A rather annoying and common case is when booting a PVonHVM guest
and exposing the PV KBD and PV VFB - as broken toolstacks don't
always initialize the backends correctly.
Normally The HVM guest is using the VGA driver and the emulated
keyboard for this (though upstream version of QEMU implements
PV KBD, but still uses a VGA driver). We provide a very basic
two-stage wait mechanism - where we wait for 30 seconds for all
devices, and then for 270 for all them except the two mentioned.
That allows us to wait for the essential devices, like network
or disk for the full 6 minutes.
To trigger this, put this in your guest config:
vfb = [ 'vnc=1, vnclisten=0.0.0.0 ,vncunused=1']
instead of this:
vnc=1
vnclisten="0.0.0.0"

Unfortunately xend creates a bogus console/0 frotend/backend entry pair
on xenstore that console backends cannot properly cope with.
Any guest behavior that is not completely ignoring console/0 is going
to either cause problems with xenconsoled or qemu.
Returning 0 or -ENODEV from xencons_probe is not enough because it is
going to cause the frontend state to become 4 or 6 respectively.
The best possible thing we can do here is just ignore the entry from
xenbus_probe_frontend.

The individual drivers' remove functions could legitimately attempt to
access this information (for logging messages if nothing else). Note
that I did not in fact observe a problem anywhere, but I came across
this while looking into the reasons for what turned out to need the
fix at https://lkml.org/lkml/2012/3/5/336 to vsprintf().

The code which takes requests from userspace already validates against the size
of this buffer so no further checks are required to ensure that userspace

Haogang Chen found out that:
There is a potential integer overflow in process_msg() that could result
in cross-domain attack.
body = kmalloc(msg->hdr.len + 1, GFP_NOIO | __GFP_HIGH);
When a malicious guest passes 0xffffffff in msg->hdr.len, the subsequent
call to xb_read() would write to a zero-length buffer.
The other end of this connection is always the xenstore backend daemon
so there is no guest (malicious or otherwise) which can do this. The
xenstore daemon is a trusted component in the system.
However this seem like a reasonable robustness improvement so we should
have it.
And Ian when read the API docs found that:
The payload length (len field of the header) is limited to 4096
(XENSTORE_PAYLOAD_MAX) in both directions. If a client exceeds the
limit, its xenstored connection will be immediately killed by
xenstored, which is usually catastrophic from the client's point of
view. Clients (particularly domains, which cannot just reconnect)
should avoid this.
so this patch checks against that instead.
This also avoids a potential integer overflow pointed out by Haogang Chen.

The 'name', 'owner', and 'mod_name' members are redundant with the
identically named fields in the 'driver' sub-structure. Rather than
switching each instance to specify these fields explicitly, introduce
a macro to simplify this.
Eliminate further redundancy by allowing the drvname argument to
DEFINE_XENBUS_DRIVER() to be blank (in which case the first entry from
the ID table will be used for .driver.name).
Also eliminate the questionable xenbus_register_{back,front}end()
wrappers - their sole remaining purpose was the checking of the
'owner' field, proper setting of which shouldn't be an issue anymore
when the macro gets used.

For xenbus_{map,unmap}_ring to work on HVM, the grant table operations
must be set up using the gnttab_set_{map,unmap}_op functions instead of
directly populating the fields of gnttab_map_grant_ref. These functions
simply populate the structure on paravirtualized Xen; however, on HVM
they must call __pa() on vaddr when populating op->host_addr because the
hypervisor cannot directly interpret guest-virtual addresses.

Add HVM implementations of xenbus_(map,unmap)_ring_v(alloc,free) so
that ring mappings can be done without using GNTMAP_contains_pte which
is not supported on HVM. This also removes the need to use vmlist_lock
on PV by tracking the allocated xenbus rings.

As when booting the kernel under Amazon EC2 as an HVM guest it ends up
hanging during startup. Reverting this we loose the fix for kexec
booting to the crash kernels.

When mapping a foreign page with xenbus_map_ring_valloc() with the
GNTTABOP_map_grant_ref hypercall, set the GNTMAP_contains_pte flag and
pass a pointer to the PTE (in init_mm).
After the page is mapped, the usual fault mechanism can be used to
update additional MMs. This allows the vmalloc_sync_all() to be
removed from alloc_vm_area().

The xenstore daemon does not have to run in the xen initial domain;
however, Linux currently uses xen_initial_domain to test if a loopback
event channel should be used instead of the event channel provided in
Xen's start_info structure. Instead, if the event channel passed in the
start_info structure is not valid, assume that this domain will run
xenstored locally and set up the event channel.

The xenbus event channel established in xenbus_init is intended to be a
loopback channel, but the remote domain was hardcoded to 0; this will
cause the channel to be unusable when xenstore is not being run in
domain 0.

Add new xs_reset_watches function to shutdown watches from old kernel after
kexec boot. The old kernel does not unregister all watches in the
shutdown path. They are still active, the double registration can not
be detected by the new kernel. When the watches fire, unexpected events
will arrive and the xenwatch thread will crash (jumps to NULL). An
orderly reboot of a hvm guest will destroy the entire guest with all its
resources (including the watches) before it is rebuilt from scratch, so
the missing unregister is not an issue in that case.
With this change the xenstored is instructed to wipe all active watches
for the guest. However, a patch for xenstored is required so that it
accepts the XS_RESET_WATCHES request from a client (see changeset
23839:42a45baf037d in xen-unstable.hg). Without the patch for xenstored
the registration of watches will fail and some features of a PVonHVM
guest are not available. The guest is still able to boot, but repeated
kexec boots will fail.

After triggering a crash dump in a HVM guest, the PV backend drivers
will remain in Connected state. When the kdump kernel starts the PV
drivers will skip such devices. As a result, no root device is found and
the vmcore cant be saved.
A similar situation happens after a kexec boot, here the devices will be
in the Closed state.
With this change all frontend devices with state XenbusStateConnected or
XenbusStateClosed will be reset by changing the state file to Closing ->
Closed -> Initializing. This will trigger a disconnect in the backend
drivers. Now the frontend drivers will find the backend drivers in state
Initwait and can connect.

During repeated kexec boots xenwatch_thread() can crash because
xenbus_watch->callback is cleared by xenbus_watch_path() if a node/token
combo for a new watch happens to match an already registered watch from
an old kernel. In this case xs_watch returns -EEXISTS, then
register_xenbus_watch() does not remove the to-be-registered watch from
the list of active watches but returns the -EEXISTS to the caller
anyway.
Because the watch is still active in xenstored it will cause an event
which will arrive in the new kernel. process_msg() will find the
encapsulated struct xenbus_watch in its list of registered watches and
puts the "empty" watch handle in the queue for xenwatch_thread().
xenwatch_thread() then calls ->callback which was cleared earlier by
xenbus_watch_path().
To prevent that crash in a guest running on an old xen toolstack remove
the special -EEXIST handling.

The xenbus bus type uses device_create_file to assign all used device
attributes. However it does not remove them when the device goes away.
This patch uses the dev_attrs field of the bus type to specify default
attributes for all devices.

Use PM_FREEZE, PM_THAW and PM_RESTORE power events for
suspend/resume/checkpoint functionality, instead of PM_SUSPEND
and PM_RESUME. Use of these pm events fixes the Xen Guest hangup
when taking checkpoints. When a suspend event is cancelled
(while taking checkpoints once/continuously), we use PM_THAW
instead of PM_RESUME. PM_RESTORE is used when suspend is not
cancelled. See Documentation/power/devices.txt and linux/pm.h
for more info about freeze, thaw and restore. The sequence of
pm events in a suspend-resume scenario is shown below.

Register_xenstore_notifier should guarantee that the caller gets
notified even if xenstore is already up.
Therefore we revert "do not notify callers from
register_xenstore_notifier" and set xenstored_read at the right time for
PV on HVM guests too.
In fact in case of PV on HVM guests xenstored is ready only after the
platform pci driver has completed the initialization, so do not set
xenstored_ready before the call to xenbus_probe().
This patch fixes a shutdown_event watcher registration bug that causes
"xm shutdown" not to work properly.

Fixed up pretty trivial conflicts in drivers/block/virtio_blk.c and
drivers/scsi/scsi_error.c as per Jens.
According to the comments, this was how it's been done years ago, but
apparently took an xbt pointer from elsewhere back then. The code was
removed because of consistency issues: cancellation wont't roll back
the saved xbdev->state.
Still, unsolicited writes to the state field remain an issue,
especially if device shutdown takes thread synchronization, and subtle
races cause accidental recreation of the device node.
Fixed by reintroducing the transaction. An internal one is sufficient,
so the xbdev->state value remains consistent.
Also fixes the original hack to prevent infinite recursion. Instead of
bailing out on the first attempt to switch to Closing, checks call
depth now.

Currently register_xenstore_notifier notifies the caller during the
registration itself if xenstore is believed to be ready. This behaviour
causes problems to PV on HVM guests, in which case callers should be
notified by xenbus_probe only after the platform pci driver is loaded.
We already make sure xenbus_probe is called at the right time, calling
it either from device_initcall (PV case) or from the platform pci
driver initialization (HVM case) so we don't need this additional
notification.

Add a xen_emul_unplug command line option to the kernel to unplug
xen emulated disks and nics.
Set the default value of xen_emul_unplug depending on whether or
not the Xen PV frontends and the Xen platform PCI driver have
been compiled for this kernel (modules or built-in are both OK).
The user can specify xen_emul_unplug=ignore to enable PV drivers on HVM
even if the host platform doesn't support unplug.

Add the xen pci platform device driver that is responsible
for initializing the grant table and xenbus in PV on HVM mode.
Few changes to xenbus and grant table are necessary to allow the delayed
initialization in HVM mode.
Grant table needs few additional modifications to work in HVM mode.
The Xen PCI platform device raises an irq every time an event has been
delivered to us. However these interrupts are only delivered to vcpu 0.
The Xen PCI platform interrupt handler calls xen_hvm_evtchn_do_upcall
that is a little wrapper around __xen_evtchn_do_upcall, the traditional
Xen upcall handler, the very same used with traditional PV guests.
When running on HVM the event channel upcall is never called while in
progress because it is a normal Linux irq handler (and we cannot switch
the irq chip wholesale to the Xen PV ones as we are running QEMU and
might have passed in PCI devices), therefore we cannot be sure that
evtchn_upcall_pending is 0 when returning.
For this reason if evtchn_upcall_pending is set by Xen we need to loop
again on the event channels set pending otherwise we might loose some
event channel deliveries.

Since the device we are resuming could be the device containing the
swap device we should ensure that the allocation cannot cause
IO.
On resume, this path is triggered when the running system tries to
continue using its devices. If it cannot then the resume will fail;
to try to avoid this we let it dip into the emergency pools.
The majority of these changes were made when linux-2.6.18-xen.hg
changeset e8b49cfbdac0 was ported upstream in
a144ff09bc but somehow this hunk was
dropped.

percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files. percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.
percpu.h -> slab.h dependency is about to be removed. Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability. As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.
* Scan files for gfp and slab usages and update includes such that
only the necessary includes are there. ie. if only gfp is used,
gfp.h, if slab is used, slab.h.
* When the script inserts a new include, it looks at the include
blocks and try to put the new include such that its order conforms
to its surrounding. It's put in the include block which contains
core kernel includes, in the same order that the rest are ordered -
alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
doesn't seem to be any matching order.
* If the script can't find a place to put a new include (mostly
because the file doesn't have fitting include block), it prints out
an error message indicating which .h file needs to be added to the
file.
The conversion was done in the following steps.
1. The initial automatic conversion of all .c files updated slightly
over 4000 files, deleting around 700 includes and adding ~480 gfp.h
and ~3000 slab.h inclusions. The script emitted errors for ~400
files.
2. Each error was manually checked. Some didn't need the inclusion,
some needed manual addition while adding it to implementation .h or
embedding .c file was more appropriate for others. This step added
inclusions to around 150 files.
3. The script was run again and the output was compared to the edits

4. Several build tests were done and a couple of problems were fixed.
e.g. lib/decompress_*.c used malloc/free() wrappers around slab
APIs requiring slab.h to be added manually.
5. The script was run on all .h files but without automatically
editing them as sprinkling gfp.h and slab.h inclusions around .h
files could easily lead to inclusion dependency hell. Most gfp.h
inclusion directives were ignored as stuff from gfp.h was usually
wildly available and often used in preprocessor macros. Each
slab.h inclusion directive was examined and added manually as
necessary.
6. percpu.h was updated not to include slab.h.
7. Build test were done on the following configurations and failures
were fixed. CONFIG_GCOV_KERNEL was turned off for all tests (as my
distributed build env didn't work with gcov compiles) and a few
more options had to be turned off depending on archs to make things
build (like ipr on powerpc/64 which failed due to missing writeq).
* x86 and x86_64 UP and SMP allmodconfig and a custom test config.
* powerpc and powerpc64 SMP allmodconfig
* sparc and sparc64 SMP allmodconfig
* ia64 SMP allmodconfig
* s390 SMP allmodconfig
* alpha SMP allmodconfig
* um on x86_64 SMP allmodconfig
8. percpu.h modifications were reverted so that it could be applied as
a separate patch and serve as bisection point.
Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

Increases the device timeout from 10s to 5 minutes, giving the user a
visual indication during that time in case there are problems. The patch
is a backport of changesets 144 and 150 in the Xenbits tree.

When printing a warning about a timed-out device, print the
current state of both ends of the device connection (i.e., backend as
well as frontend). This backports half of changeset 146 from the
Xenbits tree.

The logic of is_disconnected_device/exists_disconnected_device is wrong
in that they are used to test whether a device is trying to connect (i.e.
connecting). For this reason the patch fixes them to not consider a
Closing or Closed device to be connecting. At the same time the patch
also renames the functions according to what they really do; you could
say a closed device is "disconnected" (the old name), but not "connecting"
(the new name).
This patch is a backport of changeset 909 from the Xenbits tree.

The xenfs filesystem exports various interfaces to usermode. Initially
this exports a file to allow usermode to interact with xenbus/xenstore.
Traditionally this appeared in /proc/xen. Rather than extending procfs,
this patch adds a backward-compat mountpoint on /proc/xen, and provides
a xenfs filesystem which can be mounted there.

Avoid allocations causing swap activity on the resume path by
preventing the allocations from doing IO and allowing them
to access the emergency pools.
These paths are used when a frontend device is trying to connect
to its backend driver over Xenbus. These reconnections are triggered
on demand by IO, so by definition there is already IO underway,
and further IO would naturally deadlock. On resume, this path
is triggered when the running system tries to continue using its
devices. If it cannot then the resume will fail; to try to avoid this
we let it dip into the emergency pools.

When restoring, rebind the existing xenbus irq to the new xenbus event
channel. (It turns out in practice that this is always the same, and
is never updated on restore. That's a bug, but Xeno-linux has been
like this for a long time, so it can't really be fixed.)

When the xen block frontend driver is built as a module the module load
is only synchronous up to the point where the frontend and the backend
become connected rather than when the disk is added.
This means that there can be a race on boot between loading the module and
loading the dm-* modules and doing the scan for LVM physical volumes (all
in the initrd). In the failure case the disk is not present until after the
scan for physical volumes is complete.

Don't use alloc_vm_area()/free_vm_area() directly, instead define
xen_alloc_vm_area()/xen_free_vm_area() and use them.
alloc_vm_area()/free_vm_area() are used to allocate/free area which
are for grant table mapping. Xen/x86 grant table is based on virtual
address so that alloc_vm_area()/free_vm_area() are suitable.
On the other hand Xen/ia64 (and Xen/powerpc) grant table is based on
pseudo physical address (guest physical address) so that allocation
should be done differently.
The original version of xenified Linux/IA64 have its own
allocate_vm_area()/free_vm_area() definitions which don't allocate vm area
contradictory to those names.
Now vanilla Linux already has its definitions so that it's impossible
to have IA64 definitions of allocate_vm_area()/free_vm_area().
Instead introduce xen_allocate_vm_area()/xen_free_vm_area() and use them.

This communicates with the machine control software via a registry
residing in a controlling virtual machine. This allows dynamic
creation, destruction and modification of virtual device
configurations (network devices, block devices and CPUS, to name some
examples).
