MMC/SD card driver learns SPI

Teaching the MMC/SD block card driver about SPI.

 - Provide the SPI response type flags with each request issued.

 - Understand that multiblock SPI writes don't use STOP_TRANSMISSION.

 - Correct check for APP_CMD failure.
