA
bit with this lock held.

Add per-device dma_mapping_ops support for CONFIG_X86_64 as POWER
architecture does:
This enables us to cleanly fix the Calgary IOMMU issue that some devices
are not behind the IOMMU (http://lkml.org/lkml/2008/5/8/423).
I think that per-device dma_mapping_ops support would be also helpful for
KVM people to support PCI passthrough but Andi thinks that this makes it
difficult to support the PCI passthrough (see the above thread). So I
CC'ed this to KVM camp. Comments are appreciated.
A pointer to dma_mapping_ops to struct dev_archdata is added. If the
pointer is non NULL, DMA operations in asm/dma-mapping.h use it. If it's
NULL, the system-wide dma_ops pointer is used as before.
If it's useful for KVM people, I plan to implement a mechanism to register
a hook called when a new pci (or dma capable) device is created (it works
with hot plugging). It enables IOMMUs to set up an appropriate
dma_mapping_ops per device.
The major obstacle is that dma_mapping_error doesn't take a pointer to the
device unlike other DMA operations. So x86 can't have dma_mapping_ops per
device. Note all the POWER IOMMUs use the same dma_mapping_error function
so this is not a problem for POWER but x86 IOMMUs use different
dma_mapping_error functions.
The first patch adds the device argument to dma_mapping_error. The patch
is trivial but large since it touches lots of drivers and dma-mapping.h in
all the architecture.
This patch:
dma_mapping_error() doesn't take a pointer to the device unlike other DMA
operations. So we can't have dma_mapping_ops per device.
Note that POWER already has dma_mapping_ops per device but all the POWER
IOMMUs use the same dma_mapping_error function. x86 IOMMUs use device
argument.

When adapters have differing IRD limits, the RDMA transport will fail to
connect properly. The RDMA transport should use the client's advertised
inbound read limit when computing its outbound read limit. For iWARP
transports, there is currently no standard for exchanging IRD/ORD
during connection establishment so the 'responder_resources' field in the
connect event is the local device's limit. The RDMA transport can be
configured to use a smaller ORD by writing the desired number to the

An NFS_WRITE requires a set of RDMA_READ requests to fetch the write
data from the client. There are two principal pieces of data that
need to be tracked: the list of pages that comprise the completed RPC
and the SGE of dma mapped pages to refer to this list of pages. Previously
this whole bit was managed as a linked list of contexts with the
context containing the page list buried in this list. This patch
simplifies this processing by not keeping a linked list, but rather only
a pionter from the last submitted RDMA_READ's context to the context
that maps the set of pages that describe the RPC. This significantly
simplifies this code path. SGE contexts are cleaned up inline in the DTO
path instead of at read completion time.

The svcrdma transport takes a reference when it gets the ESTABLISHED
event from the provider. This reference is supposed to be removed when
the DISCONNECT event is received, however, the call to svc_xprt_put
was missing in the switch statement. This results in the memory
associated with the transport never being freed.

The svc_rdma_send function will attempt to reap SQ WR to make room for
a new request if it finds the SQ full. This function races with the
dto_tasklet that also reaps SQ WR. To avoid polling and arming the CQ
unnecessarily move the test_and_clear_bit of the RDMAXPRT_SQ_PENDING

The svcrdma transport provider currently allocates receive buffers
to the RQ through the xpo_release_rqst method. This approach is overly
complicated since it means that the rqstp rq_xprt_ctxt has to be
selectively set based on whether the RPC is going to be processed
immediately or deferred. Instead, just post the receive buffer when
we are certain that we are replying in the send_reply function.

Add a new IB_WR_SEND_WITH_INV send opcode that can be used to mark a
"send with invalidate" work request as defined in the iWARP verbs and
the InfiniBand base memory management extensions. Also put "imm_data"
and a new "invalidate_rkey" member in a new "ex" union in struct
ib_send_wr. The invalidate_rkey member can be used to pass in an
R_Key/STag to be invalidated. Add this new union to struct
ib_uverbs_send_wr. Add code to copy the invalidate_rkey field in
ib_uverbs_post_send().
Fix up low-level drivers to deal with the change to struct ib_send_wr,
and just remove the imm_data initialization from net/sunrpc/xprtrdma/,
since that code never does any send with immediate operations.
Also, move the existing IB_DEVICE_SEND_W_INV flag to a new bit, since
the iWARP drivers currently in the tree set the bit. The amso1100
driver at least will silently fail to honor the IB_SEND_INVALIDATE bit
if passed in as part of userspace send requests (since it does not
implement kernel bypass work request queueing). Remove the flag from
all existing drivers that set it until we know which ones are OK.
The values chosen for the new flag is not consecutive to avoid clashing
with flags defined in the XRC patches, which are not merged yet but
which are already in use and are likely to be merged soon.
This resurrects a patch sent long ago by Mikkel Hagen <mhagen@iol.unh.edu>.

The RDMACTXT_F_LAST_CTXT bit was getting set incorrectly
when the last chunk in the read-list spanned multiple pages. This
resulted in a kernel panic when the wrong context was used to
build the RPC iovec page list.
RDMA_READ is used to fetch RPC data from the client for
NFS_WRITE requests. A scatter-gather is used to map the
advertised client side buffer to the server-side iovec and
associated page list.
WR contexts are used to convey which scatter-gather entries are
handled by each WR. When the write data is large, a single RPC may
require multiple RDMA_READ requests so the contexts for a single RPC
are chained together in a linked list. The last context in this list
is marked with a bit RDMACTXT_F_LAST_CTXT so that when this WR completes,
the CQ handler code can enqueue the RPC for processing.
The code in rdma_read_xdr was setting this bit on the last two
contexts on this list when the last read-list chunk spanned multiple
pages. This caused the svc_rdma_recvfrom logic to incorrectly build
the RPC and caused the kernel to crash because the second-to-last
context doesn't contain the iovec page list.
Modified the condition that sets this bit so that it correctly detects
the last context for the RPC.

The iWARP protocol limits RDMA read requests to a single scatter
entry. NFS/RDMA has code in rdma_read_max_sge() that is supposed to
limit the sge_count for RDMA read requests to 1, but the code to do
that is inside an #ifdef RDMA_TRANSPORT_IWARP block. In the mainline
kernel at least, RDMA_TRANSPORT_IWARP is an enum and not a
preprocessor #define, so the #ifdef'ed code is never compiled.
In my test of a kernel build with -j8 on an NFS/RDMA mount, this
problem eventually leads to trouble starting with:
svcrdma: Error posting send = -22
svcrdma : RDMA_READ error = -22
and things go downhill from there.
The trivial fix is to delete the #ifdef guard. The check seems to be
a remnant of when the NFS/RDMA code was not merged and needed to
compile against multiple kernel versions, although I don't think it
ever worked as intended. In any case now that the code is upstream
there's no need to test whether the RDMA_TRANSPORT_IWARP constant is
defined or not.
Without this patch, my kernel build on an NFS/RDMA mount using NetEffect
adapters quickly and 100% reproducibly failed with an error like:
ld: final link failed: Software caused connection abort
With the patch applied I was able to complete a kernel build on the
same setup.
(Tom Tucker says this is "actually an _ancient_ remnant when it had to
compile against iWARP vs. non-iWARP enabled OFA trees.")

This file implements the RDMA transport sendto function. A RPC reply
on an RDMA transport consists of some number of RDMA_WRITE requests
followed by an RDMA_SEND request. The sendto function parses the
ONCRPC RDMA reply header to determine how to send the reply back to
the client. The send queue is sized so as to be able to send complete
replies for requests in most cases. In the event that there are not
enough SQ WR slots to reply, e.g. big data, the send will block the
NFSD thread. The I/O callback functions in svc_rdma_transport.c that
reap WR completions wake any waiters blocked on the SQ. In general,
the goal is not to block NFSD threads and the has_wspace method
stall requests when the SQ is nearly full.

This file implements the RDMA transport recvfrom function. The function
dequeues work reqeust completion contexts from an I/O list that it shares
with the I/O tasklet in svc_rdma_transport.c. For ONCRPC RDMA, an RPC may
not be complete when it is received. Instead, the RDMA header that precedes
the RPC message informs the transport where to get the RPC data from on
the client and where to place it in the RPC message before it is delivered
to the server. The svc_rdma_recvfrom function therefore, parses this RDMA
header and issues any necessary RDMA operations to fetch the remainder of
the RPC from the client.
Special handling is required when the request involves an RDMA_READ.
In this case, recvfrom submits the RDMA_READ requests to the underlying
transport driver and then returns 0. When the transport
completes the last RDMA_READ for the request, it enqueues it on a
read completion queue and enqueues the transport. The recvfrom code
favors this queue over the regular DTO queue when satisfying reads.

enqueue receive data indications on a list for the transport. The

NFSD thread context.
