Voltage regulators are optional, so functions must check they exist
before dereferencing.
Note this issue is hidden if CONFIG_REGULATORS is not set, because the
offending code is optimised away.

The issue first appears in commit 57d104c153 ("ufs: add UFS power
management support") but is inadvertently fixed in commit 60f0187031
("scsi: ufs: disable vccq if it's not needed by UFS device") which in
turn was reverted by commit 730679817d ("Revert "scsi: ufs: disable vccq
if it's not needed by UFS device""). So fix applies v3.18 to v4.5 and

If HBA initialization fails unexpectedly (exiting via probe_failed:), we
may fail to free vha->gnl.l. So that we don't attempt to double free, set
this pointer to NULL after a free and check for NULL at probe_failed: so we
know whether or not to call dma_free_coherent.

This patch provides a module parameter and sysfs interface to select
whether the queue depth for each device should be based on the value
suggested by firmware (the default) or the maximum supported by the
controller (can_queue).
Although we have a sysfs interface per sdev to change the queue depth of
individual scsi devices, this implementation provides a single sysfs entry
per shost to switch between the controller max and the value reported by
firmware. The module parameter can provide an interface for one time grub
settings and provides persistent settings across the boot.

Instead of calling qla2x00_fcport_event_handler() and letting the switch
statement inside that function decide which other function to call, call
the latter function directly. Remove the event member from the event_arg
structure because it is no longer needed. Remove the
qla_handle_els_plogi_done() function because it is never called.

The LIO core requires that the target driver callback functions
.queue_data_in() and .queue_status() call target_put_sess_cmd() or
transport_generic_free_cmd(). These calls may happen synchronously or
asynchronously. Make sure that one of these LIO functions is called in case
a command has been aborted. This patch avoids that the code for removing a
session hangs due to commands that do not make progress.

Instead of allocating a struct srb dynamically from inside .queuecommand(),
set qla2xxx_driver_template.cmd_size such that struct scsi_cmnd and struct
srb are contiguous. Do not call QLA_QPAIR_MARK_BUSY() /
QLA_QPAIR_MARK_NOT_BUSY() for SRBs associated with SCSI commands. That is
safe because scsi_remove_host() is called before queue pairs are deleted
and scsi_remove_host() waits for all outstanding SCSI commands to finish.

Since all pointers passed to the srb_t.done() and srb_t.free() functions
have type srb_t, change the type of the first argument of these functions
from void * into struct srb *. This allows the compiler to verify the
argument types for these functions. This patch does not change any
functionality.

Introduce the be_id_t and le_id_t data types for Fibre Channel source and
destination ID formats supported by the firmware instead of using an
uint8_t[3] array. Introduce functions for converting from and to the
port_id_t data types. This patch does not change the behavior of the
qla2xxx driver but improves source code readability and also allows the
compiler to verify the endianness of Fibre Channel IDs.

Such a timeout is a severe issue. Hence complain if waiting for pending
commands times out. This patch fixes a small bug: it modifies
qla82xx_chip_reset_cleanup() such that the "Done waiting" message is
reported if qla82xx_chip_reset_cleanup() succeeded instead of if that
function failed.

I'm not sure how this happened but the patch that was intended to fix abort
handling was incomplete. This patch fixes that patch as follows:
- If aborting the SCSI command failed, wait until the SCSI command
completes.
- Return SUCCESS instead of FAILED if an abort attempt races with SCSI
command completion.
- Since qla2xxx_eh_abort() increments the sp reference count by calling
sp_get(), decrement the sp reference count before returning.

In some cases, like while performing extensive expander reset or phy reset,
user may observe that drives are not visible in OS. Driver's
firmware-worker thread is blocked for more than 120 seconds resulting in a
call trace.
1. Received target add event for Device A and hence driver has registered
this device to SML by calling sas_rphy_add(). SML has half added this
device and returned the control to the driver by quitting from
sas_rphy_add() API, and started some background scanning on this device A.
2. While background scanning is going on device A, driver has received SAS
DEVICE STATUS CHANGE EVENT with RC code "Internal device reset" event and
hence driver has set tm_busy flag for this Device A from FW worker thread
context. When tm_busy flag is set then driver return scsi commands with
device busy status asking the kernel to retry the command after some time.
So background scanning for device A will be waiting for this tm_busy to be
cleared.
3. Meanwhile driver has received a target add event for Device B and hence
driver called sas_rphy_add() API to register this device with SML. But
since background scanning for Device A is still pending and SML is not
quitting from sas_rphy_add(), the driver’s firmware worker thread got
blocked.
4. Now driver has received SAS DEVICE STATUS CHANGE EVENT with RC code
"Internal device reset complete" event. But as driver’s firmware worker
thread got blocked in Step3, it can’t process this event and it was not
clearing the tm_busy flag and deadlock occurred (where SML was waiting for
tm_busy flag to be cleared and our FW worker thread is waiting for SML to
quit from sas_device_rphy_add() API).
Same deadlock will be observed even if device B is getting removed in
step3. So to limit these types of deadlocks driver will process the SAS
DEVICE STATUS CHANGE EVENT events from ISR context instead of processing
this event from worker thread context. This improvement avoids above
deadlock.

This patch is to reduce the performance drop depth observed on SATA HDD
when ATA PT command is outstanding.
Driver returns IO commands with status "SAM_STAT_BUSY" whenever ATA PT
command is outstanding. With this, IO commands will be retried until this
outstanding ATA PT to complete and hence we will observe drop in
performance.
As the driver is completing the subsequent IOs commands with SAM_STAT_BUSY
status, these IOs has to go though the block layer. Hence it adds latency
to the IOs and large performance drop is observed.
So to reduce this performance dropp, added improvement in driver to return
the subsequent IOs with SCSI_MLQUEUE_DEVICE_BUSY status instead of
completing the IOs with SAM_STAT_BUSY status when ATA PT command is
outstanding. Sending command back with SCSI_MLQUEUE_DEVICE_BUSY does not go
through complete block layer stack (as scsi_done won't be called) SML will
immediately retry the command and this method will avoid latency of block
layer stack and the performance impact will be reduced.
On Local setup, ran 512k sequential read IO operation on HGST SATA drive
with existing driver & with this improvement drivers and here is the
result,
1. With existing driver: IOs are running at bandwidth of ~230 rMB/s and
whenever any ATA PT command is outstanding (e.g issued from systemd-udevd
daemon) then this bandwidth drops to ~150 rMB/s.
2. With this improvement driver: IOs are running at bandwidth of ~230 rMB/s
and whenever any ATA PT command is outstanding then this bandwidth drops to
just ~190 rMB/s.

During HBA initialization time, if handshake operation fails due to some
firmware fault then currently driver is terminating the HBA
initialization. It is possible that HBA may come up properly if diag reset
is issued.
So improvement is made in driver in such a way that before terminating the
HBA initialization, driver checks the IOC state and if IOC state is in
fault state then issue diag reset for once. If diag reset is successful
then continue with HBA initialization else terminate the HBA
initialization.

Currently with sysfs parameter "drv_support_bitmap" driver exposes whether
driver supports toolbox memory move command or not.
And application should issue the toolbox memory move command only if driver
tell that memory move tool box command is supported through this sysfs
parameter.
In future we can utilize this sysfs parameter if any new feature is added
and need to notify the same to applications.

Host uses the Memory Move Tool to copy data from any source/destination
combination of system memory and IOC memory.
Memory Move Tool box request contains two SGE fields, First SGE field must
contains the source buffer details described by an MPI Simple SGE. The
second SGE field must contains the destination buffer details described by
an MPI Simple SGE.
Source -> Destination
1. IOC -> IOC (Both the SGE's will be filled by application)
2. HOST -> HOST (Both the SGE's will be filled by the host,
application should give sgl_offset to first SGE offset)
3. IOC -> HOST (Application will fill the first SGE and set the
sgl_offset to second SGE and hence driver fills
the second SGE)
4. HOST -> IOC (Application will fill IOC buffer information in the
first SGE and set the sgl_offset to second SGE.
Then driver will fill the second SGE with Host buffer
information and just before posting the command to the
firmware, driver will swap these two SGEs so that first
SGE contains the HOST buffer information and second SGE
contains the IOC information.
Driver has to take care only of the 4th case, other three cases are by
default supported by the current driver design.

If driver sees the NVMe drive with "DEVICE_BLOCKED" AccessStatus in its
PCIe Device Page0, then driver removes the drive from its internal list and
does not allow any IOCTL commands to be sent to the drive and will return
the IOCTLs with "-ENODEV" status.
The driver will now allow NVMe Encapsulated IOCTL issued to the NVMe device
with an access status of DEVICE_BLOCKED. This change allows the user to
flash new drive firmware online and revive the drive.
Add NVMe device only the driver's internal list even though the device is
in the blocked state so that the device will be visible to Apps. This way
Apps can send NVMe Encapsulated IOCTLs to this drive and bring the drive
online. This NVMe drive with DEVICE_BLOCKED access status won't added to
the SML, it will be added only in the driver's internal list.

SES device of managed PCIe switch will be enumerated same as NVMe drives.
The device info type for this SES device is
MPI26_PCIE_DEVINFO_SCSI (0x4),
whereas the device info type for NVMe drives is
MPI26_PCIE_DEVINFO_NVME (0x3).
Based on this device info type driver determines whether the device is NVMe
drive or a SES device of a managed PCIe switch.
This SES device doesn't have the PCIe device page 2 information like NVMe
drives, so driver won't read PCIe device page 2 information for SES device.
This SES device uses only IEEE SGL's, So driver build's IEEE SGL's whenever
it receives any SCSI commands for this SES device.

Issue:
During online Firmware upgrade operations it is possible that MaxDevHandles
filled in IOCFacts may change with new FW. With this we may observe kernel
panics when driver try to access the pd_handles or blocking_handles buffers
at offset greater than the old firmware's MaxDevHandle value.
Fix:
_base_check_ioc_facts_changes() looks for increase/decrease in IOCFacts
attributes during online firmware upgrade and increases the pd_handles,
blocking_handles, etc buffer sizes to new firmware's MaxDevHandle value if
this new firmware's MaxDevHandle value is greater than the old firmware's
MaxDevHandle value.

Driver gets a request frame from the free pool of DMA-able request frames
and fill in the required information and pass the address of the frame to
IOC/FW to pull the complete request frame. In certain places the driver
used the request frame allocated from the free pool without completely
clearing the previous data stored in it. The request contents were cleared
only for the size of the new request to be issued and that left out some
stale data in the unused part of the request. Though the IOC/FW is not
expected to access the request beyond the specified size, it is good
practice to clear complete request message frame.
So reinitialize the complete request message frame with 0s before using
it.

PCIe Lane margin tool box request requires IEEE sgl's and hence driver
fills the SGL field with IEEE sgl's while issuing the PCIe Lane margin
ioctl request to the HBA firmware.

The SMP frame response is written to the command table and not the SMP
response pointer from libsas, so don't bother DMA mapping (and unmapping)
the SMP response from libsas.

The call to kmap_atomic() in the SMP command completion code is
unnecessary, since kmap() is only really concerned with highmem, which is
not relevant on arm64. The controller only finds itself in arm64 systems.

For a system with PAGE_SIZE of 16K or 64K, the size every time we want to
alloc may be small like 4K, but for function dmam_alloc_coherent(), the
least size it allocates is PAGE_SIZE, so it will waste much memory for the
situation.
To solve the issue, limit the minimum allocation size of slot buf to
PAGE_SIZE.

For struct hisi_sas_status_buffer, it contains struct hisi_sas_err_record
and iu[1024]. The struct iu[1024] will be filled fully by the response of
disks, so it is not need to initialize them to 0, but for the struct
hisi_sas_err_record, SAS controller only fill some fields of
hisi_sas_err_record according to hw designer, so it should be initialised
to 0. After the change, cpu utilization percentage of memset() is changed
from 1.7% to 0.12%.

The value of IOST/ITCT is updated to cache first, and then synchronize to
DDR periodically. So the value in IOST/ITCT cache is the latest data and
it's important for debugging.
So, the HW cache of IOST and ITCT should be snapshot at debugfs.

In commit 1273d65f29 ("scsi: hisi_sas: change queue depth from 512 to
4096"), the depth of each queue is the same as the max IPTT in the system.
As such, as long as we have an IPTT allocated, we will have enough space on
any delivery queue.
All .get_free_slot functions were checking for space on the queue by
reading the DQ read pointer. Drop this, and also raise the code into common
code, as there is nothing hw specific remaining.

There is a small optimisation to be had by making the max IPTT the same for
all hw revisions, that being we can drop the check for read and write
pointer being the same in the get free slot function.
Change v1 hw to have max IPTT of 4096 - same as v2 and v3 hw - and
drop hisi_sas_hw.max_command_entries.

When a configurations runs with a single cpu (such as a kdump kernel),
which causes the driver to request a single vector, when the driver
subsequently requests an irq affinity mask, the mask comes back null. The
driver currently does nothing in this scenario, which leaves mappings to
hardware queues incomplete and crashes the system.
Fix by recognizing the null mask and assigning the vector to the first cpu
in the system.

Configure CDNS_UFS_REG_HCLKDIV in .hce_enable_notify() instead of
.setup_clock() because if UFSHCD resets the controller ip because of phy or
device related errors then CDNS_UFS_REG_HCLKDIV is reset to default value
and .setup_clock() is not called later in the sequence whereas
.hce_enable_notify will be called everytime controller is reenabled.

The data structure used for log messages is so large that it can cause a
boot failure. Since allocations from that data structure can fail anyway,
use kmalloc() / kfree() instead of that data structure.

The only 'new_state' values passed by upstream kernel code to
scsi_internal_device_unblock_nowait() are SDEV_RUNNING and
SDEV_TRANSPORT_OFFLINE. These are the only values that should be passed to
this function. Hence check the value of the 'new_state' argument to avoid
that scsi_internal_device_unblock_nowait() would be used to trigger an
illegal SCSI device state transition. In this context 'illegal' means not
allowed by scsi_device_set_state().

Current driver report dev_loss_tmo to 0 for NVMe devices with short cable
pull. This causes NVMe controller to be freed along with NVMe namespace.
The side affect is IO would stop. By not setting dev_loss_tmo to 0, NVMe
namespace would stay until cable is plugged back in. This allows IO to
resume afterward.

A hang was observed in the fcport delete path when the device was
responding slow and an issue-lip path (results in session termination) was
taken.
Fix this by issuing logo requests unconditionally.

- remote port delete completes
- relogin is scheduled
- "post gidpn" message appears due to rscn generation # mismatch
In short, if a device comes back online sooner than an unregister
completion, a mismatch in rscn generation number occurs, which is not
handled correctly during device relogin. Fix this by starting with a redo
of GNL.
When ql2xextended_error_logging is enabled, the re-plugged device's
discovery stops with the following messages printed:

Current code misses or fails to account for proper recovery during early
initialization failures:
- Properly unwind allocations during probe() failures.
- Protect against non-initialization memory allocations during
unwinding.
- Propagate error status during HW initialization.
- Release SCSI host reference when memory allocations fail.

For any qla2xxx async command, the SRB buffer is used to send it. In
setting up the SRB buffer, the timer for this command is started before all
memory allocation has finished. Under low memory pressure, memory alloc
can go to sleep and not wake up before the timer expires. Once timer has
expired, the timer thread will access uninitialize fields resulting into
NULL pointer crash.
This patch fixes this crash by moving the start of timer after everything
is setup.

Relogin fails to move forward due to scan_state flag indicating device is
not there. Before relogin process, Session delete process accidently
modified the scan_state flag.

Implement .cleanup_rq() callback for freeing driver private part
of the request. Then we can avoid to leak this part if the request isn't
completed by SCSI, and freed by blk-mq or upper layer(such as dm-rq) finally.

This patch implements the zone reset all operation for sd_zbc.c. We add
a new boolean parameter for the sd_zbc_setup_reset_cmd() to indicate
REQ_OP_ZONE_RESET_ALL command setup. Along with that we add support in
the completion path for the zone reset all.

If an abort times out, the Abort IOCB completion and Abort timer can race

We can request task management IOCTL command(MPI2_FUNCTION_SCSI_TASK_MGMT)
to /dev/mpt3ctl. If the given task_type is either abort task or query
task, it may need a field named "Initiator Port Transfer Tag to Manage" in
the IU.
Current code does not support to check target IPTT tag from the tm_request.
This patch introduces to check TaskMID given from the userspace as a target
tag. We have a rule of relationship between

There is a copy and paste bug here. It uses EVENT_TRIGGERS size instead of
SCSI_TRIGGERS size but fortunately both size are 84 bytes so it doesn't
affect runtime.
These days the preferred style is to just say sizeof(object) instead of
sizeof(type) so I have updated the function to the latest style as well.

In qla2x00_alloc_fcport(), fcport is assigned to NULL in the error
handling code on line 4880:

Although SAS3 & SAS3.5 IT HBA controllers support 64-bit DMA addressing, as
per hardware design, if DMA-able range contains all 64-bits
set (0xFFFFFFFF-FFFFFFFF) then it results in a firmware fault.
E.g. SGE's start address is 0xFFFFFFFF-FFFF000 and data length is 0x1000
bytes. when HBA tries to DMA the data at 0xFFFFFFFF-FFFFFFFF location then
HBA will fault the firmware.
Driver will set 63-bit DMA mask to ensure the above address will not be
used.

Commit 1697c6a64c ("scsi: fdomain: Add register definitions") somehow
missed the masking of the 'status' variable with the SCSI phase mask in
fdomain_work(), leaving the magic number intact. Fix this issue; while at
it, change the order of BSTAT_{MSG|CMD|IO} bits in the MESSAGE IN
phase *case* (with no change in the generated object file).

Gcc-9 complains for a memset across pointer boundaries, which happens as
the code tries to allocate a flexible array on the stack. Turns out we
cannot do this without relying on gcc-isms, so with this patch we'll embed
the fc_rport_priv structure into fcoe_rport, can use the normal
'container_of' outcast, and will only have to do a memset over one
structure.

Retrying immediately after we've received a 'transitioning' sense code is
pretty much pointless, we should always use a delay before retrying. So

While loading fw crashdump in function fw_crash_buffer_show(), left bytes

We should only call dma_max_mapping_size for devices that have a DMA mask
set, otherwise we can run into a NULL pointer dereference that will crash
the system.
Also we need to do right shift to get the sectors from the size in bytes,
not a left shift.

This patch is to change msleep() to usleep_range() based on
Documentation/timers/timers-howto.txt. It suggests using usleep_range()
for small msec(1ms - 20ms) since msleep() will often sleep longer than
desired value.
After changing, booting time will be 5ms-10ms faster than before. I
tested this change on two different platforms, one has 5ms faster, another
one is about 10ms. I think this is different on different platform.
Actually, from UFS host side, 1ms-5ms delay is already sufficient for its
initialization of the local UIC layer.

Adding the command and kicking the virtqueue so far was done one after
another. Make the kick optional, so that we can take into account
SCMD_LAST. We also need a commit_rqs callback to kick the device if
blk-mq aborts the submission before the last request is reached.

This allows a list of requests to be issued, with the LLD only writing the
hardware doorbell when necessary, after the last request was prepared.
This is more efficient if we have lists of requests to issue, particularly
on virtualized hardware, where writing the doorbell is more expensive than
on real hardware.
The use case for this is plugged IO, where blk-mq flushes a batch of
requests all at once.
The API is the same as for blk-mq, just with blk-mq concepts tweaked to
fit the SCSI subsystem API: the "last" flag in blk_mq_queue_data becomes a
flag in scsi_cmnd, while the queue_num in the commit_rqs callback is
extracted from the hctx and passed as a parameter.
The only complication is that blk-mq uses different plugging heuristics
depending on whether commit_rqs is present or not. So we have two
different sets of blk_mq_ops and pick one depending on whether the
scsi_host template uses commit_rqs or not.

Currently only "interrupt-based" errors have their own history, however
there are some "non-interrupt-based" errors or events which need history
to improve debugging or help know the health status of UFS devices.
Example of fatal errors:
- Link startup error
- Suspend error
- Resume error
Example of abnormal events:
- Task or request abort
- Device reset (now equals to Logical Unit Reset)
- Host reset
This patch tries to track above errors and events by existed UFS error
history mechanism.

Currently UFS error history will be reset and lost during host reset flow
by ufschd_probe_hba().
Avoid resetting it so error history can be kept as complete as possible to
improve debugging.
In addition, fix a minor display error in ufshcd_print_err_hist().

is adding a virt_boundary_mask to the SCSI host and host template to
parametrise this for NVMe devices instead of having them do a call in
slave_alloc. It's a fairly straightforward conversion except in the
two NVMe handling drivers that didn't set it who now have a virtual
infinity parameter added.

When using a virt_boundary_mask, as done for NVMe devices attached to
megaraid_sas controllers, we require an unlimited max_segment_size as the
virt boundary merging code assumes that. But we also need to propagate
that to the DMA mapping layer to make dma-debug happy. The SCSI layer
takes care of that when using the per-host virt_boundary setting, but
given that megaraid_sas only wants to set the virt_boundary for actual
NVMe devices, we can't rely on that. The DMA layer maximum segment is
global to the HBA however, so we have to set it explicitly. This patch
assumes that megaraid_sas does not have a segment size limitation, which
seems true based on the SGL format, but will need to be verified.

When scsi_init_sense_cache(host) is called concurrently from different
hosts, each code path may find that no cache has been created and
allocate a new one. The lack of locking can lead to potentially
overriding a cache allocated by a different host.
Fix the issue by moving 'mutex_lock(&scsi_sense_cache_mutex)' before
scsi_select_sense_cache().

The RISC-V architecture has a register named the "Supervisor Exception
Program Counter", or "sepc". This abbreviation triggers checkpatch.pl's
misspelling detector, resulting in noise in the checkpatch output. The
risk that this noise could cause more useful warnings to be missed seems
to outweigh the harm of an occasional misspelling of "spec". Thus drop
the "sepc" entry from the misspelling list.

During disk scan and revalidation done with sd_revalidate(), the zones
of a zoned disk are checked using the helper function
blk_revalidate_disk_zones() if a configuration change is detected
(change in the number of zones or zone size). The function
blk_revalidate_disk_zones() issues report_zones calls that are very
large, that is, to obtain zone information for all zones of the disk
with a single command. The size of the report zones command buffer
necessary for such large request generally is lower than the disk
max_hw_sectors and KMALLOC_MAX_SIZE (4MB) and succeeds on boot (no
memory fragmentation), but often fail at run time (e.g. hot-plug
event). This causes the disk revalidation to fail and the disk
capacity to be changed to 0.
This problem can be avoided by using vmalloc() instead of kmalloc() for
the buffer allocation. To limit the amount of memory to be allocated,
this patch also introduces the arbitrary SD_ZBC_REPORT_MAX_ZONES
maximum number of zones to report with a single report zones command.
This limit may be lowered further to satisfy the disk max_hw_sectors
limit. Finally, to ensure that the vmalloc-ed buffer can always be
mapped in a request, the buffer size is further limited to at most
queue_max_segments() pages, allowing successful mapping of the buffer
even in the worst case scenario where none of the buffer pages are
contiguous.

Only GFP_KERNEL and GFP_NOIO are used with blkdev_report_zones(). In
preparation of using vmalloc() for large report buffer and zone array
allocations used by this function, remove its "gfp_t gfp_mask" argument
and rely on the caller context to use memalloc_noio_save/restore() where
necessary (block layer zone revalidation and dm-zoned I/O error path).

Load balancing IO completions across all available MSI-X vectors should be
enabled for Invader and later generation controllers only. This needs to
be disabled for older controllers. Add an adapter type check before
setting msix_load_balance flag.

In megasas_get_target_prop(), driver is incorrectly calculating the target
ID for devices with channel 1 and 3. Due to this, firmware will either
fail the command (if there is no device with the target id sent from
driver) or could return the properties for a target which was not
intended. Devices could end up with the wrong queue depth due to this.
Fix target id calculation for channel 1 and 3.

The lpfc_debug_dump_all_queues() function repeatedly calls into
lpfc_debug_dump_qe() which has a temporary 128 byte buffer. This was fine
before the introduction of CONFIG_GCC_PLUGIN_STRUCTLEAK_VERBOSE because
each instance could occupy the same stack slot. However, now they each get
their own copy, which leads to a huge increase in stack usage as seen from
the compiler warning:

Avoid this by not marking lpfc_debug_dump_qe() as inline so the compiler
can choose to emit a static version of this function when it's needed or
otherwise silently drop it. As an added benefit, not inlining multiple
copies of this function means we save several kilobytes of .text section,
reducing the file size from 47kb to 43.
It is somewhat unusual to have a function that is static but not inline in
a header file, but this does not cause problems here because it is only
used by other inline functions. It would however seem reasonable to move
all the lpfc_debug_dump_* functions into lpfc_debugfs.c and not mark them
inline as a later cleanup.

Disabling the SATA drive interface cause kernel panic. When the drive
Interface is disabled, device should be deregistered after aborting all
pending I/Os. Also changed the port recovery timeout to 10000 ms for
PM8006 controller.

The function sas_wait_eh is declared static and marked EXPORT_SYMBOL, which
is at best an odd combination. Because the function is not used outside of
the drivers/scsi/libsas/sas_scsi_host.c file it is defined in, this commit
removes the EXPORT_SYMBOL() marking.

allocated to make mq more efficient by reducing the size of the
preallocated sg list. This necessitates a large number of driver
changes because the previous guarantee that if a driver specified
SG_ALL as the size of its scatter list, it would get a non-chained
list and didn't need to bother with scatterlist iterators is now
broken and every driver *must* use scatterlist iterators.
This was broken out as a separate topic because we need to convert all
the drivers before pulling the trigger and unconverted drivers kept
being found, necessitating a rebase.

Note on the SPDX license tag conversion conflicts: the SCSI tree had
done its own SPDX conversion, which in some cases conflicted with the
treewide ones done by Thomas & co.
In almost all cases, the conflicts were purely syntactic: the SCSI tree
used the old-style SPDX tags ("GPL-2.0" and "GPL-2.0+") while the
treewide conversion had used the new-style ones ("GPL-2.0-only" and
"GPL-2.0-or-later").
In these cases I picked the new-style one.
In a few cases, the SPDX conversion was actually different, though. As
explained by James above, and in more detail in a pre-pull-request
thread:
"The other problem is actually substantive: In the libsas code Luben
Tuikov originally specified gpl 2.0 only by dint of stating:
* This file is licensed under GPLv2.
In all the libsas files, but then muddied the water by quoting GPLv2
verbatim (which includes the or later than language). So for these
files Christoph did the conversion to v2 only SPDX tags and Thomas
converted to v2 or later tags"
So in those cases, where the spdx tag substantially mattered, I took the
SCSI tree conversion of it, but then also took the opportunity to turn
the old-style "GPL-2.0" into a new-style "GPL-2.0-only" tag.
Similarly, when there were whitespace differences or other differences
to the comments around the copyright notices, I took the version from
the SCSI tree as being the more specific conversion.
Finally, in the spdx conversions that had no conflicts (because the
treewide ones hadn't been done for those files), I just took the SCSI
tree version as-is, even if it was old-style. The old-style conversions
are perfectly valid, even if the "-only" and "-or-later" versions are
perhaps more descriptive.

One of the more common cases of allocation size calculations is finding
the size of a structure that has a zero-sized array at the end, along
with memory for some number of elements for that array. For example:

Make use of the struct_size() helper instead of an open-coded version
in order to avoid any potential type mistakes.
So, replace the following form:

On session deletion, current qla code would unregister an NVMe session
before flushing IOs. This patch would move the unregistration of NVMe
session after IO flush. This way FC-NVMe layer would not have to wait for
stuck IOs. In addition, qla2xxx would stop accepting new IOs during session
deletion.

This patch uses kref to protect access between fcp_abort path and nvme
command and LS command completion path. Stack trace below shows the abort
path is accessing stale memory (nvme_private->sp).
When command kref reaches 0, nvme_private & srb resource will be
disconnected from each other. Any subsequence nvme abort request will not
be able to reference the original srb.

The crash is due to a bad entry in the nvme_rport_list. This list is not
protected, and when a remoteport_delete callback is called, driver
traverses the list and crashes.
Actually, the list could be removed and driver could traverse the main
fcport list instead. Fix does exactly that.

For Aero adapters, driver provides three different performance modes
controlled through module parameter named 'perf_mode'. Below are those
performance modes:
0: Balanced - Additional high IOPS reply queues will be enabled along with
low latency queues. Interrupt coalescing will be enabled only for these
high IOPS reply queues.
1: IOPS - No additional high IOPS queues are enabled. Interrupt coalescing
will be enabled on all reply queues.
2: Latency - No additional high IOPS queues are enabled. Interrupt
coalescing will be disabled on all reply queues. This is a legacy
behavior similar to Ventura & Invader Series.
Default performance mode settings:
- Performance mode set to 'Balanced', if Aero controller is working in
16GT/s PCIe speed.
- Performance mode will be set to 'Latency' mode for all other cases.
Through module parameter 'perf_mode', user can override default performance
mode to desired one.
Captured some performance numbers with these performance modes. 4k Random
Read IO performance numbers on 24 SAS SSD drives for above three
performance modes. Performance data is from Intel Skylake and HGST SS300
(drive model SDLL1DLR400GCCA1).

The driver will use round-robin method for IO submission in batches within
the high IOPS queues when the number of in-flight ios on the target device
is larger than 8. Otherwise the driver will use low latency reply queues.

High iops queues are mapped to non-managed IRQs. Set affinity of
non-managed irqs to local numa node. Low latency queues are mapped to
managed IRQs.
Driver reserves some reply queues for high IOPS queues (through
pci_alloc_irq_vectors_affinity and .pre_vectors interface). The rest of
queues are for low latency.
Based on IO workload, driver will decide which group of reply queues
(either high IOPS queues or low latency queues) to be used.
High IOPS queues will be mapped to local numa node of controller and
low latency queues will be mapped to CPUs across numa nodes. In general,
high IOPS and low latency queues should fit into 128 reply queues
which is the max number of reply queues supported by Aero adapters.

Aero controllers support balanced performance mode through the ability to
configure queues with different properties.
Reply queues with interrupt coalescing enabled are called "high iops reply
queues" and reply queues with interrupt coalescing disabled are called "low
latency reply queues".
The driver configures a combination of high iops and low latency reply
queues if:
- HBA is an AERO controller;
- MSI-X vectors supported by the HBA is 128;
- Total CPU count in the system more than high iops queue count;
- Driver is loaded with default max_msix_vectors module parameter; and
- System booted in non-kdump mode.

For RAID5/RAID6 volumes configured behind Aero, driver will be doing 64bit
division operations on behalf of firmware as controller's ARM CPU is very
slow in this division. Later, driver calculates Q-ARM, P-ARM and Log-ARM and
passes those values to firmware by writing these values to RAID_CONTEXT.

Issue: This issue is applicable to scenario when JBOD sequence map is
unavailable (memory allocation for JBOD sequence map failed) to driver but
feature is supported by firmware. If the driver sends a JBOD IO by not
adding 255 (MAX_PHYSICAL_DEVICES - 1) to device ID when underlying firmware
supports JBOD sequence map, it will lead to the IO failure.
Fix: For JBOD IOs, driver will not use the RAID map to fetch the devhandle
if JBOD sequence map is unavailable. Driver will set Devhandle to 0xffff
and Target ID to 'device ID + 255 (MAX_PHYSICAL_DEVICES - 1)'.

Firmware does not expect FastPath IO sent through Region Lock Bypass queue.
Though firmware never exposes such settings when fastpath IO can be sent to
RL bypass queue but it's safer to remove dead code which directs fastpath
IO to RL Bypass queue.

Issue: Under certain conditions, controller goes in FAULT state after IOC
INIT fired to firmware. Such Fault can be recovered through controller
reset.
Fix: In driver probe context, if firmware fault is observed post IOC INIT,
driver would do controller reset followed by retry logic for IOC INIT
command.

Issue: There is possibility of few DCMDs timing out with 'reset_mutex' lock
held. As part of DCMD timeout handling, driver calls function
megasas_reset_fusion which also tries to acquire same lock 'reset_mutex'
and end up with deadlock.
Fix: Upon timeout of DCMDs (which are fired with 'reset_mutex' lock held),
driver will release 'reset_mutex' before calling OCR function and will
acquire lock again after OCR function returns.

On PowerPC architecture, calling disable_irq_nosync from IRQ context is not
providing the required effect.
In current megaraid_sas driver, disable_irq_nosync is being called from IRQ
context before enabling IRQ poll. But due to the issue seen on PPC, after
IRQ poll disable and legacy ISR is enabled, we are not seeing our ISR
getting called.
Fix: Call disable_irq from IRQ poll thread context instead of IRQ context.

This patch will add support for non-secure Aero adapter PCI IDs. Driver
will throw an error message when a non-secure type controller is
detected. Purpose of this interface is to avoid interacting with any
firmware which is not secured/signed by Broadcom. Any tampering on Firmware
component will be detected by hardware and it will be communicated to the
driver to avoid any further interaction with that component.

Aero adapters provides Atomic Request Descriptor as an alternative method
for posting an entry onto a request queue. The posting of an Atomic Request
Descriptor is an atomic operation, providing a safe mechanism for multiple
processors on the host to post requests without synchronization. This
Atomic Request Descriptor format is identical to first 32 bits of Default
Request Descriptor and uses only 32 bits.
If Aero adapters support Atomic descriptor, driver should use it for
posting IOs and DCMDs to firmware.

Enable msix load balance only when combined reply queue mode is disabled on
the SAS3 and above generation HBA devices.
Earlier msix load balance used to enable if the number of online cpus is
greater than the number of MSI-X vectors enabled on the HBA. Combined reply
queue mode will be disabled only on those HBA which works in shared
resources mode. I.e. on SAS3 HBAs it will be <= 8 and on SAS35 HBA devices
it will be <= 16.
- Before this patch if system has 256 logical CPUs and HBA exposes 128
MSI-X vectors, driver will enable msix load balance.
- After this patch if system has 256 logical CPUs and HBA exposes 128
MSI-X vectors, driver will disable msix load balance.
- After this patch if system has 256 logical CPUs and HBA exposes 16 MSI-X
vectors (due to combined reply queue mode being off in HW), driver will
enable msix load balance.

Even though 'smp_affinity_enable' module parameter is enabled, if the
number of online CPUs is bigger than the number of msix vectors enabled on
that HBA, then smp affinity settings should be disabled only for this HBA.
But currently the smp affinity setting is disabled globally and hence smp
affinity will be disabled for subsequent HBAs even though number of msix
vectors enabled for this HBA matches the number of online CPU.
To fix this, define a per HBA variable smp_affinity_enable. Initially this
variable is initialized with smp_affinity_enable module parameter value. If
this HBA has less number of msix vectors configured when compared to number
of online cpus, then only this HBA's variable smp_affinity_enable is set to
zero.

When enabling high iops queues, the driver should use the HBA's configured
PCIe link speed instead of looking for the maximum link speed.
I.e. enable high iops queues only if Aero/Sea HBA's configured PCIe link
speed is set to 16GT/s.

Currently default perf_mode is set to 'balanced' on Intel architecture
machines and on other machines default perf_mode is set to 'latency' mode.
This CPU architecture check is removed and the default perf_mode mode is
set to 'balanced' mode on all machines.
User can choose the required performance mode using perf_mode module
parameter.

In certain tests where the SCSI error handler issues an abort that is
already outstanding, we will cleanup the command so that the SCSI error

The reason that there was a mismatch is that the SCSI command is actual
returned from the cleanup handler. In previous testing, the type of
cleanup notification we'd get from the CQE did not trigger the code that
returned the SCSI command. To overcome the previous behavior we would put
a reference in bnx2fc_abts_cleanup() to account for the SCSI command.
However, in cases where the SCSI command is actually off, we end up with an
extra put.
The fix for this is to only take the extra put in bnx2fc_abts_cleanup if
the completion for the cleanup times out.

For bnx2fc, the source FCoE MAC is stored in the fcoe_port struct in the
data_src_mac field. Currently this is set in fcoe_ctlr_recv_flogi which
ends up setting it by simply using fc_fcoe_set_mac() which only uses the
default FCF-MAC. We still want to store the source FCoE MAC in
port->data_src_mac but we want to snoop the FLOGI response payload so as to
set it in the following method:
1. If a granted_mac is found, use that.
2. If not granted_mac is there but there is a FCF-MAP from the FCF then
create the MAC from the FCF-MAP and the destination ID from the frame.
3. If there is no FCF-MAP the use the spec. default FCF-MAP and the
destination ID from the frame.

In the case of UPIU/DME request execution failed in UFS device,
ufs_bsg_request() will complete the failed bsg job by calling
bsg_job_done(). Meanwhile, it returns this error status to blk-mq layer,
then triggers blk-mq completing this request again, this will cause the
following panic.

There is an arbitrary difference between the prototypes of
bus_find_device() and class_find_device() preventing their callers
from passing the same pair of data and match() arguments to both of
them, which is the const qualifier used in the prototype of
class_find_device(). If that qualifier is also used in the
bus_find_device() prototype, it will be possible to pass the same
match() callback function to both bus_find_device() and
class_find_device(), which will allow some optimizations to be made in
order to avoid code duplication going forward. Also with that, constify
the "data" parameter as it is passed as a const to the match function.
For this reason, change the prototype of bus_find_device() to match
the prototype of class_find_device() and adjust its callers to use the
const qualifier in accordance with the new prototype of it.
