Linus Torvalds f632a8170a Driver Core and debugfs changes for 5.3-rc1
Here is the "big" driver core and debugfs changes for 5.3-rc1
 
 It's a lot of different patches, all across the tree due to some api
 changes and lots of debugfs cleanups.  Because of this, there is going
 to be some merge issues with your tree at the moment, I'll follow up
 with the expected resolutions to make it easier for you.
 
 Other than the debugfs cleanups, in this set of changes we have:
 	- bus iteration function cleanups (will cause build warnings
 	  with s390 and coresight drivers in your tree)
 	- scripts/get_abi.pl tool to display and parse Documentation/ABI
 	  entries in a simple way
 	- cleanups to Documenatation/ABI/ entries to make them parse
 	  easier due to typos and other minor things
 	- default_attrs use for some ktype users
 	- driver model documentation file conversions to .rst
 	- compressed firmware file loading
 	- deferred probe fixes
 
 All of these have been in linux-next for a while, with a bunch of merge
 issues that Stephen has been patient with me for.  Other than the merge
 issues, functionality is working properly in linux-next :)

Anshuman Khandual 8b1e0f81fb mm/pgtable: drop pgtable_t variable from pte_fn_t functions
Drop the pgtable_t variable from all implementation for pte_fn_t as none
of them use it.  apply_to_pte_range() should stop computing it as well.
Should help us save some cycles.

Mike Rapoport 28bcf59375 arm: switch to generic version of pte allocation
Replace __get_free_page() and alloc_pages() calls with the generic
__pte_alloc_one_kernel() and __pte_alloc_one().

There is no functional change for the kernel PTE allocation.

The difference for the user PTEs, is that the clear_pte_table() is now
called after pgtable_page_ctor() and the addition of __GFP_ACCOUNT to the
GFP flags.

The conversion to the generic version of pte_free_kernel() removes the NULL
check for pte.

The pte_free() version on arm is identical to the generic one and can be
simply dropped.

Pull force_sig() argument change from Eric Biederman:
 "A source of error over the years has been that force_sig has taken a
  task parameter when it is only safe to use force_sig with the current
  task.

  The force_sig function is built for delivering synchronous signals
  such as SIGSEGV where the userspace application caused a synchronous
  fault (such as a page fault) and the kernel responded with a signal.

  Because the name force_sig does not make this clear, and because the
  force_sig takes a task parameter the function force_sig has been
  abused for sending other kinds of signals over the years. Slowly those
  have been fixed when the oopses have been tracked down.

  This set of changes fixes the remaining abusers of force_sig and
  carefully rips out the task parameter from force_sig and friends
  making this kind of error almost impossible in the future"

Christoph Hellwig 34ab03160e arm-nommu: remove the partial DMA_ATTR_NON_CONSISTENT support
The arm-nommu DMA code supports DMA_ATTR_NON_CONSISTENT allocations, but
does not provide a cache_sync operation.  This means any user of it
will never be able to actually transfer cache ownership and thus cause
coherency bugs.

Christoph Hellwig ab746573c4 ARM: dma-mapping: allow larger DMA mask than supported
Since the Linux 5.1 merge window we allow drivers to just set the
largest DMA mask they support instead of falling back to smaller ones.

But I forgot to remove a check that prohibits this behavior in the
arm DMA code, as it is rather hidden.  There is not reason for this check
as the code will do the right thing for a "too large" DMA mask, so
just remove it.

Benjamin Gaignard 779eb41ccb ARM: 8862/1: errata: 814220-B-Cache maintenance by set/way operations can execute out of order
The v7 ARM states that all cache and branch predictor maintenance operations
that do not specify an address execute, relative to each other, in program
order. However, because of this erratum, an L2 set/way cache maintenance
operation can overtake an L1 set/way cache maintenance operation, this would
cause the data corruption.

This ERRATA affected the Cortex-A7 and present in r0p2, r0p3, r0p4, r0p5.

This patch is the SW workaround by adding a DSB before changing cache levels as
the ARM ERRATA: ARM/MP: 814220 told in the ARM ERRATA documentation.

Marek Szyprowski 5f41f9198f ARM: 8864/1: Add workaround for I-Cache line size mismatch between CPU cores
Some big.LITTLE systems have I-Cache line size mismatch between
LITTLE and big cores. This patch adds a workaround for proper I-Cache
support on such systems. Without it, some class of the userspace code
(typically self-modifying) might suffer from random SIGILL failures.

Similar workaround already exists for ARM64 architecture. I has been
added by commit 116c81f427 ("arm64: Work around systems with mismatched
cache line sizes").

Doug Anderson 304009a182 ARM: 8861/1: errata: Workaround errata A12 857271 / A17 857272
This adds support for working around errata A12 857271 / A17 857272.
These errata were causing hangs on rk3288-based Chromebooks and it was
confirmed that this workaround fixed the problems.  In the Chrome OS
3.14 kernel this was treated as two errata: ERRATA_FOOBAR [1] and
ERRATA_CR711784 [2].  Apparently the two errata got lumped together at
some point in time.

Let's actually get the workaround landed.

Mike Rapoport 8a7f97b902 treewide: add checks for the return value of memblock_alloc*()
Add check for the return value of memblock_alloc*() functions and call
panic() in case of error.  The panic message repeats the one used by
panicing memblock allocators with adjustment of parameters to include
only relevant ones.

The replacement was mostly automated with semantic patches like the one
below with manual massaging of format strings.

Mike Rapoport f240ec09bb memblock: replace memblock_alloc_base(ANYWHERE) with memblock_phys_alloc
The calls to memblock_alloc_base(size, align, MEMBLOCK_ALLOC_ANYWHERE)
and memblock_phys_alloc(size, align) are equivalent as both try to
allocate 'size' bytes with 'align' alignment anywhere in the memory and
panic if hte allocation fails.

The conversion is done using the following semantic patch:

Mike Rapoport b63a07d69d arch: simplify several early memory allocations
There are several early memory allocations in arch/ code that use
memblock_phys_alloc() to allocate memory, convert the returned physical
address to the virtual address and then set the allocated memory to
zero.

Exactly the same behaviour can be achieved simply by calling
memblock_alloc(): it allocates the memory in the same way as
memblock_phys_alloc(), then it performs the phys_to_virt() conversion
and clears the allocated memory.

Replace the longer sequence with a simpler call to memblock_alloc().

Stefan Agner b7e8c9397c ARM: 8845/1: use unified assembler in c files
Use unified assembler syntax (UAL) in inline assembler. Divided
syntax is considered deprecated. This will also allow to build
the kernel using LLVM's integrated assembler.

When compiling non-Thumb2 GCC always emits a ".syntax divided"
at the beginning of the inline assembly which makes the
assembler fail. Since GCC 5 there is the -masm-syntax-unified
GCC option which make GCC assume unified syntax asm and hence
emits ".syntax unified" even in ARM mode. However, the option
is broken since GCC version 6 (see GCC PR88648 [1]). Work
around by adding ".syntax unified" as part of the inline
assembly.

Robin Murphy fc67e6f120 ARM: 8835/1: dma-mapping: Clear DMA ops on teardown
Installing the appropriate non-IOMMU DMA ops in arm_iommu_detch_device()
serves the case where IOMMU-aware drivers choose to control their own
mapping but still make DMA API calls, however it also affects the case
when the arch code itself tears down the mapping upon driver unbinding,
where the ops now get left in place and can inhibit arch_setup_dma_ops()
on subsequent re-probe attempts.

Fix the latter case by making sure that arch_teardown_dma_ops() cleans
up whenever the ops were automatically installed by its counterpart.

Vladimir Murzin 72cd4064fc ARM: 8830/1: NOMMU: Toggle only bits in EXC_RETURN we are really care of
ARMv8M introduces support for Security extension to M class, among
other things it affects exception handling, especially, encoding of
EXC_RETURN.

The new bits have been added:

Bit [6]	Secure or Non-secure stack
Bit [5]	Default callee register stacking
Bit [0]	Exception Secure

which conflicts with hard-coded value of EXC_RETURN:

In fact, we only care of few bits:

Bit [3]	 Mode (0 - Handler, 1 - Thread)
Bit [2]	 Stack pointer selection (0 - Main, 1 - Process)

We can toggle only those bits and left other bits as they were on
exception entry.

It is basically, what patch does - saves EXC_RETURN when we do
transition form Thread to Handler mode (it is first svc), so later
saved value is used instead of EXC_RET_THREADMODE_PROCESSSTACK.

Doug Berger 071d184a19 ARM: 8826/1: mm: initialize pfn limits with find_limits()
The max_low_pfn value must be set before sparse_init() is called to
keep the early memblock allocations and frees balanced for kmemleak
initialization when sparsemem is enabled.

This commit accomplishes that by replacing the local variables min,
max_low, and max_high with the global limit variables min_low_pfn,
max_low_pfn, and max_pfn respectively in bootmem_init(). The global
variables are initialized directly by find_limits() and used in the
remainder of the function.

Geert Uytterhoeven 1c31d4e96b ARM: 8820/1: mm: Stop printing the virtual memory layout
Since commit ad67b74d24 ("printk: hash addresses printed with
%p"), the virtual memory layout printed during boot up contains "ptrval"
instead of actual addresses:

All interesting information (actual section sizes) is already printed by
mem_init_print_info() just above anyway.

Ard Biesheuvel 9ec5cd0a40 ARM: 8817/1: mm: skip cleaning of idmap page tables on LPAE capable cores
Currently, init_static_idmap() installs some page table entries to
cover the identity mapped part of the kernel image (which is only
about 160 bytes in size in a multi_v7_defconfig Thumb2 build), and
calls flush_cache_louis() to ensure that the updates are visible
to the page table walker on the same core.

When running under virtualization, flush_cache_louis() may take more
than 10 seconds to complete:

[   13.078127] rcu: Hierarchical SRCU implementation.

This is due to the fact that set/way ops are not virtualizable, and so
KVM may trap each one, resulting in a substantial delay.

Since only LPAE capable CPUs may execute under virtualization, and
considering that LPAE capable CPUs are guaranteed to have cache
coherent page table walkers (per the architecture), let's only
perform this cache maintenance on non-LPAE cores.

Andrew Murray cafa780e7b perf/core, arch/arm: Use PERF_PMU_CAP_NO_EXCLUDE for exclusion incapable PMUs
For drivers that do not support context exclusion let's advertise the
PERF_PMU_CAP_NO_EXCLUDE capability. This ensures that perf will
prevent us from handling events where any exclusion flags are set.
Let's also remove the now unnecessary check for exclusion flags.

The intention in the previous patch was to only place the processor
tables in the .rodata section if big.Little was being built and we
wanted the branch target hardening, but instead (due to the way it

Marek Szyprowski reported problems with CPU hotplug in current kernels.
This was tracked down to the processor vtables being located in an
init section, and therefore discarded after kernel boot, despite being
required after boot to properly initialise the non-boot CPUs.

Arrange for these tables to end up in .rodata when required.

Arm already returns (~(dma_addr_t)0x0) on mapping failures, so we can
switch over to returning DMA_MAPPING_ERROR and let the core dma-mapping
code handle the rest.

Nathan Jones c2a3831df6 ARM: 8816/1: dma-mapping: fix potential uninitialized return
While trying to use the dma_mmap_*() interface, it was noticed that this
interface returns strange values when passed an incorrect length.

If neither of the if() statements fire then the return value is
uninitialized. In the worst case it returns 0 which means the caller
will think the function succeeded.

Chris has discovered and reported that v7_dma_inv_range() may corrupt
memory if address range is not aligned to cache line size.

Since the whole cache-v7m.S was lifted form cache-v7.S the same
observation applies to v7m_dma_inv_range(). So the fix just mirrors
what has been done for v7 with a little specific of M-class.

Chris Cole a1208f6a82 ARM: 8814/1: mm: improve/fix ARM v7_dma_inv_range() unaligned address handling
This patch addresses possible memory corruption when
v7_dma_inv_range(start_address, end_address) address parameters are not
aligned to whole cache lines. This function issues "invalidate" cache
management operations to all cache lines from start_address (inclusive)
to end_address (exclusive). When start_address and/or end_address are
not aligned, the start and/or end cache lines are first issued "clean &
invalidate" operation. The assumption is this is done to ensure that any
dirty data addresses outside the address range (but part of the first or
last cache lines) are cleaned/flushed so that data is not lost, which
could happen if just an invalidate is issued.

The problem is that these first/last partial cache lines are issued
"clean & invalidate" and then "invalidate". This second "invalidate" is
not required and worse can cause "lost" writes to addresses outside the
address range but part of the cache line. If another component writes to
its part of the cache line between the "clean & invalidate" and
"invalidate" operations, the write can get lost. This fix is to remove
the extra "invalidate" operation when unaligned addressed are used.

A kernel module is available that has a stress test to reproduce the
issue and a unit test of the updated v7_dma_inv_range(). It can be
downloaded from
http://ftp.sageembedded.com/outgoing/linux/cache-test-20181107.tgz.

v7_dma_inv_range() is call by dmac_[un]map_area(addr, len, direction)
when the direction is DMA_FROM_DEVICE. One can (I believe) successfully
argue that DMA from a device to main memory should use buffers aligned
to cache line size, because the "clean & invalidate" might overwrite
data that the device just wrote using DMA. But if a driver does use
unaligned buffers, at least this fix will prevent memory corruption
outside the buffer.

  per-cpu can't be used. We also have a problem that two of the methods
  are not called from preempt-safe paths, but thankfully these remain
  identical between all CPUs in the system. To make sure, we validate
  that these are identical during boot"

In big.Little systems, some CPUs require the Spectre workarounds in
paths such as the context switch, but other CPUs do not.  In order
to handle these differences, we need per-CPU vtables.

We are unable to use the kernel's per-CPU variables to support this
as per-CPU is not initialised at times when we need access to the
vtables, so we have to use an array indexed by logical CPU number.

We use an array-of-pointers to avoid having function pointers in
the kernel's read/write .data section.

Nicolas Pitre b99afae139 ARM: 8805/2: remove unneeded naked function usage
The naked attribute is known to confuse some old gcc versions when
function arguments aren't explicitly listed as inline assembly operands
despite the gcc documentation. That resulted in commit 9a40ac8615
("ARM: 6164/1: Add kto and kfrom to input operands list.").

Yet that commit has problems of its own by having assembly operand
constraints completely wrong. If the generated code has been OK since
then, it is due to luck rather than correctness. So this patch also
provides proper assembly operand constraints, and removes two instances
of redundant register usages in the implementation while at it.

Inspection of the generated code with this patch doesn't show any
obvious quality degradation either, so not relying on __naked at all
will make the code less fragile, and avoid some issues with clang.

The only remaining __naked instances (excluding the kprobes test cases)
are exynos_pm_power_up_setup(), tc2_pm_power_up_setup() and

cci_enable_port_for_self(. But in the first two cases, only the function
address is used by the compiler with no chance of inlining it by
mistake, and the third case is called from assembly code only. And the
fact that no stack is available when the corresponding code is executed
does warrant the __naked usage in those cases.

 "I have been slowly sorting out siginfo and this is the culmination of
  that work.

  The primary result is in several ways the signal infrastructure has
  been made less error prone. The code has been updated so that manually
  specifying SEND_SIG_FORCED is never necessary. The conversion to the
  new siginfo sending functions is now complete, which makes it
  difficult to send a signal without filling in the proper siginfo
  fields.

  At the tail end of the patchset comes the optimization of decreasing
  the size of struct siginfo in the kernel from 128 bytes to about 48
  bytes on 64bit. The fundamental observation that enables this is by
  definition none of the known ways to use struct siginfo uses the extra
  bytes.

  This comes at the cost of a small user space observable difference.
  For the rare case of siginfo being injected into the kernel only what
  can be copied into kernel_siginfo is delivered to the destination, the
  rest of the bytes are set to 0. For cases where the signal and the
  si_code are known this is safe, because we know those bytes are not
  used. For cases where the signal and si_code combination is unknown
  the bits that won't fit into struct kernel_siginfo are tested to
  verify they are zero, and the send fails if they are not.

  I made an extensive search through userspace code and I could not find
  anything that would break because of the above change. If it turns out
  I did break something it will take just the revert of a single change
  to restore kernel_siginfo to the same size as userspace siginfo.

  Testing did reveal dependencies on preferring the signo passed to
  sigqueueinfo over si->signo, so bit the bullet and added the
  complexity necessary to handle that case.

  Testing also revealed bad things can happen if a negative signal
  number is passed into the system calls. Something no sane application
  will do but something a malicious program or a fuzzer might do. So I
  have fixed the code that performs the bounds checks to ensure negative
  signal numbers are handled"

Christoph Hellwig 58b0440663 dma-mapping: consolidate the dma mmap implementations
The only functional differences (modulo a few missing fixes in the arch
code) is that architectures without coherent caches need a hook to
convert a virtual or dma address into a pfn, given that we don't have
the kernel linear mapping available for the otherwise easy virt_to_page
call.  As a side effect we can support mmap of the per-device coherent
area even on architectures not providing the callback, and we make
previous dangerous default methods dma_common_mmap actually save for
non-coherent architectures by rejecting it without the right helper.

In addition to that we need a hook so that some architectures can
override the protection bits when mmaping a dma coherent allocations.

Thomas Petazzoni 3a58ac65e2 ARM: 8799/1: mm: fix pci_ioremap_io() offset check
IO_SPACE_LIMIT is the ending address of the PCI IO space, i.e
something like 0xfffff (and not 0x100000).

Therefore, when offset = 0xf0000 is passed as argument, this function
fails even though the offset + SZ_64K fits below the
IO_SPACE_LIMIT. This makes the last chunk of 64 KB of the I/O space
not usable as it cannot be mapped.

This patch fixes that by substracing 1 to offset + SZ_64K, so that we
compare the addrss of the last byte of the I/O space against
IO_SPACE_LIMIT instead of the address of the first byte of what is
after the I/O space.

Marek Szyprowski d834c5ab83 kernel/dma: remove unsupported gfp_mask parameter from dma_alloc_from_contiguous()
The CMA memory allocator doesn't support standard gfp flags for memory
allocation, so there is no point having it as a parameter for
dma_alloc_from_contiguous() function.  Replace it by a boolean no_warn
argument, which covers all the underlaying cma_alloc() function
supports.

This will help to avoid giving false feeling that this function supports
standard gfp flags and callers can pass __GFP_ZERO to get zeroed buffer,
what has already been an issue: see commit dd65a941f6 ("arm64:
dma-mapping: clear buffers allocated with FORCE_CONTIGUOUS flag").

Souptick Joarder 50a7ca3c6f mm: convert return type of handle_mm_fault() caller to vm_fault_t
Use new return type vm_fault_t for fault handler.  For now, this is just
documenting that the function returns a VM_FAULT value rather than an
errno.  Once all instances are converted, vm_fault_t will become a
distinct type.

In this patch all the caller of handle_mm_fault() are changed to return
vm_fault_t type.

     This value is not supported in ARMv8 if ID_PFR1 bits [7:4] are zero.

0001 The implementation includes the VBAR, and the TCR.PD0 and TCR.PD1
     bits.

0010 As for 0001, plus the ability to access Secure or Non-secure
     physical memory is supported.

All other values are reserved.

This field is only valid when ID_PFR1[7:4] == 0, otherwise it holds
the value 0000.

Steven Rostedt (VMware) b4c7e2bd2e ARM: 8780/1: ftrace: Only set kernel memory back to read-only after boot
Dynamic ftrace requires modifying the code segments that are usually
set to read-only. To do this, a per arch function is called both before
and after the ftrace modifications are performed. The "before" function
will set kernel code text to read-write to allow for ftrace to make the
modifications, and the "after" function will set the kernel code text
back to "read-only" to keep the kernel code text protected.

The issue happens when dynamic ftrace is tested at boot up. The test is
done before the kernel code text has been set to read-only. But the
"before" and "after" calls are still performed. The "after" call will
change the kernel code text to read-only prematurely, and other boot
code that expects this code to be read-write will fail.

The solution is to add a variable that is set when the kernel code text
is expected to be converted to read-only, and make the ftrace "before"
and "after" calls do nothing if that variable is not yet set. This is
similar to the x86 solution from commit 1623963097 ("ftrace, x86:
make kernel text writable only for conversions").

Pull more overflow updates from Kees Cook:
 "The rest of the overflow changes for v4.18-rc1.

  This includes the explicit overflow fixes from Silvio, further
  struct_size() conversions from Matthew, and a bug fix from Dan.

  But the bulk of it is the treewide conversions to use either the
  2-factor argument allocators (e.g. kmalloc(a * b, ...) into
  kmalloc_array(a, b, ...) or the array_size() macros (e.g. vmalloc(a *

Kees Cook 6396bb2215 treewide: kzalloc() -> kcalloc()
The kzalloc() function has a 2-factor argument form, kcalloc(). This
patch replaces cases of:

        kzalloc(a * b, gfp)

with:
        kcalloc(a * b, gfp)

as well as handling cases of:

        kzalloc(a * b * c, gfp)

with:

        kzalloc(array3_size(a, b, c), gfp)

as it's slightly less ugly than:

        kzalloc_array(array_size(a, b), c, gfp)

This does, however, attempt to ignore constant size factors like:

        kzalloc(4 * 1024, gfp)

though any constants defined via macros get caught up in the conversion.

Any factors with a sizeof() of "unsigned char", "char", and "u8" were
dropped, since they're redundant.

The Coccinelle script used for this was:
