Masahiro Yamada 550116d21a scripts/spelling.txt: add "aligment" pattern and fix typo instances
Fix typos and add the following to the scripts/spelling.txt:

  aligment||alignment

I did not touch the "N_BYTE_ALIGMENT" macro in
drivers/net/wireless/realtek/rtlwifi/wifi.h to avoid unpredictable
impact.

I fixed "_aligment_handler" in arch/openrisc/kernel/entry.S because
it is surrounded by #if 0 ... #endif.  It is surely safe and I
confirmed "_alignment_handler" is correct.

I also fixed the "controler" I found in the same hunk in
arch/openrisc/kernel/head.S.

Christoph Hellwig b6a05c823f scsi: remove eh_timed_out methods in the transport template
Instead define the timeout behavior purely based on the host_template
eh_timed_out method and wire up the existing transport implementations
in the host templates.  This also clears up the confusion that the
transport template method overrides the host template one, so some
drivers have to re-override the transport template one.

Johannes Thumshirn 4633773799 scsi: bfa: fix wrongly initialized variable in bfad_im_bsg_els_ct_request()
Commit 01e0e15c8b ("scsi: don't use fc_bsg_job::request and
fc_bsg_job::reply directly") introduced a typo, which causes that the
bsg_request variable in bfad_im_bsg_els_ct_request() is initialized to
itself instead of pointing to the bsg job's request.

Christoph Hellwig 984dc46c57 scsi: bfa: remove bfa_fcs_mod_s
Just call the functions directly instead of obsfucating the call chain.
This was in reply to a patch from Kees Cook to constify the function
pointer struct bfa_fcs_mod_s, but it turns out there is no reason to
have this indirection at all.

bna & bfa firmware version 3.2.5.1 was submitted to linux-firmware on
Feb 17 19:10:20 2015 -0500 in 0ab54ff1dc ("linux-firmware: Add QLogic BR
Series Adapter Firmware").

bna was updated to use the newer firmware on Feb 19 16:02:32 2015 -0500 in
3f307c3d70 ("bna: Update the Driver and Firmware Version")

bfa was not updated. I presume this was an oversight but it broke support
for bfa+bna cards such as the following

Johannes Thumshirn 1abaede715 scsi: fc: Export fc_bsg_jobdone and use it in FC drivers
Export fc_bsg_jobdone so drivers can use it directly instead of doing
the round-trip via struct fc_bsg_job::job_done() and use it in the
LLDDs.  That way we can also unify the interfaces of fc_bsg_jobdone and
bsg_job_done.

As we've converted all LLDDs over to use fc_bsg_jobdone() directly, we
can remove the function pointer from struct fc_bsg_job as well.

Colin Ian King 5f6ac2cee2 scsi: bfa: Do not dereference port before it is null checked
Port is deferenced before it is null sanity checked, hence we
potentially have a null pointer dereference bug. Instead, initialise
trl_enabled from port->fcs->bfa after we are sure port is not null.

Dan Carpenter 4e51af9ee7 bfa: fix bfa_fcb_itnim_alloc() error handling
The caller assumes that "itnim" is NULL on error and non-NULL on success
but really "itnim" is uninitialized on error.  This function should just
use normal error handling where it returns zero on success and negative
on failure.

Denys Vlasenko d04a78f43a bfa: deinline __bfa_trc() and __bfa_trc32()
__bfa_trc() compiles to 115 bytes of machine code.
With this .config: http://busybox.net/~vda/kernel_config
there are 1494 calls of __bfa_trc().

__bfa_trc32() is very similar, so it is uninlined too.
However, it appears to be unused, therefore this patch
ifdefs it out.

Change in code size is about 130,000 bytes:

Dan Carpenter aee5618dfe bfa: use strncpy() instead of memcpy()
BFA_MFG_NAME is "QLogic" which is only 7 bytes, but we are copying 8
bytes.  It's harmless because the badding byte is likely zero but it
makes static checkers complain.

Anil Gurumurthy 3c3da12d31 bfa: Fix for crash when bfa_itnim is NULL
Fix a very corner case when the port gets disconnected and the BFA and
FCS layers clean up references to the IT nexus.  During this window if a
task management command is issued by the SCSI-ML and ends up referencing
a NULL itnim, it could lead to a crash.

Christoph Hellwig 64d513ac31 scsi: use host wide tags by default
This patch changes the !blk-mq path to the same defaults as the blk-mq
I/O path by always enabling block tagging, and always using host wide
tags.  We've had blk-mq available for a few releases so bugs with
this mode should have been ironed out, and this ensures we get better
coverage of over tagging setup over different configs.

Christoph Hellwig db5ed4dfd5 scsi: drop reason argument from ->change_queue_depth
Drop the now unused reason argument from the ->change_queue_depth method.
Also add a return value to scsi_adjust_queue_depth, and rename it to
scsi_change_queue_depth now that it can be used as the default

Christoph Hellwig c8b09f6fb6 scsi: don't set tagging state from scsi_adjust_queue_depth
Remove the tagged argument from scsi_adjust_queue_depth, and just let it
handle the queue depth.  For most drivers those two are fairly separate,
given that most modern drivers don't care about the SCSI "tagged" status
of a command at all, and many old drivers allow queuing of multiple
untagged commands in the driver.

Instead we start out with the ->simple_tags flag set before calling
->slave_configure, which is how all drivers actually looking at
->simple_tags except for one worke anyway.  The one other case looks
broken, but I've kept the behavior as-is for now.

Except for that we only change ->simple_tags from the ->change_queue_type,
and when rejecting a tag message in a single driver, so keeping this
churn out of scsi_adjust_queue_depth is a clear win.

Now that the usage of scsi_adjust_queue_depth is more obvious we can
also remove all the trivial instances in ->slave_alloc or ->slave_configure
that just set it to the cmd_per_lun default.

Christoph Hellwig 2ecb204d07 scsi: always assign block layer tags if enabled
Allow a driver to ask for block layer tags by setting .use_blk_tags in the
host template, in which case it will always see a valid value in
request->tag, similar to the behavior when using blk-mq.  This means even
SCSI "untagged" commands will now have a tag, which is especially useful
when using a host-wide tag map.

Alexey Khoroshilov dc6d2a0f97 bfa: allocate memory with GFP_ATOMIC in spinlock context
bfa_fcb_pbc_vport_create() is called only from bfa_fcs_pbc_vport_init(),
that is called only from bfad_drv_start() with bfad_lock spinlock held.
So the patch replaces GFP_KERNEL with GFP_ATOMIC to avoid
sleeping in atomic spinlock context.

Found by Linux Driver Verification project (linuxtesting.org).

In direct attach mode, if port features like QoS/Trunk/FEC/CR are
enabled, driver can receive PLOGI from the target before firmware

dropping it on the floor.

This patch includes change to enable firmware patch simplication
feature. This feature is targeted to address the requirement to have
independent patch release for firmware. Prior to 3.2.3, releasing a
patch fix for firmware requires changes to bfa, to use new firmware
images. But with these changes, if the new firmware is flashed on to the
HBA with brocade adapter management utilites, driver uses the new
firmware after checking the patch release byte in the firmware version.

Vijaya Mohan Guvva dcaf9aed99 [SCSI] bfa: Chinook quad port 16G FC HBA claim issue
Bfa driver crash is observed while pushing the firmware on to chinook
quad port card due to uninitialized bfi_image_ct2 access which gets
initialized only for CT2 ASIC based cards after request_firmware().
For quard port chinook (CT2 ASIC based), bfi_image_ct2 is not getting
initialized as there is no check for chinook PCI device ID before
request_firmware and instead bfi_image_cb is initialized as it is the
default case for card type check.

This patch includes changes to read the right firmware for quad port chinook.

Jingoo Han 08b7e10716 SCSI: remove unnecessary pci_set_drvdata()
Since commit 0998d06310
(device-core: Ensure drvdata = NULL when no driver is bound),
the driver core clears the driver data to NULL after device_release
or on probe failure. Thus, it is not needed to manually clear the

Vijaya Mohan Guvva c679b599af [SCSI] bfa: kdump fix on 815 and 825 adapters
Root cause: When kernel crashes, On brocade 815/825 adapters,
 bfa IOC state machine and FW doesn't get a notification and
hence are not cleanly shutdown. So registers holding driver/IOC
state information are not reset back to valid disabled/parking
values. This causes subsequent driver initialization to fail
during kdump kernel boot.

Fix description: during the initialization of first PCI function, reset
corresponding register when unclean shutown is detect by reading chip
registers. This will make sure that ioc/fw gets clean re-initialization.

Vijaya Mohan Guvva e1aaab89de [SCSI] bfa: Fix 1860 port initialize when ATC is enabled
On Xen kernels, if ATC (address translation cache) is enabled, the
first PCIe DMA read from the adapter fails with an error. This is due to
a bug ASIC, which leads to a failure of 1860 ports to be initialised.

This patch includes the fix to disable Invalidated Tag Match Enable
capability by setting the bit 26 of CHIP_MISC_PRG to 0, by default it is
set to 1.

Vijaya Mohan Guvva 1a898a794d [SCSI] bfa: Add dynamic diagnostic port support
D-Port is a new port type created with the intention of running link
level diagnostic tests like loopback, traffic test. In static D-port
mode, user configures the port to D-port mode and starts the test, but
in dynamic D-port, once the Brocade switch port is configured to
D-port, it will reject the regular FLOGI from HBA with reason that it is
in D-port mode. So based on the reason code HBA port will turn itself into
D-port and start diagnostic test.

Vijay Mohan Guvva 111ecbe426 [SCSI] bfa: fix faulty handling of events in lps sm
When a switch disable/enable or a reboot is done, the HBA port gets an
offline and a subsequent online notification. When the port comes up a
link up notification is sent to bfa from the firmware. The bfa then send
an FLOGI to the firmware which is sent out on the wire.
The switch port meanwhile goes offline (presumably for diagnostics)
which causes the switch not to respond to the FLOGI.
The link down notification is sent to the HBA driver. However owing to a
bug in the lps state machine handling the lps state machine does not
move to sm_init state (it remains in sm_login state and send a login
complete message to fcs). This results in a zero PID assignment as the
login is not really complete.

This fix is to correctly handle the events in lps state machine.

Greg Kroah-Hartman 6f03979051 Drivers: scsi: remove __dev* attributes.
CONFIG_HOTPLUG is going away as an option.  As a result, the __dev*
markings need to be removed.

This change removes the use of __devinit, __devexit_p, __devinitdata,
__devinitconst, and __devexit from these drivers.

Krishna Gudipati 7d0ecef383 [SCSI] bfa: Firmware image naming convention update
- Modified the firmware naming convention to contain the
  firmware image version (3.1.0.0).
- The new convention is <firmware-image>-<firmware-version>.bin
- The change will enforce loading only compatible firmware with this driver
  and also avoid over-writing the old firmware image in-order to load new
  version driver as the firmware names used to be the same.

- Made changes such that once the PWWN is acquired from the fabric through FAA,
  and if the FAPWWN configuration is modified on the switch side, driver should
  show relevant information to the user.
- Added logic to cache the reason code when the given port is disabled implicitl
  due to FAA error condition.
- If the port is disabled, while sending SCN to upper layer, update the
  reason code appropriately. With this, BFA FC port state machine will enter
  into faa_err_config state. This state will be shown to the user.

- Introduced support for D-Port which is a new port mode during which
  link level diagnostics can be run.
- Provided mechanism to dynamically configure D-Port and initiate diagnostic
  tests to isolate any link level issues.

  remote device or run data traffic.
