Create ioremap_32.c and ioremap_64.c and move respective ioremap
functions out of pgtable_32.c and pgtable_64.c
In the meantime, fix a few comments and changes a printk() to
pr_warn(). Also fix a few oversplitted lines.

Drop multiple definitions of ioremap_bot and make one common to
all subarches.
Only CONFIG_PPC_BOOK3E_64 had a global static init value for
ioremap_bot. Now ioremap_bot is set in early_init_mmu_global().

ppc_md.ioremap() is only used for I/O workaround on CELL platform,
so indirect function call can be avoided.
This patch reworks the io-workaround and ioremap() functions to
use the global 'io_workaround_inited' flag for the activation
of io-workaround.
When CONFIG_PPC_IO_WORKAROUNDS or CONFIG_PPC_INDIRECT_MMIO are not
selected, the I/O workaround ioremap() voids and the global flag is
not used.

ppc_md.iounmap() is never set, drop it.
Once ppc_md.iounmap() is gone, iounmap() remains the only user of
__iounmap() and iounmap() does nothing else than calling __iounmap().
So drop iounmap() and make __iounmap() the new iounmap().

Even when we have HugeTLB and THP disabled, kernel linear map can still be
mapped with hugepages. This is only an issue with radix translation because hash
MMU doesn't map kernel linear range in linux page table and other kernel
map areas are not mapped using hugepage.
Add config independent helpers and put WARN_ON() when we don't expect things
to be mapped via hugepages.

__ioremap_at error handling is wonky, it requires caller to clean up
after it. Implement a helper that does the map and error cleanup and
remove the requirement from the caller.

Implement code to walk all pages and warn if any are found to be both
writable and executable. Depends on STRICT_KERNEL_RWX enabled, and is
behind the DEBUG_WX config option.
This only runs on boot and has no runtime performance implications.
Very heavily influenced (and in some cases copied verbatim) from the
ARM64 code written by Laura Abbott (thanks!), since our ptdump
infrastructure is similar.

This patch maps vmalloc, IO and vmemap regions in the 0xc address range
instead of the current 0xd and 0xf range. This brings the mapping closer
to radix translation mode.
With hash 64K page size each of this region is 512TB whereas with 4K config
we are limited by the max page table range of 64TB and hence there regions
are of 16TB size.
The kernel mapping is now:
On 4K hash
kernel_region_map_size = 16TB
kernel vmalloc start = 0xc000100000000000
kernel IO start = 0xc000200000000000

Set PAGE_KERNEL directly in the caller and do not rely on a
hack adding PAGE_KERNEL flags when _PAGE_PRESENT is not set.
As already done for PPC64, use pgprot_cache() helpers instead of

Update few code paths to check for pmd_large.
set_pmd_at:
We want to use this to store swap pte at pmd level. For swap ptes we don't want
to set H_PAGE_THP_HUGE. Hence check for pmd_large in set_pmd_at. This remove
the false WARN_ON when using this with swap pmd entry.
pmd_page:
We don't really use them on pmd migration entries. But they can also work with
migration entries and we don't differentiate at the pte level. Hence update
pmd_page to work with pmd migration entries too
__find_linux_pte:
lockless page table walk need to handle pmd migration entries. pmd_trans_huge
check will return false on them. We don't set thp = 1 for such entries, but
update hpage_shift correctly. Without this we will walk pmd migration entries
as a pte page pointer which is wrong.

Now that we have removed 64K page size support, the RCU page table free can
be much simpler for nohash. Make a copy of the the rcu callback to pgalloc.h
header similar to nohash 32. We could possibly merge 32 and 64 bit there. But
that is for a later patch
We also move the book3s specific handler to pgtable_book3s64.c. This will be
updated in a later patch to handle split pmd ptlock.

On POWER9, under some circumstances, a broadcast TLB invalidation
might complete before all previous stores have drained, potentially
allowing stale stores from becoming visible after the invalidation.
This works around it by doubling up those TLB invalidations which was
verified by HW to be sufficient to close the risk window.
This will be documented in a yet-to-be-published errata.

To support memory keys, we moved the hash pte slot information to the
second half of the page table. This was ok with PTE entries at level
4 (PTE page) and level 3 (PMD). We already allocate larger page table
pages at those levels to accomodate extra details. For level 4 we
already have the extra space which was used to track 4k hash page
table entry details and at level 3 the extra space was allocated to
track the THP details.
With hugetlbfs PTE, we used this extra space at the PMD level to store
the slot details. But we also support hugetlbfs PTE at PUD level for
16GB pages and PUD level page didn't allocate extra space. This
resulted in memory corruption.
Fix this by allocating extra space at PUD level when HUGETLB is
enabled.

Radix guests do normally invalidate process-scoped translations when a
new pid is allocated but migrated guests do not invalidate these so
migrated guests crash sometime, especially easy to reproduce with
migration happening within first 10 seconds after the guest boot start
on the same machine.
This adds the "Invalidate process-scoped translations" flush to fix
radix guests migration.

commit ac29c64089 ("powerpc/mm: Replace _PAGE_USER with
_PAGE_PRIVILEGED") introduced _PAGE_PRIVILEGED for BOOK3S/64
This patch generalises _PAGE_PRIVILEGED for all CPUs, allowing
to have either _PAGE_PRIVILEGED or _PAGE_USER or both.
PPC_8xx has a _PAGE_SHARED flag which is set for and only for
all non user pages. Lets rename it _PAGE_PRIVILEGED to remove
confusion as it has nothing to do with Linux shared pages.
On BookE, there's a _PAGE_BAP_SR which has to be set for kernel
pages: defining _PAGE_PRIVILEGED as _PAGE_BAP_SR will make
this generic

Most callers users of free_hot_cold_page claim the pages being released
are cache hot. The exception is the page reclaim paths where it is
likely that enough pages will be freed in the near future that the
per-cpu lists are going to be recycled and the cache hotness information
is lost. As no one really cares about the hotness of pages being
released to the allocator, just ditch the parameter.
The APIs are renamed to indicate that it's no longer about hot/cold
pages. It should also be less confusing as there are subtle differences
between them. __free_pages drops a reference and frees a page when the
refcount reaches zero. free_hot_cold_page handled pages whose refcount
was already zero which is non-obvious from the name. free_unref_page
should be more obvious.
No performance impact is expected as the overhead is marginal. The
parameter is removed simply because it is a bit stupid to have a useless
parameter copied everywhere.

CONFIG_PPC_STD_MMU_64 indicates support for the "standard" powerpc MMU
on 64-bit CPUs. The "standard" MMU refers to the hash page table MMU
found in "server" processors, from IBM mainly.
Currently CONFIG_PPC_STD_MMU_64 is == CONFIG_PPC_BOOK3S_64. While it's
annoying to have two symbols that always have the same value, it's not
quite annoying enough to bother removing one.
However with the arrival of Power9, we now have the situation where
CONFIG_PPC_STD_MMU_64 is enabled, but the kernel is running using the
Radix MMU - *not* the "standard" MMU. So it is now actively confusing
to use it, because it implies that code is disabled or inactive when
the Radix MMU is in use, however that is not necessarily true.
So s/CONFIG_PPC_STD_MMU_64/CONFIG_PPC_BOOK3S_64/, and do some minor
formatting updates of some of the affected lines.
This will be a pain for backports, but c'est la vie.

Currently KERN_IO_START is defined as:
#define KERN_IO_START (KERN_VIRT_START + (KERN_VIRT_SIZE >> 1))
Although it looks like a constant, both the components are actually
variables, to allow us to have a different value between Radix and
Hash with a single kernel.
However that still requires both Radix and Hash to place the kernel IO
region at the same location relative to the start and end of the
kernel virtual region (namely 1/2 way through it), and we'd like to
change that.
So split KERN_IO_START out into its own variable, and initialise it
for Radix and Hash. In the medium term we should be able to
reconsolidate this, by doing a more involved rearrangement of the
location of the regions.

Currently even with STRICT_KERNEL_RWX we leave the __init text marked
executable after init, which is bad.
Add a hook to mark it NX (no-execute) before we free it, and implement
it for radix and hash.
Note that we use __init_end as the end address, not _einittext,
because overlaps_kernel_text() uses __init_end, because there are
additional executable sections other than .init.text between
__init_begin and __init_end.
Tested on radix and hash with:

The Radix linear mapping code (create_physical_mapping()) tries to use
the largest page size it can at each step. Currently the only reason
it steps down to a smaller page size is if the start addr is
unaligned (never happens in practice), or the end of memory is not
aligned to a huge page boundary.
To support STRICT_RWX we need to break the mapping at __init_begin,
so that the text and rodata prior to that can be marked R_X and the
regular pages after can be marked RW.
Having done that we can now implement mark_rodata_ro() for Radix,
knowing that we won't need to split any mappings.

With hash we update the bolted pte to mark it read-only. We rely
on the MMU_FTR_KERNEL_RO to generate the correct permissions
for read-only text. The radix implementation just prints a warning
in this implementation

Add a trace point for tlbie(l) (Translation Lookaside Buffer Invalidate
Entry (Local)) instructions.
The tlbie instruction has changed over the years, so not all versions
accept the same operands. Use the ISA v3 field operands because they are
the most verbose, we may change them in future.

Introduce a helper pgtable_gfp_flags() which
just returns the current gfp flags and adds
__GFP_ACCOUNT to account for page table allocation.
The generic helper is added to include/asm/pgalloc.h
and has two variants - WARNING ugly bits ahead
1. If the header is included from a module, no check
for mm == &init_mm is done, since init_mm is not
exported
2. For kernel includes, the check is done and required
see (3e79ec7 arch: x86: charge page tables to kmemcg)
The fundamental assumption is that no module should be
doing pgd/pud/pmd and pte alloc's on behalf of init_mm
directly.
NOTE: This adds an overhead to pmd/pud/pgd allocations
similar to x86. The other alternative was to implement
pmd_alloc_kernel/pud_alloc_kernel and pgd_alloc_kernel
with their offset variants.
For 4k page size, pte_alloc_one no longer calls
pte_alloc_one_kernel.

When changing a partition table entry on POWER9, we do a particular
form of the tlbie instruction which flushes all TLBs and caches of
the partition table for a given logical partition ID (LPID).
This instruction has a field in the instruction word, labelled R
(radix), which should be 1 if the partition was previously a radix
partition and 0 if it was a HPT partition. This implements that
logic.

POWER9 contains an off core mmu called the nest mmu (NMMU). This is
used by other hardware units on the chip to translate virtual
addresses into real addresses. The unit attempting an address
translation provides the majority of the context required for the
translation request except for the base address of the partition table
(ie. the PTCR) which needs to be programmed into the NMMU.
This patch adds a call to OPAL to set the PTCR for the nest mmu in
opal_init().

POWER9 requires the host to set up a partition table, which is a
table in memory indexed by logical partition ID (LPID) which
contains the pointers to page tables and process tables for the
host and each guest.
This factors out the initialization of the partition table into
a single function. This code was previously duplicated between
hash_utils_64.c and pgtable-radix.c.
This provides a function for setting a partition table entry,
which is used in early MMU initialization, and will be used by
KVM whenever a guest is created. This function includes a tlbie
instruction which will flush all TLB entries for the LPID and
all caches of the partition table entry for the LPID, across the
system.
This also moves a call to memblock_set_current_limit(), which was
in radix_init_partition_table(), but has nothing to do with the
partition table. By analogy with the similar code for hash, the
call gets moved to near the end of radix__early_init_mmu(). It
now gets called when running as a guest, whereas previously it
would only be called if the kernel is running as the host.

This is the third version of the patchset previously sent [1]. I have
basically only rebased it on top of 4.7-rc1 tree and dropped "dm: get
rid of superfluous gfp flags" which went through dm tree. I am sending
it now because it is tree wide and chances for conflicts are reduced
considerably when we want to target rc2. I plan to send the next step
and rename the flag and move to a better semantic later during this
release cycle so we will have a new semantic ready for 4.8 merge window
hopefully.
Motivation:
While working on something unrelated I've checked the current usage of
__GFP_REPEAT in the tree. It seems that a majority of the usage is and
always has been bogus because __GFP_REPEAT has always been about costly
high order allocations while we are using it for order-0 or very small
orders very often. It seems that a big pile of them is just a
copy&paste when a code has been adopted from one arch to another.
I think it makes some sense to get rid of them because they are just
making the semantic more unclear. Please note that GFP_REPEAT is
documented as
* __GFP_REPEAT: Try hard to allocate the memory, but the allocation attempt
* _might_ fail. This depends upon the particular VM implementation.
while !costly requests have basically nofail semantic. So one could
reasonably expect that order-0 request with __GFP_REPEAT will not loop
for ever. This is not implemented right now though.
I would like to move on with __GFP_REPEAT and define a better semantic
for it.

So we are down to the third after this patch series. The remaining
places really seem to be relying on __GFP_REPEAT due to large allocation
requests. This still needs some double checking which I will do later
after all the simple ones are sorted out.
I am touching a lot of arch specific code here and I hope I got it right
but as a matter of fact I even didn't compile test for some archs as I
do not have cross compiler for them. Patches should be quite trivial to
review for stupid compile mistakes though. The tricky parts are usually
hidden by macro definitions and thats where I would appreciate help from
arch maintainers.

__GFP_REPEAT has a rather weak semantic but since it has been introduced
around 2.6.12 it has been ignored for low order allocations. Yet we
have the full kernel tree with its usage for apparently order-0
allocations. This is really confusing because __GFP_REPEAT is
explicitly documented to allow allocation failures which is a weaker
semantic than the current order-0 has (basically nofail).
Let's simply drop __GFP_REPEAT from those places. This would allow to
identify place which really need allocator to retry harder and formulate
a more specific semantic for what the flag is supposed to do actually.
