Do not return SCSI_MLQUEUE_HOST_BUSY, but send DID_ERROR to SCSI mid-layer, if
adapter is in critical error state.  "SCSI_MLQUEUE_HOST_BUSY" will keep same
SCSI command in loop and it is not a right return value, if controller is
hardware critical error.

Bjorn Helgaas 9936906597 [SCSI] megaraid_sas: Use correct #define for MSI-X capability
Previously we used PCI_MSI_FLAGS to locate a register in the MSI-X
capability.  This did work because the MSI and MSI-X flags happen
to be at the same offsets, but was confusing.

PCI_MSIX_FLAGS_ENABLE is already defined in include/uapi/linux/pci_regs.h,
so no need to define it again.

adam radford 9c5ebd09e5 [SCSI] megaraid_sas: Dont load DevHandle unless FastPath enabled
The following patch for megaraid_sas will fix an issue where the
driver should not be loading the DevHandle unless FastPath is enabled.
 If FastPath was not enabled, this means the hardware raid map
validation failed for some reason, or the map was corrupted, which
could mean the DevHandle could be invalid.

Greg Kroah-Hartman 6f03979051 Drivers: scsi: remove __dev* attributes.
CONFIG_HOTPLUG is going away as an option.  As a result, the __dev*
markings need to be removed.

This change removes the use of __devinit, __devexit_p, __devinitdata,
__devinitconst, and __devexit from these drivers.

Based on patches originally written by Bill Pemberton, but redone by me
in order to handle some of the coding style issues better, by hand.

Xiaotian Feng c1d390d8e6 megaraid: fix BUG_ON() from incorrect use of delayed work
megaraid use INIT_WORK to declare a hotplug_work, but cast the
hotplug_work from work_struct to delayed_work and
schedule_delayed_work on it.  This is very dangerous, as other part of
delayed_work might be kernel memories allocated by others.

With commit 8852aac ("workqueue: mod_delayed_work_on() shouldn't queue
timer on 0 delay"), schedule_delayed_work() will check dwork->timer
before queue_work even when @delay is 0, this causes megaraid code to
hit the BUG_ON() in workqueue code.  Change megaraid code to use
delayed work.

Kashyap Desai bd8d6dd43a [SCSI] megaraid_sas: Move poll_aen_lock initializer
The following patch moves the poll_aen_lock initializer from
megasas_probe_one() to megasas_init().  This prevents a crash when a user
loads the driver and tries to issue a poll() system call on the ioctl
interface with no adapters present.

logic to use fpRead/WriteCapable, fpRead/WriteAcrossStripe flags
instead of the old logic.  This fixes a bug where fastpath writes
could be sent to a read only LD.

Paul Gortmaker acf3368ffb scsi: Fix up files implicitly depending on module.h inclusion
The module.h header was implicitly present everywhere, so files
with no explicit include of the module infrastructure would build
anyway.  We are now removing the implicit include, and so we need
to call out the module.h file that we need explicitly.

adam radford f575c5d3eb [SCSI] megaraid_sas: Fix instance access in megasas_reset_timer
The following patch for megaraid_sas will fix a potential bad pointer access
in megasas_reset_timer(), when a MegaRAID 9265/9285 or 9360/9380 gets a
timeout.  megasas_build_io_fusion() sets SCp.ptr to be a struct
megasas_cmd_fusion *, but then megasas_reset_timer() was casting SCp.ptr to be
a struct megasas_cmd *, then trying to access cmd->instance, which is invalid.

Just loading instance from scmd->device->host->hostdata in
megasas_reset_timer() fixes the issue.

Paul Bolle 395cf9691d doc: fix broken references
There are numerous broken references to Documentation files (in other
Documentation files, in comments, etc.). These broken references are
caused by typo's in the references, and by renames or removals of the
Documentation files. Some broken references are simply odd.

Fix these broken references, sometimes by dropping the irrelevant text

Jeff Garzik f281233d3e SCSI host lock push-down
Move the mid-layer's ->queuecommand() invocation from being locked
with the host lock to being unlocked to facilitate speeding up the
critical path for drivers who don't need this lock taken anyway.

The patch below presents a simple SCSI host lock push-down as an
equivalent transformation.  No locking or other behavior should change
with this patch.  All existing bugs and locking orders are preserved.

Additionally, add one parameter to queuecommand,
	struct Scsi_Host *
and remove one parameter from queuecommand,
	void (*done)(struct scsi_cmnd *)

Scsi_Host* is a convenient pointer that most host drivers need anyway,
and 'done' is redundant to struct scsi_cmnd->scsi_done.

Minimal code disturbance was attempted with this change.  Most drivers
needed only two one-line modifications for their host lock push-down.

If fw didn't raise the interrupt with the fw state change to driver
and fw goes to failure state, driver Will check the FW state in
driver's timeout routine and issue the reset if need.  Driver will do
the OCR upto three times until kill adapter.  Also driver will issue
OCR before driver kill adapter even if fw in operational state.

Arnd Bergmann 6038f373a3 llseek: automatically add .llseek fop
All file_operations should get a .llseek operation so we can make
nonseekable_open the default for future file operations without a
.llseek pointer.

The three cases that we can automatically detect are no_llseek, seq_lseek
and default_llseek. For cases where we can we can automatically prove that
the file offset is always ignored, we use noop_llseek, which maintains
the current behavior of not returning an error from a seek.

New drivers should normally not use noop_llseek but instead use no_llseek
and call nonseekable_open at open time.  Existing drivers can be converted
to do the same when the maintainer knows for certain that no user code
relies on calling seek on the device file.

The generated code is often incorrectly indented and right now contains
comments that clarify for each added line why a specific variant was
chosen. In the version that gets submitted upstream, the comments will
be gone and I will manually fix the indentation, because there does not
seem to be a way to do that using coccinelle.

Some amount of new code is currently sitting in linux-next that should get
the same modifications, which I will do at the end of the merge window.

Many thanks to Julia Lawall for helping me learn to write a semantic
patch that does all this.

bo yang 39a985547c [SCSI] megaraid_sas: Add Online Controller Reset to MegaRAID SAS drive
To add the Online controller reset support, driver need to do:
a). reset the controller chips -- Xscale and Gen2 which will change
the function calls and add the reset function related to this two
chips.
b). during the reset, driver will store the pending cmds which not
returned by FW to driver's pending queue.  Driver will re-issue those
pending cmds again to FW after the OCR finished.
c). In driver's timeout routine, driver will report to OS as reset.
Also driver's queue routine will block the cmds until the OCR
finished.
d). in Driver's ISR routine, if driver get the FW state as state
change, FW in Failure status and FW support online controller
reset (OCR), driver will start to do the controller reset.
e). In driver's IOCTL routine, the application cmds will wait for the
OCR to finish, then issue the cmds to FW.

Arnd Bergmann c45d15d24e scsi: autoconvert trivial BKL users to private mutex
All these files use the big kernel lock in a trivial
way to serialize their private file operations,
typically resulting from an earlier semi-automatic
pushdown from VFS.

None of these drivers appears to want to lock against
other code, and they all use the BKL as the top-level
lock in their file operations, meaning that there
is no lock-order inversion problem.

Consequently, we can remove the BKL completely,
replacing it with a per-file mutex in every case.
Using a scripted approach means we can avoid
typos.

Tejun Heo 5a0e3ad6af include cleanup: Update gfp.h and slab.h includes to prepare for breaking implicit slab.h inclusion from percpu.h
percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files.  percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.

percpu.h -> slab.h dependency is about to be removed.  Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability.  As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.

* Scan files for gfp and slab usages and update includes such that
  only the necessary includes are there.  ie. if only gfp is used,
  gfp.h, if slab is used, slab.h.

* When the script inserts a new include, it looks at the include
  blocks and try to put the new include such that its order conforms
  to its surrounding.  It's put in the include block which contains
  core kernel includes, in the same order that the rest are ordered -
  alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
  doesn't seem to be any matching order.

* If the script can't find a place to put a new include (mostly
  because the file doesn't have fitting include block), it prints out
  an error message indicating which .h file needs to be added to the
  file.

The conversion was done in the following steps.

1. The initial automatic conversion of all .c files updated slightly
   over 4000 files, deleting around 700 includes and adding ~480 gfp.h
   and ~3000 slab.h inclusions.  The script emitted errors for ~400
   files.

2. Each error was manually checked.  Some didn't need the inclusion,
   some needed manual addition while adding it to implementation .h or
   embedding .c file was more appropriate for others.  This step added
   inclusions to around 150 files.

3. The script was run again and the output was compared to the edits
   from #2 to make sure no file was left behind.

4. Several build tests were done and a couple of problems were fixed.
   e.g. lib/decompress_*.c used malloc/free() wrappers around slab
   APIs requiring slab.h to be added manually.

5. The script was run on all .h files but without automatically
   editing them as sprinkling gfp.h and slab.h inclusions around .h
   files could easily lead to inclusion dependency hell.  Most gfp.h
   inclusion directives were ignored as stuff from gfp.h was usually
   wildly available and often used in preprocessor macros.  Each
   slab.h inclusion directive was examined and added manually as
   necessary.

6. percpu.h was updated not to include slab.h.

7. Build test were done on the following configurations and failures
   were fixed.  CONFIG_GCOV_KERNEL was turned off for all tests (as my
   distributed build env didn't work with gcov compiles) and a few
   more options had to be turned off depending on archs to make things
   build (like ipr on powerpc/64 which failed due to missing writeq).

   * x86 and x86_64 UP and SMP allmodconfig and a custom test config.
   * powerpc and powerpc64 SMP allmodconfig
   * sparc and sparc64 SMP allmodconfig
   * ia64 SMP allmodconfig
   * s390 SMP allmodconfig
   * alpha SMP allmodconfig
   * um on x86_64 SMP allmodconfig

8. percpu.h modifications were reverted so that it could be applied as
   a separate patch and serve as bisection point.

Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

Noriyuki Fujii aeab3fd7b8 [SCSI] megaraid_sas: make driver PCI legacy I/O port free driver
On the large servers, I/O port resource may not be assigned to all
the PCI devices since it is limited (to 64KB on Intel Architecture[1])
and it may also be fragmented (I/O base register of PCI-to-PCI bridge
will usually be aligned to a 4KB boundary[2]).
If no I/O port resource is assigned to devices, those devices do not
work.

[1] Some machines support 64KB I/O port space per PCI segment.
[2] Some P2P bridges support optional 1KB aligned I/O base.

Therefore, I made a patch for MegaRAID SAS driver to make PCI legacy
I/O port free.  I have also tested the patch and it had no problem.

The way to make PCI legacy I/O port free is the same as Fusion-MPT
driver's and it has been merged into 2.6.30.4.

This has already been fixed in e1000 and lpfc.

As a result of the above, the driver can handle its device even when
there are a huge number of PCI devices being used on the system and no
I/O port region assigned to the device.

Mike Christie e881a172da [SCSI] modify change_queue_depth to take in reason why it is being called
This patch modifies scsi_host_template->change_queue_depth so that
it takes an argument indicating why it is being called. This will be
used so that if a LLD needs to do some extra processing when
handling queue fulls or later ramp ups, it can do so.

This is a simple port of the drivers setting a change_queue_depth
callback. In the patch I just have these LLDs adjust the queue depth
if the user was requesting it.

Driver will update the OS devices after adding and deleting the device
from FW.  When driver receive add or delete AEN from FW, driver will
send the DCMD cmd to get the System PD list from FW.  Then driver will
check if this device already in the OS: If add event and OS don't have
the device (but it is in the list), driver add the device to OS,
otherwise driver will not add.  If remove event, driver will check the
list, if is not in the list, but OS have the device, driver will
remove the device.

Al Viro 233e70f422 saner FASYNC handling on file close
As it is, all instances of ->release() for files that have ->fasync()
need to remember to evict file from fasync lists; forgetting that
creates a hole and we actually have a bunch that *does* forget.

So let's keep our lives simple - let __fput() check FASYNC in
file->f_flags and call ->fasync() there if it's been set.  And lose that
crap in ->release() instances - leaving it there is still valid, but we
don't have to bother anymore.

Jens Axboe 242f9dcb8b block: unify request timeout handling
Right now SCSI and others do their own command timeout handling.
Move those bits to the block layer.

Instead of having a timer per command, we try to be a bit more clever
and simply have one per-queue. This avoids the overhead of having to
tear down and setup a timer for each command, so it will result in a lot
less timer fiddling.

Joe Malicki 66dca9b8c5 [SCSI] megaraid_sas: remove sysfs dbg_lvl world writeable permissions
/sys/bus/pci/drivers/megaraid_sas/dbg_lvl defaults to being
world-writable, which seems bad (letting any user affect kernel driver
behavior and logging level).

This turns off group and user write permissions, so that on typical
production systems only root can write to it.

Hannes Reinecke 69cd39e946 [SCSI] megaraid_mbox: fix Dell CERC firmware problem
Newer Dell CERC firmware (>= 6.62) implement a random deletion handling
compatible with the legacy megaraid driver.  The legacy handling shifted
the target ID by 0x80 only for I/O commands (READ/WRITE/etc), whereas
megaraid_mbox shifts the target ID always if random deletion is supported.
The resulted in megaraid_mbox sending an INQUIRY to the wrong channel, and
not finding any devices, obviously.

So we disable the random deletion support if the offending firmware is
found.

Thomas Horsten 90a95af85f [SCSI] MegaRAID driver management char device moved to misc
The MegaRAID driver's common management module (megaraid_mm.c) creates a
char device used by the management tool "megarc" from LSI Logic (and
possibly other management tools).

In 2.6 with udev, this device doesn't get created because it is not
registered in sysfs.

I first fixed this by registering a class "megaraid_mm", but realized that
this should probably be moved to misc devices, instead of taking up a char
major.  This is because only 1 device is used, even if there are multiple
adapters - the minor is never used (the adapter info is in the ioctl block
sent to the driver, not detected based on the minor number as one might
think).  So it is a complete waste to have an entire major taken by this.

So it now uses a misc device which I named "megadev0" (the name that megarc
expects), and has a dynamic minor (previoulsy a dynamic major was used).

I have tested this on my own system with the megarc tool, and it works just
as fine as before (only now the device gets created correctly by udev).

James Bottomley d3f46f39b7 [SCSI] remove use_sg_chaining
With the sg table code, every SCSI driver is now either chain capable
or broken (or has sg_tablesize set so chaining is never activated), so
there's no need to have a check in the host template.

Also tidy up the code by moving the scatterlist size defines into the
SCSI includes and permit the last entry of the scatterlist pools not
to be a power of two.

Added module parameter "poll_mode_io" to support for "polling"
(reduced interrupt operation).  In this mode, IO completion interrupts
are delayed. At the end of initiating IOs, the driver schedules for
cmd completion if there are pending cmds.  A timer-based interrupt has
also been added to prevent IO completion from being delayed
indefinitely in the case that no new IOs are initiated.  Some
formatting issues in resume, suspend comment block also corrected

1. Setting the max_sectors_per_req based on max SGL supported by the
   FW. Prior versions calculated this value from controller info's
   max_sectors_1, max_sectors_2. For certain controllers/FW, this was
   resulting in a value greater than max SGL supported by the FW. Now
   we take the min of max sgl from FW and max_sectors calculation.

2. Increased MFI_POLL_TIMEOUT_SECS to 60 seconds from 10. FW may take
   a max of 60 seconds to respond to the INIT cmd.

The Megaraid Mailbox driver uses a semaphore as mutex.  Use the mutex API
instead of the (binary) semaphore.

eh_timed_out call back (megasas_reset_timer) is used to throttle io
to the adapter when it is called the first time for a scmd.
The MEGASAS_FW_BUSY flag is set and can_queue reduced to 16.
The can_queue is restored from completion routine in following
two conditions : 5 seconds has elapsed and
the # of outstanding cmds in FW is < 17.

Alexey Dobriyan e8edc6e03a Detach sched.h from mm.h
First thing mm.h does is including sched.h solely for can_do_mlock() inline
function which has "current" dereference inside. By dealing with can_do_mlock()
mm.h can be detached from sched.h which is good. See below, why.

This patch
a) removes unconditional inclusion of sched.h from mm.h
b) makes can_do_mlock() normal function in mm/mlock.c
c) exports can_do_mlock() to not break compilation
d) adds sched.h inclusions back to files that were getting it indirectly.
e) adds less bloated headers to some files (asm/signal.h, jiffies.h) that were
   getting them indirectly

Net result is:
a) mm.h users would get less code to open, read, preprocess, parse, ... if
   they don't need sched.h
b) sched.h stops being dependency for significant number of files:
   on x86_64 allmodconfig touching sched.h results in recompile of 4083 files,
   after patch it's only 3744 (-8.3%).

Cross-compile tested on

1.      Changes in Initialization to fix kdump failure.
        Send SYNC command on loading.
        This command clears the pending commands in the adapter
        and re-initialize its internal RAID structure.
        Without this change, megaraid driver either panics or fails to
        initialize the adapter during kdump's second kernel boot
        if there are pending commands or interrupts from other devices
        sharing the same IRQ.

David Howells 7d12e780e0 IRQ: Maintain regs pointer globally rather than passing to IRQ handlers
Maintain a per-CPU global "struct pt_regs *" variable which can be used instead
of passing regs around manually through all ~1800 interrupt handlers in the
Linux kernel.

The regs pointer is used in few places, but it potentially costs both stack
space and code to pass it around.  On the FRV arch, removing the regs parameter
from all the genirq function results in a 20% speed up of the IRQ exit path
(ie: from leaving timer_interrupt() to leaving do_IRQ()).

Where appropriate, an arch may override the generic storage facility and do
something different with the variable.  On FRV, for instance, the address is
maintained in GR28 at all times inside the kernel as part of general exception
handling.

Having looked over the code, it appears that the parameter may be handed down
through up to twenty or so layers of functions.  Consider a USB character
device attached to a USB hub, attached to a USB controller that posts its
interrupts through a cascaded auxiliary interrupt controller.  A character
device driver may want to pass regs to the sysrq handler through the input
layer which adds another few layers of parameter passing.

I've build this code with allyesconfig for x86_64 and i386.  I've runtested the
main part of the code on FRV and i386, though I can't test most of the drivers.
I've also done partial conversion for powerpc and MIPS - these at least compile
with minimal configurations.

This will affect all archs.  Mostly the changes should be relatively easy.
Take do_IRQ(), store the regs pointer at the beginning, saving the old one:

	struct pt_regs *old_regs = set_irq_regs(regs);

And put the old one back at the end:

	set_irq_regs(old_regs);

Don't pass regs through to generic_handle_irq() or __do_IRQ().

In timer_interrupt(), this sort of change will be necessary:

	-	update_process_times(user_mode(regs));
	-	profile_tick(CPU_PROFILING, regs);
	+	update_process_times(user_mode(get_irq_regs()));
	+	profile_tick(CPU_PROFILING);

I'd like to move update_process_times()'s use of get_irq_regs() into itself,
except that i386, alone of the archs, uses something other than user_mode().

Some notes on the interrupt handling in the drivers:

 (*) input_dev() is now gone entirely.  The regs pointer is no longer stored in
     the input_dev struct.

 (*) finish_unlinks() in drivers/usb/host/ohci-q.c needs checking.  It does
     something different depending on whether it's been supplied with a regs
     pointer or not.

 (*) Various IRQ handler function pointers have been moved to type
     irq_handler_t.

Eric W. Biederman f5ebbeb57e [SCSI] megaraid: Use the proper type to hold the irq number.
When testing on a Unisys machine it was discovered that the megaraid driver
would not initialize as it was requesting irq 162 instead of irq 1442 it
was assigned.  The problem was the irq number had been truncated by being
stored in an unsigned char.

This patches fixes that problem and the driver now appears to work.

The ioctl interface appears fundamentally broken as it exports the irq
number to user space in an unsigned char.

With patch, the driver access PCIconfiguration space with dedicated
offset to read a signature. If the signature read, it means that the
controller has capability to handle 64-bit DMA.
Without this patch, the driver used to blindly claim 64-bit DMA
capability.

The patch adds support for a ZCR controller (Device ID : 0x413).

It also has a critical bug fix :

Disable controller interrupt before firing INIT cmd to FW.  Interrupt

When abort failed, the driver gets reset handleer called.  In the reset
handler, driver calls 'scsi_done()' callback for same SCSI command packet
(struct scsi_cmnd) multiple times if there are multiple SCSI command packet
in the pend_list.  More over, if there are entry in the pend_lsit with
IOCTL packet associated, the driver returns it to wrong free_list so that,
in turn, the driver could end up with 'NULL pointer dereference..' during
I/O command building with incorrect resource.

Sumant Patro 1341c93922 [SCSI] megaraid_sas: new template defined to represent each type of controllers
This patch defines a new template to represent each type of
controllers (identified by the processor used). The template has
members that is set with appropriate values during driver
initialisation. This change is done to support new controllers with
minimal change to existing code. In future, for a new controller
support, a template will be declared and its members initialised
appropriately.

Olaf Hering 733482e445 [PATCH] changing CONFIG_LOCALVERSION rebuilds too much, for no good reason
This patch removes almost all inclusions of linux/version.h.  The 3
#defines are unused in most of the touched files.

A few drivers use the simple KERNEL_VERSION(a,b,c) macro, which is
unfortunatly in linux/version.h.

There are also lots of #ifdef for long obsolete kernels, this was not
touched.  In a few places, the linux/version.h include was move to where
the LINUX_VERSION_CODE was used.

recent kernels call the eh_ methods without the host lock held.
megaraid_sas doesn't need it but drops it before calling a sleeping
routine and reqcquires it afterwards.  Just remove the
spin_unlock/spin_lock calls.

Christoph Hellwig 12413197ee [SCSI] remove scsi_set_device
scsi_add_host is the proper place to set the device, but people copy
the scsi_set_device usage from older drivers again and again.

note that this leaves some legacy drivers like qlogicisp/qlogicfc
without pci association in sysfs, but they're scheduled to go away soon
anyway.

Linus Torvalds 1da177e4c3 Linux-2.6.12-rc2
Initial git repository build. I'm not bothering with the full history,
even though we have it. We can create a separate "historical" git
archive of that later if we want to, and in the meantime it's about
3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.

Let it rip!
